Интерес к изучению хронических заболеваний кишечника связан с ростом заболеваемости данной патологией во всем мире [22,38,39,46].

Глютенчувствительная целиакия (ГЦ) — заболевание тонкой кишки, характеризующееся развитием гиперрегенераторной атрофии слизистой оболочки тонкой кишки в ответ на введение глютенсодержащих злаковых продуктов (пшеницы, ржи, ячменя и овса) у лиц, генетически предрасположенных к заболеванию. Несмотря на длительную историю изучения заболевания, многие вопросы, касающиеся основных аспектов проблемы, остаются недостаточно изученными.

Миф о целиакии как о редком заболевании постепенно развеялся благодаря применению современных методов диагностики, и в настоящее время по разным оценкам около 1% населения страдает этим заболеванием [142, 213,255].

Не уточнены группы риска и частота выявления в них ГЦ, в первую очередь среди пациентов с хронической диареей [202,356], железодефицитной анемией неясной этиологии [256,364],- первичным бесплодием [175,389] и другой патологией [32,391,487 и др.].

Многообразие вариантов клинического течения, а также отсутствие классификации, охватывающей основные клинические формы заболевания; включая атипичные, бессимптомные и латентные, создают значительные сложности для ранней диагностики целиакии [43,185,243,249 и др.]. Недостаточное знакомство практикующих врачей с этим заболеванием приводит к тому, что даже при типичном течении диагноз устанавливается в среднем, через 11,7 лет [154]. Несвоевременное назначение адекватного этиотропного лечения ухудшает прогноз заболевания, а также увеличивает частоту аутоиммунных и онкологических осложнений [67,99].

Морфологическое исследование является основным методом, диагностики целиакии. В то же время постановка диагноза целиакии и неправомерное назначение аглютеновой диеты (АГД), больным на основании только серологических проб или неправильной интерпретации гистологических изменений, приводит не только к гипердиагностике заболевания, но и затрудняет дальнейшую лечебную тактику [26]. В связи с этим необходимо уточнение чувствительности и специфичности скрининговых серологических методов диагностики ГЦ, а также определение возможной их взаимосвязи с морфологическими изменениями слизистой оболочки* тонкой кишки=(СОТК) и вариантами клинического течения заболевания.

Этиотропное лечение целиакии предполагает строгое и пожизненное соблюдение АГД. Однако соблюдение только диетических рекомендаций не всегда приводит к быстрой клинической ремиссии и зачастую требует дополнительной медикаментозной коррекции [149,302,432,462]. В то же время многообразие вариантов клинического течения ГЦ обосновывает необходимость разработки дифференцированных подходов ^ к лечению. В настоящее время, обоснованность и продолжительность медикаментозной терапии, а также критерии оценки её эффективности, в зависимости от клинической формы заболевания не разработаны.

Отдаленные результаты и прогноз заболевания во многом; зависят от тщательности соблюдения АГД. Недостаточная осведомленность врачей и диетологов о составе продуктов питания, содержащих «скрытый» глютен, отсутствие производства в России безглютеновых продуктов (БП); с соответствующей маркировкой, создают значительные сложности соблюдения больными АГД. Дороговизна и трудность приобретения БП зарубежного производства ограничивают их применение, а необходимость пожизненного соблюдения АГД приводят тому, что часть больных периодически её нарушает. Поэтому одной-из важных проблем при целиакии является медико-социальная адаптация больных к новому образу жизни с необходимостью пожизненного соблюдения АГД.

В России отсутствуют система и общепризнанные рекомендации по реабилитации больных целиакией. Анализ зарубежной литературы также показывает расхождение во взглядах на эту проблему, а многие рекомендации не основаны на фактических данных [439]. Всё вышеизложенное позволяет говорить об отсутствии системы реабилитации и диспансерного наблюдения за больными ГЦ и обосновывает необходимость её разработки.

