АКТУАЛЬНОСТЬ. В настоящее время, благодаря интенсивным исследованиям последних десятилетий, не вызывает сомнений, что физиологическая и репаративная регенерация органов происходит благодаря наличию в них тканеспецифичных стволовых клеток, которые обнаружены в скелетных мышцах, роговице, эпидермисе кожи, красном костном мозге и многих других тканях и органах [292]. На этом фоне кажется парадоксальным, что в печени, удивительные способности к регенерации которой известны с античных времен, стволовая клетка до сих пор не идентифицирована.

Начиная с 60-х годов двадцатого столетия, на роль стволовой клетки печени рассматривались, так называемые овальные клетки [186, 359], фенотип которых идентичен фенотипу пренатальных гепатобластов [36, 40, 63, 64, 101, 129, 132, 180, 270, 410]. Овальные клетки появляются и участвуют в восстановлении печени после повреждения органа некоторыми канцерогенами на фоне частичной гепатэктомии [35, 116, 117, 130, 180, 188, 330, 352, 353, 404, 421, 428, 429]. Однако овальных клеток, нет во взрослой здоровой печени и в печени, которая регенерирует после частичной гепатэктомии [307, 310]. Поэтому главный вопрос — за счёт каких стволовых клеток восстанавливалась печень после частичной гепатэктомии до сих не решен.

Во-первых, это связано с тем, что большинство работ по изучению регенерации печени после частичной гепатоэктомии было выполнено в середине 20-го столетия, когда уровень знаний о клеточном составе печени и строении синусоидов, а также техническая оснащенность лабораторий, не позволяли идентифицировать отдельные клеточные типы, участвующие в регенерации.

Во-вторых, описание овальных клеток привело к тому, что большинство исследований проведено на моделях регенерации органа с участием овальных клеток, а не на ЧГ. Однако в конце 20-го столетия исследование «классической» частичной гепатэктомии позволило практически по часам расписать включение различных генов, изменение концентрации цитокинов, а также установить вовлеченность в регенераторный ответ синусоидных клеток, в частности перисинусоидальных клеток печени. Перисинусоидальные клетки накапливают и сохраняют в своей цитоплазме ретиноиды [66], которые являются важными факторами морфогенеза эпителиев, модифицируют внеклеточный матрикс [233], синтезируют большое количество разнообразных цитокинов и факторов роста, среди которых фактор стволовых клеток [160], фактор роста гепатоцитов [393], эритропоэтин [301, 120]. Более того, перисинусоидальные клетки реагируют на любое повреждение печени независимо от его тяжести и характера [156]. Однако вопрос о том, являются ли перисинусоидальные клетки стволовыми, прогениторными, или они создают необходимое микроокружение для стволовых клеток печени до сих пор остается невыясненным.

Открытие специфических маркеров, позволяющих идентифицировать стволовые клетки и клетки предшественники, стимулировало новый виток исследований по идентификации стволовой клетки печени. Так, в работе Урываевой с соавторами [25] было установлено, что после частичной гепатэктомии некоторые гепатоциты приобретают фенотипические характеристики стволовых клеток. Гепатоциты изначально рассматривались как основной и чуть ли не единственный клеточный тип, вовлеченный в процессы гипертрофии печеночных долек [307, 310], которая, в свою очередь, по мнению ряда исследователей, лежит в основе репаративной регенерации печени после частичной гепатэктомии [345, 505]. Однако гипертрофия проявляется лишь увеличением толщины печеночных балок до размера двух гепатоцитов [46, 309], что не объясняет механизмов внутренней перестройки железы. Этот механизм внутренней перестройки морфофункциональных единиц печени пытались объяснить ветвлением воротной вены [162]. Но воротная вена - это лишь одна из структур портального тракта, где обязательно присутствуют также ветвь печеночной артерии и желчный проток, поэтому перестройка структуры железы не может быть объяснена только с позиции ветвления воротной вены. Более того, клетки желчных протоков - это еще один эпителиальный клеточный тип печени, который, как предполагают, в ходе пренатального развития печени может образовываться из перипортальных гепатоцитов [111, 461]. Что же происходит после частичной гепатэктомии — повторение пренатального гистогенеза или спраутинг протоковых структур, до сих пор не известно, также как неизвестно каким образом перестраиваются притоки печеночной вены.

