Остеоартроз представляет собой гетерогенную группу заболеваний суставов различной этиологии со сходными биологическими, морфологическими и клиническими признаками [10]. В основе ОА лежит нарушение равновесия между анаболическими и катаболическими процессами в тканях сустава, прежде всего в гиалиновом хряще - основном и первичном месте патологических изменений [39]. Помимо гиалинового хряща при ОА патологически изменяются и другие ткани сустава: синовиальная оболочка, субхондральная кость, суставная капсула, внутрисуставные связки и околосуставные мышцы [37, 62, 69, 71].

Истинную распространенность ОА трудно оценить, так как многие рентгенопозитивные по этому заболеванию больные не имеют клинических симптомов. Однако, несомненно, что по распространенности он лидирует среди других ревматических болезней. ОА встречается у 10 % населения; каждый второй пациент с этим заболеванием старше 50 лет [5, 7]. С возрастом частота ОА резко возрастает: его рентгенологические признаки определяются у 90% женщин и у 80% мужчин старше 70 лет [2]. В первую очередь при ОА поражаются так называемые нагрузочные суставы, что значительно ухудшает качество жизни больных и представляет серьезную социально-экономическую проблему [2, 8]. ОА не влияет непосредственно на жизненный прогноз, но является одной из основных причин преждевременной потери трудоспособности и инвалидности [10].

Патогенез остеоартроза очень сложен. Очень трудно или даже невозможно разделить долю и значимость отдельных факторов в развитии ОА. Однако в последние годы внимание исследователей фокусируется на идентификации и значимости отдельных звеньев патогенеза. Эти данные важны для более успешной фармакотерапии остеоартроза. Большой интерес представляют данные последних исследований о взаимосвязи метаболического синдрома и остеоартроза [24, 26]. Показано, что висцеральная жировая ткань, накапливающаяся при метаболическом синдроме, является активным эндокринным органом, который продуцирует провоспалительные цитокины и большое число адипокинов [11, 12, 14, 17]. Особенно важна информация о лептине - продукте жировой ткани, действующего и как гормон, и как цитокин [184]. Показано, что лептин и его рецепторы принимают участие в метаболизме хондроцитов [9, 184].

В последние годы показано, что ожирение является одной из основных причин развития остеоартроза [355, 356]. При нем происходит не только повышенная механическая нагрузка на суставы, но и развивается метаболический синдром и связанные с ним биохимические и системные нарушения в метаболизме липидов [4, 20, 21]. Метаболический синдром характеризуется увеличением массы висцерального жира, снижением чувствительности периферических тканей к инсулину и гиперинсулинемией, которые вызывают развитие нарушений углеводного, липидного и пуринового обмена [18, 19, 28, 32, 34]. Выделение метаболического синдрома имеет большое клиническое значение, поскольку, с одной стороны, это состояние является обратимым, а с другой стороны, оно предшествует возникновению целого ряда заболеваний, таких как сахарный диабет 2 типа, атеросклероз, остеоартроз [27, 72, 73]. Чрезвычайно трудно найти единственную первопричину в патогенезе метаболического синдрома, поскольку все причины тесно взаимосвязаны и взаимообусловлены. В качестве пускового фактора рассматривается абдоминальное ожирение, в результате которого адипоциты висцеральной жировой ткани секретируют ряд адипокинов [72, 82]. Одним из них является лептин, который по данным литературы регулирует чувство насыщения [184]. При метаболическом синдроме концентрация лептина в крови повышается, однако из-за утраты чувствительности к нему гипоталамических центров развивается относительная лептинрезистентность. В результате происходит повышение активности симпатической и снижение тонуса парасимпатичекой систем, что приводит к увеличению сердечного выброса, частоты сердечных сокращений, снижению вариабельности сердечного ритма, повышению общего периферического сосудистого сопротивления [97, 105]. Кроме того, происходит нарушение функции гипатоламо-гипофизарно-надпочечниковой системы. Лептин активно влияет на метаболизм, как показал ряд исследований, он также функционирует подобно провоспалительному адипокину [105, 299]. Предполагается, что лептин является связующим звеном между иммунными и воспалительными процессами [299]. Есть данные о том, что лептин воздействуя на Т-клетки, играет ключевую роль в патогенезе ряда воспалительных заболеваний, таких как аутоиммунный энцефалит, воспалительные заболевания кишечника, а также дегенеративные и воспалительные заболевания суставов [231, 232]. Гиперэкспрессия лептина в хряще при ОА коррелирует со степенью его разрушения [105]. Внутрисуставные инъекции лептина стимулируют анаболические процессы в хондроцитах и вызывают синтез ростовых факторов в хряще и в хондроцитах [299]. Все это свидетельствует о том, что лептин является связующем звеном между остеоартрозом и ожирением и, возможно, играет важную роль в метаболизме хряща [360].

