Остеартроз (ОА) - наиболее распространенная форма суставной патологии, которая приводит к значительным потерям в экономической, социальной и психологических сферах в связи с частым развитием временной и стойкой нетрудоспособности [1,21.

В последние годы произошли определенные изменения в понимании процессов, происходящих при ОА, ранее рассматриваемого как дегенеративное заболевание суставов. Современная ревматология рассматривает ОА как хроническое воспалительное заболевание с поражением всех тканей диартрозного сустава (суставного хряща, субхондральной кости, синовиальной оболочки, внутрисуставных связок, суставной капсулы и периартикулярных мышц) и является результатом действия механических и биологических процессов, дестабилизирующих нормальное равновесие процессов деградации и регенерации в хондроцитах суставного хряща, внеклеточном матриксе и субхондральной кости [3-5]. На сегодняшний день ОА - «патологический процесс, захватывающий все ткани сустава, т. е. сустав рассматривается как орган, реагирующий всеми структурами на возникший патологический процесс, хотя преимущественное поражение хряща - безусловная черта ОА» [6].

Высокая социальная значимость и распространенность ОА определяет актуальность поиска адекватных методов диагностики и способов эффективного лечения заболевания на более ранних стадиях.

Определенные надежды на возможность ранней патогенетической терапии ОА появились в последние десятилетия в связи с успехами в изучении его патогенеза. Исследования последних лет показали, что процесс деструкции суставного хряща при ОА сопровождается смещением динамического равновесия биосинтетической активности хондроцитов и процессов деградации суставного хряща, происходящие в результате действия экзогенных и эндогенных факторов. При этом все большее значение придается медиаторам воспаления, выявляемым при исследовании хряща, субхондральной кости, синовиальной оболочки и синовиальной жидкости интерлейкин-1, фактор некроза опухоли, оксид азота, простагландинов и др) и, по мнению авторов, приводящим к дальнейшей деградации суставного хряща [7-13]. Эти данные стали основой исследований в области фармакотерапии OA, направленных на установление доказательств эффективности различных препаратов, уточнение степени их безопасности и возможности проведения длительной терапии, уменьшения клинических проявлений, сохранения функциональной активности пациентов, замедления прогрессирования заболевания и улучшения прогноза OA [14].

Основу патогенетического действия могут составлять лекарственные препараты, представляющие собой естественные компоненты матрикса -глкжозамин сульфат и хондроитин сульфат, действие которых направлено на подавление процессов деструкции и восстановления структуры хряща. Эффективность и безопасность, положительное влияние на течение заболевания данных препаратов доказана в многочисленных экспериментальных исследованиях in vitro и in vivo, а также в клинических исследованиях [15-39]. Создание нового класса нестероидных противовоспалительных препаратов (НПВП), избирательно подавляющих провоспалительную циклооксигеназу-2 (ЦОГ-2), позволяет предположить, что препараты данной группы могут обладать определенными патогенетическими свойствами, подавляя синтез простагландинов в зоне воспаления. Кроме того, преимущественные ингибиторы ЦОГ-2, по данным экспериментальных исследований in vitro, обладают свойствами изменять метаболизм протеогликанов и коллагена за счет подавления синтеза простагландинов в зоне воспаления, уменьшения продукции токсических свободных радикалов, ингибирования металлопротеиназ матрикса, тормозят активацию фактора транскрипции NF-кВ, что служит основанием для того, чтобы рассматривать препараты данной группы как структурно-модифицирующие средства при OA [40-48]. Реальные возможности применения НПВП с целью изменения течения заболевания в экспериментальных исследованиях на животных in vivo не проводились, единичные клинические исследования посвящены изучению влияния НПВП на хрящ [49]. На сегодняшний день остается спорным вопрос о роли простых анальгетиков, главным образом ацетаминофена, в терапии OA, поскольку лишь единичные работы посвящены изучению его эффективности в условиях длительного наблюдения за больными [50,51].

