Актуальность проблемы

Сахарный диабет 2 типа (СД 2) характеризуется большой распространенностью сосудистой мозговой патологии, являющейся морфологическим субстратом когнитивных расстройств. Наиболее частыми клиническими формами цереброваскулярной патологии являются хронические расстройства мозгового кровообращения - хроническая ишемия мозга (ХИМ) [11]. К моменту выявления СД 2 типа более 50-75% больных уже имеют артериальную гипертонию (АГ) и дислипидемию, что вносит вклад в развитие и прогрессирование цереброваскулярной патологии. Риск развития хронической ишемии мозга возрастает в 2-6 раз при сочетании СД 2 типа и АГ. Это обусловлено тем, что для СД 2 типа характерно наличие дополнительных специфических факторов риска ХИМ [85, 281]. До настоящего времени проблема хронической цереброваску-ларной патологии у больных сахарным диабетом 2 типа изучена недостаточно, отсутствует стратегия по максимально раннему выявлению ХИМ и своевременному лечению, не изучены ранние маркеры развития данной патологии при СД 2 типа с учетом современных представлений о ключевой роли дисфункции эндотелия и нарушений в системе гемостаза в формировании сосудистой мозговой патологии [64]. Актуальность изучения ХИМ у больных СД 2 типа определяется не только высокой распространенностью ХИМ при данном заболевании, но и высоким риском острых сосудистых мозговых катастроф и тесной ассоциацией с развитием тяжелых когнитивных расстройств [180]. Ряд авторов предлагают внести когнитивные расстройства в перечень осложнений сахарного диабета 2 типа как частое и значимое осложнение [93]. В клинической практике отсутствуют рекомендации по использованию доступных и информативных методов нейропсихологического тестирования для выявления ранних стадий сосудистых когнитивных расстройств у больных с сахарным диабетом 2 типа, тогда как смертность среди группы больных с легкими когнитивными расстройствами сосудистого генеза в 2,4 раза выше, чем у лиц соответствующей возрастной группы без таких нарушений [234]. Принимая во внимание отсутствие очагового неврологического дефицита на начальных стадиях цереб-роваскулярной патологии, применение МРТ головного мозга, нейропсихологи-ческого тестирования и изучение сосудистых факторов риска представляется необходимым для выявления ранних и потенциально курабельных форм когнитивных расстройств у больных сахарным диабетом 2 типа с ХИМ.

Цель исследования: изучить клинико-диагностические особенности ранних когнитивных расстройств у больных СД 2 типа с хронической ишемией головного мозга, их ассоциацию с факторами сердечно-сосудистого риска.

Задачи исследования:

1. Изучить особенности ХИМ и когнитивных расстройств у больных с СД 2 типа по данным МРТ головного мозга и нейропсихологического тестирования.

2.0ценить вклад в развитие ХИМ и когнитивных расстройств у больных СД 2 типа маркеров дисфункции эндотелия.

3.Изучить вклад нарушений системы гемостаза в развитие ХИМ и когнитивных расстройств.

4,Оценить связь диабетических микро- и макрососудистых осложнений с развитием ХИМ и когнитивных расстройств.

5.Изучить диагностическую роль нейронспецифических и глиальных белков у больных СД 2 типа с ХИМ и когнитивными расстройствами.

6.Оценить эффективность терапии гликозаминогликанами у больных СД 2 типа с ХИМ, когнитивными расстройствами и диабетической нефропати-ей на стадии микроальбуминурии.

Научная новизна.

Впервые в Российской Федерации на основе МРТ визуализации проведено изучение особенностей и частоты встречаемости ХИМ у больных СД 2 типа без очагового неврологического дефицита. Установлено, что бессимптомная (без очаговых неврологических изменений) ХИМ выявляется у подавляющего большинства больных СД 2 типа (96,8%). В то же время ранние когнитивные расстройства выявляются у 81% этих пациентов. Впервые на основе валидиро-ванных нейропсихологических тестов представлена структура ранних когнитивных расстройств у больных СД 2 типа с ХИМ. На основе изучения широкого спектра сердечно-сосудистых факторов риска определены механизмы снижения когнитивных функций у больных СД 2 типа с ХИМ. Впервые показано, что препарат из группы гликозаминогликанов - сулодексид наряду с позитивным влиянием на течение диабетической нефропатии на стадии микроальбуминурии у больных СД 2 типа с ХИМ, улучшает когнитивные функции. Эти эффекты опосредованы благоприятным воздействием на эндотелий, профиб-ринолитическим и антитромботическим действием препарата.

