Контактные линзы за последнее десятилетие, широко используются в коррекции аномалий рефракции и для лечения ряда патологических состояний глаз. В настоящее время в мире контактные линзы носят более 100 млн. человек и эта цифра неуклонно растет. Так, по данным профессора Б. Холдена, в 2004 году контактные линзы в мире будут носить 130 млн. человек. Но, не смотря на большую популярность и высокую эффективность контактной коррекции, контактные линзы могут стать причиной различных заболеваний глаз.

Контактные линзы являются причиной симптоматического синдрома «сухого глаза», аллергического конъюнктивита, способствуют обострению хронических заболеваний век и конъюнктивы.

В литературе описаны случаи тяжелого кератита, язвы роговицы, генерализации инфекции внутрь глаза и гибель глазного яблока, как результат контактной коррекции. Преимущественное поражение лиц молодого и трудоспособного возраста, а также длительное лечение, существенно влияющее на качество жизни — все это определяет важную медицинскую и социальную значимость проблемы.

Причины возникновения различных осложнений контактной коррекции в литературе находятся в стадии изучения.

В настоящее время известно, что для безопасного ношения контактных линз огромное значение имеет целостность прекорнеальной слезной пленки, которая покрывает переднюю поверхность глаза и является первым иммунным барьером. Прекорнеальная слезная пленка при ношении контактных линз отличается по многим параметрам от интактной слезной пленки.

Сведения о патогенетической роли деструкции прекорнеальной слезной пленки в развитии осложнений контактной коррекции в отечественной литературе отсутствуют. Биохимическая структура прекорнеальной слезной пленки в литературе представлена единичными исследователями, состав ее в настоящее время дискутируется.

Следовательно, углубленное изучение различных аспектов данной проблемы является высоко актуальным. С учетом вышеизложенного были сформулированы цель и задачи исследования.

Цель настоящей работы - изучение структуры, биохимического состава прекорнеальной слезной плёнки в норме, при контактной коррекции и восстановление ее деструкции при заболеваниях глаз.

Для достижения поставленной цели были определены следующие задачи:

1. Исследовать структуру и биохимический состав прекорнеальной слезной плёнки у лиц молодого возраста в норме.

2. Изучить структуру и биохимический состав прекорнеальной слезной пленки у лиц при длительном ношении контактных линз.

3. Описать клинику различных осложнений контактной коррекции: кератита, кератоувеита, симптоматического синдрома «сухого глаза», аллергического и мебомиевого конъюнктивита.

4. Исследовать биохимический состав прекорнеальной слезной пленки при различной патологии переднего отрезка глаза у лиц с контактной коррекцией.

5. Оптимизировать восстановление прекорнеальной слезной пленки при ее деструкции путем использования современных слёзозамещающих препаратов.

Научная новизна

Впервые установлен биохимический состав прекорнеальной слезной пленки: общих белков, общих липидов и сиаловых кислот у здоровых лиц молодого возраста.

При длительной контактной коррекции выявлены изменения структуры и биохимического состава слезной пленки: снижение общих белков и сиаловых кислот.

Доказано, что контактная коррекция вызывает деструкцию прекорнеальной слезной пленки, развитие микробного и асептического инфильтративного кератита, при которых установлено разрушение прекорнеальной слезной пленки и выраженные изменения ее биохимического состава.

Контактная коррекция может способствовать развитию симптоматического синдрома «сухого глаза» при медленно развивающейся деструкции прекорнеальной слезной пленки и уменьшении всех компонентов, особенно сиаловых кислот.

Использование современного слезозамещающего препарата «Видисик» позволило оптимизировать структуру и восстановить биохимический состав слезной пленки до нормального уровня.

Разработана методика использования препарата «Видисик» 2-4 раза в день в зависимости от степени деструкции прекорнеальной слезной пленки. При заболеваниях переднего отдела глаза назначается традиционное лечение в комбинации с препаратом «Видисик», что в 2 раза ускоряет процессы регенерации роговицы.

Практическая значимость

В связи с установленным положением о том, что контактная коррекция может приводить к изменению структуры и биохимического состава прекорнеальной слезной пленки, необходимо у всех больных при ношении контактных линз исследовать время разрыва по Норну и слезопродукцию с помощью теста Ширмера.

Было обосновано, что при длительной контактной коррекции (10 и более лет) и изменеиях биохимического состава ПСП, необходимы профилактические курсы препаратом «Видисик» в течение 1 месяца.

Описанная клиника асептического и микробного кератита при контактной коррекции и выявленные при этих заболеваниях изменения структуры и биохимического состава прекорнеальной слезной пленки требуют комбинированного лечения с использованием слезозамещающих препаратов.

При симптоматическом синдроме «сухого глаза» на фоне контактной коррекции, аллергического и мейбомиевого конъюнктивита целесообразно исследовать не только структуру ПСП, но и ее биохимический состав, что позволяет оптимизировать дифференцированное лечение разных заболеваний переднего отдела глаза.

Положения, выносимые на защиту

1. Структура и биохимический состав прекорнеальной слезной плёнки в норме у лиц молодого возраста: общие белки - 0,33 мкг/мл; общие липиды -0,022 мкг/мл; сиаловые кислоты - 0,170 Пмоль/мл.

