ВВЕДЕНИЕ

Актуальность исследования. В последние годы было выявлено множество факторов, влияющих на течение и клинические проявления депрессии, одним из которых является пол пациента (Тювина Н.А., 2003; Краснов В.Н., Zierau et al., 2002). Несмотря на то что некоторые исследователи подчеркнули необходимость учета гендерного фактора при диагностике депрессии (Cyranowsky J.M. et al., 2000), международные классификаторы болезней (DSM, МКБ) до сих пор не включают специфические гендерные симптомы в свои диагностические критерии.

Известно, что депрессия чаще встречается у женщин, чем у мужчин (Verhagen M. et al., 2010; Fernandez-Pujals A.M. et al., 2015). Это сместило фокус исследований на поиск причин такой распространенности депрессии среди женщин (Salk R.H., Hyde J.S., Abramson L.Y., 2017), в результате чего особенности физиологических и психологических процессов у мужчин, влияющих на клинику заболевания, остались недостаточно изученными (Flint J., Kendler K.S., 2014).

Мужчины совершают суициды в несколько раз чаще (Yang X. et al., 2017; Oliffe J.L., 2019), вдвое чаще злоупотребляют алкоголем и наркотиками (Yucel K. et al., 2013) и совершают больше насильственных действий (Chuang J-Y et al., 2017). Многочисленные исследования показывают, что агрессивное поведение и раздражительность являются основными симптомами, отличающими мужскую депрессию от женской (Rajkumar R.P., Kumaran A.K., 2015; Call J.B., Shafer K., 2018). Анализ стратегий преодоления угрозы маскулинности выявил склонность мужчин к гипертрофированным мужским реакциям и избеганию женских стилей поведения (Cheryan S. et al., 2015). Агрессия часто используется ими как способ снижения угрозы и восстановления маскулинности (Bosson J.K.,

2009). Применение таких малоэффективных стратегий удлиняет течение депрессии, затрудняет ее выявление и откладывает начало лечения (Brownhill S. et al., 2005; Hausmann A., Rutz W., Meise U., 2008).

Депрессия значительно чаще наблюдается у пациентов с соматическими заболеваниями по сравнению с физически здоровыми людьми (Egede L.E., 2007; Moussavi S. et al., 2007; Bhattacharya R. et al., 2014; Boing A.F. et al., 2012). Существуют данные о специфических особенностях депрессии при различных соматических заболеваниях, что усложняет их диагностику (Корнетов Н.А., Жоров В.А., 2008). Наличие депрессии также может влиять на клинические проявления, течение и терапию основного заболевания (Крючкова М.Н., Солдаткин В.А., 2017). Известно, что мужчины реже обращаются за медицинской помощью, как по поводу физических, так и психических проблем (Galdas P.M. et al., 2005; Fisher K.A. et al., 2021). Низкая активность мужчин в поиске помощи приводит к ряду проблем, поскольку раннее вмешательство повышает выживаемость и снижает бремя прямых и косвенных расходов (Galdas P.M. et al., 2005).

Депрессия у мужчин может проявляться нетипичными симптомами, отличающимися от стандартных признаков депрессивного синдрома, что затрудняет ее диагностику по общепринятым критериям (Kilmartin C.T., 2013; Salk R.H. et al., 2017). Поскольку различия в распространенности депрессии обычно не выявляются до подросткового возраста, исследования предполагают, что это может быть связано с особенностями работы эндокринной системы у мужчин и женщин (Тювина Н.А., Балабанова Е.О., 2018; Adreano J.M. et al., 2018; Hwang W.J. et al., 2020). Известно, что гонадотропные гормоны, особенно тестостерон, влияют на регуляцию эмоциональных реакций и настроения, что может объяснять половые различия в эмоциональном статусе (Klein K. et al., 2018; Grotzinger A.D. et al., 2018; Kaldewaij R. et al., 2019).

Ряд исследований свидетельствует о повышенном риске развития депрессии у пациентов со сниженным уровнем тестостерона (Amanatkar H.R. et al., 2016; Wooderson S.C. et al., 2015). Кроме этого, тестостерон влияет на дофаминергическую систему, увеличивая выброс дофамина в мезолимбической системе, стриатуме и прилежащем ядре (Oyola M.G., Handa R.J., 2017), одновременно с этим отмечается возможное увеличение плотности серотониновых рецепторов, доступных для связывания (Kranz G.S. et al., 2015).