Таким образом, отсутствие в России клинической классификации ГЦ, противоречивость данных по распространенности её в популяции, а также сведений о её частоте среди групп риска, отсутствие морфологических и иммунологических характеристик в зависимости от клинических форм заболевания, а также алгоритма обследования и лечения больных ГЦ, системы реабилитации и диспансерного наблюдения, предопределили актуальность и обосновали необходимость данного исследования.

ЦЕЛЬ РАБОТЫ

Выявить особенности клинической картины и морфологических изменений слизистой оболочки тонкой кишки у больных с различными формами глютенчувствительной целиакии; разработать дифференцированные методы лечения и систему диспансерного наблюдения.

ЗАДАЧИ

1. Определить частоту выявления целиакии среди больных с хронической диареей, железодефицитной анемией неясной этиологии и нарушением репродуктивной функции.

2. Изучить особенности клинических проявлений и. разработать клиническую классификацию глютенчувствительной целиакии.

3. Выявить особенности морфологических изменений слизистой оболочки тонкой кишки при различных формах целиакии.

4. Определить особенности иммунного статуса у больных с различными клиническими формами целиакии по уровню антител к глиадину (АГА), тканевой трансглютаминазе (АТтТГ) и цитокиновому профилю сыворотки крови.

5. Оценить влияние базисной терапии (аглютеновой диеты) и медикаментозного лечения на клиническое течение, морфологические изменения слизистой оболочки тонкой кишки и иммунный статус больных целиакией.

6. Разработать дифференцированный подход к лечению в зависимости от клинических форм целиакии и предложить критерии оценки его эффективности.

7. Разработать рекомендации по реабилитации и диспансерному наблюдению за больными целиакией.

НАУЧНАЯ НОВИЗНА

• Впервые предложена клиническая классификация ГЦ с выделением 5 основных форм: типичной, латентной, рефрактерной, атипичной и бессимптомной и установлена частота их выявления среди общей популяции больных целиакией. Показано, что типичная (классическая) форма ГЦ встречается только в 8,2% случаев, а наиболее распространенной формой является латентная форма, которая была выявлена в 80,7 % случаев.

• Впервые выявлена частота ГЦ среди больных с факторами риска, включая лиц с хронической диареей, железодефицитной анемией неясного генеза и нарушением репродуктивной функции, что обосновывает необходимость более углубленного обследования таких пациентов с целью раннего выявления заболевания.

• Определена взаимосвязь между тяжестью клинических проявлений при ГЦ и гистологическими изменениями СОТК: более выраженным структурным изменениям в СОТК соответствует тяжелая степень СНВ.

• Впервые показано, что наиболее тяжелая степень мальабсорбции (СНВ III степени тяжести) достоверно чаще наблюдалась среди больных с рефрактерной формой по сравнению с типичной (р<0,05) и одинаково часто с латентной формой. В то время как для больных с атипичной формой ГЦ характерна более легкая степень СНВ.

• Впервые установлены частота и уровень повышения антител к глиадину (АГА) и тканевой трансглютаминазе (АТтТГ) класса IgA и IgG при различных клинических формах ГЦ. Наиболее часто повышение титра ATA IgA отмечалось у больных с типичной и латентной формой, а наиболее высокие уровни концентрации АГА IgA были выявлены при латентной и атипичной формах. Наибольшая частота повышения уровней АТтТГ IgA определялись при латентной и атипичной формах. Для этих форм были характерны и более высокие титры антител по сравнению с другими формами.

• Разработан алгоритм диагностики, направленный на выявление ГЦ как среди больных с клиническими симптомами заболевания кишечника, так и среди пациентов групп риска, в том числе и на ранних стадиях заболевания с минимальными изменениями в СОТК.

• Разработаны критерии оценки эффективности терапии и предложена схема дифференцированного лечения в зависимости от клинической формы ГЦ.

• Разработана система реабилитации и диспансерного наблюдения за больными ГЦ, заключающаяся в динамическом контроле за качеством соблюдения АГД и способствующая своевременному выявлению аутоиммунных заболеваний и профилактике осложнений ГЦ.

