Актуальность проблемы
Туберкулез как инфекционную болезнь характеризует высокая восприимчивость к нему человека; многообразие клинических проявлений, связанных с поражением различных органов, главным образом - дыхательной системы; в основном хроническое течение; тесная связь с условиями жизни и недостаточная эффективность лечения. Заболевание имеет преимущественно антропонозный характер с аспирационным механизмом передачи возбудителя -патогенных микобактерий туберкулезного комплекса, среди которых наиболее значимым является Mycobacterium tuberculosis.
Глобальное ухудшение эпидемической ситуации по туберкулезу в последнее десятилетие обусловлено социально-экономическими, демографическими факторами и распространением вируса иммунодефицита человека, оказывающими неблагоприятное влияние на сопротивляемость организма возбудителю (Dye С. et al., 1999). Приобрела остроту проблема лекарственной устойчивости микобактерий, особенно в странах Восточной Европы (России, Эстонии, Латвии), а также в Иране и Китае, где отмечено широкое распространение штаммов М. tuberculosis, обладающих множественной лекарственной устойчивостью (МЛУ) к основным противотуберкулезным препаратам (ЛТП) - изониазиду и рифампицину (Espinal М. et al., 2001). Установлено, что ряд хромосомных мутаций, обеспечивающих устойчивость к этим препаратам, формирующим мультирезистентность, не снижает жизнеспособности и вирулентности микобактерий туберкулеза (МБТ), способствуя поддержанию резервуара возбудителя в условиях широкого применения химиотерапии (Ordway D. et al., 1995; Billington O. et al., 1999; Pym A. et al., 2002). В этой связи, потенциальную эпидемическую опасность представляет циркуляция мультирезистентных штаммов М. tuberculosis, способных вызывать групповые заболевания подобно клону W-Beijing (Bifani P. et al., 2002).
Подъем основных эпидемиологических показателей в России в конце XX века, особенно в 1999-2000 г.г., связан с истинным ухудшением эпидемической ситуации по туберкулезу и введением учета всех новых случаев заболевания по месту выявления (Перельман М.И., 2000). В 2001 г. показатели заболеваемости и смертности несколько снизились по сравнению с 2000 г. и составили 88,2 и 18,9 против 90,7 и 20,4 на 100 000 населения. Однако с учетом сохранения современных тенденций социально-экономических условий жизни и организации противотуберкулезной помощи ожидать стабилизации эпидемического процесса в ближайшее десятилетие преждевременно (Шилова М.В., 2001). Высокий уровень распространения туберкулеза в России в современных условиях отличает рост числа впервые выявленных больных относительно молодого возраста с тяжелым прогрессирующим течением болезни за счет экзогенной инфекции и суперинфекции, вызванных лекарственно устойчивыми вариантами возбудителя (Хоменко А.Г., 1997а; Шилова М.В., 2001). Указанные особенности эпидемического процесса в значительной мере обусловлены изменчивостью таких важнейших биологических свойств М. tuberculosis как вирулентность и лекарственная чувствительность (Вишневский Б.И. и др., 2002).
Повышение эффективности лечебно-профилактических мероприятий и эпидемиологического надзора за туберкулезом требует совершенствования микробиологической диагностики и слежения за популяциями М. tuberculosis на основе молекулярно-генетических методов выявления и маркирования возбудителя (Шагинян И.А. 1996; Вишневская Е.Б., 2001; Голышевская В.И. и др., 2001; Rhee J. et al., 2000; Van Soolingen D. 2001). В этой связи, в последнее десятилетие появились работы, посвященные генотипической характеристике российских штаммов М. tuberculosis (Шагинян И.А. и др., 1997; Генерозов Э.В. и др., 1999; Нарвская ОБ. и др., 1999, 2002; Степаншина В.Н. и др., 2000; Тунгусова О.С., 2001; Черноусова JI.H. и др., 2001; Шемякин И.Г. и др., 2002; Kurepina N. et al., 2001; Drobniewsky F. et al., 2002).
Недостаток сведений о структуре популяций на территориях, о частоте и спектре мутаций, связанных с лекарственной устойчивостью М. tuberculosis различных генотипов, наряду с отсутствием данных об их эпидемиологическом и патогенетическом значении обусловливают актуальность системных исследований генетического разнообразия возбудителя туберкулеза с учетом современных тенденций развития эпидемического процесса в Российской Федерации.
