Атеросклероз является многофакторным заболеванием. Его развитие начинается с увеличения проницаемости эндотелия, которое может возникать вследствие механического повреждения турбулентными потоками крови, нарушений метаболизма, воздействия инфекций, иммунных комплексов и химических препаратов.

Гиперлипидемия является общепризнанным фактором риска атеросклероза. Накопление и окисление ЛНП в стенке сосудов стимулирует экспрессию молекул адгезии на эндотелии. Это способствует связыванию моноцитов, их проникновению в стенку сосудов и превращению в макрофаги, которые усиливают окисление ЛНП, поглощают окисленные ЛНП и превращаются в пенистые клетки. После насыщения липидами пенистые клетки разрушаются с освобождением окисленного холестерина и его эфиров, которые вызывают дальнейшее повреждение эндотелия. Макрофаги секретируют факторы, стимулирующие пролиферацию гладкомышечных клеток и синтез белков внеклеточного матрикса, а также тканевой фактор - основной активатор свертывания крови. Вследствие этих процессов образуется атеросклеротическая бляшка с ядром, содержащим липиды, которое заключено в фиброзную капсулу [33,181]. Результаты больших плацебо-контролируемых исследований показали, что коррекция гиперлипидемии приводит к быстрому снижению частоты клинических проявлений атеросклероза. Считается, что основной механизм защитного действия гиполипидемических препаратов заключается в стабилизации покрышки атеросклеротической бляшки. Другие механизмы могут состоять во влиянии на синтез вазоактивных соединений в эндотелии и активацию свертывания крови [15,22].

Образование фибрина само по себе может приводить к нарушению целостности эндотелия и таким образом способствовать развитию атеросклеротической бляшки, однако основная роль системы гемостаза в возникновении осложнений атеросклероза заключается в тромбировании поврежденной поверхности сосудов. Морфологические и ангиографические исследования убедительно доказали тромботическую природу острых коронарных синдромов, что послужило созданию концепции атеротромбоза [128,164].

Данные больших проспективных исследований свидетельствуют о том, что повышение фибриногена, фактора Виллебранда и фактора VII, а также снижение фибринолитической активности связаны с увеличением риска инфарктов и инсультов [56,169,246,310]. Фг и ФВ играют ключевую роль в адгезии, активации и агрегации тромбоцитов. Активированные тромбоциты обеспечивают протекание реакций, ведущих к образованию тромбина, и секретируют фактор роста и ингибитор активаторов плазминогена (ИАП-1), стабилизирующий фибрин. Тромбин, фибрин и тромбоцитарный фактор роста, действуя через разные механизмы, стимулируют миграцию и пролиферацию гладкомышечных клеток. Таким образом, компоненты системы гемостаза не только участвуют в тромбировании пораженных участков сосудов, но и могут играть важную роль в процессе формирования и прогрессирования атеросклеротических стенозов [33,181,371].

В большинстве проспективных исследований Фг проявился как независимый фактор риска, а сочетание высокого Фг с гиперлипидемией v или повышенным артериальным давлением [56,437] приводит к дополнительнму увеличению риска. Нарушение обмена углеводов, андроидный тип ожирения, гипертония, повышение триглицеридов при снижении ЛВП являются признаками синдрома резистентности к инсулину, предрасполагающего к развитию диабета типа II и атеросклероза. У таких больных часто наблюдаются изменения в системе гемостаза, которые в целом могут быть охарактеризованы как гиперкоагуляция и снижение фибринолитической активности крови [238].

В изучении структуры и функции компонентов системы фибринолиза достигнут значительный прогресс, но в физиологии лизиса тромбов остается много вопросов, выяснение которых имеет как теоретическое, так и практическое значение [1,25]. Природа создала фибринолиз как медленный процесс, который должен быть отложен во времени от формирования фибрина на период, необходимый для восстановления поврежденной стенки сосуда. При этом тканевой активатор плазминогена является единственным ферментом системы гемостаза, который непрерывно секретируется эндотелием в полностью активной форме. Детали молекулярных механизмов, смещающих во времени образование и лизис фибрина, во многом остаются невыясненными [60,115]. Ингибитор активаторов плазминогена типа 1 является основным фактором, определяющим фибринолитическую активность крови [261], и в ряде исследований была обнаружена связь повышения активности ИАП-1 с риском ИБС и ее осложнений [238,378]. Повышение активности ИАП-1 наблюдается при многих заболеваниях и поэтому исследование возможности медикаментозной коррекции его уровня представляется актуальной задачей.