Длительная контактная коррекция 10 и более лет сопровождается медленно прогрессирующим нарушением структуры и биохимического состава прекорнеальной слезной плёнки.

2. Описаны осложнения контактной коррекции: микробный и асептический инфильтративный кератит, при которых изучены структура и биохимический состав прекорнеальной слезной пленки.

3. Представлена клиника симптоматического синдрома «сухого глаза» на фоне контактной коррекции, аллергического и мейбомиевого конъюнктивита, при которых изменения структуры и биохимического состава прекорнеальной слезной пленки зависели от тяжести заболевания.

4. Использование современного слезозамещающего препарата «Видисик» в комплексном лечении патологии переднего отдела глаза при деструкции прекорнеальной слезной плёнки позволило восстановить общие белки, общие липиды и сиаловые кислоты, ее водный, липидный и муциновый слой.

Внедрение результатов работы в практику

Материалы диссертации внедрены в работу поликлинических кабинетов -МУЗГП № 3, Центра патологии рефракции и лазерной хирургии ГБ № 2, Медицинского центра «Медикор», Глазного центра «Зрение».

Материалы диссертации включены в программу преподавания раздела офтальмологии: «Синдром сухого глаза, патология роговицы и конъюнктивы» при подготовке клинических врачей, обучающихся на тематических курсах, а также врачей-интернов, клинических ординаторов, проходящих обучение на базе кафедры офтальмологии ГОУ ДПО Уральской государственной медицинской академии дополнительного образования (г. Челябинск).

Публикации

По материалам диссертации выступала на Всероссийской Научно-практической конференции с международным участием «Новые технологии в лечении заболеваний роговицы» - материалы опубликованы в центральной печати (Москва 2004); на научно-практической конференции «Актуальные проблемы медицинской науки, технологий и профессионального образования УГМАДО-2003» (Челябинск); на заседаниях областного общества офтальмологов; на межрегональной конференции (Челябинск 2004). По материалам диссертации опубликовано 13 печатных работ, из них 2 в центральной печати.

Структура и объем диссертации

Диссертация изложена на 139 страницах машинописного текста, состоит из разделов введения, обзора литературы, главы материал и методы, пяти глав собственных результатов исследования, заключения, выводов, библиографического списка. Работа иллюстрирована 44 рисунками, 20 таблицами. Библиографический список содержит 315 работ, из которых 20 -отечественных авторов, 295- зарубежных.

ВЫВОДЫ

1. Установлен биохимический состав прекорнеальной слезной пленки при неизмененной структуре у здоровых лиц, который представлен общими белками (0,33±0,04 мкг/мл), общими липидами (0,022±0,007 мкг/мл) и сиаловыми кислотами (0,170±0,007 Пмоль/мл).

2. Доказано, что при длительной контактной коррекции происходит изменение структуры и биохимического состава прекорнеальной слезной пленки: снижение общих белков на 21%, сиаловых кислот на 36%.

3. Осложнениями контактной коррекции являются: микробный и асептический инфильтративный кератит, симптоматический синдром «сухого глаза», при которых выявлены выраженные изменения структуры и биохимического состава прекорнеальной слезной пленки.

4. Микробный инфильтративный кератит сопровождается выраженной деструкцией прекорнеальной слезной пленки и значительным изменением ее биохимического состава: увеличением количества общих белков в 2 раза; уменьшением общих липидов в 2 раза; резким снижением сиаловых кислот в 3 раза.

5. При асептическом инфильтративном кератите деструкция прекорнеальной слезной пленки и соответственно ее биохимический состав менее выражены: увеличены общие белки в 1,5 раза, незначительно снижены общие липиды и сиаловые кислоты.

6. При симптоматическом синдроме «сухого глаза» на фоне контактной коррекции, аллергического конъюнктивита и мейбомиевого блефароконъюнктивита изменения структуры и биохимического состава прекорнеальной слезной пленки зависят от степени тяжести заболевания. При тяжелом течении отмечается деструкция прекорнеальной слезной пленки, что соответствует снижению общих белков, общих липидов и сиаловых кислот.

7. Использование современного слезозамещающего препарата «Видисик» позволяет восстановить структуру и биохимический состав прекорнеальной слезной пленки: общих белков, общих липидов и сиаловых кислот до нормального уровня при симптоматическом синдроме «сухого глаза», ускоряет процессы регенерации роговицы при микробном инфильтративном кератите.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ.

1. Изучение структуры прекорнеальной слезной пленки - время разрыва по Норну и слезопродукции - тест Ширмера, должны быть обязательными методами исследования при подборе контактных линз.

2. Было обосновано, что при длительной контактной коррекции (10 и более лет) и нарушении структуры слезной пленки, необходимы профилактические курсы препарата «Видисик» в течении 1 месяца.

3. При развитии синдрома «сухого глаза» пациентам с контактной коррекцией назначать специальные биосовместимые контактные линзы.,

4. При симптоматическом синдроме «сухого глаза» на фоне контактной коррекции, аллергического и мейбомиевого конъюнктивита целесообразно исследовать не только структуру ПСП, но и ее биохимический состав, что позволяет оптимизировать дифференцированное лечение разных заболеваний переднего отдела глаза.
