Актуальность проблемы

Эректильная дисфункция определяемая, как постоянная неспособность мужчины достигать и/или поддерживать эрекцию, достаточную для осуществления полового акта [201], является в настоящее время одним из наиболее распространённых заболеваний мужской части населения планеты, частота встречаемости которого характеризуется выраженной тенденцией к росту [180]. Исследования патогенеза эректильной дисфункции показали, что в большинстве случаев она имеет сосудистое происхождение и связана с нарушением притока крови к кавернозным телам (так называемая артериогенная эректильная дисфункция) [181]. Это подтверждается также и данными эпидемиологических исследований: вероятность развития эректильной дисфункции резко возрастает у больных сахарным диабетом, артериальной гипертензией, гиперхолестеринемией, ожирением, а также курящих [61,80,112,179,208,214,226,238,239]. К настоящему времени нет общепризнанного объяснения этой взаимосвязи, однако известно, что все перечисленные заболевания оказывают неблагоприятное влияние на эндотелий, функциональное состояние которого играет важную роль в обеспечении адекватного кровоснабжения органов и тканей [6,109,121]. При эндотелиальной дисфункции, которая в настоящее время рассматривается в качестве первого этапа развития атеросклероза [6,22,23], имеет место снижение биоактивности оксида азота NO, являющегося не только основным вазодилататором, но и ключевым медиатором эрекции, обеспечивающим увеличение притока артериальной крови к кавернозным телам [38]. Таким образом, эндотелиальная дисфункция может являться одним из важных патогенетических механизмов развития эректильной дисфункции.

Исследование эндотелиальной функции артерий у человека in vivo впервые было выполнено Ludmer P.L. и соавт (1986), которые показали увеличение диаметра здоровых коронарных артерий и сужение поражённых атеросклерозом при введении в них ацетилхолина во время коронароангиографии [167]. В настоящее время широкое применение в диагностике эндотелиальной функции получила методика ультразвукового исследования посткомпрессионных изменений диаметра периферических артерий [156]. Подобный метод, применённый при исследовании кавернозных артерий [263], может явиться альтернативой существующим в настоящее время инвазивным тестам, однако данная методика в настоящее время окончательно не разработана. Также малоизученными остаются вопросы о состоянии системной эндотелиальной функции у больных эректильной дисфункцией и влиянии на функциональное состояние эндотелия пероральных ингибиторов фосфодиэстеразы 5 типа, являющихся на сегодняшний день препаратами выбора в лечении эректильной дисфункции.

В связи с очевидной актуальностью проблемы создания новых, неинвазивных методов диагностики артериогенной эректильной дисфункции, небольшим числом работ, посвященных объективным доказательствам роли локальной и системной эндотелиальной дисфункции в патогенезе эректильных нарушений, а также неясностью закономерностей влияния ингибиторов фосфодиэстеразы 5 типа на функциональное состояние эндотелия, мы сочли возможным предложить читателям нашу точку зрения на эти вопросы.

Цель работы:

Улучшение результатов лечения больных эректильной дисфункцией на основе специфической оценки функционального состояния эндотелия.

Для достижения данной цели мы поставили перед собой следующие задачи:

1. Исследовать состояние системной эндотелиальной функции у больных эректильной дисфункцией с применением метода ультразвукового исследования посткомпрессионных изменений диаметра плечевой артерии.

2. Оценить возможности предлагаемой усовершенствованной оригинальной методики исследования эндотелиальной функции кавернозных артерий в определении патогенетической формы эректильной дисфункции.

3. Изучить взаимосвязь между результатами ультразвукового исследования посткомпрессионных изменений диаметра кавернозных и плечевой артерий.

4. Выработать алгоритм оценки эндотелиальной функции с применением метода ультразвукового исследования посткомпрессионных изменений диаметра периферических артерий у больных эректильной дисфункцией.

5. Оценить влияние ингибиторов фосфодиэстеразы 5 типа на состояние эндотелиальной функции плечевой и кавернозных артерий и исследовать корреляцию между действием этих препаратов на эндотелиальную функцию и их эффективностью в лечении эректильной дисфункции.

Научная новизна:

1. На основании данных ультразвукового исследования посткомпрессионных изменений диаметра плечевой артерии показано присутствие системного сосудистого поражения у всех больных артериогенной эректильной дисфункцией.

2. На основании исследования состояния эндотелиальной функции кавернозных артерий с применением усовершенствованной оригинальной методики ультразвукового исследования посткомпрессионных изменений их диаметра установлена роль локальной эндотелиальной дисфункции в генезе артериогенной эректильной дисфункции.

3. На основании исследования системной и локальной эндотелиальной функции показано благоприятное влияние на функциональное состояние эндотелия приёма ингибиторов фосфодиэстеразы 5 типа.

4. Продемонстрирована перспективная возможность применения исследования эндотелиальной функции кавернозных артерий после приёма ингибиторов фосфодиэстеразы 5 типа в прогнозировании их клинического эффекта.