Для точной диагностики процессов, происходящих в суставе, необходим комплексный подход, при котором тщательное клиническое обследование сочетается с инструментальными методами, среди которых в настоящее время применяют рентгенографию, ультразвуковую томографию (УЗТ), артроскопию и магнитно-резонансную томографию (МРТ), позволяющих оценить толщину суставного хряща и синовиальной оболочки, выявить наличие эрозий в хряще, определить характер и количество жидкости в различных отделах сустава.

Использование комплексного клинико-инструментального обследования позволяет оценить влияние фармакотерапии на патологический процесс при OA. К моменту начала нашего исследования не было проведено ни одной работы по изучению сравнительной эффективности ацетаминофена, глюкозамин сульфата, хондроитин сульфата и мелоксикама при длительном применении. В известных исследованиях не была использована комплексная методология оценки медленно действующих препаратов для лечения OA и применения данных препаратов с целью оценки структурных изменений в суставе. Ни в одном исследовании не применялись УЗТ, МРТ и артроскопия для оценки влияния исследуемых препаратов на суставной хрящ при остеоартрозе коленных суставов (ОАКС).

Цель работы: изучение влияния современной фармакотерапии остеоартроза (ацетаминофена, глюкозамин сульфата, хондроитин сульфата и мелоксикама) на течение болезни у 80 пациентов с остеоартрозом коленных суставов в условиях открытого рандомизированного 18-месячного проспективного исследования в параллельных группах на основании комплексного клинического и инструментального обследования.

Задачи исследования:

1. Изучить сравнительную клиническую эффективность и переносимость ацетаминофена (Ац), глюкозамин сульфата (ГС), хондроитин сульфата (ХС) и мелоксикама (М) у больных с ОАКС;

2. Установить особенности воздействия изучаемых препаратов на структуры коленного сустава с использованием рентгенографии, УЗТ, МРТ и артроскопии.

Материалы, изложенные в диссертации, являются результатом исследований, выполненных на базе ГУ института ревматологии РАМН (директор - чл.-корр. РАМН, профессор Насонов Е. Л.).

Научная новизна:

1. Впервые проведено сравнительное рандомизированное исследование эффективности и переносимости Ац, ГС, ХС и М у больных с ОАКС в условиях длительного открытого рандомизированного исследования;

2. Использование комплекса современных инструментальных методов (рентгенографии, УЗТ, МРТ и артроскопии) позволило визуализировать структурные изменения при ОАКС и объективизировать оценку фармакотерапии;

3. Впервые МРТ и артроскопический SFA-счет позволили дать количественную оценку хондропатии при ОАКС в динамике;

4. Установлено, что по противовоспалительной активности М, ГС и ХС сопоставимы между собой и при длительном применении способны замедлять прогрессирование ОАКС.

Практическая значимость:

1. Показана высокая клиническая эффективность и хорошая переносимость стандартных доз ГС, ХС, и М.

2. Использование стандартизованной рентгенографии, УЗТ, МРТ и артроскопии позволяет улучшить диагностику структурных повреждений при ОАКС.

Основные положения, выносимые на защиту:

1. ГС, ХС и М оказывают выраженное анальгетическое и противовоспалительное действие при ОАКС.

2. ГС, ХС и М в условиях длительного применения способны замедлять прогрессирование ОАКС.

3. Ац обладает худшей переносимостью в отношении гепатотоксичности, в сравнении с другими группами исследуемых препаратов, частота развития других нежелательных явлений не различается на фоне терапии разными препаратами.

4. Для оценки влияния препаратов на течение ОАКС целесообразно использовать комплекс клинических и инструментальных методик, позволяющих объективизировать полученные данные.

Внедрение в практику: Использованные современные методы лечения и диагностики ОАКС внедрены в практику ГУ ИР РАМН.

Публикации:

Результаты научных исследований отражены в 10 печатных работах.

Апробация работы: Материалы диссертации доложены на Невском радиологическом форуме «Из будущего в настоящее» (Санкт-Петербург, 2003), на конгрессе ревматологов (Саратов 2003), на ревматологической секции Московского общества терапевтов (Москва, 2005), на IV Съезде ревматологов России (Казань, 2005), на VI Европейском Конгрессе Ревматологов (Вена, 2005), на V Северо-западной конференции по ревматологии (Санкт-Петербург, 2005).

