ВВЕДЕНИЕ

АКТУАЛЬНОСТЬ ИССЛЕДОВАНИЯ

Рак молочной железы (РМЖ) на протяжении последних десятилетий

занимает лидирующие позиции среди злокачественных новообразований

женского населения. Количество заболевших в России составляет 38.2 на

ЮО'ООО населения. Пик развития рака приходится на возраст от 50 до 60 лет.

Среди данной популяции смертность вызываемая раком молочной железы

достигает 25-40% от числа заболевших [1]. В общей группе больных с

метастатическим поражением головного мозга РМЖ занимает 2-е место,

уступая только раку легкого. В то время как при ретроспективном анализе,

частота метастазирования в головной мозг при раке молочной железы

составляет 10-20%, по данным аутопсии метастазы в головной мозг

диагностируют в 30% случаев [21, 22]. Наличие метастазов повышает

смертность при раке молочной железы [22]. Так, независимо от возможных

методов лечения продолжительность жизни пациентов не превышает 12-24

месяцев. Таким образом, необходимость поиска и внедрения в клиническую

практику новых специфичных маркёров ранней диагностики метастазирования

при раке молочной железы, очевидна. В настоящее время, помимо онкогенов и

генов-супрессоров опухолевого роста, регулирующих формирование первичной

опухоли, выделяют новый класс генов, функцией которых является регуляция

метастазирования. Изучение данного класса генов расширяет понимание

механизмов опухолевой прогрессии, а также имеет практический интерес с

точки зрения поиска новых мишеней для терапии онкологических заболеваний.

Среди генов-супрессоров метастазирования известны такие гены как KAI, NM23,

а также KISS1 [23-25]. Впервые ген-супрессор KISS1 был обнаружен в клетках

метастазирующей меланомы в 1996 году [26]. Позднее Miele и Shirasaki с соавт,

определили локализацию данного гена в локусе 6ql6.3-q23 хромосомы, и

выяснили что в результате делеций KISS1 гена способность к метастазированию

клеток меланомы увеличивается [27, 28]. Несмотря на значительное количество накопившихся к настоящему времени экспериментальных данных, касающихся возможной роли этого гена в трансформации и метастазировании опухолевых клеток, механизм его действия до конца неизвестен. Изучение последствий снижения экспрессии гена ШББ! позволит выяснить физиологическую роль продукта данного гена-мишени. Известно, что наряду с генетическими событиями, обуславливающими инактивацию клеточных генов, вследствие изменения последовательности ДНК, важную роль в процессе канцерогенеза играют также эпигенетические явления, не затрагивающие первичную структуру ДНК, но значительно изменяющие экспрессию генов [2,4,13]. Одним из таких эпигенетических механизмов является абберантное гиперметилирование специфических последовательностей Срв-островков, расположенных в 5'-регуляторных районах многих генов, которое влечет за собой подавление экспрессии гена. Практически половина молчащих генов супрессоров и генов репарации инактивируются в результате метилирования промоторного района [29]. Метилирование ДНК является важнейшим механизмом инактивации генов супрессоров самых различных морфологических типах опухолей [30]. Анализ наиболее часто метилируемых генов способствует созданию систем диагностических и прогностических маркеров.

ЦЕЛЬ ИССЛЕДОВАНИЯ: изучить особенностии экспрессии гена-супрессора К1881 при метастазировании опухолевых клеток рака молочной железы в головной мозг для определения его прогностической значимости в развитии заболевания.

ЗАДАЧИ ИССЛЕДОВАНИЯ

1. Исследовать уровень экспрессии гена KISS1 в клетках нормальной ткани, РМЖ, церебральных метастазах (МРМЖ) и определить взаимосвязь с клинико-морфологическими признаками первичной опухоли молочной железы.

2. Подтвердить измененный характер транскрипции гена KJSS1 методом полимеразной цепной реакции в реальном времени.

3. Исследовать статус и частоту метилирования гена KISS1 в клетках нормальной ткани, РМЖ и церебральных метастазах.