Практическая значимость.

Полученные в работе данные позволяют расширить представление о когнитивных расстройствах у больных СД 2 типа с ХИМ. Показано, что пациенты с СД 2 типа являются группой риска развития ХИМ и ранних когнитивных расстройств. Данные проведенного исследования позволяют рекомендовать применение нейровизуализации и простых и доступных нейропсихологичеких тестов для раннего выявления ХИМ и конитивных расстройств.

Изученные потенциальные факторы риска развития ХИМ и когнитивных расстройств у больных СД 2 типа являются основанием для назначения базисной многокомпонентной терапии, направленной на различные звенья патогенеза.

Полученные данные об эффективности и безопасности присоединения гликозаминогликанов (сулодексид) к базисной терапии больных СД 2 типа с диабетической нефропатией на стадии микроальбуминурии и ХИМ с когнитивными расстройствами позволяют рекомендовать лечение данным препаратом с целью много факторного воздействия на несколько сосудистых регионов.

Апробация работы.

Основные положения диссертации доложены на межотделенческой конференции ФГУ ЭНЦ Росмедтехнологий 22.10.2008г. Результаты работы были представлены в виде докладов на: 43 конгрессе Европейской ассоциации по изучению диабета (EASD, Амстердам, Нидерланды, 2007г, постерный доклад), 44 конгрессе Европейской ассоциации по изучению диабета (EASD, Рим, Италия, 2008г, постерный доклад), на IV всероссийском диабетологическом конгрессе (19-21 мая 2008, Москва, устный доклад). По теме диссертации опубликовано 10 работ, из них 3 - в зарубежной печати.

выводы

1. По данным МРТ головного мозга у 96,8% больных с СД 2 типа выявлены структурные изменения, соответствующие хронической ишемии мозга: лей-коареоз в 63,2%, локальные изменения в 64,8%, расширение периваскулярных пространств в 95,1% случаев. У 40,9% больных СД 2 типа с хронической ишемией мозга представлены одновременно все описанные структурные изменения.

2. Снижение когнитивных функций у больных СД 2 типа с хронической ишемией мозга на основании комплекса нейропсихологических тестов выявлено в 81% случаев: у 55% больных когнитивное расстройство соответствовало критериям легких когнитивных расстройств, у 26% выявлены начальные когнитивные расстройства, и у 19% когнитивных расстройств не выявлено.

3. Легкие когнитивные расстройства при СД 2 типа характеризуются наибольшей выраженностью локальных ишемических изменений и величиной индекса третьего желудочка мозга. Для легких когнитивных расстройств характерна: г большая частота атеросклероза сонных артерий, выраженность дисфункции эндотелия и нарушений в системе гемостаза.

4. Проведенный корреляционный анализ свидетельствует о патогенетической роли артериальной гипертонии, дисфункции эндотелия и нарушений в системе гемостаза, инсулинорезистентности, микро- и макрососудистых осложнений СД 2 типа в развитии когнитивных расстройств. В меньшей степени в формирование когнитивных расстройств при СД 2 типа вносит вклад фактор старения.

5. Не установлено достоверной связи уровня глиальных и нейрональных белков с наличием и выраженностью хронической ишемии мозга и ранних когнитивных расстройств у больных СД 2 типа.

6. Присоединение гликозаминогликанов (сулодексид) к базисной терапии больным СД 2 типа с ХИМ, диабетической нефропатией на стадии микроальбуминурии и когнитивными расстройствами, приводит к достоверному улучшению когнитивных функций и снижению экскреции альбумина с мочой.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

1. Современные методы нейровнзуализации (МРТ) позволяют прижизненно оценить состояние ткани головного мозга с последующей количественной оценкой, их следует использовать для диагностики ХИМ у больных СД 2 типа.

2. Полученные результаты доказывают необходимость ранней диагностики когнитивных расстройств у больных СД 2 типа. Нейропсихологические скрининговые тесты просты в исполнении и могут быть рекомендованы для выявления ранних когнитивных расстройств в практике диабетолога.

3. При наличии микроальбуминурии у больных СД 2 типа с ХИМ присоединение сулодексида в дозе 200 мг в сутки в течение 6 месяцев к базовой терапии оказывает позитивное действие на когнитивные функции и экскрецию альбумина с мочой.
