В последние годы большой интерес вызывают исследования, связанные с изучением и применением стволовых клеток в медицине для заместительной клеточной терапии поврежденных тканей. При этом до сих пор ведется много дискуссий на тему, что же понимать под термином «стволовые клетки», и можно ли их использовать в клинике. Кроме клинического применения, наличие такой линии способствовало бы изучению механизмов регуляции дифференцировки клеток, а также изучению влияния на нее различных факторов. Для того чтобы детально их изучать, необходимо наличие постоянной линии стволовых клеток с тем, чтобы исключить вариации каких-либо признаков, которые можно наблюдать при использовании материала от разных доноров. Кроме того, такая линия являлась бы постоянным источником материала для клеточных технологий. На данный момент из взрослого организма удается выделять в чистом виде только гемопоэтические мезенхимные стволовые клетки. Однако линии стволовых клеток взрослого организма до сих пор не созданы.

Было обнаружено, что в ряде тканей взрослого организма существуют ниши, в которых находятся стволовые клетки, например, в коже в базальном слое кератиноцитов. Кожный покров человека и животных постоянно обновляется за, счет пролиферации кератиноцитов и их последовательной дифференцировки по направлению от базальной мембраны к поверхности кожи. Согласно существующим представлениям покоящиеся стволовые клетки эпидермиса сосредоточены у базальной мембраны и в волосяных фолликулах. Они дают начало так называемым транзиторным клеткам, способным интенсивно размножаться, за счет чего эпидермис постоянно обновляется. После нескольких делений они вступают на путь дифференцировки. В результате образуются разные слои эпидермиса, содержащие кератиноциты, находящиеся на разных уровнях дифференцировки (Хэм, Кормак, 1983).

При переводе в культуру кератиноцитов базального слоя эпидермиса они способны размножаться и воспроизводить такой же процесс последовательной дифференцировки, как это происходит в коже. Образуется многослойный пласт эпидермальных клеток, который может быть использован в технологиях заместительной клеточной терапии (Limat, Klein, 1993; Romero et al., 1999).

В связи с тем, что доказано существование стволовых клеток в коже человека, представляется актуальным получить популяцию кератиноцитов, обогащенную стволовыми клетками, которые обладают большим пролиферативным потенциалом. Пул таких клеток можно было бы использовать как для фундаментальных исследований, так и для применения в заместительной клеточной терапии. Предполагается, что это позволит повысить эффективность технологий восстановления структурной и функциональной целостности кожного покрова.

Решить задачу получения линии стволовых клеток или, по крайней мере, обогащения ими гетерогенной популяции базальных кератиноцитов, можно было бы осуществить путем выявления стволовых клеток методом проточной цитометрии с помощью специфических маркеров. В литературе описан ряд подобных маркеров (Michel et al., 1996; Pellegrini et al., 2001). Однако на наш взгляд они не являются строго специфичными для этого типа клеток, так как они носят скорее количественный характер и найдены и в клетках других тканей (Fradette et al., 1998; Reis-Filho et al., 2002). Кератиноциты разной степени дифференцировки различаются лишь только по величине содержания в них маркерных белков (Moles, Watt, 1997; Yang et al., 1998).

В связи с отсутствием строго специфичных маркеров необходимо искать другие экспериментальные подходы для четкого выделения этих клеток или, по крайней мере, для обогащения ими культивируемой популяции кератиноцитов. Стволовые клетки эпидермиса локализованы на базальной мембране и, следовательно, тесно взаимодействуют с входящими в ее состав белками (Spradling et al., 2001). В связи с этим представлялось перспективным для указанной цели применить метод селективной адгезии выделенных кератиноцитов к разным белкам внеклеточного матрикса (ВКМ) базальной мембраны, с которыми они взаимодействуют в неповрежденной коже. Для проверки данного предположения и разработки метода селективного разделения гетерогенной популяции клеток необходимо прежде всего выяснить, какие из белков базальной мембраны могут быть использованы для этой цели. Кроме того, важно установить степень взаимодействия с этими белками как свежевыделенной популяции кератиноцитов, так и после их культивирования.

Таким образом, четкое определение и идентификация стволовых клеток, обладающих высоким пролиферативным потенциалом, а также возможность получения из общей популяции эпидермальных клеток фракции стволовых кератиноцитов, представляет собой насущную задачу как для решения ряда фундаментальных вопросов, так и с точки зрения применения этих клеток в медицине с целью сокращения сроков заживления ран.

6. выводы

1. Первичные кератиноциты кожи человека отличаются по способности к взаимодействию с различными белками внеклеточного матрикса. Наиболее адгезивными для них являются коллагены I и IV типов, а также фибронектин.

2. Недифференцированные кератиноциты, выявляемые окрашиванием антителами на кератины 5 и 14, обнаруживают наибольшую степень сродства к субстрату при коротких временах адгезии до 30 минут.

3. Совмещение морфометрического анализа прикрепившихся клеток к субстрату с окрашиванием на кератин 19, считающийся маркером стволовых клеток кожи, позволило выявить субпопуляцию мелких круглых клеток, предположительно представляющих собой субпопуляцию стволовых кератиноцитов.

4. Проведенные исследования с использованием таких маркеров стволовых клеток эпидермиса, как рбЗ и кератина 19, показали, что с их помощью возможно лишь выявлять и охарактеризовать группы клеток по наличию в них данных белков, однако они не могут являться строго специфичными только для стволовых клеток кожи в условиях in vitro.

5. С целью выявления маркерных белков, специфичных для стволовых клеток кожи, получены антитела на поверхностные белки кератиноцитов, которые выявляют более четкие различия между дифференцированными и недифференцированными клетками.

6. Совокупность полученных данных свидетельствует о том, что с помощью селективной адгезии с использованием специфических маркеров возможно проводить обогащение гетерогенной популяции кератиноцитов стволовыми клетками.
