ВВЕДЕНИЕ
Актуальность и степень разработанности темы исследования
Ревматоидный артрит (РА) - самое частое аутоиммунное ревматическое заболевание, характеризующееся хроническим эрозивным артритом и системным поражением внутренних органов, приводящее к ранней инвалидизации и сокращению продолжительности жизни пациентов [1, 2]. Согласно данным исследования «Глобальное бремя болезней 2017», распространенность РА колеблется от 0,1% в Юго-Восточной Азии до 0,38% в Северной Америке [3]. По данным мета-анализа, включавшем 67 когортных исследований, проведенных в 41 стране за период с 1986 по 2014 гг., общая распространенность РА в популяции достигает 0,46% [4]. В нашей стране число больных РА составляет 610 на 100 тыс. населения [5].
Ведение пациентов с РА основано на применении патогенетической терапии, направленной на контроль системного воспаления и ограничение прогрессирова-ния деструктивных изменений в суставах. В клинической практике стандартом лечения РА является стратегия T2T («treat-to-target»), предполагающая раннее назначение базисных противовоспалительных препаратов (БПВП) с целью достижения ремиссии или низкой активности заболевания (НАЗ) [6]. Расширение терапевтических возможностей за счет внедрения генно-инженерных биологических препаратов (ГИБП), включая ингибиторы фактора некроза опухоли-а (иФНОа), блокатор костимуляции Т-лимфоцитов - абатацепт (АБЦ), анти-CD20-препарат - ритуксимаб (РТМ), ингибиторы интерлейкина-6 и его мембранных и растворимых рецепторов (иИЛ-6), а также таргетных синтетических базисных противовоспалительных препаратов (тсБПВП), представленных ингибиторами янус-киназ (иЯК) - тофацитинибом, упа-дацитинибом и барицитинибом, позволило улучшить прогноз течения заболевания [7, 8].
Однако, несмотря на появление широкого спектра ГИБП и иЯК, цели лечения РА - достижения ремиссии/НАЗ - удается достичь лишь примерно у 50-60% пациентов. По данным К. Lauper и соавт. [9], которые провели анализ 19 регистров больных РА, получавших ГИБП и иЯК (всего 31 846 курсов), ремиссия/НАЗ по CDAI (Clinical Disease Activity Index) при использовании иФНОа зарегистрирована у 16%
и 54% больных, АБЦ - у 12% и 50%, иИЛ-6 - у 16% и 55%, иЯК - у 15% и 56%. Согласно результатам мета-анализа Y. H. Lee и соавт. [10], которые выполнили сравнение эффективности и безопасности терапии иИЛ-6 (олокизумаб [11], тоци-лизумаб [12], сарилумаб [13]), снижение активности РА на уровне ACR50 (American College of Rheumatology) варьирует от 43,9% до 50,0%. В мета-анализе 8 исследований (n = 2720) была проведена оценка эффективности ритуксимаба при РА. Согласно полученным данным, лишь 29% пациентов достигли ACR50, а 22% -ремиссии по DAS28 (Disease Activity Score) [14]. Достижение цели терапии РА на фоне терапии иЯК демонстрирует мета-анализ Y. Lee и соавт. [15] среди пациентов, получавших тофацитиниб, упадацитиниб и барицитиниб, ремиссии по DAS28-СРБ достигли 30%, 38%, 40% пациентов, по CDAI - 19%, 25%, 22% пациентов соответственно.
Недостаточный эффект терапии или развитие серьезных нежелательных реакций (НР) при использовании ГИБП или иЯК требуют «переключения» на другой вид терапии [16-18]. В зависимости от выбранной стратегии переключение может осуществляться путем назначения другого препарата в рамках одной фармакологической группы, в частности иФНОа (стратегия cycling), либо посредством перехода на препарат с иным механизмом действия (стратегия swapping) [8, 19, 20]. Несмотря на то, что ранее предпринимались попытки разработки алгоритмов для рациональной смены ГИБП/иЯК при их недостаточной эффективности или непереносимости [21], рекомендации Ассоциации ревматологов России, EULAR и ACR не указывают на предпочтительное использование какой-либо стратегии. Предполагается, что в каждом конкретном случае решение врача о переключении терапии должно основываться на оценке анамнеза, особенностей течения РА и коморбид-ной патологии [19, 20].
