Актуальность проблемы

Вирус простого герпеса (ВПГ) и цитомегаловирус (ЦМВ) чрезвычайно широко распространены в популяции и вызывают широкий спектр патологии человека [237, 273]. Предположение о присутствии ВПГ и ЦМВ в эякуляте было впервые высказано в начале 70-х годов прошлого века [90, 180, 202]. Проблема выявления вирусов в половых клетках на протяжении многих лет привлекает внимание исследователей по нескольким причинам.

Во-первых, выделение вируса с эякулятом вносит существенный вклад в его горизонтальное распространение в популяции [21, 133].

Во-вторых, в последние годы возрастает число исследований, свидетельствующих о том, что инфекции органов мочеполовой системы, в том числе вирусные, занимают важное место в структуре причин мужского бесплодия [41, 72, 119, 177]. Мнения исследователей относительно роли ВПГ и ЦМВ в этиологии нарушений фертильности у мужчин расходятся от полного отрицания влияния герпесвирусов на процесс созревания мужских половых клеток [73, 128, 248] до установления прямой корреляции между вирусным инфицированием эякулята и мужским бесплодием [72, 183, 188, 250].

В-третьих, инфицирование половых клеток предполагает возможность вертикальной передачи вируса при оплодотворении яйцеклетки пораженной гаметой, что может стать причиной самопроизвольного аборта или внутриутробной инфекции плода [7, 10]. Особую актуальность проблема вирусного инфицирования сперматозоидов приобрела в связи с развитием вспомогательных репродуктивных технологий (ВРТ), поскольку искусственная селекция мужских гамет может нарушить естественные механизмы отбраковки инфицированных гамет и стать причиной заражения яйцеклетки.

Кроме того показано, что у лиц, выделяющих герпесвирусы с эякулятом, возрастает риск заражения другими инфекционными агентами, в частности ВИЧ [50, 120].

Цель и задачи исследования

Цель настоящего исследования заключалась в оценке влияния ВПГ и ЦМВ на сперматогенез.

Для достижения указанной цели были поставлены следующие задачи:

1. Оценить частоту выявления ВПГ и ЦМВ в эякуляте пациентов с нарушением фертильности по сравнению со здоровыми мужчинами.

2. Изучить влияние ВПГ и ЦМВ на основные показатели качества спермы (концентрацию сперматозоидов, количество подвижных и морфологически нормальных форм сперматозоидов) и на созревание половых клеток.

3. Оценить возможность внутригаметной локализации герпесвирусов.

4. Разработать модель герпесвирусной инфекции (ГВИ) мужских гонад in vitro.

5. Оценить влияние острой ВПГ-инфекции (ВПГИ) на сперматогенный эпителий мыши.

6. Выявить половые и соматические клетки мужских гонад, чувствительные к герпесвирусам.

7. Изучить изменения структуры диплоидных фибробластов человека под действием ЦМВ при низкой множественности инфицирования (МИ).

Научная новизна и практическая значимость

1. Впервые показаны сезонные различия частоты выделения ВПГ и ЦМВ с эякулятом.

2. Установлена связь между наличием ВПГ в эякуляте и первичным бесплодием у мужчин.

3. Выявление ВПГ и ЦМВ во фракции подвижных сперматозоидов указывает на необходимость исследования эякулята на маркеры герпесвирусов перед осуществлением ВРТ, в ходе обследования кандидатов на донорство спермы, а также при планировани семьи.

4. Для изучения влияния герпесвирусов на сперматогенез разработана модель ГВИ мужских гонад in vitro.

5. Впервые вирусные капсиды типа С выявлены в ядрах сперматогоний и сперматоцитов.

6. Впервые на ультраструктурном уровне описана локализация сверхраннего белка 1Е-рр72 ЦМВ в околоядерном включении — предполагаемом сайте окончательной сборки вируса.

7. Выявление 1Е-рр72 ЦМВ в плотных тельцах, обладающих способностью проникать в клетки, указывает на то, что выявление указанного белка в клетке без других маркеров инфекции может быть следствием пассивного переноса вирусных антигенов.

Основные положения, выносимые на защиту

1. Показано, что у мужчин без клинических признаков инфекции мочеполового тракта частота выявления ВПГ с эякулятом составляет в среднем 27,3%, ЦМВ - 8,8%, с максимумом выделения герпесвирусов в зимние месяцы (ВПГ - 34%, ЦМВ - 15,7%).

2. Установлена прямая корреляция между инфицированностью эякулята ВПГ и первичным бесплодием.

3. Инфекционно-активный ВПГ и ЦМВ и ДНК герпесвирусов выявлены не только в цельном эякуляте, но и во фракции подвижных сперматозоидов.

4. На ультраструктурном уровне выявлена способность ВПГ проникать в ядра клеток-предшественников сперматозоидов мьппи, а также высокая чувствительность клеток интерстиция к ВПГ.

5. При заражении фибробластов ЦМВ с низкой МИ на поздних стадиях инфекции выявлено накопление сверхраннего белка 1Е-рр72 в околоядерном включении в составе плотных телец.

Выводы

1. У мужчин без клинических проявлений инфекции урогенитального тракта частота выявления маркеров ВПГ в эякуляте составляет 27,3%, ЦМВ -8,8%. Выделение герпесвирусов с эякулятом возрастает в зимние месяцы.

2. ВПГ чаще встречается у мужчин с первичным бесплодием, при этом выявлена корреляция между присутствием ВПГ и ухудшением показателей качества спермы, а также нарушением процесса созревания мужских половых клеток. Этиологическая роль ЦМВ в развитии бесплодия не доказана.

3. ВПГ и ЦМВ выявлены как в цельном эякуляте, так и во фракции подвижных сперматозоидов.

4. ВПГ оказывает гаметотоксическое воздействие на герминативный эпителий.

5. Показана возможность заражения как соматических клеток семенника, так и половых клеток, находящихся на ранних этапах цитодифференцировки.

6. В фибробластах человека, зараженных ЦМВ с низкой множественностью, в околоядерном включении впервые обнаружен сверхранний белок IE-1 рр72, находящийся в составе ПТ.

7. Разработка и внедрение в практику алгоритмов исследования эякулята на присутствие герпесвирусов позволяет оцепить значение герпесвирусов в этиологии бесплодия и использовать этиотропное лечение, что уменьшает вероятность вертикальной передачи ВПГ и ЦМВ.