Новым стимулом к изучению роли системы фибринолиза в атеротромбозе послужили данные о связи повышенной концентрации тАП в плазме с увеличением риска сердечно-сосудистых осложнений [235,238, 378,419,446]. В качестве возможного объяснения парадоксальной связи тАП, активирующего фибринолиз, с тромботическими осложнениями было предположено, что повышение концентрации тАП является следствием увеличения доли неактивного комплекса тАП-ИАП-1, имеющего большую продолжительность циркуляции в кровотоке, чем свободный тАП [106], однако результаты исследования Роттердам [446] свидетельствуют о том, что увеличение активности тАП в плазме тоже может быть связано с развитием ИМ. В экспериментах показано, что тромбин стимулирует секрецию тАП и ИАП-1 клетками эндотелия [137,447], и поэтому повышение этих компонентов в плазме больных может быть следствием увеличения образования тромбина. Эндотелию принадлежит важная роль в регуляции физиологической активности тромбина. Связывание тромбина с тромбомодулином (ТМ) ингибирует прокоагулянтные активности тромбина и активацию этим ферментом клеточного рецептора [198]. Поэтому снижение экспрессии ТМ на эндотелии может повышать как прокоагулянтную, так и пролиферативную активности тромбина.

Таким образом, к началу 90-х гг. были получены убедительные доказательства важной роли тромбообразования в патогенезе атеросклероза, но многие вопросы, связанные с механизмами активации свертывания крови и факторами, определяющими объем и стабильность образующихся тромбов, оставались невыясненными. К этому времени были разработаны методы, позволяющие не только количественно определять индивидуальные компоненты системы гемостаза, но и оценивать степень их активации у больных, что существенно расширило возможности диагностики и анализа влияния терапии на выявляемые нарушения в системе гемостаза.

Цель и задачи исследования. Изучить содержание ключевых компонентов системы гемостаза у больных с различными клиническими проявлениями атеросклероза и оценить влияние антитромботических, вазоактивных и гиполипидемических препаратов на маркеры активации свертывания крови и коагуляционные факторы риска сердечнососудистых заболеваний.

В работе решались следующие задачи:

1. Получение и характеристика активатора протеина С, фибриногена и фибрин-мономера.

2. Разработка и апробация методов определения протеина С, тканевого активатора плазминогена и его ингибитора.

3. Разработка инструментального метода регистрации лизиса фибрина. Анализ активации лизиса фибрина стрептокиназой и тканевым активатором плазминогена.

4. Исследование влияния активированного протеина С на тромбообразование в эксперименте.

5. Исследование активности ИАП-1, маркеров активации тромбоцитов, образования тромбина и фибрина при заболеваниях сердечно-сосудистой системы.

6. Оценка прогностической значимости маркеров активации свертывания крови и фибринолиза.

7. Анализ влияния медикаментозной терапии на маркеры активации свертывания и факторы риска сердечно-сосудистых заболеваний.

Научная новизна работы. Показано, что лизис фибрина можно количественно регистрировать по изменению светопропускания геля. Выявлены новые закономерности в активации лизиса фибрина тАП. При свертывании фибриногена образование полимера происходит быстрее, чем формирование кофакторной для тАП структуры фибрина. Продолжительность лаг-фазы превышает время полуингибирования тАП в плазме. Эта лаг-фаза представляет собой один из механизмов, обеспечивающих смещение во времени процессов образования и лизиса фибрина.

Установлено, что активность ИАП-1 у больных инфарктом миокарда повышена по сравнению с больными стабильной и нестабильной стенокардией. Анализ динамики ИАП-1 в остром периоде инфаркта миокарда показал, что активность этого ингибитора нарастает пропорционально сроку от начала заболевания и достигает максимума через 12-15 часов. Повышение активности ИАП-1 в остром периоде может быть одной из причин снижения эффективности тромболитической терапии при позднем ее начале. У больных с восстановленным в первые 6 часов заболевания коронарным кровотоком активность ИАП-1 повышается в меньшей степени и быстрее снижается, чем у больных без признаков реперфузии. У больных с ранней реперфузией оккпюзированных сосудов обнаружено снижение протеина С в первые часы развития ИМ, которое может отражать активацию протеина С и образвание нестабильных тромбов, поддающихся эндогенному лизису.

В проведенных экспериментах показано, что активированный протеин С в дозах, не оказывающих выраженного влияния на параметры свертывания крови, замедляет образование тромбов в поврежденной анодным током брюшной артерии крыс.

