Актуальность темы исследования.

Трансплантация почки является общепризнанным высокоэффективным методом замещения утраченной функции почек у больных в терминальной стадии хронической почечной недостаточности (ХПН) / Кирпатовский И.Д., 1969/. В течение многих лет после операции не только поддерживается жизнь больных, но и достигается достаточно полная медицинская, а нередко и социальная, трудовая реабилитация. В настоящее время ежегодно в мире выполняется около 26 тысяч таких операций, а общее число реципиентов с пересаженной почкой в Европе достигло в 1994 году 96 на 1 млн. населения, что составляет 1/3 часть всех больных ХПН /Оре1г в. & а1., 1995/.

Переломным моментом в развитии трансплантологии на пути от первых экспериментов к широкой клинической практике можно считать открытие в конце пятидесятых антигенов главного комплекса гистосовместимости. Существенно улучшился прогноз при включении в схему иммуносупрессии препарата нового поколения - циклоспорина А. Следующим этапом стало применение в борьбе с острым отторжением поли-, а затем и моноклональных антител к лимфоцитам человека. Однако в настоящее время, несмотря на успехи развития иммуносупрессивной терапии, острый криз отторжения все еще остается наиболее частой причиной гибели трансплантата. Если диагноз «острое отторжение» поставлен, следующими проблемами становятся быстрый выбор терапевтической тактики и определение продолжительности супрессии. Биохимические параметры позволяют судить об эффективности антикризовой терапии не ранее, чем через 3-5 суток, особенно при изначальном отсутствии функции трансплантата.

Повторное использование антилимфоцитарных препаратов может быть ограничено образованием антител к ним в организме человека. Было показано, что количество наиболее серьезных инфекционных осложнений напрямую связано с дозой и продолжительностью применения антилимфоцитарных препаратов. С целью снижения частоты инфекционных осложнений некоторые авторы предлагают уменьшать ежедневную дозу препарата. Однако, в литературе практически не приводится убедительных доказательств эффективности подобных схем. Поэтому необходимо выработать надежные критерии контроля эффективности антикризовой терапии, а также определить пути профилактики возможных осложнений.

Наиболее частой причиной смерти больных после трансплантации почки являются инфекционные осложнения /Сеска J. et al., 1990/. Снижение иммунитета в результате уремии, анемии, применения иммуносупрессивных препаратов, множественные гемотрансфузии приводят не только к значительному увеличению их числа (некоторых - в десятки раз по сравнению с общей популяцией), но и к усилению их тяжести, а также развитию атипичных форм /Wolff S. et al., 1993/. Более 60% больных ХПН инфицировано различными видами гепатита, 50 % -цитомегаловирусом (ЦМВ), уточняются данные об инфицировании туберкулезом. В условиях иммуносупрессии большое значение имеет не только первичное инфицирование, но и реактивация инфекции /Miller В. et al., 1995/. Кроме того, в последнее время обнаружены способности к иммуномодулирующему действию некоторых вирусов. Решающее влияние на результаты лечения инфекционных осложнений может оказать разработка надежных тестов ранней диагностики.

Широкие перспективы открываются перед использованием таких новейших методов исследования, как иммуноферментный и ДНК-полимеразный /Andrews P. et al., 1994/. При этом ведущая роль в снижении смертности от инфекционных осложнений и увеличении выживаемости трансплантатов, по всей видимости, принадлежит разработке мер профилактики и превентивной терапии.

Одним из наиболее существенных факторов, предрасполагающих к развитию отсроченной функции трансплантата, является токсичность циклоспорина. Серьезной проблемой терапии Циклоспорином А является его узкая терапевтическая широта, что создает серьезные трудности в обеспечении достаточного уровня иммуносупрессии. Циклоспорин проходит биотрансформацию в тонком эндоплазматическом ретикулуме печени под действием ферментов цитохрома Р -450. На процесс метаболизма, особенно в раннем послеоперационном периоде, значительное воздействие оказывает состояние печеночного / кровотока и функционального состояния печени. В этой связи, крайне актуально определение четких критериев контроля эффективности действия препаратов, и разработка мер профилактики токсичности.