Изложенное определило цель и задачи настоящей работы. Цель исследования
Изучить генетическое разнообразие популяции возбудителя туберкулеза, а также оценить эпидемиологическую и клинико-патогенетическую значимость штаммов М. tuberculosis различных генотипов, циркулирующих на Северо-Западе России.
Задачи работы:
1. Изучить геномный полиморфизм штаммов М. tuberculosis, циркулирующих на Северо-Западе России, с использованием методов сполиготипирования и IS6770-RFLP.
2. Установить превалирующие генотипы в изученной популяции М. tuberculosis.
3. Определить частоту лекарственной устойчивости и профили мутаций в генах rpoB, katG, embB и rpsL, ассоциированных с устойчивостью к рифампицину, изониазиду, этамбутолу и стрептомицину, соответственно, у штаммов Ы. tuberculosis различных генотипов.
4. Оценить вклад штаммов М. tuberculosis различных генотипов в развитие тяжелых деструктивных форм туберкулеза легких и формирование эпидемических очагов.
Научная новизна
Впервые на основе методов сполиготипирования, IS67 /O-RFLP-типирования и определения мутаций в генах rpoB, katG3l5, етЬВЗОб и rpsL43, ассоциированных с устойчивостью возбудителя туберкулеза к рифампицину, изониазиду, этамбутолу и стрептомицину, проведено исследование геномного полиморфизма штаммов и получены новые знания о структуре популяции М. tuberculosis на Северо-Западе России. Установлено превалирование и доказана эпидемиологическая значимость штаммов генотипа Beijing в изученной популяции возбудителя туберкулеза.
Показано, что мультирезистентность штаммов М. tuberculosis, циркулирующих на Северо-Западе России, связана с генотипом Beijing и обусловлена мутациями в генах гроВ (кодоны 531, 516 и 526) и katG (кодон 315).
Показан вклад мультирезистентных штаммов генотипа Beijing в развитие тяжелых деструктивных форм туберкулеза легких, а также обоснована роль экзогенной суперинфекции штаммами этого генотипа при рецидивах заболевания. Установлена этиологическая роль мультирезистентного штамма ВО генотипа Beijing при групповых внутрибольничных заболеваниях с вовлечением персонала.
Обоснована эффективность изучения комплекса маркеров геномного полиморфизма штаммов (сполиготип, профиль IS6//0-RFLP, профили мутаций в генах rpoB, katG315, етЬВЗОб и rpsL43) в эпидемиологических исследованиях при туберкулезе.
Практическая значимость исследования
Создан компьютерный банк данных, содержащий профили сполиготипирования и профили IS6//0-RFLP штаммов М. tuberculosis. Банк данных служит основой налаживания микробиологического слежения за структурой популяции и циркуляцией эпидемиологически значимых мультирезистентных штаммов М. tuberculosis на территории Северо-Запада России, а в перспективе - и в других регионах страны, а также позволяет участвовать в международных исследованиях глобальной эпидемиологии туберкулеза и филогении возбудителя.
Использование метода сполиготипирования в целях быстрой идентификации и генотипирования микобактерий туберкулезного комплекса непосредственно в исследуемом материале (окрашенные препараты для микроскопии) способствует совершенствованию микробиологической диагностики туберкулеза.
Выявление одновременной устойчивости штаммов М. tuberculosis различных генотипов к рифампицину и изониазиду, формирующим мультирезистентность, путем определения мутаций в генах гроВ (кодоны 531, 516 и 526) и katG315 с помощью быстрых методов, основанных на применении полимеразной цепной реакции (ПЦР), способствует совершенствованию микробиологической диагностики и повышению эффективности лечения туберкулеза.
Использование метода IS6//0-RFLP для генотипирования изолятов М. tuberculosis существенно повышает эффективность эпидемиологического обследования очагов туберкулеза, в т.ч. нозокомиальных, позволяя обосновать источник или общность источника М. tuberculosis при групповых заражениях. При этом может быть подтверждено инфицирование от предполагаемого источника, а в противном случае - показана необходимость дополнительного эпидемиологического расследования.
