Актуальность исследования

Течение и прогноз острого инфаркта миокарда (ИМ) тесно связаны со своевременной и эффективной реперфузией миокарда. Общепринятая терапия, включая применение тромболитических препаратов, имеет существенные ограничения и отрицательные стороны. В связи с этим в последнее время нашла широкое применение механическая реперфузия с помощью транслюминальной баллонной коронарной ангиопластики (ТБКА) без предшествующего тромболизиса (ТЛТ) - первичная ТБКА.

Многоцентровые исследования (CADILLAC, FRESCO) подтвердили эти выводы. Однако проблема сохраняет свою актуальность и в настоящее время, особенно у больных с острым инфарктом миокарда (PAMI), поскольку риск смертельно опасных осложнений ТБКА в условиях острого атеротромбоза значительно выше, чем при чрескожных коронарных вмешательствах (4KB) у больных в фазе стабильного течения заболевания (Е.И.Чазов, 2003). До конца не определены оптимальные сроки 4KB и показания для их применения при различных клинических вариантах острого коронарного синдрома (K.Rentrop, 1999).

Использование в клинической практике стентирования коронарных артерий уменьшает число случаев рецидивов ишемии и ранних рестенозов по сравнению с первичной ТБКА (А.Н.Самко, 1999). Однако первичное стентирование при остром ИМ нашло применение лишь в последнее время (CJ.Grines, 1998). Кроме того, остается спорным вопрос об эффективности применения первичного стентирования при остром ИМ (М.Я.Руда, 1999), поскольку исходный массивный тромбоз провоцирует ретромбоз даже после имплантации стента и восстановления адекватного кровотока по инфаркт-зависимой артерии. Неясны вопросы течения заболевания и прогноз у больных после первичного стентирования.

Потому оценка эффективности и безопасности инвазивных методов лечения различных форм острого ИМ, сравнение их ближайших и отдаленных результатов является очень актуальным в настоящее время.

Цель исследования: разработка способов профилактики и лечения осложнений внутрикоронарных вмешательств у больных острым инфарктом миокарда.

Задачи исследования

1. Сравнить эффективность и безопасность методов экстренных и отсроченных чрескожных коронарных вмешательств у больных инфарктом миокарда с подъемом сегмента ST.

2. Изучить частоту возникновения и характер ангиографических осложнений в инфаркт-связанной артерии во время и после коронарных вмешательств у больных острым инфарктом миокарда.

3. Выявить клинические, ангиографические и процедурные факторы, связанные с развитием ангиографических осложнений коронарных вмешательств, оценить их прогностическое значение.

0. Оценить роль блокатора Ilb/IIIa гликопротеиновых рецепторов тромбоцитов эптифибатида в профилактике и лечении ангиографических осложнений коронарных вмешательств у больных острым инфарктом миокарда.

Научная новизна

Впервые на большой, группе больных острым ИМ проведено проспективное сравнительное изучение эффективности и безопасности инвазивных подходов (ТБКА и коронарное стентирование) к лечению острого ИМ в зависимости от сроков заболевания, его клинической формы и наличия ангиографических осложнений.

Установлено, что инвазивные вмешательства, выполненные у больных острым ИМ, значительно улучшают прогноз. При кардиогенном шоке инвазивные вмешательства особенно эффективны в сочетании с внутриаортальной баллонной контрпульсацией, в такой комбинации они увеличивают выживаемость.

Имплантация, стента в инфаркт-связанную коронарную артерию и внутривенное применение блокатора IIb/Ша гликопротеиновых рецепторов тромбоцитов эптифибатида повышают эффективность ангиопластики у больных острым ИМ.

Впервые из репрезентативной выборки больных острым ИМ, перенесших коронарное вмешательство, выделены пациенты, у которых случались ангиграфические осложнения — выраженная диссекция, ретромбоз и дистальная эмболизация артерии. Выявлены клинические, ангиографические и процедурные факторы, которые в реальной клинической практике статистически значимо связаны с развитием ангиграфических осложнений.

Практическая значимость

Показано, что использование 4KB при различных формах острого ИМ ведет к снижению летальности и частоты других нефатальных осложнений, и может служить методом выбора в клиниках, располагающих катетерными лабораториями.

Применение коронарного стентирования при остром ИМ улучшает непосредственные и ближайшие результаты по сравнению с ангиопластикой.

У больных острым ИМ использование блокатора Ilb/IIIa гликопротеиновых рецепторов тромбоцитов эптифибатида увеличивает эффективность 4KB и уменьшает частоту ангиографических осложнений.

Положения, выносимые на защиту

ТБКА и коронарное стентирование являются высокоэффективными и безопасными методами лечения острого инфаркта миокарда, которые улучшают прогноз.

Такой подход особенно эффективен у больных острым инфарктом миокарда, осложненным кардиогенным шоком.

Внутривенное введение блокатора Ilb/IIIa гликопротеиновых рецепторов тромбоцитов эптифибатида повышает эффективность и безопасность коронарных вмешательств при остром инфаркте миокарда.

выводы

1. Чрескожные коронарные вмешательства высокоэффективны у больных острым инфарктом миокарда. В первые сутки острого инфаркта проведение экстренного вмешательства приводит к реперфузии миокарда у 90% больных, при этом оптимальный кровоток восстанавливается в 82%. Отсроченное вмешательство на 2-21 сутки острого инфаркта позволяет достичь реперфузии миокарда у 94% больных с оптимальным кровотоком в 83% случаев.

2. Как после экстренного, так и после отсроченного чрескожного коронарного вмешательства при остром инфаркте миокарда доля больных без госпитальных сердечно-сосудистых осложнений достигает 89%.

3. При экстренном коронарном вмешательстве как выраженная диссекция, так и дистальная эмболизация возникают у 11% больных, а ретромбоз - у 14%. При отсроченном вмешательстве выраженная диссекция отмечается в 8%, а дистальная эмболизация и ретромбоз возникают в 19%. Среди 144 больных с возникшими ангиографическими осложнениями у 109 больных они были успешно устранены.

4. Ангиографические осложнения чаще возникали у женщин, у больных сахарным диабетом и у пациентов с гиперхолестеринемией. Ангиографическими признаками, увеличивающими риск осложнений во время вмешательства, стали извитость до стеноза, изгиб в месте стеноза, кальцинированный стеноз, протяженный стеноз и исходная окклюзия.

О.При проведении баллонной ангиопластики без стентирования у больных острым инфарктом миокарда риск ангиографических осложнений возрастал, а при профилактическом применении блокатора Ilb/IIIa гликопротеиновых рецепторов тромбоцитов эптифибатида — уменьшался.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ.

0. Балонная ангиопластика со стентированием инфаркт-связанной коронарной артерии с одновременным использованием временной электрокардиостимуляции является эффективным методом лечения больных с острым инфарктом миокарда с подъемом сегмента ST и зубцом Q и может быть рекомендована во всех стационарах, где имеется необходимое оборудование и высококвалифицированные кадры.

0. При сложности в проведении коронарного проводника дистальнее окклюзии инфаркт-связанной коронарной артерии целесообразно выполнять селективный интракоронарный тромболизис 250000 Ед стрептокиназы через установленный гайд-катетер.

0. Коронарную ангиопластику при остром инфаркте миокарда целесообразно завершать коронарным стентированием для уменьшения частоты непосредственных и госпитальных осложнений.

0. Ангиографические результаты коронарного вмешательства у больных острым инфарктом миокарда улучшаются при одновременном назначении блокатора гликопротеиновых Ilb/IIIa рецепторов тромбоцитов эптифибатида.
