ВЬБОДЫ

1. Реальная практика фармакотерапии внебольничных инфекций (острого тонзиллофарингита, острого синусита, острого среднего отита, обострения хронического бронхита, внебольничной пневмонии) в поликлиниках городов России во многих случаях не соответствует современным стандартам.

2. К числу основных проблем, касающихся фармакотерапии перечисленных заболеваний, относятся:

- Назначение антибактериальных препаратов без учета наиболее вероятных возбудителей соответствующих инфекций и особенностей их антибиотико-резистентности, а также фармакокинетических параметров и профиля безопасности антибиотиков;

- Высокая частота парентерального применения антибактериальных препаратов вопреки общепринятым рекомендациям;

- Необоснованное назначение комбинаций антибактериальных препаратов;

- Широкое использование лекарственных средств с недоказанной при инфекциях дыхательных путей клинической эффективностью - антигистаминных, противогрибковых, витаминов, иммуномодуляторов, минеральных добавок.

3. При остром тонзиллофарингите не проводится бактериологическая диагностика, направленная на идентификацию бета-гемолитического стрептококка группы А, предусмотренная современным стандартом, что является причиной необоснованно частого назначения антибактериальной терапии.

4. При остром тонзиллофарингите практикуется заниженный по длительности курс антибиотикотерапии, не достаточный для эрадикации возбудителя в случае стрептококковой этиологии заболевания - 7 дней вместо 10.

5. При остром синусите для микробиологической диагностики ошибочно используется метод взятия мазка из зева, имеющий низкую информативность.

6. При обострении хронического бронхита антибактериальная терапия назначается неоправданно часто - в 80-90%, хотя роль бактериального фактора доказана не более чем в 50% случаев заболевания.

7. Отмечены позитивные тенденции в структуре антибактериальной терапии обострения хронического бронхита и внебольничной пневмонии в 2003-2004 гг. по сравнению с 1998 г.:

- При обострении хронического бронхита в 10 раз возросла частота назначения амоксициллина препарата, соответствующего современному стандарту, в то время как использование менее приемлемого ампициллина сократилось в 2 раза, а потенциально токсичного ко-тримоксазола - в 10 раз.

- При внебольничной пневмонии частота назначения амоксициллина увеличилась в 20 раз при сокращении применения ампициллина и ко-тримоксазола в 5 раз, а гентамицина - 2 2,5 раза. Рекомендации фармацевтов посетителям аптек, обращающимся по поводу симптомов инфекций дыхательных путей, во многих случаях являются некорректными:

- Менее чем в 50% таких ситуаций даются советы об обращении к врачу;

- Почти в половине случаев предлагается самолечение с использованием антибактериальных препаратов.

В результате проведения телефонного опроса населения 10 городов России у 4 6% респондентов выявлен низкий уровень знаний об антибактериальных препаратах, у 37%

- средний, и только у 17% - высокий.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

Низкий уровень качества фармакотерапии внебольничных инфекций в амбулаторных условиях является основанием к активному внедрению и доведению до каждого поликлинического врача в указанных регионах национальных стандартов и рекомендаций по диагностике и лечению этих заболеваний и разработке эффективных мер контроля их выполнения.

В целях дальнейшей оптимизации фармакотерапии внебольничных инфекций необходимо периодически повторять проведение региональных и многоцентровых фармакоэпидемиологических исследований. Необходимо пересмотреть меры государственного контроля за рецептурной продажей антибактериальных препаратов в аптечных учреждениях.

Необходимо разработать на федеральном уровне регламентирующие меры для запрета рекламы антибактериальных препаратов в средствах массовой информации.

Выявленные отклонения от стандартов терапии внебольничных инфекций позволяют определить главные направления методических рекомендаций на федеральном и региональном уровне, создания образовательных программ для врачей и фармацевтов, научно-обоснованных информационных материалов для населения.
