ВЫВОДЫ

1. Кальцификация клапанных структур без формирования порока сердца выявлялась у 23,2% женщин в постменопаузе и локализовалась только в левых отделах сердца. При этом преобладали сочетанные поражения колец и створок, преимущественно аортального клапана.

2. Женщины в постменопаузе с кальцификацией клапанных структур без порока сердца были старше по возрасту и имели более высокий уровень общего холестерина по сравнению с группой женщин без кальцификации клапанных структур сердца. Распространённость гипертонической болезни у женщин в постменопаузе с кальцификацией клапанных структур сердца (42,2%) была выше, чем у женщин, не имевших кальцификации клапанных структур (31,2%), достигая статистически значимых различий при кальцинированном аортальном стенозе (68,1%). Больные с кальцинированным аортальным стенозом чаще имели хроническую болезнь почек и сахарный диабет 2 типа, по сравнению с женщинами без кальцификации клапанных структур сердца.

3. Женщины в постменопаузе с различной степенью выраженности кальцификации клапанных структур сердца (с пороком и без формирования порока сердца) имели более низкий уровень скорости клубочковой фильтрации и более высокие уровни мочевой кислоты и фосфора, чем женщины, не имевшие кальцификации клапанных структур сердца. Частота гиперурикемии у женщин с кальцинированным аортальным стенозом достигла 31,9%, существенно превышая этот показатель в других группах (Р<0,05).

4. Нарушения минеральной плотности кости, преимущественно остеопороз, выявлены у 75,7% женщин с кальцификацией клапанных структур без порока сердца и у 71,7% женщин с кальцинированным аортальным стенозом, что существенно превышает распространённость остеопороза у женщин без калыдафикации клапанных структур (36,7%) (р<0,05). Выраженность снижения МПК у женщин в постменопаузе до формирования порока сердца имеет прямую корреляционную связь со снижением СКФ.

5. Различные сердечно-сосудистые осложнения (инфаркт миокарда, инсупьт; фибрилляция предсердий) зарегистрированы главным образом у женщин с кальцификацией клапанных структур без порока сердца и с кальцинированным аортальным стенозом. Число женщин с сердечнососудистыми осложнениями, в том числе летальными, было наибольшим в группе с кальцинированным аортальным стенозом (44,7%) и достоверно превышало число женщин с осложнениями в группе без кальцификации клапанных структур сердца (4,59%) (р<0,001).

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

1. В план диспансерного обследования женщин в постменопаузе необходимо включать ЭхоКГ и рентгеновскую денситометрию для выявления и оценки выраженности кальцификации клапанных структур сердца и нарушений МПК.

2. Нягтичие кальцификации хлапапкых структур сердца и/или КАС у женщин в постменопаузе является показанием для углублённого обследования сердечно-сосудистой системы, расчёта СКФ и коррекции модулируемых факторов риска ССО, ведущим из которых является АГ.

3. Женщинам с нарушениями МПК следует рекомендовать ЭхоКГ для уточнения наличия и степени выраженности кальцификации клапанных структур сердца.

4. Для своевременного выявления признаков формирования КАС необходимо проводить повторные ЭхоКГ женщинам с кальцификацией клапанных структур сердца. При развитии КАС показана консультация кардиолога для уточнения выраженности клапанной дисфункции, определения лечебной тактики, в том числе показаний к оперативной коррекции порока сердца.