Вместе с тем, переключение на другие ГИБП/иЯК не всегда обеспечивает терапевтический успех [20, 22-25]. Так, в исследовании R. van Vollenhoven и соавт. было показано, что назначение УПА при неэффективности или непереносимости ГИБП позволило достичь ремиссии/НАЗ у 45% и 79% пациентов, соответственно,
[26]. Однако лишь 25% и 43% пациентов сохранили первоначальный хороший ответ на лечение после 6 месяцев наблюдения. В исследовании R. СарогаН и соавт. [25] оценивалась эффективность ГИБП/иЯК, назначенных 503 пациентам с неудачей при первом назначении иФНОа. Переключение на другой иФНОа позволило добиться ремиссии у 40,3%, иЯК - у 67,7% больных.
Неудачное переключение терапии приводит к повторным сменам ГИБП/иЯК. По данным британского регистра, включавшего 22934 пациента с РА, после первого назначения ГИБП или иЯК, у 47,2% больных пришлось провести одно, у 22,0% - два, у 9,3% - три, у 3,3% - четыре и более переключений этих препаратов [17]. Отражением данной проблемы стало выделение особой категории больных с D2T (difficult-to-tгeat) - трудно поддающийся лечению РА, у которых выбор терапии вызывает наибольшие затруднения [27-36]. Согласно критериям EULAR, трудно поддающийся лечению РА определяется при неэффективности двух и более последовательно назначенных ГИБП/иЯК с разным механизмом действия; сохранении активности заболевания и/или симптомов, снижающих качество жизни; если лечение считается проблематичным с точки зрения врача и/или пациента [27]. По разным данным, эта категория составляет от 5 до 20% всех пациентов с РА [27, 28, 32, 37-39].
Выявление факторов, ассоциированных с недостаточной эффективностью терапии при переключении ГИБП и иЯК, имеет принципиальное значение для предупреждения формирования РА, трудно поддающегося лечению. Предикторы неэффективного переключения ГИБП и иЯК традиционно подразделяются на факторы, связанные с сохраняющейся воспалительной активностью заболевания, включая синтез противолекарственных антител (ПЛА) [40], исходно высокую активность РА, длительность применения глюкокортикоидов [35], возраст дебюта болезни, патотипы синовита [41, 42], иммуногенетические особенности [43-46]), и не связанные напрямую с аутоиммунным воспалением. Среди последних важное значение имеет хроническая «невоспалительная» боль [36, 47] (фибромиалгия, вторичный остеоартрит, последствия травм), проблемы с переносимостью препарата и развитием НР, коморбидность (ожирение, патология легких, инфекции, перио-
донтит, заболевания сердечно-сосудистой системы и др.), социально-экономические проблемы (недоступность дорогостоящих препаратов, некомплаентность больных) и др. [29, 46, 47]. Большое значение также придается иммуногенетиче-ским особенностям, определяющим рефрактерность к терапии, в частности мутации генов (различные однонуклеотидные полиморфизмы, SNP) и влиянию эпигенетических факторов. Было показано, что ряд SNP связан с более низким клиническим ответом на ГИБП [48-50] и повышенным восприятием боли пациентами [5153]. В свою очередь, эпигенетические механизмы (метилирование ДНК, посттранскрипционные и посттрансляционные изменения) могут влиять на синтез цитоки-нов и их клеточных рецепторов, что определяет эффективность терапии [54].
Областью серьезного научного интереса является изучение хемокинового профиля пациентов. Хемокины - семейство белков, индуцирующих направленное движение и активацию лейкоцитов, а также других иммунных клеток, включая эн-дотелиальные и эпителиальные. Выделяют четыре основных подсемейства хемоки-нов - СХС, СС, СХзС и С [55-60]. Хемокины проявляют свои биологические эффекты, взаимодействуя с трансмембранными рецепторами, связанными с G-бел-ком, расположенными на поверхности клеток-мишеней [46]. Хемокины играют важную роль в активации иммунных реакций, а также участвуют в патогенезе многих заболеваний в т. ч. РА [61]. Установлено, что уровень хемокинов (например, СХСЬ13) положительно коррелирует с показателями активности РА. Ряд этих субстанций (ССЬ2, ССЬ3, ССЬ4 и др.) может быть использован в качестве прогностических инструментов при проведении терапии ГИБП/иЯК [59].