126 Выводы

1. В результате рандомизированного проспективного 18-месячного исследования 80 пациентов с ОАКС установлено, что глюкозамин сульфат (ГС), хондроитин сульфат (ХС) и мелоксикам (М) обладают симптоматическим эффектом и способностью замедлять прогрессирование ОАКС.

2. Анальгетическое действие подтверждено для всех исследуемых препаратов по индексу \ЛЮМАС по шкапе боли, боли при ходьбе без статистически значимых различий между группами (р=1,0 и р=0,64 соответственно).

3. Антивоспалительное действие ГС, ХС и М доказано положительной динамикой индекса \ЛГОМАС по шкапе скованность(р<0,001), боли в покое (р<0,001) и ночной боли (р<0,0001), противовоспалительный эффект ацетаминофена (Ац) не подтвержден (р>0,05).

4. При сравнении частоты и выраженности нежелательных явлений (НЯ) в анализируемых группах выявлена гепатотоксичность^при применении Ац (р=0,006), в отношении частоты других НЯ не отмечено статистически значимых различий между группами исследуемых препаратов (р>0,05).

5. Не отмечено рентгенологического прогрессирования заболевания в условиях весовой нагрузки на фоне терапии ГС, ХС и М (р=0,055, р=0,051 и р=0,55 соответственно). В группе пациентов, принимавших Ац, установлено статистически достоверное уменьшение ширины суставной щели в медиальном отделе коленного сустава (р=0,00007).

6. Положительное влияние ГС, ХС и М при ОАКС подтверждено УЗТ и проявляется достоверным уменьшением сонографических признаков синовита (р=0,005, р=0,005, р=0,002 соответственно) и стабилизацией толщины суставного хряща в медиальном отделе коленного сустава (р=0,63, р=0,06, р=0,43).

7. При оценке хондропатии по данным МРТ установлено, что наиболее часто изменения обнаруживаются в области надколенника (33,75%), а также в зоне медиального отдела бедренной кости (18,75%), медиального отдела большеберцовой кости (15%), в латеральном отделе бедренной (12,5%) и большеберцовой кости (7,5%).

8. Во всех анализируемых зонах коленного сустава (по данным МРТ) на фоне приема ГС, ХС и М отмечена стабилизация хондропатии и достоверное уменьшение признаков синовита (р=0,03, р=0,01, р=0,03 соответственно).

9. Положительное влияние ГС, ХС и М на суставной хрящ подтверждено данными артроскопии при анализе количественной оценки хондропатии (SFA-счет). Через 18 месяцев терапии отмечена стабилизация SFA-счета в данных группах (р=0,59, р=0,08, р=0,29 соответственно), в группе Ац положительной динамики не установлено (р=0,04).

10. Использование комплекса современных инструментальных методов (рентгенографии, УЗТ, МРТ и артроскопии) позволяет визуализировать структурные изменения при ОАКС и объективизировать оценку фармакотерапии.

Практические рекомендации:

1. Лечение глюкозамин сульфатом, хондроитин сульфатом и мелоксикамом должно проводиться длительно с момента установления диагноза (в том числе на ранних стадиях заболевания).

2. Ацетаминофен может быть рекомендован в качестве симптоматической терапии у больных ОА со слабым и умеренно выраженным болевым синдромом при отсутствии признаков воспаления в суставе.

3. Терапия ацетаминофеном должна проводиться при тщательном мониторинге содержания трансаминаз в сыворотке крови каждые 2-3 месяца.

4. Артросонографическое исследование рекомендуется проводить всем пациентам с ОАКС для уточнения характера, степени выраженности и локализации поражения суставного хряща и воспаления.

5. С целью контроля эффективности проводимой терапии больных ОАКС, помимо рентгенографии, целесообразно использовать МРТ и артроскопию.