Таким образом, в настоящее время до сих пор остаются открытыми следующие основные вопросы: что же собой представляет стволовая клетка печени, как восстанавливается не только клеточная масса, но и структура железы в ходе регенерации, какие изменения происходят в сосудистой системе на уровне портальных трактов, притоков печеночных вен и синусоидов?

ЦЕЛЬ РАБОТЫ - изучение изменения микроархитектоники печени и активации ее стволовых клеток в ходе регенерации после частичной гепатэктомии у крыс.

ЗАДАЧИ ИССЛЕДОВАНИЯ:

1. Выполнить морфометрическое исследование размеров печеночных долек через 1, 2, 3, 5, 7 суток после ЧГ.

2. Изучить пролиферацию гепатоцитов, холангиоцитов, перисинусоидальных клеток и макрофагов после ЧГ.

3. Изучить реакцию элементов портального тракта (желчные протоки, ветви воротной вены и печеночной артерии) и центральной вены после ЧГ.

4. Выявить клетки, экспрессирующие маркер стволовых и прогениторных клеток C-kit, в печени крыс после ЧГ.

НАУЧНАЯ НОВИЗНА. В диссертации впервые на основе совмещения морфометрического и иммуногистохимического анализа получены доказательства того, что в гипертрофированных после частичной гепатэктомии долях печени происходит образование новых ацинусов. Было установлено, что в основе данного процесса лежит пролиферация гепатоцитов и перисинусоидальных клеток с последующей перестройкой морфофункциональных единиц. Образование новых притоков печеночных вен происходит за счёт перестройки синусоидов, а новые трубчатые структуры портальных трактов появляются в ходе преобразования терминальных артериол и венул в артерии и вены портальных трактов, с одновременным образованием рядом с ними новых желчных протоков. Впервые установлено, что при регенерации печени после частичной гепатэктомии не только гепатоциты, но и перисинусоидальные клетки проявляют фенотипические признаки стволовых и прогениторных клеток, экспрессируя C-kit.

ТЕОРЕТИЧЕСКАЯ И ПРАКТИЧЕСКАЯ ЗНАЧИМОСТЬ. Результаты работы позволили по новому оценить и понять закономерности изменения микроструктуры железы в ходе регенерации печени после частичной гепатэктомии. Проведенные исследования способствовали выявлению новых фактов как о характере, так и о последовательности клеточных реакций, возникающих в ходе регенерации печени после частичной гепатэктомии, и участии в этих процессах отдельных клеточных типов печени (гепатоцитов, холангиоцитов, клеток Купфера и перисинусоидальных клеток). Результаты работы расширяют и углубляют представления о роли перисинусоидальных клеток в регенерации печени, о процессах взаимодействия различных клеточных типов в ходе регенерации. Теоретически значимым является установление факта изменения фенотипа перисинусоидальных звездчатых клеток в ходе регенерации и экспрессия этими клетками рецептора к фактору стволовых клеток. Совокупность полученных данных позволила предложить, что перисинусоидальные звездчатые клетки могут претендовать на роль стволовых клеток печени.

Полученные результаты могут быть использованы для дальнейшего изучения проблемы регенерации и активации стволовых клеток печени, а таюке в учебном процессе. В ходе выполнения работы разработаны новые методические подходы для двойного иммуногистохимического окрашивания, которые могут быть рекомендованы для лабораторной практики.

ОСНОВНЫЕ ПОЛОЖЕНИЯ, ВЫНОСИМЫЕ НА ЗАЩИТУ:

1. Восстановление оставшихся долей печени после частичной гепатэктомии происходит поэтапно и начинается с 1) гипертрофии печеночных ацинусов, происходящей за счёт пролиферации гепатоцитов и перисинусоидальных клеток, и 2) заканчивается перестройкой морфофункциональных единиц путем спраутинга афферентных и эфферентных сосудов и желчных протоков.

2. В ходе регенерации печени после частичной гепатэктомии перисинусоидальные клетки являются клеточным типом, который наряду с гепатоцитами экспрессирует маркер стволовых и прогениторных клеток C-kit.

