Лечение мужского и женского бесплодия методамивспомогательных репродуктивных технологий в настоящее времяявляется наиболее эффективным. Успех программыэкстракорпорального оплодотворения (ЭКО) во многом зависит отпричин бесплодия, возраста пациентки, от количества и качестваполученных и трансплантированных в матку эмбрионов. Низкаярезультативность лечения данным методом, как правило, наблюдается упациенток старшей возрастной группы (35 лет и старше) и у женщин снеадекватным ответом яичников на гормональную стимуляцию (низкийответ, синдром гиперстимуляции яичников). В таких случаях числополученных ооцитов может быть невелико, качество ооцитов иэмбрионов и, как следствие, их жизнеспособность значительно снижены.Повышение жизнеспособности эмбрионов in vitro - одна изключевых задач современной репродуктивной медицины. Значительнаячасть исследований в данной области посвящена разработкемодификаций культуральных сред и условий культивирования с цельюулучшения качества ооцитов и эмбрионов in vitro (Behr et al, 2004;Quinn, 2004; Summers et al, 2003). Многими исследователями былопоказано, что нарушения развития и снижение качества эмбрионов (втом числе остановка в развитии, фрагментация и др.), полученных впрограммах ЭКО, является следствием апоптоза отдельных бластомеровили целых эмбрионов.При лечении бесплодия методом ЭКО созревание большогоколичества ооцитов индуцируют повышенными дозами гонадотропныхгормонов, многие пациентки имеют нарушения иммунной системы,страдают заболеваниями репродуктивной и эндокринной систем. Эти- 5 факторы могут являться причиной сверхэкспрессии некоторых геновапоптоза в ходе оогенеза (Chi et al., 2000; Dharma et al., 2003; Perez et al.,1997;Katagirie?a/., 1997).Открытиями последних десятилетий в области генетики имолекулярной биологии показана возможность модуляции геннойэкспрессии экзогенными генетическими конструкциями (Gleave et al.,2005; Morishita et al., 2003; Putnam, 1996). Это положило началоразвитию нового направления молекулярной медицины - геннойтерапии, одним из методов которой является применениеантиСмысловых олигонуклеотидов. Преимущество подходаантисмыслового ингибирования на посттранскрипционном уровне переддругими методами генной терапии заключается в том, что в результатевоздействия терапевтического агента (антисмысловой олигодезоксинуклеотидной последовательности - АсОДН) не затрагиваетсяцелостность генома, так как все биохимические процессы протекают внеядра: в клетку вводится АсОДН, идентичная участку кодирующей частигена-мишени, которая комплементарно связывается с мРНК данногогена в цитоплазме, образуя двуцепочечный комплекс, что препятствуетсинтезу белка (Putnam, 1996; Tomita et al., 2003).Использование АсОДН в генной терапии для лечения различныхзаболеваний обсуждается с конца семидесятых годов прошлого века. Внастоящее время уже проводятся клинические испытания препаратовАсОДН для лечения таких заболеваний, как раковые поражения органови их систем, сердечно-сосудистые патологии, ревматоидные артриты,псориаз, астма, гепатит С, СНИД, и многие другие (Kurreck, 2003). Вданном исследовании нами была предложена методика примененияАсОДН для регуляции генной экспрессии на доимплантационныхстадиях развития эмбрионов человека.Целью данного исследования являлось изучение возможностииспользования АсОДН как действующих агентов для генной терапии наранних эмбриональных стадиях развития. В связи с этим былипоставлены следующие задачи: исследовать возможныеэмбриотоксические эффекты воздействия АсОДН надоимплантационные эмбрионы млекопитающих при культивировании invitro и возможные отсроченные тератогенные эффекты послевоздействия АсОДН на доимплантационных стадиях развития(модельный объект - мышь); исследовать проникновение ираспределение АсОДН в клетках доимплантационных эмбрионов прикультивировании ш vitro (модельный объект - мышь); исследоватьвозможность повышения качества эмбрионов человека прикультивировании in vitro в среде с добавлением АсОДН к мРНК геновHRK и FASL, индуцирующих апоптоз.- 7

VI. ВЫВОДЫ

1. При культивировании эмбрионов мыши в течение двух суток в среде с добавлением АсОДН отсутствуют эмбриотоксический и отсроченный тератогенный эффекты.

2. АсОДН эффективно проникают в клетки ранних эмбрионов млекопитающих, локализуются преимущественно в перинуклеарной области бластомеров и не имеют полной колокализации с лизосомами.

3. Количество аномальных эмбрионов у пациенток экспериментальных групп (среда с АсОДН к мРНК гена HRK и среда с АсОДН к мРНК гена FASL) достоверно меньше, чем в контрольных группах.

4. Терапевтический эффект АсОДН распространяется на эмбрионы со сниженным качеством (умеренная степень фрагментации, 25-50%), но не на эмбрионы с возможной летальной патологией (высокая степень фрагментации, более 50%).

5. Анализ изменения качества эмбрионов на 2-е и 3-й сутки развития показал положительный эффект смеси HRK и FASL АсОДН и отсутствие положительного эффекта некомплементарной последовательности.

6. Частота наступления беременности после внутриматочной трансплантации эмбрионов выше во всех экспериментальных группах с максимальным эффектом при сочетанном воздействии HRK и FASL

АсОДН: HRK - 41,38% (эксперимент) и 31,40% (контроль); FASL -42,37% и 27,59%; смесь HRK и FASL - 51,6% и 30,5%, соответственно.

7. При сочетанном воздействии HRK и FASL АсОДН шанс достигнуть беременности методом ЭКО в 2,56 раз больше, чем в группе интактного контроля (р=0,056), и в 4,78 раз больше, чем в группе неспецифического контроля (р=0,01).