Основные положения диссертации, выносимые на защиту:
1. Популяция М. tuberculosis на Северо-Западе России неоднородна по DR-локусу хромосомы и маркеру IS 6110.
2. В изученной популяции М. tuberculosis преобладают штаммы клонального генетического семейства Beijing.
3. Штаммы генотипа Beijing по сравнению со штаммами других генотипов характеризует более высокая частота устойчивости к отдельным препаратам и мультирезистентности, связанной с мутациями в генах гроВ и katG 315.
4. Мультирезистентные штаммы генотипа Beijing могут вызывать тяжелое течение туберкулеза легких, а также рецидивы заболевания.
5. Мультирезистентный штамм ВО генотипа Beijing способен вызывать групповые внутрибольничные заболевания.

Внедрение результатов исследования в практику
На основе результатов работы подготовлены следующие документы:
1. Аналитический обзор "Геномная дактилоскопия М. tuberculosis в системе эпидемиологического надзора за туберкулезом" (используется Департаментом Госсанэпиднадзора Минздрава России при подготовке справочно-информационных документов).
2. Пособие для врачей "Генетическое маркирования возбудителя туберкулеза и его применение в эпидемиологическом обследовании очагов" (Утв. МЗ РФ 20.12.02, протокол № 5).
3. Заявка о выдаче патента на изобретение "Способ экспрессного определения устойчивости к рифампицину у микобактерий туберкулеза" (приоритетная справка на выдачу патента № 2003115763 от 30.05.03).
Результаты исследований внедрены в практическую деятельность Санкт-Петербургского НИИ фтизиопульмонологии МЗ РФ. Материалы работы используются в учебном процессе Санкт-Петербургской МАЛО.
Публикации результатов исследования
По теме диссертации опубликована 31 печатная работа, две работы находятся в печати.

ВЫВОДЫ
1. Популяция М. tuberculosis на Северо-Западе России относительно неоднородна по DR-локусу хромосомы: среди штаммов 69 сполиготипов, известных в настоящее время, преобладают представители генетических семейств Beijing, LAM, Haarlem и Т.
2. Доля штаммов генотипа Beijing в изученной популяции М. tuberculosis составляет 51,3%. Штаммы М. tuberculosis генотипа Beijing характеризует клональная изменчивость по маркеру IS6110.
3. Устойчивость к изониазиду, рифампицину, стрептомицину, этамбутолу и мультирезистентность штаммов изученной популяции М. tuberculosis связаны с генотипом Beijing. У резистентных к рифампицину штаммов генотипа Beijing доминирует аллель rpoB531 TTG.
4. Мультирезистентные штаммы генотипа Beijing вызывают тяжелое прогрессирующее течение туберкулеза легких, в частности, у лиц сравнительно молодого возраста, и рецидивы заболевания за счет экзогенной инфекции и суперинфекции.
5. Штамм ВО М. tuberculosis генотипа Beijing, выделенный при групповом внутрибольничном заболевании, характеризует высокая трансмиссивность, наличие 17 фрагментов рестрикции, имеющих определенное взаиморасположение в профиле IS6770-RFLP, а также мультирезистентность к основным противотуберкулезным препаратам (рифампицину, изониазиду, стрептомицину и этамбутолу), связанная с мутациями rpoB531TTG, katG315ACC, embB306BTG и rpsL43.
6. Маркирование штаммов М. tuberculosis с использованием комплекса молекулярно-генетических методов, включающих сполиготипирование, IS6770-RFLP и определение спектра мутаций, ассоциированных с лекарственной устойчивостью, повышает эффективность эпидемиологических исследований при туберкулезе.
ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ
Для повышения эффективности микробиологической диагностики, лечения и предупреждения распространения туберкулеза, вызванного мультирезистентными штаммами МВТ, целесообразно:
- внедрить быстрые методы геноидентификации и определения лекарственной устойчивости изолятов МВТ, основанные на применении ПЦР;
- ввести в практику эпидемиологического обследования очагов туберкулеза генотипирование изолятов МВТ с использованием методов геномной дактилоскопии;
- создать систему динамического слежения за возбудителем в масштабах страны на основе единого компьютерного банка профилей сполиготипирования и 18(577#-КРЬР циркулирующих штаммов МВТ.