ЦЕЛЬ ИССЛЕДОВАНИЯ: разработка комплекса методов, повышающих надежность диагностики, эффективность лечения и профилактики острого отторжения у больных хронической почечной недостаточностью после пересадки почки.

ЗАДАЧИ ИССЛЕДОВАНИЯ:

1. Разработать иммунологические критерии диагностики острого отторжения после пересадки почки.

2 .Разработать комплекс мероприятий дифференциальной диагностике дисфункций трансплантата в ближайшем послеоперационном периоде. 3. Определить оптимальный срок начала антикризовой терапии препаратами моно- и поликлональных антител при лечении острого отторжения. 4. Определить влияние острого отторжения на возникновение наиболее опасных урологических осложнений.

5. Провести сравнительную оценку применяемых препаратов моно- и поликлональных антител при лечении острого криза отторжения.

6. Выделить критерии оценки эффективности антикризовой терапии у реципиентов после пересадки почки.

7. Провести сравнительную оценку развития побочных эффектов при использовании препаратов моно- и поликлональных антител к лимфоцитам человека.

8. Определить целесообразность последовательного и параллельного применения циклоспорина А с препаратами антител к лимфоцитам человека у пациентов с отсроченной функцией трансплантата.

9. Определить алгоритм наиболее эффективного и безопасного применения циклоспорина А при профилактике острого отторжения в ближайшем послеоперационном периоде.

10. Определить целесообразность применения препаратов моно- и поликлональных антител к лимфоцитам человека в профилактике острого отторжения.

11. Определить эффективность применения моноклональных антиСО-25 антител в профилактике острого отторжения.

12. Разработать комплекс мероприятий по профилактике системных инфекционных осложнений при проведении антикризовой терапии.

13. Провести сравнительный анализ традиционной базовой иммуносупрессии и модифицированной схемы с включением мофетила микофенолата.

Научная новизна.

1. Впервые дана комплексная клиническая, морфологическая, иммунологическая характеристика явлению острого отторжения при пересадке почки, разработан новый патогенетически обоснованный подход к его ранней диагностике в ближайшем и отдаленном периодах после пересадки почки.

2. Впервые проведена многоцентровая оценка влияния различных иммуносупрессивных препаратов на функциональное состояние основных субпопуляций лимфоцитов.

3. Впервые разработан метод контроля эффективности применения моно- и поликлональных антител при лечении и профилактики острого отторжения с использованием многопараметрового цитофлоуметрического анализа.

4. Изучено влияние острого отторжения на основные урологические осложнения при пересадке почки.

5. Впервые проведен комплексный анализ влияния различных методов иммуносупрессии на морфологические изменения в ренальном трансплантате.

6. Впервые проведен корреляционный анализ методики прямого и косвенного определения основных фамакокинетических характеристик циклоспорина А и определена их связь с эффективностью иммуносупрессии, нефротоксичностью и развитием инфекционных осложнений.

7. Впервые проведено определение влияния методов иммуносупрессии на развитие вирусной инфекции, вызываемой вирусом Эпштейн-Барра, парвовирусом, полиомавирусом, вирусами герпеса 1,2,3,7,8 типов у реципиентов после пересадки почки.

Практическая ценность работы.

В результате данного исследования разработан ряд методов профилактики и лечения острого отторжения после пересадки почки, разработаны критерии оценки эффективности антикризовой терапии, меры профилактики осложнений, модифицированы схемы применения основных иммуносупрессантов.

Основные положения, выносимые на защиту.

1. Для дифференциальной диагностики острого отторжения и других дисфункций трансплантата в раннем послеоперационном периоде необходим комплекс мероприятий, включающий ежедневный мониторинг биохимических параметров, ультрасонографию с доплерографией, исследовании экспрессии рецепторов лимфоцитов периферической крови.