Таким образом, выделение клинических, генетических и иммунологических (включая хемокины) предикторов недостаточного ответа на терапию при переключении ГИБП и иЯК может позволить прогнозировать результаты лечения РА. Это обеспечит возможность персонализации терапии, повысит ее эффективность и снизит риск формирования когорты пациентов, трудно поддающихся лечению. Однако, до настоящего времени в нашей стране не проводилось комплексных исследований в этом направлении.
Цель исследования
Изучить клинические, иммунологические и молекулярно-генетические особенности пациентов с РА с недостаточной эффективностью генно-инженерных биологических препаратов и ингибиторов янус-киназ (ГИБП/иЯК) при их переключении.
Задачи исследования
1. Представить клиническую характеристику пациентов с РА, нуждающихся в переключении терапии ГИБП/иЯК на другие ГИБП/иЯК.
2. Определить частоту достижения ремиссии и низкой активности заболевания (НАЗ) по стандартным индексам активности РА (DAS28, SDAI, CDAI) и динамику параметров, оцениваемых самим пациентом через 6 месяцев после переключения ГИБП/иЯК.
3. Провести сравнительное изучение полиморфизмов генов, ответственных за иммунный ответ и восприятие боли, у пациентов с РА, достигших и недостигших ремиссии и НАЗ при переключении на другой ГИБП/иЯК.
4. Сравнить исходный цитокиновый и хемокиновый профиль у пациентов с РА после переключения ГИБП/иЯК и здоровых доноров.
5. Оценить факторы, влияющие на эффективность терапии при переключении ГИБП/иЯК и разработать прогностическую модель недостаточного клинического ответа.
Научная новизна
Впервые в России представлена клиническая характеристика пациентов с РА, нуждающихся в переключении терапии ГИБП/иЯК из-за неэффективности предшествующей терапии. В ходе динамического наблюдения определена частота достижения ремиссии и НАЗ по DAS28-СРБ при переключении ГИБП/иЯК, представлена динамика параметров, оцениваемых самим пациентом. Впервые проведено сравнительное изучение полиморфизмов генов, ответственных за иммунный ответ и восприятие боли, а также цитокинового и хемокинового профиля у пациентов с РА, достигших и не достигших ремиссии и НАЗ после переключения ГИБП/иЯК.
Впервые в российской популяции пациентов с РА выявлены клинико-лаборатор-ные предикторы неэффективности терапии при переключении ГИБП/иЯК.
Практическая значимость работы
В работе охарактеризованы клинико-лабораторные особенности больных РА, нуждающихся в переключении ГИБП и/или иЯК. Выявлены предикторы неэффективности терапии при переключении ГИБП/иЯК, что позволяет на этапе принятия клинического решения выделять пациентов с повышенным риском отсутствия ответа на терапию. На основании полученных данных построена прогностическая модель клинического ответа при переключении терапии ГИБП и иЯК у пациентов с РА, ранее не ответивших на данные классы препаратов. Использование модели способствует персонализированному подходу к выбору последующей терапии и оптимизации стратегии лечения данной категории пациентов.
Материалы и методы исследования
Диссертационная работа выполнена на основе проспективного исследования, включавшего анализ результатов терапии 180 пациентов с достоверно установленным диагнозом РА и наличием неэффективности ранее назначенной ГИБП/иЯК. В данной выборке была определена частота основных клинических показателей, отражающих активность РА, особенности течения заболевания и параметры, оцениваемые самим пациентом (ПОСП). Через 6 месяцев после переключения ГИБП/иЯК на другие препараты этих групп была произведена оценка эффективности терапии с помощью стандартных индексов DAS28, SDAI, CDAI, а также ПОСП. Согласно полученным результатам терапии (достижение ремиссии или НАЗ), пациенты были разделены на 2 группы: пациенты группы 1 достигли ремиссии и НАЗ; пациенты группы 2 не достигли целей терапии. В этих группах проводился сравнительный анализ исходных клинических, иммунологических и генетических параметров с целью поиска факторов, связанных с неэффективностью переключения терапии.
Основные положения, выносимые на защиту
1. Пациенты с РА, нуждающихся в переключении терапии ГИБП/иЯК, характеризуются умеренной или высокой активностью заболевания, большой длительностью РА, выраженной болью и потребностью в приеме ГК.