ВНЕДРЕНИЕ РЕЗУЛЬТАТОВ ИССЛЕДОВАНИЯ.

Иммуногистохимическая оценка регенераторного процесса при заболеваниях печени успешно внедрена в практику ГМУ «Республиканская клиническая больница» (г. Казань), ГУЗ «Республиканская клиническая инфекционная больница» (г. Казань). Результаты проведенного исследования внедрены в учебный процесс на кафедре нормальной анатомии и кафедре гистологии Казанского государственного медицинского университета.

АПРОБАЦИЯ РАБОТЫ. Результаты исследований доложены и обсуждены на XII Всероссийской научно-практической конференции «Молодые ученые в медицине» (Казань, 2007), научно-практической конференции «Современные аспекты гистогенеза и вопросы преподавания гистологии в ВУЗе», посвященной 100-летию со для рождения Л.И.Фалина (Москва, 2007), 162-м международном симпозиуме Фальк (Dresden, Germany,

2007), 167-м международном симпозиуме Фальк (Mainz, Germany, 2008), IV Международной Пироговской научной медицинской конференции (Москва, 2009), XIV Всероссийской научно-практической конференции «Молодые ученые в медицине» (Казань, 2009), 44-ом ежегодном съезде Европейской ассоциации изучения печени (Копенгаген, Дания, 2009).

СТРУКТУРА И ОБЪЕМ ДИССЕРТАЦИИ. Диссертация изложена на 198 страницах машинописи и состоит из введения, обзора литературы, описания материалов и методов, результатов собственных исследований с обсуждением полученных данных, заключения, выводов и библиографии. Диссертация содержит 7 таблиц и 39 рисунков. Список литературы включает 511 наименований, из которых 27 русскоязычных и 484 иностранные работы.

6. ВЫВОДЫ

1. При регенерации печени после частичной гепатэктомии у крыс в течение первых трех суток происходит увеличение размеров оси ацинуса -расстояний между близлежащими портальными трактами - в 1,6 раза и площади печеночных долек в 2,5 раза, а с четвертых по седьмые - уменьшение до нормальных значений.

2. Увеличение размера печеночных долек в первые 3 суток после ЧГ обеспечивается пролиферацией клеток печени, уменьшение с четвертых по седьмые - образованием новых печеночных долек.

3. Формирование новых печеночных долек обеспечивается ветвлением центральной вены, желчных протоков, ветвей воротной вены и печеночной артерии.

4. В процессе регенерации печени после частичной гепатэктомии происходит увеличение числа внутрипеченочных желчных протоков за счет пролиферации холангиоцитов, пик пролиферации которых приходится на 3 сутки (36%). Однако через 7 суток индекс пролиферации холангиоцитов сохраняется высоким (12%), что указывает на незавершенность восстановления желчных протоков.

5. При регенерации печени после частичной гепатэктомии происходит увеличение числа клеток Купфера, и оно достигает максимума через 2 суток после операции. Увеличение численности клеток Купфера, главным образом, происходит за счет миграции моноцитов крови.

6. В процессе репаративной регенерации печени после частичной гепатэктомии происходит неполная активация перисинусоидальных клеток, которая заключается в пролиферации и усилении экспрессии десмина. Максимальное увеличение десмин-позитивных перисинусоидальных клеток происходит через 3 суток (26±4 клетки в поле зрения), однако, через 7 суток активация перисинусоидальных клеток не завершается (9,1±3,2 десмин-позитивные клетки в поле зрения).

7. Экспрессии C-kit в перисинусоидальных клетках и гепатоцитах начинается через сутки после частичной гепатэктомии, достигает пика в перисинусоидальных клетках через 3-5 суток, в гепатоцитах - через 3 суток, далее происходит снижение числа C-kit-позитивных клеток. Через 7 суток в печени присутствуют единичные C-kit-позитивные перисинусоидальные клетки и гепатоциты.

7. ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

Рекомендовать внедрение в практику. инфекционных и гастроэнтерологических отделений комплексную иммуногистохимическую оценку регенераторного процесса, включающую иммуногистохимическое окрашивание антителами к C-kit, при хронических гепатитах и циррозах печени для повышения эффективности диагностических мероприятий и оценки эффективности лечения.