2. Основные урологические осложнения при трансплантации почки определяются редукцией кровообращения мочеточника, которая находится в прямой зависимости от выраженности острого отторжения.

3. Проведение курса антикризовой терапии препаратами моно- и поликлональных антител требует контроля эффективности путем определения абсолютного количества лимфоцитов периферической крови и их фенотипирования.

4. Эффективность применения moho- и поликлональных антител для лечения и профилактики острого отторжения определяется содержанием активированных Т-лимфоцитов периферической крови.

5. Эффективность применения моно- и поликлональных антител к лимфоцитам человека зависит от обратимости морфологических изменений трансплантата и прямо связана со сроками начала антикризовой терапии.

6. Иммуносупрессивный эффект и токсичность циклоспорина А прямо связаны с величиной площади под кривой его концентрации в цельной крови.

7. Разработанные косвенные методы определения площади под кривой циклоспорина А позволяют адекватно оценивать иммуносупрессивный эффект препарата и существенно снизить затраты.

8. Для профилактики острого отторжения необходима постоянная базовая иммуносупрессия, включающая не менее трех компонентов.

204 Выводы.

1. При развитии острого отторжения после пересадки почки отмечено достоверное повышение экспрессии антигенов CD25, HLA-DR, CD95, CD45RA, CDllb, CD38 лимфоцитами периферической крови.

2. .Комплексное обследование, включающее мониторирование биохимических, иммунологических параметров крови, ультрасонографических характеристик трансплантата позволяет повысить эффективность диагностики острого отторжения после пересадки почки и сократить сроки до начала соответствующей терапии.

3. Раннее применение (в течение первых двух суток с момента начала острого криза отторжения) препаратов моно- и поликлональных антител к лимфоцитам человека позволяет существенно повысить их эффективность в отношении купирования острого криза отторжения независимо от предшествующей пульс-терапии кортикостероидами.

4. Вероятность развития наиболее опасных урологических осложнений, прежде всего некрозов мочеточника, достоверно выше у пациентов с двумя и более острыми кризами отторжения и меньше у пациентов с безкризовым течением послеоперационного периода.

5 Применение при лечении острых кризов отторжения препаратов моноклональных антител к CD3 рецепторам лимфоцитов человека позволяет снизить количество некупированных кризов отторжения в среднем на14,3% и повысить годовую выживаемость трансплантатов на 12,0 - 20,0% по сравнению с препаратами поликлональных антител.

6. Основными прогностическими признаками эффективности препаратов моноклональных антител к CD3 рецепторам лимфоцитов человека можно считать снижение уровней CD3, CD4, CD8, CD25 положительных лимфоцитов периферической крови до 0,1 - 1% при относительном росте CD 19+, CD20+ клеток через 3-6 часов после первого введения. Основными прогностическими признаками эффективности препаратов поликлональных антител можно считать снижение уровней CD3, CD4, CD8, CD25 положительных лимфоцитов периферической крови до 1 - 5% через 12 - 24 часа после первого введения.

7. .Применение моноклональных антител в лечении острого отторжения позволяет существенно снизить выраженность апластического эффекта по сравнению с применением поликлональных антител, однако сопровождается увеличением частоты развития цитомегаловирусной инфекции на 13,9%.

8. Последовательное применение препаратов поликлональных антител и циклоспорина А в раннем послеоперационном периоде позволяет снизить количество инфекционных осложнений, прежде всего вирусных, нефротоксичность и, соответственно, период восстановления функции, а также увеличить клубочковую фильтрацию на 10 -15%.

9. Прямое определение площади под кривой при мониторировании концентрации циклоспорина А является наиболее достоверным критерием эффективности действия препарата, а также коррелирует с риском развития нефротоксичности и основных инфекционных осложнений. Частота развития морфологических признаков острого отторжения в ближайшем послеоперационном периоде также зависит от площади под кривой концентрации циклоспорина А в крови.

