Актуальность темы

Патологические симптомы неизбежно возникают в ходе развития онкологического заболевания и могут быть связаны непосредственно с заболеванием или с последствиями проводимой терапии (64: 74: 184: 211: 268: 270: 281: 2911 Недостаточно контролируемые симптомы у онкогематологического больного значительно ухудшают его качество жизни. Симптомы могут явиться препятствием для проведения лечения основного заболевания и оказать негативное влияние на его эффективность [74: 225]. Спектр обсуждаемых симптомов достаточно велик, но наиболее часто встречаются боль, слабость, одышка, тошнота/рвота, депрессия, нарушения сна [64:74].

По мере прогресснрования заболевания контроль симптомов и улучшение качества жизни приобретают первостепенное значение и становятся важнейшими задачами паллиативной медицины [81]. Контроль симптомов предполагает сложную мультидисциилинарную систему оказания помощи онкогематологическим пациентам. Устранение или облегчение тягостных проявлений болезни во многом зависит от точной и достоверной оценки симптомов у каждого больного |4: 7: 9].

Адекватное исследование характера и степени выраженности симптомов зависит от методологии их изучения, чрезвычайно важной составляющей которой является положение о том. что симптом представляет собой субъективный феномен, информация о котором может быть получена только с помощью больного. Кроме этого, симптом имеет многофакторную природу, что требует его многомерной оценки с учетом таких показателей, как интенсивность, временные параметры, степень дистресса, влияние на показатели КЖ и т.п. Качественные и количественные характеристики симптома изменяются во времени, что предопределяет необходимость его оценки в динамике. Оценка симптомов предполагает использование специальных инструментов - шкал и опросников.

Применение опросников для оценки симптомов позволяет систематизировать обмен информацией, обеспечить универсальный язык общения, документировать полученные данные |4: 7: 9].

Результаты оценки симптомов являются основой для планирования и осуществления эффективной помощи пациентам.

В настоящее время разработаны опросники для оценки боли [67: 68: 76; 80: 92: 105: 106: 138: 157: 169: 178: 189: 210: 244: 262: 271: 3131. слабости [72:120: 144: 179: 190: 227: 232: 264]. тревоги и депрессии [93: 250]. тошноты и рвогы [101: 198: 208]. Однако большинство пациентов, страдающих онкологическим заболеванием, испытывают сразу несколько симптомов [64: 74]. Исходя из этого, у таких пациентов уместно применение инструмента, позволяющего оценить комплекс симптомов.

В 2000 году был создан опросник оценки основных симптомов - MD Anderson Symptom Inventory (MDASI) для оценки степени выраженности основных симптомов и их влияния на параметры качества жизни онкологических больных (Cleeland С. el. al). Преимуществами инструмента явились краткость, простота в использовании, легкость перевода на другие языки и возможность применения в клинической практике даже у самых тяжелых пациентов [64: 74]. Опросник MDASI успешно используется в клинических исследованиях и переведен на несколько языков [9: 181: 199: 201:218].

Инструменты для оценки комплекса симптомов в отечественной онкологии и онкогематологии до настоящего времени отсутствуют. В этой связи создание русской версии опросника для оценки основных симптомов имеет большое практическое значение и открывает возможности полноценного анализа спектра и выраженности симптомов у онкогематологнческих больных. Изучение спектра и выраженности симптомов у онкогематологнческих больных, а также их влияния на параметры качества жилни пациентов может способствовать решению ряда важных задач. Среди них наиболее актуальными являются: исследование механизмов развития симптомов, разработка новых лечебных алгоритмов по купированию симптомов, улучшение качества медицинской помощи больным со злокачественными заболеваниями крови.

Цель исследования

Изучить выраженность и частоту встречаемости основных симптомов у онкогематологических больных, а также их влияние на качество жизни пациентов.

Задачи исследования

1. Разработать русскоязычную версию опросника оценки основных симптомов M.D. Anderson Symptom Inventory (MDAS1) для оценки симптомов у онкогематологических и онкологических больных.

2. Определить выраженность и частоту встречаемости основных симптомов у больных злокачественными заболеваниями системы крови.

3. Сравнить выраженность и частоту встречаемости основных симптомов у больных злокачественными заболеваниями системы крови и солидными опухолями.

4. Изучить влияние симптомов на качество жизни больных со злокачественными заболеваниями системы крови.

