Актуальность проблемы.

Вирусные гепатиты продолжают оставаться одной из серьезных проблем здравоохранения во всем мире. Наиболее сильный ущерб здоровью населения наносят вирусные гепатиты с парентеральным механизмом инфицирования (В, С, D, TTV, G), так как они приводят не только к развитию острого заболевания печени, которое может реализоваться в том числе в фульминантной форме но и к развитию хронического гепатита, цирроза печени и даже гепатоцеллюлярной карциномы.

Представителями группы риска инфицирования вирусными гепатитами с парентеральным механизмом инфицирования являются больные с соматическими заболеваниями, наркоманы, работники здравоохранения и др. Присоединение вирусного гепатита у больных с соматическими заболеваниями приводит к ухудшению их состояния, сужает терапевтические возможности в отношении основной патологии и в целом ухудшает прогноз этих пациентов. При этом есть основания считать, что данные о частоте вирусных гепатитов у пациентов соматических стационаров занижены, поскольку большинство авторов сообщают лишь о клинически выраженных формах инфекции. Атипичные формы болезни диагностируются редко и им не уделяется должного внимания, хотя в эпидемиологическом отношении они наиболее опасны. (Uchihara М., Izumi N. Yoshiniori S. et al., 1998).

Изучение особенностей течения вирусных гепатитов у детей с соматической патологией имеет большое значение, т.к. дает возможность определения оптимальной тактики ведения больных с вирусными гепатитами на фоне тяжелой соматической патологии, а также разработке рациональной противовирусной терапии.

Лечение хронических гепатитов представляет собой серьезную и далеко не до конца разрешенную проблему. Наиболее широко применяемым препаратом для лечения хронических вирусных гепатитов является рекомбинантный а-интерферон (ИФН). Однако, большинство работ, посвященных лечению хронических вирусных гепатитов в педиатрической практике касаются детей, не страдающих соматическими заболеваниями. Особенно это относится к вопросам разработки комбинированного лечения хронических вирусных гепатитов у детей с применением препаратов рекомбинантного ИФН-а и противовирусных средств (рибавирин, ремантадин, амантадин, фосфоглив и др.).

В доступной нам литературе мы не нашли данных, касающихся эффективности интерферонотерапии вирусных гепатитов у детей с тяжелыми соматическими заболеваниями. До настоящего времени не изучена распространенность генотипов вирусов гепатита Ви С у российских детей, страдающих соматической патологией и не определено влияние интерферонотерапии хронических вирусных гепатитов у данной категории больных. Не разработаны схемы лечения хронических вирусных гепатитов у детей с соматической патологией. С учетом огромной значимости проблемы, настоящая работа посвящена изучению особенностей течения вирусных гепатитов и эффективности интерферонотерапии хронических вирусных гепатитов у детей с соматической патологией.

ЦЕЛЬ И ЗАДАЧИ ИССЛЕДОВАНИЯ.

Цель работы: Изучить особенности течения и совершенствовать терапию хронических вирусных гепатитов у детей с тяжелыми соматическими заболеваниями.

Задачи исследования:

1. Исследовать этиологическую структуру острых и хронических вирусных гепатитов у детей с различными соматическими заболеваниями.

2. Установить частоту выявления различных генотипов HBV и HCV среди детей с хроническим гепатитом В и С, протекающим на фоне соматической патологии.

3. Исследовать особенности течения острого и хронического гепатита В, С и смешанной этиологии у детей с соматической патологией на современном этапе.

4. Изучить эффективность интерферонотерапии хронического гепатита В и С у детей с соматической патологией.

НОВИЗНА ИССЛЕДОВАНИЯ.

Определена этиологическая структура острых и хронических вирусных гепатитов у детей с различными соматическими заболеваниями на современном этапе.

Исследованы особенности течения острых и хронических гепатитов В, С и смешанной этиологии у детей с соматической патологией.

Установлена частота выявления различных генотипов HBV и HCV среди детей с хроническим гепатитом В и С, протекающим на фоне соматической патологии.

Изучена эффективность интерферонотерапии хронического гепатита В и С у детей с соматической патологией.

ПРАКТИЧЕСКАЯ ЗНАЧИМОСТЬ

Установлены особенности течения острых и хронических вирусных гепатитов различной этиологии у детей с соматическими заболеваниями.

Изучено влияние противоопухолевой полихимиотерапии на течение острых и хронических вирусных гепатитов различной этиологии у детей с гемобластозами и злокачественными солидными опухолями.

Изучена эффективность различных схем интерферонотерапии хронического гепатита В и С у детей с тяжёлыми заболеваниями в зависимости от генотипа вируса и характера соматической патологии.

ВНЕДРЕНИЕ ПОЛУЧЕННЫХ РЕЗУЛЬТАТОВ В ПРАКТИКУ.

Результаты диссертации внедрены в практику работы Российской Детской клинической больницы МЗ РФ, НИИДОГ РОНЦ им Н.Н.Блохина РАМН, детской городской клинической больницы №9 и Морозовской детской клинической больницы г. Москвы.

Основные положения работы используются при проведении учебных занятий и чтении лекций на кафедре детских инфекционных болезней с курсом вакцинопрофилактики ФУВ РГМУ, а также включены в методические пособия для студентов, интернов, ординаторов и врачей.