4. Провести анализ изменения пролиферации и миграции клеток рака молочной железы после нокдауна гена KISS1.

5. Провести сравнительный анализ уровня KISS1 в сыворотке больных РМЖ и церебральных метастазах.

6. Изучить прогностическое значение экспрессии гена-супрессора KISSI в ткани опухоли для продолжительности жизни пациентов.

НАУЧНАЯ НОВИЗНА

Изучение молекулярно-биологических маркеров метастазирующих злокачественных опухолей имеет фундаментальное значение, и определяет направление в создании новых диагностических и противоопухолевых подходов к лечению злокачественных опухолей в клинике.

Впервые определена экспрессия кисспептина в церебральных метастазах РМЖ. Изучена взаимосвязь относительного содержания гена KISS1 в ткани опухоли молочной железы с рецепторным статусом опухоли. Для выявления корреляции между экспрессией белка и уровнем мРНК, проведен сравнительный анализ содержания мРНК гена мишени методом количественной ПЦР. Отмечено соответствие между экспрессией мРНК гена KISS1 и уровнем белка в клетках первичной опухоли РМЖ и церебральных метастазов.

Впервые показано, что в сыворотке крови больных с МРМЖ содержится более высокий уровень KJSS1 по сравнению с уровнем этого показателя в сыворотке крови пациентов РМЖ. Для изучения функции гена KISS1 был произведен нокдаун гена в клетках рака молочной железы и впоследствии изучена пролиферативная и миграционная активность данных клеток.

Впервые, изучено прогностическое значение экспрессии KISS1 для продолжительности жизни пациентов метастатическим РМЖ. Проведен анализ частоты метилирования гена в клетках нормальной ткани молочной железы, первичной опухоли и МРМЖ.

НАУЧНО-ПРАКТИЧЕСКАЯ ЗНАЧИМОСТЬ

В результате проведенного исследования оценена клиническая значимость и информативность определения уровня экспрессии гена KISS1 в ткани РМЖ и МРМЖ в зависимости от клинико-морфологического состояния опухоли. Полученные данные позволяют установить значимость гена-супрессора KISS1 в процессе метастазирования опухолевых клеток рака молочной железы в головной мозг. Исследованный маркер может быть использован в практической онкологии для оценки риска метастазирования опухолевых клеток в отдалённые органы. Данное исследование направлено на создание вспомогательных инструментов в определении опухолевого роста на ранних этапах болезни, прогнозирования осложнений в результате миграции клеток в другие органы и последующего развития вторичного опухолевого роста.

ГЛАВА 5 ВЫВОДЫ

1. Полученные данные свидетельствуют о достоверном снижении уровня экспрессии белка и мРНК гена-супрессора KISS1 в клетках церебральных метастазов по отношению к первичной опухоли рака молочной железы (РМЖ).

2. Выявлена положительная корреляция экспрессии гена KISS1 с положительным прогестероновым статусом опухоли и ассоциация с клинической стадией рака.

3. В сыворотке крови больных РМЖ обнаружен более высокий уровень белка KISS1 (6.2±1.25 мкг/мл) по сравнению с уровнем этого показателя в крови пациентов с церебральными метастазами (1,2±1.25 мкг/мл, соответственно, р<0,05).

4. С помощью однофакторного анализа показана связь экспрессии гена KISS1 с удлинением продолжительности жизни больных.

5. Наибольшая частота абберантного метилирования промоторного региона гена KISS1 была обнаружена в клетках церебральных метастазов (90.9%) в сравнении с образцами первичной опухоли РМЖ (41%).

6. Методом РНК-интерференции проведен нокдаун гена-супрессора в клетках РМЖ (MDA-231BR). Специфичность метода оценена по снижению уровня мРНК и белка изучаемого гена.

7. Показано, что нокдаун гена KISS1 в клетках РМЖ (MDA-231BR) влияет на их миграционную способность в экспериментах in vitro.
