Сахарный диабет 2 типа (СД) — одно из распространенных хронических неинфекционных заболеваний человека. В 2000 г. в мире было около 155 млн. больных СД, а к 2025 г. их число может возрасти до 300 млн. человек [1].

У больных СД 2 типа риск развития ишемической болезни сердца (ИБС) в два-четыре раза выше, чем у лиц без диабета [2] . Наличие СД 2 типа существенно ухудшает прогноз сопутствующей ИБС [3]. Дислипидемия в 2—4 раза увеличивает риск развития сердечно-сосудистых заболеваний (ССЗ) и смерти от ССЗ у больных с СД 2 типа[4]. Даже небольшое повышение уровня артериального давления (АД) сопровождается значительным увеличением риска развития ССЗ[5]. В то же время, как было показано в исследовании НОТ (Hypertension Optimal Treatment) [6] при одинаковой степени снижения уровня АД, относительный риск ССЗ у больных с СД 2 типа снижается в большей степени, чем у лиц с артериальной гипертонией, но без СД.

В нескольких рандомизированных клинических исследованиях было доказано, что медикаментозная терапия, направленная на коррекцию модифицируемых сердечно-сосудистых факторов риска (ФР), в частности, артериальной гипертонии и дислипидемии [7] , а также терапия аспирином [8] существенно уменьшают риск развития ССЗ у больных СД 2 типа. В то же время, по данным некоторых исследований, лишь небольшая часть больных СД получает препараты, способные снизить риск развития ССЗ [9] .

Мы не нашли в литературе работ, посвященных оценке качества медикаментозной коррекции модифицируемых сердечно-сосудистых ФР у больных СД 2 типа в России. Поэтому представляется актуальным проведение такой оценки и анализ ее результатов для выработки подходов к уменьшению риска развития ССЗ у данной категории больных.

Новизна исследования

Впервые в рамках рандомизированного клинического исследования проведена оценка эффективности терапевтического обучения и амбулаторного наблюдения на качество лечения ССЗ у больных СД 2 типа.

Цель исследования:

Оценить эффективность структурированного терапевтического обучения и амбулаторного наблюдения для улучшения качества медикаментозной профилактики и лечения ССЗ у больных СД 2 типа.

Задачи

1. На основе научно доказанных фактов, по данным литературы изучить основные современные подходы к профилактике и лечению ССЗ у больных СД 2 типа.

2. Оценить качество медикаментозной профилактики и лечения ССЗ больных СД 2 типа, госпитализированных в эндокринологические отделения многопрофильного стационара.

3. В рамках рандомизированного клинического испытания оценить возможность улучшения качества медикаментозной профилактики и лечения ССЗ у больных СД 2 типа с помощью структурированного терапевтического обучения и тщательного амбулаторного наблюдения кардиологом.

4. На основании полученных данных выработать рекомендации по практической реализации современных подходов к улучшению профилактики и лечения ССЗ у больных СД 2 типа.

Выводы

Проведенный анализ показал недостаточно высокое качество медикаментозной профилактики сердечно-сосудистых факторов риска у пациентов, наблюдающихся в эндокринологическом центре. Интегрированный подход с участием не только эндокринолога, но и кардиолога существенно повышает качество профилактики и лечения ССЗ у больных СД 2 типа.

Структурированное терапевтическое обучение и мониторинг больных с СД 2 типа с высоким риском ССО позволяют существенно улучшить качество их медикаментозного лечения, сопровождаются повышением степени выполнения больными врачебных медикаментозных и немедикаментозных рекомендаций.

В результате структурированного обучения и амбулаторного наблюдения в группе вмешательства 63,6% пациентов АГ и СД 2 типа достигли целевого уровня АД

У больных прошедших терапевтическое обучение отмечается тенденция к увеличению частоты применения статинов и улучшению показателей липидов крови

Обучение и мониторинг больных с СД 2 типа с высоким риском ССО сопровождаются достоверным улучшением субъективной оценки качества жизни пациентами

Практические рекомендации

1. Более широкое внедрение в практику стационаров и поликлиник интегрированного подхода с непременным участием кардиологов, позволит улучшить качество профилактики и лечения ССЗ у больных СД.

2. Применение структурированного терапевтического обучения и мониторинга после выписки из стационара больных СД 2 типа с высоким риском ССО целесообразно использовать в учреждениях практического здравоохранения для улучшения качества профилактики и лечения модифицируемых сердечно-сосудистых ФР.

3. Целесообразно расширить программу обучения больных СД, за счет введения дополнительных занятий по следующим темам: основные сердечно-сосудистые факторы риска, медикаментозные и нефармакологические подходы к профилактике и лечению ССЗ у больных СД.

4. При проведении терапевтического обучения больных СД 2 типа с высоким риском ССО наибольшее внимание следует уделять следующим разделам: 1)контроль гликемии, 2)контроль АД , 3)коррекция дислипидемии, 4) применение низких доз аспирина, 5)контроль массы тела.