Таким образом, изучение клинико-патогенетического значения лептина у больных с остеоартрозом является актуальной задачей и может способствовать созданию новых подходов к терапии этого заболевания.

ЦЕЛЬ РАБОТЫ

Изучить клинико-патогенетическое значение определения уровня лептина в сыворотке крови больных с остеоартрозом.

ОСНОВНЫЕ ЗАДАЧИ ИССЛЕДОВАНИЯ

1. Изучить концентрацию лептина в сыворотке крови в группе здоровых лиц.

2. Изучить концентрацию лептина в сыворотке крови у пациентов с остеоартрозом.

3. Изучить взаимосвязь между уровнем лептина в сыворотке крови и клиническими проявлениями остеоартроза.

4. Изучить взаимосвязь между уровнем лептина и лабораторными маркерами воспаления при остеоартрозе.

5. Изучить взаимосвязь между динамикой уровня лептина в сыворотке крови и изменения клинического состояния больных остеоартрозом на фоне снижения веса.

НАУЧНАЯ НОВИЗНА

Впервые на большом клиническом материале у больных остеоартрозом методом иммуноферментного анализа изучен уровень лептина в сыворотке крови. Определена связь между уровнем лептина и клиническими проявлениями остеоартроза. Прослежена взаимосвязь между уровнем лептина и лабораторными маркерами воспалительного процесса, такими как СОЭ, С-реактивный белок. Полученные данные позволяют предположить, что лептин может быть патогенетическим фактором, участвующим в прогрессировании при ОА.

ПРАКТИЧЕСКАЯ ЗНАЧИМОСТЬ РАБОТЫ

Изучена диагностическая ценность определения лептина в сыворотке крови при ОА. Разработаны критерии тяжести течения ОА в зависимости от уровня лептина крови. Показано, что при его высокой концентрации наблюдается более тяжелое течение ОА. Имеется корреляционная связь между уровнем лептина в сыворотке крови с одной стороны и суставными индексами, лабораторными маркерами воспаления - с другой.

Продемонстрирована уменьшение концентрации лептина при ОА на фоневеса пациентов более 2 килограмм, которое коррелировало с улучшением клиническое картины заболевания. Все это позволяет рекомендовать метод определения лептина в сыворотке крови для мониторинга тяжести состояния больных ОА в клинической практике.

ОСНОВНЫЕ ПОЛОЖЕНИЯ, ВЫНОСИМЫЕ НА ЗАЩИТУ

Показано, что у больных ОА повышен уровень лептина в сыворотке крови по сравнению с группой здоровых лиц. Наибольшее повышение концентрации лептина характеризовалось следующим симптомокомплексом: наличием вторичного синовита, большей продолжительностью заболевания, рентгенологической стадией III и IV, функциональной недостаточностью II - III. Имеется корреляционная связь между уровнем лептина и лабораторными маркерами, отражающими тяжесть ОА. У пациентов с ОА имеется корреляционная связь между уровнем лептина и индексами WOMAC, альгофункциональным индексом Лекена для гонартроза, альгофункциональным индексом Лекена для коксартроза. Полученные данные могут свидетельствовать об участии лептина в патогенезе ОА как фактора, способствующего прогрессированию суставного процесса.

