Выводы

1. Крупно фракционная дистанционная лучевая терапия первичной меланомы кожи в режиме интенсивно-концентрированного предоперационного курса в РОД 10 Гр и СОД 50 Гр за пять последовательных дней является важным и эффективным этапом комбинированного лечения первичной меланомы кожи, которая в сочетании с широким иссечением опухоли позволяет достигнуть практически полного локального контроля заболевания. Среди 502 больных, последовательно включенных в исследование, не было выявлено ни одного локального рецидива.

2. Исследование сторожевых лимфатических узлов у больных меланомой кожи с применением радиоизотопных методов визуализации регионарных лимфатических бассейнов позволяет с высокой степенью достоверности дифференцировать сторожевой лимфатический узел, а использование ручного гамма-детектора провести его прицельную биопсию. Метод позволяет с высокой достоверностью оценить статус регионарных лимфатических узлов без необоснованного расширения объема лечебно-диагностических мероприятий и может быть рекомендован в качестве альтернативы профилактической лимфаденэктомии. Его практическое применение позволяет значительно сократить сроки реабилитации (в среднем на 4-5 недель) и стоимость лечения практически у 70% больных локализованной меланомой кожи.

3. Дистанционная лучевая терапия на область регионарных лимфатических узлов может быть методом выбора органосохраияющего лечения больных меланомой кожи с клинически интактными лимфатическими узлами в случае отсутствия возможности исследования сторожевых лимфатических узлов.

4. Отсутствие локальных рецидивов в исследуемой группе, а следовательно, первичного источника для развития метастазов меланомы в дальнейшем, позволяет рассматривать регионарные рецидивы в ранее интактных лимфатических коллекторах и отдаленные метастазы, как клиническую манифестацию субклинических микрометастазов, уже существовавших на момент проведения комбинированного и комплексного лечения. Это позволило провести ретроспективный анализ вероятности наличия метастатической фазы заболевания на момент проведения специфического лечения в зависимости от клинических и морфологических характеристик опухоли и, таким образом, оценить их прогностическую ценность.

5. Кроме уже известных морфологических характеристик меланом, оказывающих влияние на эффективность лечения, таких как толщина, изъязвление и уровень инвазии опухоли - лимфоидная инфильтрация ложа меланомы так же обладает независимым и достоверным влиянием на риск развития отдаленных метастазов и результаты лечения недиссеминированной меланомы кожи. Десятилетняя актуриальная выживаемость в группе меланом с выраженной лимфоидной инфильтрацией составила 79,5±7,1% против 36±5,3% в группе опухолей со слабой лимфоидной инфильтрацией и 40,0±5,9% в группе, где она отсутствовала вовсе (р<0,001 и р<0,01, соответственно).

6. В клинической картине недиссеминированной меланомы кожи следует выделять, по крайней мере, десять клинических характеристик, ассоциированных с неблагоприятным прогнозом, отражающих степень развитости и биологическую агрессивность опухоли. При наличии любого из них следует прогнозировать более чем 60% вероятность наличия метастатической фазы заболевания. В случае сочетания пяти наиболее значимых симптомов, таких, как воспаление, кровоточивость, изъязвление, быстрый рост и инфильтрация основания опухоли, этот риск в исследуемой группе составил 82,3% против 26,3% без них (р<0,001), а десятилетняя выживаемость — 23,0±5,0% против 82,7±82,7% соответственно (р<0,001).

7. Интерпретация клинической картины недиссеминированной меланомы кожи с учетом прогностических факторов позволяет незамедлительно и с высокой степенью достоверности оценить риск развития регионарных и отдаленных метастазов, а, следовательно, выбрать наиболее оптимальный объем хирургического и комбинированного этапов лечения. В свою очередь, исследование морфологических характеристик меланомы позволяет уточнить прогноз, а также необходимость и объем адъювантной терапии.

8. Регионарные метастазы меланомы кожи являются основным неблагоприятным фактором прогноза у больных недиссеминированной меланомой кожи. Более чем в половине случаев регионарные метастазы меланомы кожи имеют субклинический характер, при том, что их размер не оказывает значимого влияния на прогноз. Следствием этого может быть недооценка стадии заболевания, а также выбор неадекватного объема лечения.

9. Число регионарных лимфатических узлов, содержащих метастазы меланомы, является единственной характеристикой, оказывающей значимое влияние на вероятность развития отдаленных метастазов и эффективность комбинированной и комплексной терапии пациентов с III стадией заболевания. Отдаленные метастазы в группе больных с метастазом в один лимфатический узел наблюдали в 57,4% случаев против 82,0% в группе с двумя и большим числом пораженных лимфатических узлов, а десятилетняя выживаемость составила 43,2±7,6% против 14,7±5,7, соответственно (р<0,05).

10. Несмотря на высокий риск развития отдаленных метастазов у больных меланомой кожи с региональными метастазами (III стадия) регионарная лимфаденэктомпя у таких пациентов должна быть обязательным компонентом лечения. В этом случае каждый четвертый больной имеет шанс пережить десятилетний срок наблюдения. В нашем исследовании десятилетняя выживаемость больных с регионарными метастазами, диагностированными на момент проведения лечения, составила 27,7±4,6%.

11. В исследовании не было получено доказательств эффективности применявшихся режимов полихимиотераппи. Десятилетняя выживаемость в группе комбинированной терапии составила 59,4±4,9%, в группе комплексного лечения с 4-компонентной ХТ - 52±3,7%, 2-компонентной - 57,9±7,4% (р>0,2 и р>0,5, соответственно). Показатели десятилетней безметастатической выживаемости в этих же группах составили 56,9±4,8%, 50,4±3,7% и 52,0±6,1%, соответственно (р>0,2 и р>0,5). Не было выявлено различий и в стратификационных статистических анализах по основным прогностическим факторам.

12. В клиническом исследовании экспрессия Fas-лиганда клетками первичной меланомы кожи не оказывала влияния на уровень лимфоидной инфильтрации ложа опухоли и отдаленные результаты лечения. Удельный вес FasL+ опухолей в группе меланом с выраженной лимфоидной инфильтрацией составил 44,4% против 46,5% в группе меланом без лимфоидной инфильтрации (р>0,5), в то время как пятилетняя выживаемость в группе ЛИ(+++) FasL+ составила 86,7±12,4% против 88,2±11,1% в группе ЛЩ+++) FasL- в сравнении с группой ЛИ(-) FasL+ -53,0±11,4% против 24,2±9,8% в группе ЛИ(-) FasL-.