ПРАКТИЧЕСКАЯ ЗНАЧИМОСТЬ

1. Предложенная классификация с выделением клинических форм ГЦ позволяет более целенаправленно оценить клинические симптомы заболевания, включая латентное, атипичное и бессимптомное течение и существенно улучшить качество диагностики.

2. Знание особенностей клинического течения ГЦ позволяет диагностировать заболевание на более ранних стадиях, своевременно назначать этио-тропное лечение, что способствует быстрой реабилитации больных и улучшает прогноз заболевания.

3. Установленная частота различных клинических форм изменяет традиционное представление о ГЦ как о редком заболевании, так как типичное течение встречается только у 8,2% больных, в остальных случаях клиническая картина не укладывается в классическое представление об этом заболевании.

4. Целенаправленное обследование лиц в группах риска с применением скриннинговьтх методов, способствует выявлению и своевременному лечению латентных и атипично протекающих форм ГЦ.

5. Отмечается параллелизм между степенью атрофии СОТК и степенью тяжести СНВ.

6. Разработанный алгоритм диагностики облегчает диагностический поиск, включая группы риска, ориентирует врача на выявление ГЦ на ранних стадиях.

7. Разработанные критерии оценки эффективности терапии позволяют объективизировать лечебный процесс, в том числе в зависимости от клинической формы ГЦ.

8. Предложенный алгоритм диспансерного наблюдения за больными ГЦ дает возможность пролонгировать клиническую ремиссию заболевания и способствует профилактике осложнений.

ОСНОВНЫЕ ПОЛОЖЕНИЯ, ВЫНОСИМЫЕ НА ЗАЩИТУ

1. Глютенчувствительная целиакия протекает в виде различных клинических форм: типичной, латентной, рефрактерной, атипичной и бессимптомной. Наиболее часто встречается латентная форма, наиболее редко — рефрактерная и бессимптомная.

2. Высокая частота выявления целиакии среди больных с хронической диареей, железодефицитной анемией и нарушением репродуктивной функции позволяет отнести их в группу риска и рекомендовать активное выявление целиакии среди данной категории больных.

3. Не отмечается отчетливой взаимосвязи между вариантами клинического течения целиакии и степенью атрофии слизистой оболочки тонкой кишки: При различных вариантах клинического-течения могут наблюдаться сходные гистологические изменения в слизистой оболочке тонкой кишки, но начальные признаки поражения СОТК могут свидетельствовать о наличии бессимптомной целиакии.

4. Частота выявления и уровень повышения, антител к глиадину (АГА IgA и АГА IgG) и тканевой трансглютаминазе (АТтТГ IgA и IgG) различаются в зависимости от клинической формы целиакии.

5. Лечение больных целиакией должно осуществляться дифференцированно в зависимости от степени тяжести синдрома нарушенного всасывания и клинической формы заболевания. Оценка эффективности лечения должна осуществляться по клиническим, иммунологическим и гистологическим критериям.

6. В основе реабилитации и диспансерного наблюдения за больными целиакией лежит контроль за соблюдением АГД и своевременное выявление аутоиммунных болезней, сопутствующих целиакии и профилактика осложнений.

ПУБЛИКАЦИИ

По материалам диссертационной работы опубликовано 40 научных работ, из них 10 статей - в ведущих рецензируемых изданиях, 1 методические рекомендации. Приоритетность полученных результатов подтверждена двумя патентами (№ 2293989 от 12 июля 2005 г., № 2324189 от 03 ноября 2006 г.)

ВНЕДРЕНИЕ РЕЗУЛЬТАТОВ ИССЛЕДОВАНИЯ В ПРАКТИКУ

• Наиболее значимые результаты исследования внедрены в научно-практическую работу клинических отделений в Центральном научно-исследовательском институте гастроэнтерологии Департамента здравоохранения г. Москвы.

262 Выводы

1. Среди 802 больных с патологией тонкой кишки, находившихся на лечении в ЦНИИГ с 1993 по 2007гг, глютенчувствительная целиакия (ГЦ) установлена у 207 человек, т.е. у 25,8 %. Выделено 5 клинических форм ГЦ: латентная (у 80,7%), типичная (у 8,2%), атипичная (у 6,3%), рефрактерная (у 2,4%) и бессимптомная целиакия (у 2,4%).