В контексте фармакологической помощи важно отметить, что мужчины обращаются за терапией депрессии реже, чем женщины (Liddon L. et al., 2018). Однако, если они все же начинают лечение, у них чаще наблюдается склонность к самостоятельному прекращению приема препаратов. Это указывает на необходимость проведения более интенсивной психообразовательной работы с мужчинами и разъяснения важности продолжения поддерживающей терапии, даже при отсутствии симптомов депрессии (Kilmartin C.T., 2013).

Несмотря на десятилетия исследований гендерных различий в эффективности антидепрессантов, четких выводов о наличии таких различий до сих пор нет (Sramek J.J. et al., 2016). Существуют работы, указывающие на положительное влияние заместительной терапии тестостероном на симптомы депрессии (Walther A. et al., 2019). Однако делать однозначные заключения преждевременно, поскольку дизайн большинства этих исследований имеет существенные недостатки (Giltay E.J. et al., 2017).

Степень разработанности темы исследования. Существует значительное количество исследований, посвященных гендерной специфике женской депрессии и ее отличиям от мужской. Однако число работ, изучающих особенности мужской депрессии на фоне дефицита тестостерона, крайне

ограничено, и многие из них были проведены достаточно давно (Salk R.H., Hyde J.S., Abramson L.Y., 2017). Как в мировой, так и в отечественной науке недостаточно исследований, оценивающих психопатологические особенности депрессий у данной группы пациентов (Zierau F. et al., 2002; Winkler D. et al., 2004; Brownhill S. et al., 2006; Степанов И.Л., Горячева Е.К., 2012; Иванец Н.Н., 2018). Кроме того, несмотря на многолетние исследования гендерных различий в терапии антидепрессантами, до сих пор нет четких выводов об эффективности схем лечения депрессий на фоне гипотестостеронемии (Sramek J.J. et al., 2016; Giltay E.J. et al., 2017).

Таким образом, особый интерес представляет изучение депрессии у мужчин с дефицитом тестостерона. Результаты данного исследования помогут снизить социальные и личностные проблемы пациентов, что поспособствует повышению эффективности терапевтического и реабилитационного процесса. Анализ состояния проблемы депрессии у мужчин со сниженным уровнем тестостерона свидетельствует об актуальности вопроса, его приоритетности и нерешенности поставленных задач.

Цель исследования. Повышение эффективности комплексной терапии депрессии у мужчин с дефицитом тестостерона посредством разработки лечебно-диагностического алгоритма.

Задачи исследования. В соответствие с поставленной целью были выделены следующие задачи исследования:

1. Изучить и сопоставить психопатологические особенности депрессии у мужчин с дефицитом тестостерона и с нормальным его уровнем.

2. Сравнить эффективность подходов к терапии депрессии у мужчин с дефицитом тестостерона: тимоаналептической терапии, гормональной коррекции и комплексной терапии.

3. Интегрировав полученные данные, разработать алгоритм комплексной диагностики и терапии мужчин с дефицитом тестостерона, страдающих депрессией.

Научная новизна. Получены новые данные о клинической особенностях депрессии у мужчин на фоне дефицита тестостерона. Впервые получены данные об эффективности комбинированной схемы лечения антидепрессантом (сертралином) и препаратом тестостерона, монотерапии антидепрессантом и монотерапией препаратом тестостерона в сравнении друг с другом.

Теоретическая и практическая значимость работы. Полученные результаты вносят научный вклад в изучение депрессии у мужчин на фоне дефицита тестостерона, позволяют глубже понимать феноменологические особенности изучаемой патологии. Изучение особенностей психопатологической картины, имеющих место при депрессии при сопутствующей гипотестостеронемии, позволяет открыть новые возможности для диагностики данного расстройства. Установленные в ходе исследование особенности эффективности разных схем лечения при депрессии у мужчин на фоне дефицита тестостерона способствуют оптимизации оказания качественной медицинской помощи данным пациентам. Выявленные закономерности могут быть использованы в повседневной деятельности врачей-психиатров, врачей-урологов, врачей-андрологов и врачей-эндокринологов стационарного и амбулаторного звена.