2. По результатам исследования установлено, что через 6 месяцев после переключения ГИБП/иЯК 49,4% пациентов не достигают ремиссии/низкой активности РА (DAS28-СРБ < 3,2), из которых 85% не удовлетворены своим состоянием, а 60,5% пациентов сохраняют потребность в приеме ГК. Пациенты, не достигшие ремиссии/низкой активности РА после переключения ГИБП/иЯК характеризуются исходно более высоким ИМТ, высокой активностью РА по индексам DAS28, SDAI, CDAI, более продолжительной утренней скованностью, выраженной болью и утомляемостью, большей потребностью в ГК; чаще имеют симптомы нейропатической боли и фибромиалгии, признаки центральной сенситизации, тревоги и депрессии, большее число переключений ГИБП/иЯК в анамнезе.
3. Установлено, что наличие мутантного аллеля полиморфизмов TNFSF13B (BAFF, rs9514828) и KCNS1 (rs734784) ассоциировано с повышением эффективности терапии после переключения ГИБП/иЯК.
4. Недостаточный ответ при переключении ГИБП/иЯК ассоциируется с более низким сывороточным уровнем ряда провоспалительных цитокинов (ИЛ-lß, ИЛ-2), противовоспалительных цитокинов (ИЛ-10) и хемокинов, ассоциирующихся с активностью моноцитов (CCL3, CCL7, CCL13), Т-лимфоцитов (CCL22, CCL25, фракталкина (CX3CL1)), нейтрофилов (CXCL1, CXCL6), В-лимфоцитов (CXCL12), эозинофилов (CCL11, CCL26).
5. Разработанная прогностическая модель демонстрирует, что вероятность недостаточного эффекта терапии после переключения ГИБП/иЯК у пациентов с РА связана с более высоким индексом массы тела, числом припухших суставов, количеством применяемых ГИБП/иЯК в анамнезе, наличием признаков нейропатиче-ской боли по опроснику PainDetect и приемом глюкокортикоидов. В то же время серопозитивность по РФ и более высокие показатели качества жизни по опроснику EQ-5D ассоциируются со снижением риска неэффективности терапии.
Апробация и представление результатов исследования
Основные положения и результаты диссертационного исследования были доложены и обсуждены на российских и международных научных форумах. Материалы работы представлены на III Всероссийской научно-практической конференции «Скелетно-мышечная боль при ревматических заболеваниях» (Москва, 2023 г.), а также на Ежегодной научно-практической конференции ФГБНУ НИИР им. В. А. Насоновой (Москва, 2023 г.).
Результаты исследования были представлены на XXIII Всероссийской школе ревматологов имени академика В. А. Насоновой «Вопросы рефрактерности в ревматологии» (Москва, 2024 г.) и на VII Форуме «Мультидисциплинарный больной», посвященном 125-летию со дня рождения П. Е. Лукомского (Екатеринбург, 2024 г.).
Материалы диссертационной работы также были представлены на международных научных конгрессах WCO IOF-ESCEO (2023, 2024, 2025 гг.), APLAR (Сингапур, 2024 г.; Япония, Фукуока, 2025 г.) и CORA (Италия, Венеция, 2025 г.).
Кроме того, результаты исследования были доложены на XXIV Всероссийской школе ревматологов имени академика В. А. Насоновой «Прогресс в ревматологии: смена парадигм» (Москва, 2025 г.) и на XXV юбилейном Всероссийском конгрессе с международным участием «Дни ревматологии в Санкт-Петербурге 2025» (Санкт-Петербург, 2025 г.).
Внедрение результатов исследования
Результаты, полученные в ходе выполнения диссертационного исследования, внедрены в практическую деятельность ФГБНУ НИИР им. В. А. Насоновой (директор - член-корреспондент Российской академии наук, профессор, доктор медицинских наук А. М. Лила). Эмпирические данные и выводы работы были использованы при выполнении фундаментальной научной темы «Разработка персонализированной программы лечения рефрактерного ревматоидного артрита на основе изучения молекулярно-генетических и молекулярно-биологических предикторов» (№ государственной регистрации 1021051503137-7 РК 122040400051-3). Материалы диссертации интегрированы в образовательный процесс и применяются при
обучении врачей и ординаторов в рамках лекционных курсов и практических занятий, проводимых на базе ФГБНУ НИИР им. В. А. Насоновой.