2. При морфологическом исследовании СОТК выявлена коррелятивная связь между степенью ее атрофии и тяжестью СНВ. Установлена отрицательная корреляционная связь между степенью тяжести СНВ и высотой ворсин (Rs= - 0,61, р<0,01), а также между степенью тяжести СНВ и соотношением высота ворсины/глубина крипты (Rs = - 0,56, р<0,01). Выявлена прямая связь между степенью тяжести СНВ и количеством МЭЛ (Rs = 0,40, р<0,01) и степенью лимфоплазмоцитарной инфильтрации (Rs = 0,40, р<0,01). Не обнаружено отчетливой связи между степенью атрофии СОТК и вариантами клинического течения целиакии. У больных с различными вариантами клинического течения целиакии могли наблюдаться сходные гистологические изменения СОТК. Тем не менее, начальные признаки ее поражения встречались только у больных с бессимптомной целиакией.

3. Частота выявления и титр антител к глиадину (АГА) зависели от особенностей клинического течения целиакии. Повышенные значения АГА IgA особенно часто наблюдались у больных типичной (90,0%) и латентной (98,7%) формами, несколько реже - у больных атипичной (70%) и бессимптомной (60%) целиакией. Повышенные значения АГА IgG наблюдались у 60% больных типичной, 96% -латентной, 60% - атипичной и 40% - бессимптомной формой. Концентрация этих антител в IgA и IgG при типичной форме болезни превышала контрольные значения в 7,3 и 3 раза соответственно, при латентной — в 24,4 и 10,5 раза, при атипичной - в 20,5 и 8,4 раза и бессимптомной - в 6 и 2,5 раза соответственно.

4. Частота выявления и титр антител к тканевой трансглютаминазе (АТтТГ) при ГЦ зависят от клинической формы заболевания. Среди больных типичной целиакией повышение AT тТГ IgA и IgG отмечалось у 80% , латентной целиакией — у 100% и 98,7%, атипичной — у 90% и 70% и бессимптомной - у 80% и 60%, соответственно. Концентрация AT тТГ IgA и IgG у больных типичной целиакией превышала контрольные значения в 10,8 и 17,3 раза, у больных латентной целиакией - в 19,6 и 29, 1 раза, атипичной — в 23,2 и 21,2 раза и бессимптомной - в 6 и 13,2 раза, соответственно. Более высокая частота выявления и титр АТтТГ по сравнению с АГА подтверждает более высокую чувствительность АТтТГ для диагностики всех клинических форм ГЦ, включая бессимптомную.

5. Установлена прямая корреляционная связь между уровнем АГА IgA (Rs = 0,51, р<0,01) и AT тТГ IgA (Rs = 0,67, р<0,01) и степенью атрофии СОТК. Эти данные позволяют на основании титра этих антител судить о степени выраженности патологических изменений слизистой оболочки тонкой кишки и предполагать наличие бессимптомной ГЦ у лиц с незначительным повышением концентрации AT.

6. У больных нелеченной целиакией наблюдалось повышение в сыворотке крови цитокинов: ИЛ-10, ФНО-а, ИЛ-6 и ИЛ-4, и их снижение на фоне АГД. Эти данные подтверждают представление о целиакии как воспалительном заболевании иммунной этиологии, демонстрируют этиотропную роль аглютеновой диеты и позволяют судить об эффективности лечения и прогнозе заболевания.

7. Из 209 больных, поступивших в клинику с хронической диареей, морфологически подтвержденная целиакия выявлена у 35 (16,9%) больных, что позволяет отнести хроническую диарею к одному из факторов риска целиакии.

8. Из 61 больного с железодефицитной анемией неясной этиологии, морфологически подтвержденная целиакия установлена у 29 (47,5%). У 16,6% обследованных больных ЖДА являлась единственным проявлением целиакии. Применение АГД у всех больных привело к повышению уровня железа в сыворотке крови и гемоглобина.