Основные положения, выносимые на защиту.

1. Депрессия у мужчин с дефицитом тестостерона характеризуется специфической клинической картиной — сексуальными нарушениями (снижение либидо и/или эректильной функции), выраженной эмоциональной лабильностью, повышенной раздражительностью и нарушениями сна

(поверхностным и чутким), что приводит к преобладанию астено-депрессивного синдрома.

2. Мужчины с депрессией на фоне дефицита тестостерона характеризуются более поздним дебютом заболевания (Ме 47 лет [44 ; 51] против 38 лет [35 ; 42]; р < 0,001). Это возрастное смещение обусловливает более высокую долю пациентов, состоящих в браке, имеющих > 1 ребёнка и закончивших средне-специальное образование, тогда как статистически значимых различий по уровню дохода и характеру занятости между группами не выявлено.

3. Лечение депрессии у мужчин с дефицитом тестостерона оптимально при комбинированной терапии: монотерапия тестостероном не обеспечивает достаточного антидепрессивного эффекта, а монотерапия антидепрессантом не корректирует сопутствующие сексуальные и гормональные нарушения. Сочетание антидепрессанта с коррекцией уровня тестостерона позволяет добиться более выраженного терапевтического эффекта, хотя требует тщательного мониторинга нежелательных явлений.

Степень достоверности результатов проведенного исследования.

Степень достоверности проведенного научного исследования обеспечена репрезентативностью выборки (114 наблюдений - пациенты с депрессией на фоне дефицита тестостерона), корректностью и комплексностью методов исследования (клинико-психопатологический, психометрический,

лабораторный и статистический) соответствуют поставленным задачам. Подробный анализ степени разработанности темы позволил корректно сформулировать цель и задачи исследования, обосновать выбор объекта исследования, а также выполнить научно квалификационную работу с опорой на общепринятые в данной отрасли науки методологические подходы и научно-

методический аппарат. Результаты проведенного исследования соответствуют области исследования научной специальности «Психиатрия и наркология».

Апробация результатов исследования. Внедрение результатов исследования. Данные, полученные в диссертационном исследовании, внедрены в практическую деятельность ГБУ Ростовской области «Психиатрическая больница»; используются в образовательном процессе на кафедре психиатрии и наркологии ФГБОУ ВО РостГМУ.

Публикация результатов исследования. Основные результаты научных исследований изложены в 20 опубликованных печатных работах, из них 6 - в научных изданиях, рекомендованных ВАК РФ, 1 руководство, 1 патент.

Личный вклад автора. Разработка дизайна исследования и определение методологии исследования, анализ литературных данных по теме исследования, реализация программы и этапов настоящего исследования, формирование выборки, изучение клинических особенностей при депрессии у мужчин на фоне дефицита тестостерона, изучение эффективности рассматриваемых схем лечение, анализ и обобщение полученных данных, статистическая обработка материала и написание всех глав диссертации выполнено лично автором.

Соответствие диссертации паспорту специальности. Диссертация соответствует паспорту специальности ВАК 3.1.17 - «Психиатрия и наркология». Диссертация включает в себя такие области исследований, как общая и частная психопатология, частная психиатрия, клиника, диагностика и лечение психических расстройств, профилактика психических заболеваний.

Объем и структура диссертации. Диссертация оформлена по национальному стандарту РФ ГОСТ Р7.0.11- 2011. Диссертация изложена на 219 страницах машинописного текста. Состоит из введения, 5 глав собственных исследований, заключения, выводов, списка литературы, 3 приложений. В списке литературы представлено 397 источников, из них 56 отечественных и 341 зарубежных. Работа содержит 18 таблиц и 24 рисунка. Представлен анализ 2 клинических наблюдений.