Проведено исследование комплекса показателей, характеризующих агрегацию тромбоцитов in vitro и их активацию в кровотоке (ß-Tr, 4-ТФ), образование и инактивацию тромбина (Ф1+2, TAT) и плазмина (ПАП), антигена и активности тАП и ИАП-1, а также маркера внутрисосудистого образования и деградации фибрина (Д-димера) у больных с различной протяженностью атеросклеротических поражений. Показано, что активация свертывания крови нарастает с увеличением протяженности поражений сосудов и в наибольшей степени проявляется в повышении уровней р-Тг/4-ТФ, Ф1+2 и Д-димера. Содержание в плазме Д-димера коррелирует положительно с Ф1+2 и комплексом ПАП и отрицательно с активностью ИАП-1. У больных с повышенным уровнем Д-димера, но не имевших на момент обследования клинических признаков острого тромбоза, обнаружена повышенная секреция тАП при выполнении физической нагрузки.

Сопоставление содержания в плазме маркеров, отражающих активацию разных звеньев системы гемостаза, свидетельствует о том, что именно активация фибринолиза является одним из факторов, ограничивающих рост тромбов и обеспечивающих проходимость пораженных участков сосудов при повышенной активации тромбоцитов и образования тромбина у больных с атеросклеротическими поражениями сосудов. Это состояние характеризуется высоким риском тромботических осложнений, которые развились в течение 4-х лет наблюдения у 26% больных АПНК и в половине случаев закончились летально. Показано, что высокий уровень Д-димера и "низкие" показатели агрегации тромбоцитов in vitro являются независимыми предикторами тромботических осложнений у больных АПНК и ПХ.

Практическая значимость работы. Предложены методы выделения фибриногена, активатора протеина С и получения растворимого фибрина. Эти белки использовались для разработки методов определения фибриногена, протеина С, активности тканевого активатора плазминогена и его ингибитора, которые применялись в данном исследовании.

Разработан метод фотометрической регистрации лизиса фибрина. Определение скорости лизиса фибрина по изменению светопропускания геля может обеспечить более высокую по сравнению с пептидными субстратами специфичность в определении компонентов системы фибринолиза. Фибрин является как кофактором активации плазминогена, так и сложным субстратом, свойства которого зависят от других компонентов плазмы и изменяются в процессе лизиса. Возможности метода продемонстрированы на примерах определения Пг и CK, а также исследования активации лизиса фибрина тАП и стрептокиназой.

Данные проспективного наблюдения за больными с АПАНК и многофакгорный анализ прогностической значимости коагулологических показателей свидетельствуют о том, что количественное определение Д-димера является лабораторным тестом, который может дополнить информативность инструментальных методов исследования сосудов для выявления больных с высоким риском осложнении атеросклероза.

Повышенное содержание маркеров активации свертывания и фибринообразования у больных с атеросклеротическими поражениями артерий свидетельствует о целесообразности проведения антитромботи-ческой терапии даже при отсутствии клинических признаков острого тромбоза. Показано, что повышенный уровень общего холестерина и Д-димера являются фактором, которые могут влиять на увеличение толерантности к физической нагрузке при монотерапии индобуфеном или пентоксифиллином у больных АПНК и ПХ.

Показано, что лечение гиперлипидемии На колестидом сопрвождается снижением уровня фактора II свертывания крови и антигена ИАП-1.

Проведено сравнительное исследование влияние монотерапии гидрохлортиазидом и силазаприлом на коагулологические и метаболические факторы риска у больных ГБ МБ стадии. Показано, что оба препарата снижают фактор VII. Терапия СЛЗ не оказала влияния на уровни инсулина и антигена ИАП-1, повышение которых отмечалось у больных, принимавших ГХТ.

II. ОБЗОР ЛИТЕРАТУРЫ

Основная задача системы гемостаза состоит в сохранении жидкого состояния крови в пределах сосудов и быстром локальном образовании тромбов при повреждении стенки сосудов. Гемостатический процесс может быть разделен на 5 стадий: локальная вазоконстиркция; адгезия и активация тромбоцитов; формирование тромбоцитарного тромба; стабилизация его фибрином; и растворение тромба после регенерации поврежденной стенки сосуда. Условно система гемостаза может быть подразделена на 3 тесно взаимосвязанные системы: свертывания, противосвертывания и фибринолиза.

V. выводы

1. Проведено сравнительное исследование влияния активированного ПрС и гепарина на тромбообразование у крыс. Показано, что дозы активированного ПрС, оказывающие сопоставимое с гепарином влияние на время наступления тромботической окклюзии поврежденной артерии, не вызывают заметного влияния на свертываемость крови in vitro.

