Эндотелиальная дисфункция (ЭД) является независимым предиктором прогрессирования атеросклероза и риска развития сердечно-сосудистых событий как у больных, страдающих ишемической болезнью сердца (ИБС), так и у здоровых лиц [42, 204, 214, 441].

Как известно, во многих странах ИБС является главной причиной смертности населения. В 2006 г в России смертность от болезней системы кровообращения составила 56,5% от общей, из них на ИБС приходится около половины. Обострение ИБС получило название «острый коронарный синдром», объединив наиболее опасные ее формы - острый инфаркт миокарда (ОИМ) и нестабильную стенокардию (НС) [31]. Госпитальная смертность при остром коронарном синдроме без подъема сегмента 8Т (ОКСбпЗТ) составляет 5-7%, а в течение следующих 6 месяцев возрастает до 12-13% [159].

Доказано, что эндотелиальная дисфункция играет важную роль не только в патогенезе ИБС, но и в развитии ее обострения, основным морфологическим проявлением которого является разрыв атеросклеротической бляшки и образование на этом месте тромба [121, 506]. Тогда как здоровый эндотелий секретирует антиагреганты, благодаря чему к нему не адгезируют тромбоциты, обладает антикоагулянтными и фибринолитическими свойствами, развитие эндотелиальной дисфункции связано с повышением его тромбогенного потенциала, парадоксальной вазоконстрикцией, усиливающей механическое воздействие на бляшку, а также формированием оксидантного стресса и инициации воспалительной реакции [335, 358].

В последнее время наибольшей популярностью стали пользоваться неинвазивные методы оценки функции эндотелия, такие, например, как ультразвуковой (УЗ) тест с реактивной гиперемией плечевой артерии (тест

ЭЗВД ПА). Он позволяет количественно оценить эндотелий зависимую вазодилятацию (ЭЗВД), не представляя угрозы для состояния больного.

Однако в литературе имеются единичные сообщения, в которых исследовалась роль эндотелиальной дисфункции у больных с ОКСбп8Т, при этом наиболее часто использовались инвазивные методы оценки функции эндотелия - в/а инфузия ацетилхолина или нитропруссида натрия [167, 484]. Неинвазивные методы оценки функции эндотелия использовались в основном в работах с больными, страдающими стабильными формами ИБС или у здоровых лиц с факторами риска ИБС [91]. В исследовании Кага1г1з ЕЛМ и соавт. тест ЭЗВД ПА проводился пациентам с ОКСбпБТ, но оценивалась только его прогностическая значимость по развитию острых сердечнососудистых событий в течение последующих 24,8 ±5,9 месяцев [272].

В то же время вопрос о влиянии эндотелиальной дисфункции на госпитальное течение ОКСбпБТ остается неизученным. Остается также неизвестным, вызывает ли лечение, применяемое у таких пациентов, изменения в функциональном состоянии эндотелия. Нашу работу мы посвятили этим актуальным на сегодняшний день вопросам.

ЦЕЛЬ ИССЛЕДОВАНИЯ.

Изучение взаимосвязи функционального состояния эндотелия и течения острого коронарного синдрома без подъема сегмента ЭТ при различной тактике лечения.

ЗАДАЧИ ИССЛЕДОВАНИЯ.

1. Оценить функциональное состояние эндотелия у больных с острым коронарным синдромом без подъема сегмента 8Т.

2. Изучить взаимосвязь эндотелиальной дисфункции и клинического течения госпитального периода острого коронарного синдрома без подъема сегмента ЭТ.

3. Изучить взаимосвязь функции эндотелия и состояния коронарных артерий по данным коронарографии у больных с острым коронарным синдромом без подъема сегмента БТ.

4. Оценить влияние чрескожных коронарных вмешательств на функцию эндотелия у больных с острым коронарным синдромом без подъема сегмента ЭТ в течение госпитального периода.

ПОЛОЖЕНИЯ, ВЫНОСИМЫЕ НА ЗАЩИТУ.

1. У пациентов с острым коронарным синдромом без подъема сегмента 8Т отмечается нарушение функции эндотелия при сравнении с лицами без ишемической болезни сердца, и более выраженное, чем у больных стабильными формами ишемической болезни сердца.

2. Выраженность эндотелиальной дисфункции при остром коронарном синдроме без подъема сегмента БТ связана с дальнейшим течением заболевания.

3. Содержание тканевого активатора плазминогена и уровень активности ингибитора тканевого активатора плазминогена 1 типа в плазме крови связаны с клиническим течением острого коронарного синдрома без подъема сегмента 8Т.

4. Уровень активности фактора фон Виллебранда в плазме крови связан с выраженностью эндотелиальной дисфункции и состоянием коронарных артерий пациентов с острым коронарным синдромом без подъема сегмента 8Т.

5. Проведение ангиопластики со стентированием коронарных артерий улучшает функциональное состояние эндотелия у больных с острым инфарктом миокарда без подъема сегмента БТ.

НАУЧНАЯ НОВИЗНА.

Впервые была исследована взаимосвязь между функциональным состоянием эндотелия и клиническим течением острого коронарного синдрома без подъема сегмента 8Т и оценена взаимосвязь между данными теста эндотелий зависимой вазодилатации плечевой артерии и уровнем биохимических маркеров эндотелиальной дисфункции у больных с острым коронарным синдромом без подъема сегмента БТ в течение госпитального периода.