ДЕПРЕССИЯ У МУЖЧИН С ДЕФИЦИТОМ ТЕСТОСТЕРОНА: КЛИНИЧЕСКИЕ И ТЕРАПЕВТИЧЕСКИЕ АСПЕКТЫ

(ОБЗОР ЛИТЕРАТУРЫ)

В последние годы исследования депрессии выявили множество особенностей и различий в ее течении, обусловленных разнообразными факторами. Одним из факторов, существенно влияющих на клинику, лечение и профилактику депрессии, является пол пациента (Тювина Н.А., 2003; Краснов В.Н., Zierau et al., 2002). Так, клинические проявления депрессии у мужчин могут значительно отличаться от таковых у женщин, что часто затрудняет ее диагностику и повышает различные риски в этой гендерной группе. Хотя некоторые исследователи подчеркивают необходимость учета гендерного фактора при диагностике депрессии (Cyranowsky J.M. et al., 2000), международные классификации болезней (DSM, МКБ) все еще не включают гендерно специфические симптомы в свои диагностические критерии.

Понятия мужского здоровья и депрессии редко рассматриваются вместе. Однако в последнее время внимание к мужскому здоровью и обсуждению роли тестостерона значительно возросло (Martin L.A. et al., 2013; Derry H.M. et al., 2015). Накопление научных данных позволяет уверенно предполагать тесную связь между мужским здоровьем, уровнем тестостерона и депрессией (Martin L.A. et al., 2013; Thapar A. et al., 2012; Flint J., Kendler K.S., 2014).

1.1. Распространенность мужских депрессий и возможные факторы, влияющие на их выявляемость

Наблюдаемый факт, что распространённость депрессии среди женщин вдвое выше (Verhagen и др., 2010; Fernandez-Pujals и др., 2015), чем

среди мужчин, привёл к тому, что большинство научных исследований в этой области сосредоточены на выяснении причин, по которым женщины находятся в группе высокого риска (Salk и др., 2017). Объяснения предлагались в рамках различных теоретических подходов: женщины более склонны к негативной оценке событий, межличностных отношений и восприятию собственного тела (Moscicki, 1995; Sue, 2003); они демонстрируют усиленный ответ на стресс (Солдаткин, 2010; Swetlitz, 2021); генетические исследования показали, что наследственная предрасположенность к депрессии более характерна для женщин (Owens и Nemeroff, 2002; Sassarini, 2016). Были выявлены гендерно -специфические различия в функционировании системы моноаминов (Sakai и др., 2006) и эндокринной системы (Young и др., 1984). Кроме того, женщины более подвержены воздействию факторов, провоцирующих воспалительные процессы, которые, как известно, могут влиять на развитие депрессии (Staley и др., 2001), и в связи с этим чаще обращаются за помощью к психиатрам (Meltzer, 2001; Swetlitz, 2021; Sassarini, 2016).

Однако такой акцент на объяснении высокой распространённости депрессии среди женщин привёл к тому, что мужчины и особенности их физиологических и психологических процессов, способных изменять клиническую картину заболевания, оказались вне поля зрения исследователей (Flint и Kendler, 2014). Между тем, авторы других работ указывают на то, что у мужчин проявляются "альтернативные" симптомы депрессии, и при их учёте разница в распространённости значительно снижается (Sofuoglu и др., 2004). Так, Zierrau с соавторами (2002), используя Готландскую шкалу мужской депрессии, обнаружили, что при учёте дополнительных симптомов, характерных для мужчин, предположительный диагноз депрессии можно было установить в 39% случаев, тогда как при использовании стандартных опросников признаки депрессии выявлялись лишь у 17% мужчин.

Исследование, включавшее 78 458 участников из шести европейских стран, также подтвердило большую распространённость депрессии среди женщин, но только в рамках большого депрессивного расстройства (American Psychiatric Association. DSM-5, 2013); при снижении необходимого количества диагностических критериев эта разница уменьшалась и почти полностью исчезала (Van der Plas и др., 2010). Авторы пришли к выводу, что различия в распространённости депрессии могут зависеть от количества симптомов, о которых сообщают пациенты. Мужчины с депрессией склонны сообщать о меньшем количестве симптомов вследствие своих психологических особенностей, и если диагностические инструменты требуют наличия определённого минимального числа симптомов, это может приводить к недодиагностированию депрессии у мужчин.

Несмотря на то что распространённость депрессии среди женщин вдвое выше, мужчины совершают суициды в несколько раз чаще (Yang и др., 2017; Солдаткин и др., 2011; Negri-Cesi и др., 2004; Oliffe, 2019), вдвое чаще злоупотребляют алкоголем и наркотиками (Gilmore и др., 2007; Boes и др., 2008; Yucel и др., 2013) и совершают большее количество насильственных действий (Chuang и др., 2017). Эти показатели могут отражать недиагностированную мужскую депрессию, остающуюся без необходимой терапии.