2. Разработан фотометрический метод регистрации лизиса фибрина. Применение этого метода позволило прямо продемонстрировать, что комплекс стрептокиназа-плазминоген является только активатором плазминогена и не обладает способностью лизировать фибрин.

3. Показано, что в активации лизиса фибрина тканевым активатором плазминогена наблюдается лаг-фаза, продолжительность которой превышает время полуингибирования тАП в плазме. Лаг-фаза представляет собой один механизмов, обеспечивающих смещение во времени процессов образования и лизиса фибрина.

4. Активность ингибитора активаторов плазминогена у больных в остром периоде инфаркта миокарда повышена по сравнению с лицами контрольной группы и больными стабильной и нестабильной стенокардией.

5. В остром периоде инфаркта миокарда активность ИАП-1 нарастает пропорционально сроку от начала приступа стенокардии. У больных с восстановленным кровотоком исходная активность ИАП-1 ниже, раньше достигается пик активности и его максимум ниже, чем у больных без признаков реперфузии в первые 6 часов заболевания. Повышение активности ИАП-1 во время развития ИМ может быть одной из причин снижения эффективности тромболитической терапии при позднем ее начале.

6. При атеросклерозе наблюдается активация свертывания крови, проявляющаяся в повышенном уровне маркеров активации тромбоцитов, образования тромбина и фибрина. Степень активации нарастает по мере увеличения распространенности атеросклеротических поражений. У больных с повышенным уровнем Д-димера, но не имеющих клинических признаков острого тромбоза, наблюдается повышенная секреция тАП.

7. У больных с атеросклеротическими поражениями артерий нижних конечностей уровень Д-димера и величина амплитуды первой волны агрегации тромбоцитов являются факторами, связанными с увеличением риска тромботических осложнений.

8. У больных с атеросклеротическими поражениями артерий нижних конечностей и перемежающейся хромотой терапия индобуфеном повышает толерантность к физической нагрузке, снижает показатели агрегации тромбоцитов in vitro, но не оказывает достоверного влияния на уровни маркеров образования тромбина и фибрина. Терапия пентоксифиллином повышает чувствительность тромбоцитов к АДФ и снижает содержание в плазме фрагмента 1 +2 активации протромбина и антигена ИАП-1.

9. У больных гиперхолестеринемией типа На прием колестипол гидрохлорида в дозе 8 г. в сутки снижает уровень общего холестерина и холестерина ЛНП. Основное снижение липидов наблюдается в течение первых 2-х месяцев лечения. Прием колестида приводит к снижению содержания в плазме фактора II свертывания крови и антигена ИАП-1.

10. Исследовано влияние монотерапии силазаприлом или гидрохпортиазидом на метаболические и коагулологические факторы риска у больных гипертонической болезнью IIB стадии. Показано, что оба препарата снижают уровень фактора VII свертывания крови. Силазаприл не оказывает влияния на содержание в плазме инсулина и ингибитора активаторов плазминогена, повышение которых наблюдалось у больных, принимавших гидрохпортиазид.

11. У больных стабильной стенокардией II-III функционального класса после приема 20 мг изосорбид-5-мононитрата наблюдается снижение чувствительности тромбоцитов к АДФ. Прием препарата не влияет на уровни антигена и активности тканевого активатора плазминогена и его ингибитора.

VI. ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

Хроматофокусирование в области рН от 9 до 7 позволяет в одну стадию выделить из яда Адк1з1гос1оп сопЬйпх препараты активатора протеина С, которые не активируют свертывание крови и не расщепляют хромогенные субстраты протеина С. Получаемые препараты активатора могут использоваться для определения протеина С в плазме больных.

Осаждение фибриногена в присутствии е-АКК позволяет получить препараты белка, обладающего высокой свертываемостью и не содержащего примеси плазминогена. ДезАА-фибрин может быть солюбилизирован 10 мМ НС1. Полученные таким способом препараты фибрин-мономеров обладают сходными с ВгС1Ч-фрагментами фибриногена кофакторными свойствами в активации плазминогена тАП и могут использоваться для определения активности тАП и его ингибитора в плазме больных.

Разработанный метод фотометрической регистрации лизиса фибрина может использоваться для тестирования активности фибринолитических препаратов. Измерение светопропускания фибрина, являющегося естественным субстратом плазмина и кофактором активации плазминогена, может обеспечить более высокую специфичность определения, чем измерение скорости расщепления хромогенных субстратов.

Количественное пределение Д-димера является лабораторным тестом, который может дополнить информативность инструментальных методов исследования сосудов для выявления больных с повышенным риском осложнений атеросклероза.