Научная новизна исследования Разработана русская версия опросника оценки симптомов MDASI. являющаяся новым инструментом для комплексной оценки симптомов у онкологических и онкогематологических больных. Впервые в России с помощью данного инструмента определены выраженность н частота встречаемости симптомов у онкогематологических больных: изучено влияние симптомов на качество жизни больных со злокачественными заболеваниями системы крови и проведен сравнительный анализ выраженности и частоты встречаемости основных симптомов у больных злокачественными заболеваниями системы крови и солидными опухолями.

Научно-практическая значимость работы и ее реализация

Установлено, что русская версия опросника оценки основных симптомов MDASI может быть использована для изучения симптомов у онкогематологических больных и больных солидными опухолями в клинических исследованиях. Опросник оценки основных симптомов MDASI также может применяться в клинической практике для комплексной оценки симптомов, разработки адекватного плана лечения и мониторинга эффективности контроля симптомов. Методологические подходы к исследованию симптомов с использованием русской версии опросника оценки основных симптомов MDASI в настоящее время применяются в многоцентровых научных исследованиях Национальною медико-хирургического центра им. Н.И.Пирогова Росздрава (Москва), Научно-методического центра мониторинга качества жизни (Москва) и Межнационального центра исследования качества жизни (Санкт-Петербург).

Апробация работы Основные материалы исследования доложены и обсуждены на между народной конференции "Исследование качества жизни в здравоохранении" (Санкт-Петербург. 2002 год). Результаты исследования опубликованы в 13-ти печатных работах.

Положения, выносимые на защиту

1. В результате определения психометрических свойств доказана возможность применения русскоязычной версии опросника оценки основных симптомов MDASI у больных гемобластозами и солидными опухолями.

2. Спектр симптомов, испытываемых больными гемобластозами, достаточно широк. Подавляющее большинство больных испытывает хотя бы один симптом. Наиболее часто встречающимися симптомами у больных гемобластозами являются слабость, нарушения сна. подавленность и печаль.

3. Симптомы оказывают значительное влияние на все параметры качества жизни больных гемобластозами физическое, психологическое и социальное функционирование.

Объем и структура работы

Диссертация состоит из введения. 3-х глав, заключения, выводов, практических рекомендаций, указателя литературы и 3-х приложений. Работа изложена на 141 страницах машинописного текста, содержит 16 таблиц. 6 рисунков. Указатель литературы включает 11 отечественных. 305 зарубежных источников.

ВЫВОДЫ

1. Русская версия опросника оценки основных симптомов МБАБ! является адекватным инструментом для оценки симптомов и их влияния на основные параметры качества жизни онкогематологических больных. Результаты оценки надежности и конструктивной валидности доказывают хорошие психометрические свойства инструмента, который может применяться для оценки и мониторинга симптомов у данной категории пациентов.

2. Слабость является наиболее распространенным и выраженным симптомом у больных гемобластозами. ее испытывают 93% больных. При этом значительную часть составляют больные с умеренно и сильно выраженной слабостью (5 баллов и выше по цифровой оценочной шкале от 0 до 10 баллов). Другими часто встречающимися симптомами являются нарушение сна и симптомы нарушения психологического функционирования (подавленность и печаль). Их испытывают три четверти больных.

3. Более половины больных гемобластозами отмечают от одного до четырех симптомов, выраженность которых выше 5 баллов. У 20.5% пациентов зарегистрированы одновременно пять или более столь же выраженных симптомов.

4. Наиболее выраженными и часто встречающимися симптомами как при гематологических, так при и солидных опухолях, являются слабость и нарушения сна. Следующими по выраженности симптомами при гематологических опухолях являются боль, одышка и нарушение памяти, а при солидных боль и симптомы нарушения психологического функционирования (чувство подавленности и чувство печали). Боль при гемобластозах встречается реже, чем при солидных опухолях.

5. Симптомы оказывают значительное влияние на физическое, психологическое и социальное функционирование онкогематологических больных. С увеличением степени выраженности и количества симптомов ухудшаются все показатели качества жизни больных. Критическое снижение качества жизни происходит у больных гемобластозами. которые одновременно испытывают пять и более симптомов интенсивностью выше 5 баллов.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

1 Опросник оценки основных симптомов МОАБ1 может быть использован в клинической практике и клинических исследованиях для оценки выраженности основных симптомов и их влияния на основные параметры качества жизни онкогемалогических больных.

2. Применение опросника оценки основных симптомов МЭА81 может быть рекомендовано для комплексной оценки состояния больного, как в процессе лечения, так и во время динамического наблюдения, а также для оценки аффективности терапии наряд) с традиционными кл и н и ч ее ки м и параметрам и.