АПРОБАЦИЯ ДИССЕРТАЦИИ.

СТРУКТУРА И ОБЪЕМ ДИССЕРТАЦИИ.

Диссертация состоит из введения, обзора литературы, 5 глав собственных исследований, заключения и указателя литературы, включающего 27 отечественных и 316 иностранных источников. Диссертация изложена на 243 стр. машинописного текста, содержит 15 выписок из истории болезни, 35 таблиц и 62 рисунка.

ВЫВОДЫ.

1. Больные онкогематологическимн заболеваниями, гемофилией и пациенты гемодиализа составляют группу высокого риска инфицирования HBV и HCV. Через 3 года от начала наблюдения маркёры гепатитов В и С обнаруживаются в 60-80% случаев. Заражение происходит преимущественно через препараты крови.

2. Клинические проявления гепатитов В и С у детей с соматической патологией принципиально не отличаются от таковых у детей без соматической патологии. Однако, заболевание в подавляющем большинстве случаев ( 94,5% при гепатите В и 75,6% - при гепатите С) протекает как первично-хронический гепатит.

3. У детей с соматической патологией течение ХГВ характеризуется постепенным снижением выраженности клинико-лабораторных проявлений, тогда как при гепатите С наоборот, патологический процесс неуклонно прогрессирует. При этом, у детей, получавших полихимиотерапию цитостатиками хронические гепатиты В и С протекают тяжелее, чем у пациентов, не получавших цитостатическую терапию.

4. Более чем у половины детей с HBV-инфекцией (53,3%), протекающей на фоне соматических заболеваний обнаруживается генотип D. Генотип А выявляется у 5,6%, Е - у 4,5%, одновременно несколько генотипов - у 13,3%. Почти у 1А больных генотип HBV не классифицируется. Генотипы В, С, F не обнаруживаются.

5. Течение ХГВ у детей с соматической патологией зависит от генотипа HBV: у больных с генотипом D выявляются более выраженные клинико-лабораторные проявления болезни, чем у больных, имеющих другие генотипы HBV.

6. При хроническом гепатите В у детей с соматической патологией, леченых препаратами рекомбинантного интерферона а, первичная биохимическая ремиссия формируется у 10%, стабильная биохимическая ремиссия - у 13,4%, полная длительная ремиссия - у 24,5%, отсутствие ремиссии - у 40% больных.

7. Эффективность интерферонотерапии при хроническом гепатите В у детей с соматической патологией не зависит от генотипа HBV.

8. При HCV-инфекции у детей с соматическими заболеваниями наиболее часто обнаруживается генотип lb (21,6%). Доля генотипа 1а составляет 9,5%, 2а - 12,2%, 2Ь - 9,5%, За - 10,8%, ЗЬ - 9,5%, 4 - у 17,6%. У 9,3% больных одновременно определяются несколько генотипов HCV. Генотипы 5 и 6 не выявляются.

9. При хроническом гепатите С у детей с соматическими заболеваниями, леченых препаратами рекомбинантного интерферона а, первичная биохимическая ремиссия формируется у 8,1%, длительная биохимическая ремиссия - у 9,5%, длительная вирусологическая ремиссия - у 10,8%, полная длительная ремиссия - у 21,6%, отсутствие ремиссии - у 43,2% больных.

10. У больных с генотипом lb лечение препаратами рекомбинантного интерферона а не эффективно, ремиссия отсутствует. У пациентов с генотипом 4 длительная полная ремиссия формируется в 38,5%, у детей с другими генотипами HCV - в 28,9% случаев.

11. Эффективность лечения при хронических гепатитах В и Су детей с соматической патологией не имеет различий при использовании парентеральных и ректальных форм рекомбинантного интерферона альфа.

Практические рекомендации.

1. Все больные с гемофилией, хронической почечной недостаточностью, дети с гемобластозами, злокачественными солидными опухолями должны быть обследованы не только на HBsAg, и анти-HCV, но также на анти- НВсоге, анти-HGV, ДНК HBV, РНК HCV, РНК HDV, РНК HGV сразу после установления диагноза соматического заболевания.

2. При возникновении вирусного гепатита любой этиологии у детей с гемобластозами и злокачественными солидными опухолями возможно проведение циклов противоопухолевой полихимиотерапии, так как она не усугубляет в значительной мере патологический процесс в печени.

3. Наличие соматического заболевания (гемобластозы, злокачественные солидные опухоли, гемофилия, хроническая почечная недостаточность) не является противопоказанием для интерферонотерапии и комбинированной терапии препаратами рекомбинантного ИФН-а и Фосфогливом.

4. Определение генотипа HBV перед началом интерферонотерапии хронического гепатита В у детей с соматической патологией может не проводиться, так как генотип HBV не оказывает влияния на эффективность лечения.

5. Определение генотипа HCV перед началом интерферонотерапии хронического гепатита С у детей с соматической патологией является обязательным, так как у больных с генотипом lb на фоне лечения ремиссия отсутствует, тогда как у пациентов с генотипом 4 длительная полная ремиссия формируется в 38,5%, у детей с другими генотипами HCV - в 28,9% случаев.

6. При хронических вирусных гепатитах у детей с соматической патологией лечение рекомбинантными интерферонами при отсутствии эффективности следует прекратить через 3-6 месяцев.
