Выявление взаимосвязи структуры и функции материала является общей-естественнонаучной проблемой. При этом стабильность объекта, являясь, с одной стороны, характеристикой структуры, с другой - напрямую определяется его функцией. Таким образом, возникает триада функция — структура - стабильность. В этой триаде стабильность является понятием достаточно общим, включающим в себя устойчивость к термическому, механическому, химическому и другим воздействиям. Не умаляя значение остальных аспектов, отметим, что для физической химии термическая стабильность играет особую роль, так как на основе именно этой характеристики мы определяем (или оцениваем) основные термодинамические функции (или их изменение) и, как следствие, используем аппарат термодинамики для описания состояния системы и ее изменения.

В определении физической химии как науки, «объясняющей на основании положений и опытов физики то, что происходит в смешанных телах при химических операциях» (М.В. Ломоносов), не содержится ограничений на выбор объекта физико-химического исследования. Однако мы понимаем, что строгое количественное описание в рамках общепризнанных моделей применяется для простых систем. Для сложных, многокомпонентных, многофазных объектов количественная характеристика затруднительна. Это не означает, что такие объекты не подчиняются основным физико-химическим законам. Первым этапом для описания сложных систем является выявление качественных закономерностей поведения и оценка самых общих термодинамических свойств, что дает возможность анализа структурно-функциональной связи в объекте. К таким свойствам относится термическая стабильность.

Соединительные ткани в организме выполняют одну функцию - механическую, но в разных вариантах: защита, опора, сочленение, объединение, передача нагрузки и прочее. Главным компонентом соединительных тканей является коллаген, который составляет треть от всех белков у млекопитающих. Все вариации надмолекулярной организации, архитектоники и функций соединительных тканей от твердой кости и прочной связки до прозрачной роговицы или гелеобразного пульпозного ядра определяются способом укладки этого белка и его взаимодействием с другими макромолекулами внеклеточного матрикса. Решение вопроса о том, как термическая стабильность коллагена отражает структуру и функцию ткани представляется, на наш взгляд, интересным аспектом освещения проблемы функция - структура - стабильность. Физико-химические исследования соединительных тканей в некоторой степени облегчаются тем, что доля клеток по массе и объему невелика (-10%), а большая часть представлена внеклеточным матриксом.

Данные о термостабильности выделенного коллагена (в растворе и волокнах) широко освещены в литературе и количественные термодинамические характеристики перехода макромолекул белка из нативной упорядоченной конформации в форму случайного клубка - температура и энтальпия денатурации (Тд и ДНд) установлены. Такая информация для коллагена в ткани крайне скудна и не систематизирована, хотя получение ее методом дифференциальной сканирующей калориметрии не представляет труда. Рассматривая же триаду структура-функция-стабильность Тд и ДНд могут быть использованы для характеристики состояния коллагена в ткани, в том числе и для тканей с измененной структурой, а следовательно, и функцией. Практическая выгода такого подхода очевидна. Во-первых, это диагностика патологических изменений в ткани, которые приводят к отклонениям от оптимального функционирования. Во-вторых, это первый шаг структурного исследования заменителей ткани (т.н. коллагенсодержащих биопротезов и скаффолдов). Наконец, это метод определения, состояния коллагена, в ткани, подвергнутой целевой модификации. Разработка подходов модификации соединительных тканей для изменения ее механических свойств в настоящее время не только обсуждается в литературе, но и внедряется в медицинскую практику.

Значительное место в современных медицинских технологиях отводится методу локальной гипертермии, когда первичный эффект либо напрямую связан с денатурацией коллагена, либо достигается при нагреве выбранного участка ткани до температуры, соответствующей или несколько превышающей начало денатурации. Локальный нагрев разных тканей осуществляется с помощью либо ИК лазера, либо СВЧ поля, либо электрическим током. К сожалению, условия воздействия (такие как, например, температура и время нагрева) устанавливаются на основе небольшой выборки экспериментальных данных, и результаты обработок зачастую оказываются непредсказуемы и субоптимальны. Таким образом, существует насущная потребность научного обоснования применения метода локального нагрева в клинической практике.

Лазерная медицина как научное направление является частью биофотоники — междисциплинарного раздела о взаимодействии света с живыми системами. Физическую основу биофотоники составляют оптика и спектроскопия. Однако описание результатов лазерной модификации биологических систем в научном сообществе, естественно, дается на основе аппарата химической термодинамики или кинетики, и, заметим, не всегда корректно. При анализе экспериментальных исследований редко принимается во внимание отличительные черты состава и архитектоники ткани. Однако специалисты-химики зачастую не участвуют в обсуждении результатов и разработке научных основ локальной гипертермии соединительных тканей.