1.2 Клинические особенности течения депрессии у мужчин

В настоящее время диагностика депрессии основывается преимущественно на клиническом мышлении врача, который способен взвешивать индивидуальные особенности пациента, а не только полагаться на шкалы и классификаторы.

Исследование 217 пациентов (113 мужчин) выявило следующие гендерные различия в проявлениях депрессии: мужчины чаще жаловались на снижение либидо, агрессию, импульсивное поведение и были более

ипохондричны. Однако стоит отметить, что хотя предъявляемые жалобы зависели от пола, тяжесть депрессии такой взаимосвязи не имела (Stoodley C.J., Schmahmann J.D., 2010). Magovcevic M. и Addis M.E. (2008) также продемонстрировали большую экстернализацию симптомов депрессии у мужчин и высокий процент соматических жалоб. Однако это было характерно для мужчин, придерживающихся традиционных представлений о мужественности, в отличие от тех, кто придерживается менее традиционных взглядов (Tekin S., Cummings J.L., 2002; Zhang H. и др., 2016). Внешнюю ориентированность симптомов у мужчин отмечают и другие авторы (Lemogne C. и др., 2012; Thayer J.F. и др., 2003; Koenigs M. и др., 2008), относя к этому понятию раздражительность, рискованное поведение, чрезмерную увлечённость работой или спортом и злоупотребление психоактивными веществами (ПАВ) (Kronmüller K.T., et al., 2009).

Злоупотребление ПАВ остаётся важным клиническим проявлением при мужских депрессиях (Salk R.H., Hyde J.S., 2017). Мужчины чаще склонны к употреблению алкоголя в период депрессии (Sramek J.J., Cutler N.R., 2011; Oliffe J.L. и др., 2019), а в исследовании коморбидности употребления ПАВ и депрессии было обнаружено, что среди пациентов с депрессией в общесоматическом звене 8,3% женщин и 19% мужчин сообщали о злоупотреблении алкоголем (Lorenzetti V., 2009). Именно злоупотребление ПАВ может стать поводом для обращения к специалисту, что необходимо учитывать при выявлении депрессии.

Многочисленные исследования показали, что агрессивное поведение и раздражительность являются одними из основных симптомов, отличающих проявления мужских депрессий от женских, и выявляются в большинстве работ (Angst J. и др., 2002; Rajkumar R.P., Kumaran A.K., 2015; Call J.B., Shafer K., 2018). Однако остаётся неясным, является ли агрессия у мужчин с депрессией симптомом заболевания или попыткой преодолеть болезнь в соответствии с представлениями о маскулинности (Benazzi F., Akiskal H.S., 2005). Так, в анализе стратегий преодоления угроз

маскулинности было обнаружено, что мужчины склонны к гипертрофированному мужскому стилю реагирования и избеганию «женского» стиля (Cheryan S. и др., 2015). Они часто используют агрессию как один из возможных способов снижения угрозы и восстановления маскулинности (Bosson J.K., 2009). Использование подобных, малоэффективных стратегий удлиняет время течение депрессии, снижает её выявляемость и откладывает начало лечения (Brownhill S. и др., 2005; Hausmann A., Rutz W., Meise U., 2008).

Однако индивидуальные реакции достаточно разнообразны. Существует множество мужчин, которые не любят спорт, и множество женщин, которые его любят. Так же как многие мужчины могут быть более эмоциональными, чем некоторые женщины. Описываемый внешний тип реагирования более соотносится с понятием маскулинности, чем с понятием «мужчина», поскольку некоторые мужчины могут проявлять классические симптомы депрессии, в то время как некоторые женщины могут обнаруживать признаки маскулинной депрессии. Однако из-за культуральных особенностей, связанных с соответствием гендерным ожиданиям, большинство испытывающих маскулинную депрессию всё же могут её скрывать (Kilmartin C.T., 2013). Это может объяснить и тот факт, что мужчины в два раза реже сообщают о желании плакать или слезливости, чем женщины (Angst J. и др., 2002). Также мужчины чаще сообщали о сниженном настроении и переживании депрессии в группах, которые собирались только один раз; если же группы собирались несколько раз, то мужчины, в отличие от женщин, склонны занижать количество депрессивных симптомов. Это можно объяснить тем, что мужчины воспринимают отсутствие дальнейшего контакта с группой как менее опасную ситуацию для их статуса (Sigmon S. и др., 2005).