Публикационная активность по результатам исследования Материалы и результаты, полученные в ходе выполнения диссертационной работы, нашли отражение в четырех научных публикациях, включая одну обзорную статью и три оригинальных исследования, основанных на собственных данных автора. Указанные работы опубликованы в рецензируемых научных журналах, входящих в перечень изданий, рекомендованных Высшей аттестационной комиссией Российской Федерации для апробации и опубликования результатов диссертационных исследований. Помимо этого, результаты диссертации были доложены и опубликованы в виде 15 тезисов в материалах отечественных и зарубежных научных форумов, включая конференции, съезды и конгрессы. Объем и структура диссертационной работы
Диссертационная работа изложена на 192 страницах. В структуру исследования входят введение, четыре главы, включающие анализ современного состояния проблемы, описание материалов и методов, изложение результатов собственного исследования и их обсуждение, а также выводы и практические рекомендации. Работа завершается списком использованной литературы и приложениями. Библиографический список включает 370 источников, из которых 32 являются отечественными и 338 - зарубежными. Приложения содержат 11 справочных и методических материалов, относящихся к разделу «Материалы и методы» и включающих использованные в исследовании опросники. Иллюстративный материал представлен 22 рисунками и 25 таблицами.
Вклад автора в выполнение диссертационного исследования В ходе выполнения диссертационного исследования осуществлен анализ отечественных и зарубежных публикаций по изучаемой проблеме, по результатам которого подготовлен и опубликован обзор современного состояния вопроса. Совместно с научным руководителем определены цель и задачи исследования, сформулированы критерии включения и невключения пациентов, а также разработана и
обоснована методология работы. В рамках исследования создана индивидуальная регистрационная карта пациента.
Проведено клиническое обследование больных, осуществлен сбор анамнестических сведений и анализ медицинской документации. Сформирован биобанк образцов цельной крови и сыворотки пациентов, включенных в исследование. В течение всего периода наблюдения обеспечивались ведение и динамическая кура-ция пациентов.
В рамках работы освоены и применены методы полимеразной цепной реакции в режиме реального времени, а также методы количественного определения уровней цитокинов и хемокинов. Полученные данные систематизированы и внесены в электронную базу данных, разработанную в процессе исследования, после чего выполнена их статистическая обработка. Завершающим этапом стал анализ и интерпретация результатов с сопоставлением с данными международных исследований, на основании чего сформулированы научные положения, выводы и практические рекомендации.
Глава 1. ОБЗОР ЛИТЕРАТУРНЫХ ДАННЫХ 1.1. Проблема эффективности ГИБП и иЯК у больных РА
Ревматоидный артрит (РА) - хроническое аутоиммунное ревматическое заболевание, характеризующееся персистирующим синовитом, системными проявлениями, наличием специфических иммунных феноменов - РФ и АЦЦП [1,2]. По оценкам исследования Глобальное бремя болезней 2021, число больных РА в 2020 году составило 17,6 миллионов человек, стандартизованная по возрасту распространенность заболевания - 208,8 случаев на 100 000 населения [62]. Число больных РА с 1990 г. по 2020 г. увеличилось на 14,1% [63]. В нашей стране распространенность РА составляет 0,61% [5].
Недостаточно эффективная терапия РА приводит к неуклонному прогрессиро-ванию заболевания, инвалидизации больных и ранней смерти. Как было показано в исследованиях, посвященных изучению раннего РА, через 2-3 года после начала заболевания стойкая утрата трудоспособности отмечается у 20-30% больных [64,65]. Имеется прямая взаимосвязь между нарушением функции и смертностью при РА: ее двукратное увеличение у пациентов с умеренным нарушением функции (HAQ от 1 до 2 баллов) и в 4-5 раз у пациентов с тяжелым нарушением функции (HAQ от 2 до 3 баллов) [64, 66]. По результатам мета-анализа 17 популяционных работ, в ходе наблюдения за 486098 пациентами с РА летальный исход отмечался у 63988 (13,2%), что было в 1,5 раза выше в сравнении с общей популяцией [67]. Наиболее частыми причинами смерти при РА являются сердечно-сосудистые заболевания (у пациентов с РА на 50% выше риск смертности от кардиоваскулярных и цереброваскулярных катастроф, чем в популяции), заболевания органов дыхания (в особенности интерстици-альное поражение легких) и инфекций [67-70].