10. Эффективность применения косвенных (расчетных) методов определения площади под кривой концентрации циклоспорина А в большой степени зависит от индивидуальных особенностей метаболизма, а не от вида применяемого генерика.

11. Для профилактики острого отторжения в раннем послеоперационном периоде после пересадки почки четырехкомпонентная стартовая иммуносупрессия с включением поликлональных антилимфоцитарных антител более эффективна по сравнению с применением моноклональных анти CD3 антител.

12. Применение моноклональных гуманизированних антител к CD25 рецепторам лимфоцитов человека с целью профилактики острого отторжения не приводит к достоверному улучшению выживаемости трансплантата и улучшению его функции в ближайшем послеоперационном периоде.

13. Уменьшение дозы применяемых препаратов моно- и поликлональных антител к лимфоцитам человека не приводит к существенному уменьшению побочных эффектов, но приводит к уменьшению выживаемости трансплантатов на 25,0%.

14. Замена в традиционной трехкомпонентной стартовой иммуносупрессии азатиоприна на мофетила микофенолат позволяет снизить количество кризов отторжения.

Практические рекомендации.

1. Цитофлоуметрия должна быть включена в обязательный протокол обследования для дифференциальной диагностики острого отторжения и канальцевого некроза.

2. В мероприятия по дифференциальной диагностике острого отторжения необходимо включить мониторирование биохимических, иммунологических параметров периферической крови, ультрасонографические (включая доплерографию) исследования, которые при отсутствии четкой клинической картины должны дополняться морфологическим исследованием биопсийного материала, включая иммуногистохимический метод.

3. Повышение CD25+ лимфоцитов в периферической крови более 5% является показанием к применению анти - CD3 моноклональных антел.

4.С целью профилактики некроза мочеточника у пациентов группы повышенного риска целесообразно профилактическое включение в протокол стартовой иммуносупрессии препаратов поликлональных антител.

5. При наличии клинических признаков острого отторжения, отсутствии эффекта от пульс-терапии кортикостероидами и повышении относительного числа CD25+ лимфоцитов в периферической крови методом выбора является назначение анти CD3 моноклональных антител.

6. Основными критериями окончания курса терапии препаратами моно- и поликлональными являются восстановление функции трансплантата и возвращение к исходному уровню CD3+, CD4+, CD8+, CD19+, CD20+, CD95+ лимфоцитов периферической крови.

7. .Для профилактики инфекционных вирусных осложнений при применении препаратов моно- и поликлональных антител необходимо назначать одновременно курс ганцикловира, независимо от наличия в крови и других биологических жидкостях пациента маркеров цитомегаловируса.

8. При отсутствии немедленной функции трансплантата целесообразно отсроченное (по окончании курса профилактической терапии препаратами поликлональных антител) включение в протокол стартовой иммуносупрессии циклоспорина А.

9. В раннем послеоперационном периоде для наибольшей эффективности и безопасности применения циклоспорина А необходимо поддерживать его концентрацию в крови, соответствующую площади под кривой 4500-5500.

10. С целью профилактики острого отторжения и повышения эффективности применения циклоспорина А в ближайшем послеоперационном периоде необходимо косвенное определение площади под кривой препарата на основе комбинации не менее двух точек, которые необходимо выбирать учитывая индивидуальные особенности фармакокинетики пациентов.

11. При уровне CD25 положительных лимфоцитов менее 5% в периферической крови для лечения и профилактики острого криза отторжения показано применение препаратов поликлональных антител.

12. С целью профилактики острого отторжения у пациентов высокого риска с отсроченной функцией трансплантата показано применение поликлональных антител к лимфоцитам человека.

13. Для лечения и профилактики острого отторжения показано назначение АЛГ в дозе 15-20 мг/кг, АТГ -в дозе 4-6 мг/кг/, ОКТЗ в дозе 5 мг/сут.

14. В стартовой иммуновупрессии целесообразно применение трех базовых препаратов: циклоспорина А, метилпреднизолона и мофетила микофенолата.