В работах Marcus S.M. et al. (2005) и Gaynes B.N. (2008) исследовательский фокус сместился с психологических особенностей депрессии на клинические проявления у мужчин. Так, было показано, что у

мужчин имеет место более позднее начало первого депрессивного эпизода и его меньшая продолжительность. Иванец Н.Н. с соавторами (2018) подробно исследовали общепсихопатологические проявления депрессии у мужчин, обнаружив следующие клинические различия: для мужчин было характерно изменение двигательной активности в виде заторможенности и снижение мотивации. Среди психических симптомов у женщин достоверно чаще встречались идеи самообвинения и самоуничижения, а также избегание контактов с окружающими, что несколько противоречит результатам работы J. Angst et al. (2002), где отмечается большая вовлечённость женщин, страдающих депрессией, в социальные контакты, тогда как мужчины чаще сокращали социальные взаимодействия.

Одним из вопросов, которые исследователи поднимают относительно специфики мужских симптомов депрессии, является то, представляет ли она собой отдельный тип депрессии или же является коморбидным расстройством с такими состояниями, как расстройства личности, зависимость от психоактивных веществ (ПАВ), проявления биполярного аффективного расстройства или синдром дефицита внимания с гиперактивностью (Барденштейн Л.М., Славгородский Я.М., 2013; Shah R., Zanarini M.C., 2018). Некоторые авторы утверждают, что поскольку нарушение стрессоустойчивости, включая раздражительность, часто рассматривается как наиболее распространённый симптом мужской депрессии, следует отметить, что этот же симптом вдвое чаще встречается при биполярном аффективном расстройстве (Rihmer Z., 2019). Также существует мнение, что мужская депрессия может быть более связана с биполярным аффективным расстройством, чем с униполярной депрессией. При этом биполярная депрессия чаще ассоциируется с нарушениями поведения в подростковом возрасте, а риск суицида в группе пациентов с БАР значительно выше, чем при униполярной депрессии (Rihmer Z., Kiss K., 2002; Осипова Н.Н. и соавт., 2018). Если эти данные подтвердятся, то в

терапии мужских депрессий необходимо применять алгоритмы лечения БАР.

Принимая во внимание устоявшуюся традицию российской психиатрии, которая придает значительное значение не только доминирующим симптомам у пациента, но и определяющему синдрому, мы провели исследование клинических характеристик депрессивного синдрома в основной и контрольной группах. Определяющий синдром, безусловно, способен усилить наше понимание текущего состояния пациента, его диагноза и возможного прогноза, и может повлиять на выбор терапевтической стратегии (Иванец Н.Н. и соавт., 2018).

В отечественной психиатрии традиционно уделяется внимание не только преобладающему симптому, но и ведущему синдрому, что позволяет более точно оценить статус пациента, поставить диагноз, прогнозировать исход (Снежневский А.В., 1960; Бухановский А.О. и соавт., 2016), а также влияет на выбор терапии (Солдаткин А.А., Мавани Д.Ч., 2016). Исходя из этого подхода, различные авторы выделяют несколько вариантов клинического проявления депрессивного синдрома (Степанов И.Л., Горячева Е.К., 2012; Распопова Н.И. и соавт., 2017; Ветроградова О.П., 1980; Цыганков Б.Д., 2011), среди которых чаще всего упоминаются тревожный, тоскливый (меланхолический), адинамический, ларвированный, атипичный, дереализационно-деперсонализационный и анестетический. Тревожный тип сопровождается внутренней тревогой, нервозностью и повышенной возбудимостью, часто переходящей в соматоформные симптомы, что указывает на выраженный эмоциональный дистресс. Тоскливый (меланхолический) вариант характеризуется глубоким подавленным настроением, анизохронностью сна и психомоторной заторможенностью, свидетельствующими о нарушении регуляции эмоциональных состояний. При адинамической депрессии отмечается заметное снижение активности, апатия и отсутствие инициативы, что негативно влияет на социальную и профессиональную сферу.