Настоящая работа, направленная на исследование стабильности коллагена в соединительных тканях различного типа, состоит из двух частей. В первой части обосновывается использование термической стабильности, как структурной и функциональной характеристики матрикса соединительных тканей различного типа, а во второй - исследуется устойчивость этих тканей в условиях неоднородного лазерного нагрева. Работа выполнена в соответствии с планом научно-исследовательских работ Химического факультета МГУ имени М.В. Ломоносова, тема «Химические реакции в разрядах и при фотохимическом и лазерном инициировании», № гос. регистрации 01.200.115171. В течение 10 лет исследования осуществлялись при финансовой поддержке РФФИ (проекты №№ 99-02- 16855-а, 00-02-16263-а, 01-02-17743-а, 02-02-16246-а, 04-02-16743-а, 05-02-16902-а, 06-08-00950-а, 07-02-12080-офи, 07-08-00448-а, 08-02-00583-а, 09-02-00714-а), в том числе проектом № 07-02-00749-а, руководителем которого автор диссертации непосредственно являлся. Работа поддерживалась также грантами АФГИР (1ШР2-2660-М0-05) и МНТЦ (3360). Цели работы

1. Охарактеризовать состояние коллагена в соединительных тканях и установить взаимосвязь между структурными и термодинамическими параметрами стабильности коллагена в этих тканях с применением современных физико-химических методов.

2. Определить особенности действия локального лазерного нагрева на матрикс соединительных тканей и выявить факторы, определяющие стабильность коллагена в этих условиях.

Научная новизна

- Впервые проведено систематическое исследование термической стабильности коллагена в тканях с разной архитектоникой и химическим составом матрикса. Выявлена взаимосвязь между термодинамическими параметрами денатурации коллагена и свойствами матрикса. Установлено, что иммобилизация фибрилл и волокон коллагена за счет взаимодействия с сеткой протеогликановых агрегатов или внедрения их в более плотную ткань является важнейшим фактором стабильности коллагена в ткани. Нарушение целостности единой структуры органа приводит к значительному уменьшению термической стабильности коллагена.

- Впервые показано, что использование термодинамических характеристик денатурации коллагена в соединительных тканях совместно с химическим анализом и данными оптических методов визуализации, позволяет определять состояние коллагена в тканях и его изменения в результате патологических процессов или целевой модификации.

Разработанный в диссертации подход является достаточно эффективным и информативным методом определения структурных и функциональных особенностей матрикса соединительных тканей, а также их изменений при различных воздействиях на ткань.

- Впервые выявлено принципиальное различие в поведения коллагеновой сети в тканях с волокнистой структурой при осуществлении гидротермального и ИК лазерного нагрева умеренной интенсивности. Это различие обусловлено развитием механической дезорганизации упаковки коллагена в условиях нестационарного и неоднородного в пространстве температурного поля при ИК лазерном воздействии (фототермомеханический эффект). Показано, что при ИК лазерном нагреве денатурация коллагена в тканях возможна при температурах более низких, чем температура денатурации коллагена в калориметрической ячейке. Предложена схема модификации волокнистых структур ИК лазерным излучением умеренной интенсивности.

- Впервые показано, что характер изменений матрикса и его коллагеновой подсистемы зависит от состава и надмолекулярной организации матрикса ткани. В частности, в гиалиновом хряще носовой перегородки. дезорганизацию претерпевает главным образом протеогликановая подсистема, а в жировой ткани — подсистема триацилглицеридов. Практическая значимость

- Разработанная методология комплексного физико-химического анализа соединительных тканей, включающая термический и химический анализы, а также методики оптической диагностики, однозначно характеризует состояние коллагена в тканях, что дает молекулярную основу для разработки новых методов коррекции функции тканей.

- Обнаруженный фототермомеханический эффект воздействия - ИК лазерного излучения умеренной интенсивности на соединительные коллагенсодержащие ткани, приводящий к дезорганизации матрикса ткани, необходимо учитывать при разработке клинических методов модификации ткани с использованием лазерных технологий. Выявленная корреляция между составом и архитектоникой ткани и ее откликом на неоднородный нагрев дает важный вклад в разработку научных основ метода локальной гипертермии коллагенсодержащих тканей с патологическими изменениями.

Полученные в работе данные позволяют предсказать степень деградации соединительной ткани при термическом и ИК лазерном воздействии и дают научную основу выбора режимов ИК лазерного воздействия, обеспечивающего желаемый терапевтический эффект.

