выводы

1. У больных АФС-нефропатией (при первичном и вторичном АФС) выявлены нарушения внутрипочечной гемодинамики, свидетельствующие о наличии гипоперфузии паренхимы почек. Отмеченное у этих больных снижение СКФ (в среднем до 51±19 мл/мин.) коррелировало (г=0,41) с нарушением внутрипочечного кровотока в междолевых почечных артериях.

2. Из всех параметров УЗДГ внутрипочечных сосудов линейные скорости кровотока оказались наиболее информативными для диагностики АФСН и оценки эффективности антикоагулянтной терапии.

3. У больных АФСН с высоким титром антител к кардиолипину отмечалось более тяжелое нарушение внутрипочечного кровотока с развитием диффузной гипоперфузии паренхимы почек.

4. Лечение низкомолекулярным гепарином клексаном, применявшимся у больных АФСН с экстраренальными проявлениями (поражением ЦНС, сердца, кожи), при активации внутрисосудистого свертывания крови, а также при беременности, в 70% случаев приводило к уменьшению выраженности проявлений АФСН: уменьшению уровня протеинурии на 50%, снижению или нормализации АД, повышению СКФ на 25%.

5. Лечение непрямым антикоагулянтом варфарином, применявшимся у больных АФСН в случаях изолированного поражения почек и без признаков активации внутрисосудистого свертывания крови, приводило к стабилизации функции почек, замедляя прогрессирование почечного процесса у 92% больных.

6. Антикоагулянтная терапия АФСН сопровождалась улучшением внутрипочечной гемодинамики с полным восстановлением внутрипочечного кровотока у 42% больных.

7. Лечение клексаном и варфарином хорошо переносилось больными. При лечении клексаном побочных эффектов отмечено не было; на фоне приема варфарина у 46% больных развивались «малые» геморрагические осложнения (синяки, носовые кровотечения, кровоточивость десен, мено-метроррагии), не требующие отмены препарата.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

1. Низкомолекулярный гепарин клексан показан больным АФСН с экстраренальными проявлениями (поражением ЦНС, сердца, кожи), активацией внутрисосудистого свертывания крови, а также при беременности у пациенток с АФСН.

2. Непрямой антикоагулянт варфарин показан при изолированной АФСН без признаков активации внутрисосудистого свертывания крови, а также после завершения курса лечения клексаном.

3. Пациентам с первичным и вторичным АФС целесообразно проведение УЗДГ внутрипочечных сосудов с оценкой линейных скоростей для диагностики АФС-нефропатии (гипоперфузии паренхимы почек) и контроля за антикоагулянтной терапией.

4. Оптимальным видом антикоагулянтной терапии больных АФСН при первичном и вторичном АФС является комбинированное применение прямых и непрямых антикоагулянтов.