9. Среди 37 женщин с нарушением репродуктивной функции, связанной с первичным бесплодием, невынашиванием беременности и генетическим инфантилизмом у 14 (37,8%) обнаружено повышение уровня антител к тТГ и у четырех из них (10,8%) при гистологическом исследовании СОТК выявлена целиакия. Длительное применение АГД при бесплодии, ассоциированном с целиакией, может способствовать восстановлению репродуктивной способности.

10. Лечение больных целиакией зависит от особенностей клинической картины. Больным с диарейным синдромом, помимо соблюдения АГД, показано последовательное применение кишечных антисептиков и пробиотиков, регуляторов моторики и адсорбентов. Больным с СНВ должно проводиться дифференцированное патогенетическое лечение в зависимости от степени тяжести мальабсорции, направленное на восстановление нарушенного обмена веществ. Для обеспечения более полной и быстрой реабилитации больных с СНВ III степени тяжести и рефрактерной формой ГЦ следует применять кортикостероиды.

11. Комплексное лечение в стационаре у всех больных приводит к уменьшению клинических симптомов и нормализации лабораторных показателей СНВ: в 32,3% он отсутствует, в 62,5% отмечается СНВ I степени тяжести и только в 2,5% - СНВ II степени тяжести. Через 1 год соблюдения АГД у 37,9% происходит полное восстановление структуры СОТК, а в сроки до 5 лет - у 64,1% больных. Нормализация уровня АГА IgA и АТтТГ IgA в сроки наблюдения до 1 года происходит у 48,8% и 72,3% больных, а в сроки до 5 лет у 81% и 88,1% - соответственно.

12. По данным длительного (до 30 лет) динамического наблюдения установлено, что только 72,4% больных тщательно соблюдают АГД, 9,3% - не достаточно строго придерживаются диеты, а 18,3 % больных отказались от её соблюдения, что отражается на качестве реабилитации больных ГЦ.

13. Несмотря на строгое следование АГД в 38,5% случаев наступали обострения заболевания, которые характеризовались появлением клинических и лабораторных симптомов мальабсорбции или отсутствовало полное восстановление СОТК. Причинами недостаточной эффективности лечения могли служить неосознанное нарушение АГД, связанное с отсутствием маркировки продуктов, системы обеспечения больных безглютеновыми продуктами или рефрактерное течение болезни.

Практические рекомендации

1. Оценками эффективности лечения больных ГЦ должны служить: 1) клинический критерий (полная клинико-лабораторная ремиссия у больных типичной, латентной и атипичной целиакией и частичная - у больных с рефрактерной целиакией), 2) гистологический критерий (полное восстановление или заметное улучшение морфологической структуры СОТК у больных типичной, латентной, атипичной и бессимптомной целиакией и частичное - у больных рефрактерной целиакией), 3) иммунологический критерий (снижение титра АГА и АТТГ под влиянием лечения).

2. Система реабилитации больных ГЦ должна включать следующие аспекты: диагностические (ранняя диагностика с учетом особенностей клинического течения заболевания); терапевтические (соблюдение АГД и медикаментозная коррекция в зависимости от степени тяжести СНВ) и организационные (введение маркировки безглютеновых продуктов и создание системы обеспечения больных ГЦ продуктами, не содержащими глютен).

3. Система диспансерного наблюдения за больными ГЦ должна включать: а) контроль за соблюдением АГД, б) определение степени ограничения трудоспособности, в) своевременное выявление аутоиммунных и онкологических болезней, г) диагностику и лечение сопутствующих заболеваний.

4. С целью улучшения диагностики целиакии больным с хронической диареей, ЖДА неустановленной этиологии и первичным бесплодием необходимо исследовать АГА и/или АТТГ в сыворотке крови. При положительных серологических тестах — назначать множественную биопсию (не менее 3-х фрагментов) СОТК.

5. При подозрении на латентную и атипичную формы целиакии показано применение серологических тестов (АТтТГ IgA), так как при этих формах наиболее часто и в высоких титрах отмечается повышение антител к тканевой трансглютаминазе.
