Сахарный диабет типа 1 (СД типа 1) - широко распространенное, тяжело протекающее заболевание, приводящее к ранней инвалидности и преждевременной смерти больных. СД типа 1 относится к одному из наиболее серьезных эндокринных заболеваний человека. Для него характерно аутоиммунное разрушение бета-клеток островков Лангерганса поджелудочной железы и последующая полная зависимость от введения экзогенного инсулина, который необходим для регуляции уровня глюкозы в крови. В Российской Федерации около 0,4% индивидов страдает этой формой диабета, относящейся к наследственным аутоиммунным заболеваниям. СД типа 1 представляет собой не только медицинскую, но и серьезную социальную проблему. В целом средняя продолжительность жизни больных СД типа 1, заболевших в детстве, на 20 лет меньше, чем средняя продолжительность жизни в общей популяции.

Известно, что СД типа 1 развивается почти у половины генетически идентичных близнецов и только у 3-6% сибсов. В настоящее время СД типа 1 относят к многофакторным, полигенным заболеваниям. На полигенную природу указывает отмеченный всеми исследователями низкий уровень семейного риска, не совместимый ни с одной из гипотез моногенного наследования без привлечения дополнительного предположения о неполной пе-нетрантности предполагаемого гена, что с формально генетической точки зрения совпадает с полигенной гипотезой. Многофакторная модель наследования предполагает, что проявление болезни определяется взаимодействием средовых и генетических факторов. Под генетическими факторами при этом подразумевают определенные аллели ряда полиморфных генов, вовлеченных в развитие СД типа 1, которые в клинической практике получили название аллелей, предрасполагающих к развитию СД типа 1.

До настоящего времени в русской популяции с использованием подхода «ген-кандидат» удалось надежно идентифицировать только две хромосомные области, содержащие гены, связанные с предрасположенностью к развитию СД типа 1. Это локус МНС (главный комплекс гистосовместимо-сти) и ген CTLA4, кодирующий поверхностный рецептор Т-клеток [1,2].

В последние годы обнаружена ассоциация с СД типа 1 ряда новых ло-кусов, в том числе и на основе полных геномных поисков с использованием микрочипов высокой плотности. Однако эти данные относятся, главным образом, к больным СД типа 1, происходящим из Великобритании [3-5]. Необходимость проведения аналогичных исследований на русской популяции не вызывает сомнений, так как известно, что вклад различных генов в формирование предрасположенности к СД типа 1 существенно различается в разных популяциях, а вычисление генетического риска для пациента требует наличия информации о распределении частот аллелей и генотипов для популяции, которой он принадлежит, что исключает использование данных, полученных для других популяций.

Установление ассоциации полиморфных маркеров с СД типа 1 поможет разработать дифференцированный подход к профилактике и лечению данной патологии.

Цели и задачи работы

Целью данной работы было изучение ассоциации полиморфных маркеров ряда генов-кандидатов с развитием сахарного диабета типа 1.

Для достижения этой цели были поставлены следующие задачи:

1. Определить частоты аллелей и генотипов полиморфных маркеров генов, кодирующих тирозиновые фосфатазы типа 2, 11 и 22 (PTPN2, PTPN11, PTPN22), тирозиновую киназу, являющуюся рецептором эпи-дермальных факторов роста типа 3 (ERBB3), представителя семейства лектинов типа С (CLEC16A), адаптерный белок Lnk (SH2B3), проинсу-лин (INS) и альфа-цепь рецептора интерлейкина 2 (IL2RA) в группе больных СД типа 1 и группе здоровых индивидов среди русских г. Москвы.

2. Провести сравнительный анализ распределения аллелей и генотипов полиморфных маркеров данных генов-кандидатов в исследованных выборках больных и здоровых индивидов для выявления ассоциации изученных маркеров с развитием болезни и определения вклада данных генов в наследственную предрасположенность к патологии. Научная новизна и практическая значимость работы В данной работе впервые в России исследована ассоциация полиморфных маркеров -23HphI (rs689) гена INS, Trp262Arg (rs3184504) гена SH2B3, С1858Т (rs2476601) гена PTPN22, rs2847281, rs2542151, rs3737361, rs547268 и rs2542156 гена РТРN2, rs2903692 и rs725613 гена CLEC16A, rs2292239 гена ERBB3, rs17696736, rs12425405, rs11066284, rs7974468 и rs11066301 гена PTPN11, rs41295061 и rs11594656 гена IL2RA с сахарным диабетом типа 1.

Обнаружена ассоциация полиморфных маркеров генов INS, SH2B3, PTPN22, PTPN11, PTPN2 и CLEC16A с развитием СД типа 1. Все вышеизложенные результаты получены впервые.

Показано, что исследованные полиморфные маркеры генов INS, SH2B3, PTPN22, PTPN11, PTPN2 и CLEC16A могут использоваться для количественной оценки генетического риска возникновения сахарного диабета типа 1 в популяции русских. Несомненно, что понимание механизма патогенетического развития СД типа 1 со временем позволит создать новые лекарственные средства, предохраняющие от развития этого заболевания у генетически предрасположенных индивидов.

6. Обнаружена ассоциация полиморфного маркера rsl 1066284 гена PTPN11 с развитием СД типа 1. Установлено, что носители аллеля Т данного полиморфного маркера имеют повышенный риск развития СД типа 1, тогда как носители аллеля А имеют пониженный риск развития СД типа 1.

7. Обнаружена ассоциация полиморфного маркера rs2903692 гена CLEC16A с развитием СД типа 1. Установлено, что носители аллеля G данного полиморфного маркера имеют повышенный риск развития СД типа 1, тогда как носители аллеля А имеют пониженный риск развития СД типа 1.

8. Полученные данные (значения OR и частоты аллелей для полиморфных маркеров) позволят количественно оценивать генетический риск возникновения сахарного диабета типа 1 в популяции русских.

ЗАКЛЮЧЕНИЕ.

На основании полученных результатов можно сделать вывод о том, что к развитию СД типа 1 приводит комплекс регуляторных нарушений, вызванных неблагоприятной комбинацией предрасполагающих аллелей в отдельных локусах - HLA, INS, CTLA4, PTPN22, PTPN2, SH2B3 и др. (таблица 29). Полученные данные (значения OR и частоты аллелей для полиморфных маркеров) позволят количественно оценивать генетический риск возникновения сахарного диабета типа 1 в популяции русских. Понимание генетических основ развития заболевания приближает нас к идентификации этиологических мутаций в генах, определяющих предрасположенность к СД типа 1. Несомненно, что понимание механизма патогенетического развития СД типа 1 со временем позволит создать новые лекарственные средства, предохраняющие от развития этого заболевания у генетически предрасположенных индивидов.