Данные об успехах фармакотерапии РА несколько противоречивы. Один из важнейших выводов, к которому пришли эксперты при анализе крупнейших когорт больных с РА на протяжении нескольких десятилетий, заключается в том, что, несмотря на весь арсенал фармакотерапии, в целом достижение ремиссии по индексу
DAS28 не изменилось c 2010 года [71-73]. В то же время по результатам мета-анализа, проведенного L. Carpenter и соавт., за последние 20 лет удалось улучшить результаты лечения пациентов согласно индексам активности РА и показателям качества жизни, однако показатели уровня боли, усталости, инвалидизации не имели положительных изменений [74]. Аналогично в исследовании Т. Fujii и соавт., по данным когорты KURAMA в результате 10-летнего наблюдения (с 2012 по 2021 гг.) наблюдалось улучшение качества жизни больных и исходов лечения РА по CDAI преимущественно за счет имплементации ГИБП/иЯК [71]. Например, по данным японского когортного исследования IORRA, показатель 5-летней выживаемости при РА не менялся на протяжении последних 17 лет [75].
В настоящее время общепризнанной стратегией ведения больных РА является Treat-to-target (Т2Т) - «лечение до цели» [6, 19]. Эта концепция включает в себя такие компоненты, как выбор принципиальной цели лечения и метода оценки ее достижения; выбор времени оценки; готовность изменить лечение, если цель не достигнута [76, 77]. В качестве основной цели лечения принято достижение ремиссии и НАЗ, что зафиксировано в международных и российских клинических рекомендациях [19, 77, 78]. Для определения ремиссии и НАЗ при РА настоятельно рекомендуется использовать стандартные индексы активности, такие как DAS28 [79], SDAI (Simplified Disease Activity Index) или CDAI (Clinical Disease Activity Index) [80,81]. Для пациентов с ранним РА рекомендуется проводить оценку индексов активности каждые 3 месяца, для пациентов с развернутой стадией РА - не реже одного раза в 6 месяцев [19]. Данная рекомендация основана на принципе «плотного контроля» (tight control), который позволяет оптимально использовать окно возможностей для минимизации риска необратимого повреждения суставов [2]. Особенно важно после оценки эффективности терапии совместно с пациентом принять решение о продолжении или изменении терапии [77].
Для достижения указанных целей используют БПВП, среди которых выделяют синтетические БПВП (сБПВП) такие как метотрексат (МТ), лефлуномид, сульфасалазин, гидроксихлорохин, а также биологические БПВП (ГИБП) и таргет-ные синтетические БПВП (тсБПВП, или иЯК) [19, 78, 82] (Рисунок 1).

Однако даже применение ГИБП и иЯК далеко не всегда позволяет добиться заявленной цели лечения РА. В крупном систематическом обзоре C. Hughes и со-авт. представлен анализ частоты достижения ремиссии на основе данных 48 исследований по эффективности комбинированной терапии БПВП, ГИБП, иЯК у паци-
ентов с РА: из 11259 пациентов, получавших различные виды противоревматической терапии лишь 3013 (26,8%) достигли ремиссии [83]. В другом обзоре Н. West-erlind и соавт., включавшем 8 регистров Швеции, Дании и Норвегии, ремиссии достигли от 32% до 75% пациентов по DAS28, от 12% до 58% по SDAI через 6 месяцев терапии иФНОа [84]. Согласно систематическому обзору Cochrane 22% больных, получавших РТМ, достигают ремиссии по DAS28 в течение одного года наблюдения [14]. При назначении иЯК ремиссия РА по DAS28-CPE достигалась в 38%, 40%, 30% для тофацитиниба [85], барицитиниба [86], упадацитиниба [87] соответственно. В крупных систематических обзорах приведены данные по эффективности иФНОа при лечении пациентов с РА. Так, значение ACR50 было достигнуто у 32% больных РА при назначении инфликсимаба (ИНФ) [88], у 48% этанерцепта (ЭТЦ) [89], у 41,5% - адалимумаба (АДА) [90], у 38% - голимумаба (ГОЛ) [91], у 36% -цертолизумаба пэгол (ЦЗП) [92]. При изучении эффективности терапии иИЛ-6 на 24 неделе установлено, что назначение тоцилизумаба (ТЦЗ), сарилумаба (САР), левилимаба (ЛЕВ), олокизумаба (ОКЗ) позволило достичь значений ACR50 у 44% [12], 45,7% [93], 47,1% [94] и 50,1% [11] пациентов соответственно. В крупных РКИ было показано, что применение АБЦ позволяло достичь ACR50 у 41,7% пациентов [95], а РТМ - у 34% больных РА [96]. Достижение ACR50 наблюдалось в 46%, 51% и 54% случаев при использовании тофацитиниба [97], барицитиниба [86], упадаци-тиниба [87] соответственно.