Практическая значимость:

1. Обоснована необходимость исследования системной эндотелиальной функции с применением метода ультразвукового исследования посткомпрессионных изменений диаметра плечевой артерии в обследовании больных эректильной дисфункцией.

2. Предложена усовершенствованная оригинальная методика исследования эндотелиальной функции кавернозных артерий и разработаны диагностические критерии оценки её результатов.

3. Показана высокая информативность усовершенствованной оригинальной методики исследования эндотелиальной функции кавернозных артерий в диагностике артериогенной эректильной дисфункции при применении порогового значения процентного увеличения диаметра кавернозных артерий 50%.

4. Разработан алгоритм обследования больных эректильной дисфункцией.

5. Проведена оценка влияния ингибиторов фосфодиэстеразы 5 типа на эндотелиальную функцию плечевой и кавернозных артерий.

6. Разработаны критерии прогнозирования клинической эффективности ингибиторов фосфодиэстеразы 5 типа по результатам исследования их влияния на эндотелиальную функцию кавернозных артерий.

Положения, выносимые на защиту:

1. Нарушение локальной эндотелиальной функции кавернозных артерий является ведущим патофизиологическим механизмом развития артериогенной эректильной дисфункции.

2. Основным диагностическим показателем оценки состояния локальной эндотелиальной функции кавернозных артерий является процентное увеличение диаметра кавернозных артерий, значения которого менее 50% характерны для артериогенной эректильной дисфункции.

3. Системная эндотелиальная дисфункция развивается раньше, чем нарушение локальной эндотелиальной функции и имеет место у всех больных артериогенной эректильной дисфункцией.

4. Препараты из группы ингибиторов фосфодиэстеразы 5 типа улучшают как системную, так и локальную эндотелиальную функцию, при этом степень улучшения зависит от исходного состояния функциональной способности эндотелия. Чем хуже исходное состояние, тем более выражено улучшение.

5. Клиническая эффективность препаратов из группы ингибиторов фосфодиэстеразы 5 типа у больных артериогенной эректильной дисфункцией определяется их действием на функциональное состояние эндотелия кавернозных артерий.

Структура и объём работы

Диссертация изложена на 188 страницах, состоит из введения, обзора литературы (глава 1), общей характеристики больных, методов диагностики и лечения (глава 2), 2-х глав собственных исследований, заключения, выводов, практических рекомендаций, списка литературы (30 отечественных и 260 зарубежных источников) и приложения. Работа содержит 32 таблицы и 40 рисунков.

ВЫВОДЫ

1. Признаки системной эндотелиальной дисфункции по результатам ультразвукового исследования посткомпрессионных изменений диаметра плечевой артерии имеют место у всех больных артериогенной эректильной дисфункцией, в то время как у больных нейрогенной, веноокклюзивной и психогенной эректильной дисфункцией их частота не отличается от таковой в контрольной группе.

2. Усовершенствованная оригинальная методика ультразвукового исследования посткомпрессионных изменений диаметра кавернозных артерий позволяет установить диагноз артериогенной эректильной дисфункции.

3. Нарушение функции эндотелия плечевой артерии наступает раньше, чем кавернозных, что делает необходимым выполнять ультразвуковое исследование посткомпрессионных изменений диаметра плечевой артерии на первом этапе алгоритма обследования больных эректильной дисфункцией.

4. Приём ингибиторов фосфодиэстеразы 5 типа сопровождается улучшением состояния эндотелиальной функции плечевой и кавернозных артерий, при этом, чем ниже исходные показатели этой функции, тем более выражено её улучшение.

5. Клиническая эффективность ингибиторов фосфодиэстеразы 5 типа у больных артериогенной эректильной дисфункцией коррелирует с их действием на эндотелиальную функцию кавернозных артерий.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

1. Выполнение ультразвукового исследования посткомпрессионных изменений диаметра кавернозных артерий показано больным эректильной дисфункцией для выявления артериогенной эректильной дисфункции.

2. Ультразвуковое исследование посткомпрессионных изменений диаметра кавернозных артерий следует выполнять дистальнее уровня компрессии, оценивая динамику изменений обеих артерий.

3. Критерием оценки состояния эндотелиальной функции кавернозных артерий является процентное увеличение диаметра кавернозных артерий, значение которого менее 50% указывает на артериогенный генез эректильной дисфункции.

4. Начинать оценку эндотелиальной функции у больных эректильной дисфункцией рекомендуем с ультразвукового исследования посткомпрессионных изменений диаметра плечевой артерии. При его увеличении менее либо равном 15% следует проводить исследование кавернозных артерий; при увеличении более 15% от исследования кавернозных артерий можно воздержаться.

5. Целесообразно назначать препараты из группы ингибиторов фосфодиэстеразы 5 типа больным артериогенной эректильной дисфункцией, у которых процентное увеличение диаметра кавернозных артерий после их приёма превышает 50%.