Впервые была исследована и оценена взаимосвязь между состоянием коронарных артерий у больных с острым коронарным синдромом без подъема сегмента 8Т и функциональным состоянием эндотелия.

Впервые было исследовано и оценено влияние коронарной ангиопластики со стентированием на функцию эндотелия у больных с острым инфарктом миокарда без подъема сегмента БТ в течение госпитального периода.

ПРАКТИЧЕСКАЯ ЗНАЧИМОСТЬ.

В настоящей работе показано, что пациентам с острым коронарным синдромом без подъема сегмента БТ в течение первых 3 дней от его развития целесообразно определение функционального состояния эндотелия с помощью теста эндотелий зависимой вазодилатации плечевой артерии для оценки дальнейшего течения заболевания и выбора соответствующей тактики лечения. Такой метод оценки функционального состояния эндотелия является доступным и относительно простым в освоении и использовании. Он может сыграть значимую роль в практической кардиологии для уменьшения количества сердечно-сосудистых событий у больных с острым коронарным синдромом.

Также в настоящей работе показано, что выявление относительно повышенного содержания тканевого активатора плазминогена (ТАП) и уровня активности ингибитора тканевого активатора плазминогена 1 типа (ИТАП-1) в плазме крови у больных на 1-3 сутки от развития острого коронарного синдрома без подъема сегмента БТ может облегчить выбор группы лиц с повышенным риском развития осложнений заболевания, а, следовательно, нуждающихся в максимально интенсивном лечении.

Учитывая полученные в настоящей работе данные по положительному влиянию на функцию эндотелия ангиопластики со стентированием коронарных артерий, выявление относительно повышенного содержания тканевого активатора плазминогена и уровня активности ингибитора тканевого активатора плазминогена 1 типа в плазме крови и/или значительно сниженной эндотелий зависимой вазодилатации плечевой артерии у больных с острым инфарктом миокарда без подъема сегмента БТ на 1-3 сутки от его развития является дополнительным показанием к проведению чрескожных коронарных вмешательств (ЧКВ).

выводы.

1. У пациентов с острым коронарным синдромом без подъема сегмента 8Т на 1-3 сутки от его развития отмечается более низкая эндотелий зависимая вазодилатация плечевой артерии по сравнению с больными стенокардией напряжения и здоровыми лицами. К 7-14 суткам эндотелий зависимая вазодилатация плечевой артерии больных острым коронарным синдромом без подъема сегмента 8Т достоверно улучшается и достигает уровня больных стенокардией напряжения.

2. Эндотелий зависимая вазодилатация плечевой артерии, определенная на 1-3 сутки острого коронарного синдрома без подъема сегмента ЭТ, достоверно ниже среди тех больных, у которых в госпитальный период наблюдалось осложненное течение заболевания, по сравнению с неосложненным течением.

3. У пациентов с острым коронарным синдромом без подъема сегмента 8Т на 1-3 сутки от его развития эндотелий зависимая вазодилатация плечевой артерии обратно коррелирует с уровнем активности фактора фон Виллебранда в плазме крови.

4. У больных, куривших до развития острого коронарного синдрома без подъема сегмента 8Т, уровень тканевого активатора плазминогена в плазме крови при поступлении в стационар достоверно ниже по сравнению с не курившими больными.

5. Уровень активности фактора фон Виллебранда коррелирует с количеством пораженных атеросклерозом ветвей коронарных артерий у больных с острым коронарным синдромом без подъема сегмента 8Т на 13 сутки от его развития.

6. Содержание тканевого активатора плазминогена и уровень активности ингибитора тканевого активатора плазминогена 1 типа в плазме крови у больных с осложненным течением острого коронарного синдрома без подъема сегмента 8Т в госпитальный период достоверно выше на 1-3 сутки по сравнению с содержанием тканевого активатора плазминогена и уровнем активности ингибитора тканевого активатора плазминогена 1 типа в плазме крови больных с неосложненным течением острого коронарного синдрома без подъема сегмента 8Т.

7. У больных с острым инфарктом миокарда без подъема сегмента 8Т, которым была проведена ангиопластика со стентированием коронарных артерий, эндотелий зависимая вазодилатация плечевой артерии на 7-14 день достоверно выше, по сравнению с больными, которым ангиопластика не проводилась.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ.

1. Пациентам с острым коронарным синдромом без подъема сегмента БТ в течение первых 3 дней от его развития целесообразно определение функционального состояния эндотелия с помощью теста эндотелий зависимой вазодилатации плечевой артерии и определения содержания тканевого активатора плазминогена и уровня активности ингибитора тканевого активатора плазминогена 1 типа в плазме крови для оценки дальнейшего течения заболевания.

2. Выявление значительно сниженной эндотелий зависимой вазодилатации плечевой артерии и/или относительно повышенного содержания тканевого активатора плазминогена и уровня активности ингибитора тканевого активатора плазминогена 1 типа в плазме крови у больных с острым коронарным синдромом без подъема сегмента 8Т на 1-3 сутки от его развития является дополнительным показанием к проведению чрескожных коронарных вмешательств.