1. Предложен подход определения состояния коллагена в соединительных тканях на всех уровнях структурной иерархии, включающий в себя:

- определение содержания основных компонентов матрикса, а именно, коллагена с оценкой его типового состав и гликозаминогликанов;

- определение конформации (тройная спираль, случайный клубок) макромолекул коллагена (методы дифференциальной сканирующей калориметрии, спектроскопии комбинационного рассеяния, устойчивость к неспецифическим протеолитическим ферментам);

- определение упаковки макромолекул в фибриллы и фибрилл в волокна (метод электронной микроскопии);

- определение укладки фибрилл в волокна и волокон в пучки и/или ламеллы (методы многофотонной микроскопии с регистрацией генерации второй гармоники, световой микроскопии);

- определение общей анизотропии ткани (методом кросс-поляризационной оптической когерентной томографии);

- определение температуры перехода Тд (температура денатурации) макромолекул коллагена из конформации тройной спирали в случайный клубок (метод дифференциальной сканирующей калориметрии).

2. На примерах тканей глаза (склера, Тенонова капсула) показана связь между отклонениями физико-химических характеристик» матрикса соединительных тканей от контроля и направлением патологических изменений коллагенового каркаса ткани. Предложен и охарактеризован метод стабилизации матрикса за счет действия сшивающих агентов на коллагеновую сеть.

3. Установлено, что температура денатурации является характеристикой состояния коллагена, а ее отклонение от Тд выделенного фибриллярного коллагена определяется архитектоникой ткани, а именно:

- иммобилизацией (полной или частичной) фибрилл коллагена сеткой протеогликановых агрегатов в ткани гиалинового хряща;

- закреплением волокон коллагена фиброзного кольца в костной ткани;

- закреплением волокон коллагена надхрящницы формообразующих хрящей в зонах гиалинового хряща.

Факторы закрепления и иммобилизации влияют на энтропию случайного клубка полипептидных цепей коллагена, определяя температуру денатурации Тд=ДНд/Д8д.

4. Установлена закономерность последовательного увеличения термической стабильности коллагена в тканях, работающих преимущественно на растяжение, сжатие и сдвиг. В матриксе мениска обнаружено две подсистемы коллагена с разной термической стабильностью - круговые пучки и поперечные стяжки. Согласно установленной закономерности для этих подсистем выполнено отнесение функций: круговые пучки противостоят растяжению, а поперечные стяжки — сдвиговым деформациям.

5. Показано, что неоднородный нагрев волокнистых тканей под действием ИК лазерного излучения приводит к повреждениям термомеханического характера. При этом деградация коллагеновой сети на всех уровнях структурной иерархии происходит при температурах, существенно более низких, чем при гидротермальном нагреве. Фактор натяжения волокон, стабилизирующий коллаген при гидротермальном нагреве, оказывает противоположное дестабилизирующее действие.

6. Предложена схема модификации коллагена в соединительной' ткани- с волокнистой структурой в условиях нагрева ткани ИК лазерным излучением умеренной интенсивности. Эта схема включает последовательно следующие стадии:

- локальные конформационные переходы макромолекул коллагена из тройной спирали в случайный клубок;

- локальная деформация и разрывы в фибриллярной структуре;

- перераспределение натяжения по соседним макромолекулам и разрыв ослабленных фибрилл и волокон;

- объемное плавление коллагена.

Каждая стадия предложенной схемы обусловлена фототермическим или фототермомеханическим эффектом лазерного воздействия, либо их совместным влиянием.

7. Показано, что лазеро-индуцированные превращения матрикса соединительной ткани зависят от химического состава, морфологии и материальных свойств (в том числе гидравлической проницаемости) матрикса. Наименее стабильная подсистема подвергается деградации в первую очередь и определяет непосредственный отклик матрикса. В волокнистых тканях (связка, фиброзное кольцо) дезорганизация начинается с механической деструкции коллагеновой сети, в ткани гиалинового хряща - с разрушения протеогликановой подсистемы, в жировой ткани - с плавления, разрушения ассоциатов и окисления липидов. Химические (возрастные и патологические) изменения компонентов и состава матрикса приводят к изменению механических свойств ткани, тем самым специфически влияя на первичный отклик ткани к неоднородному лазерному нагреву. Найденные закономерности дают научную основу выбора оптимального режима ИК лазерного воздействия на соединительные ткань разных типов с целью получения желаемого терапевтического эффекта.