ВНЕДРЕНИЕ В ПРАКТИКУ

Критерии определения тяжести течения ОА в зависимости от уровня лептина в сыворотке крови внедрены в работу Муниципального учреждения здравоохранения «Городская клиническая больница скорой медицинской помощи № 25» г. Волгограда. С результатами работы, возможностями оценки тяжести ОА путем определения концентрации лептина знакомятся практические врачи на курсах усовершенствования, научно-практических конференциях.

ПУБЛИКАЦИИ

Основные положения диссертации изложены в 16 печатных работах, из которых 3 в центральной, 3 в зарубежной печати. Результаты работы были представлены и доложены на ежегодных научных конференциях студентов и молодых ученых Волгоградского государственного медицинского университета (2008 - 2011 г), IV Всероссийской конференции «Инновационные технологии в ревматологии» (Нижний Новгород, 5-7 мая, 2008 год), Всероссийской научно-практической конференции «Актуальные проблемы в терапевтической практике» с международным участием (май, 2010 год, Казань), 63-м Европейском конгрессе ревматологов ЕХХАЯ (16-19 июня 2010 года, Рим), на II Всероссийском конгрессе ревматологов (апрель, 2011 года, Ярославль), национальной конференции по вопросам внутренней медицины в Республике Молдова с международным участием (май, 2011 год, Кишинев, Молдова), на 64-м Европейском конгрессе ревматологов ЕЦЪАК (24-29 мая 2011 года, Лондон).

ОБЪЕМ И СТРУКТУРА ДИССЕРТАЦИИ

Диссертация изложена на 170 страницах машинописного текста и состоит из введения, части I - обзора литературы, представленной 3 главами, в которых изложены современные представления о патогенезе остеоартроза, о функциональной и эндокринной роли жировой ткани, а также о роли лептина в патогенезе различных заболевания, включая ОА, части II - собственных исследований, состоящей из 3 глав, содержащих клиническую характеристику больных, методы исследования, полученные результаты и их обсуждение, выводы, практические рекомендации и приложения.

Выводы

1.В сыворотке крови больных с остеоартрозом в 32,3% случаев случаев выявлено повышение концентрации лептина выше границ нормы (42,3% женщин с ОА и 17,3% мужчин с ОА).

2. Для мужчин с О А, имеющих высокий уровень лептина, был характерен следующий симптомокомплекс: узелковая форма с наличием вторичного синовита, с продолжительностью заболевания более 10 лет, рентгенологической стадией III и IV, функциональной недостаточностью суставов II - III. Для женщин с О А, имеющих высокий уровень лептина, был характерен следующий симптомокомплекс: остеоартроз с наличием вторичного синовита, с продолжительностью заболевания более 10 лет, рентгенологической стадией III и IV, функциональной недостаточностью суставов II - III.

3. При О А имеется прямая корреляционная связь между уровнем лептина в сыворотке крови и уровнями С-реактивного белка и СОЭ.

4. У пациентов с ОА имелась корреляционная связь между уровнем лептина и индексами WOMAC, альгофункциональным индексом Лекена для гонартроза, альгофункциональным индексом Лекена для коксартроза.

Практические рекомендации

1. Для определения уровня лептина рекомендуется непрямой твердофазный иммуноферментный метод. Нормальной концентрацией лептина следует считать диапазон у мужчин от 2,0 до 5,6 нг/мл, у женщин от 3,7 до 11,1 нг/мл. Показатели лептина более 5,6 нг/мл у мужчин и более 11,1 нг/мл женщин следует считать повышенными.

2. Для контроля за тяжестью течения остеоартроза рекомендуется определять уровень лептина в сыворотке крови. Повышение его уровня более 5,6 нг/мл у мужчин и более 11,1 нг/мл женщин говорит о прогрессировании и более тяжелом варианте течения ОА, с наличием вторичного синовита, генерализованным поражением суставов, с большим классом функциональной недостаточности суставов.

3. На фоне снижения веса пациентов с остеоартрозом более чем на 5 кг наблюдалось снижение уровня лептина в сыворотке крови и улучшение клинического течения остеоартроза и уменьшению проявлений инсулинорезистентности.
