Введение
В настоящее время аутоиммунные заболевания относятся к числу основных медицинских проблем, требующих внимания. На данный момент известно более 80 различных аутоиммунных заболеваний человека. Причиной аутоиммунных заболеваний являются нарушения функционирования иммунной системы, в результате которых клетки иммунной системы атакуют органы и ткани собственного организма. Для ряда аутоиммунных состояний есть основания предполагать, что причиной развития может являтся клональная экспансия аутореактивных Т-клеток. К таким заболеваниям относятся различного рода артриты, диабет I типа, рассеянный склероз.
Одним из аутоиммунных заболеваний, относящихся к группе артритов, является анкилозирующий спондилит (АС, болезнь Бехтерева, заболевание Штрумпелля-Мари). Генетические причины возникновения и развития этого заболевания окончательно не выяснены. По современным представлениям его можно отнести к мультигенным заболеваниям, так как показана корреляция между присутствием в геноме ряда аллельных вариантов генов с повышенным риском развития АС. Учитывая жесткую сцепленность риска развития АС с одним из НЬА-В аллелей, кодирующим вариант молекулы МНС-1 типа, в настоящее время предполагается активное участие Т-клеток в развитии заболевания. Существующие гипотетические модели развития АС предполагают вклад в развитие заболевания различных типов взаимодействия внешних и внутренних факторов, так как наследуемость АС не является 100% среди монозиготных близнецов, подразумевая вклад факторов окружающей среды. В настоящий момент считается, что определяющим этапом и характеристическим признаком развития АС является изменение репертуара Т-лимфоцитов.
Выявление и характеристика клональной экспансии Т-лимфоцитов у больных АС, изучение динамики клонов Т-клеток в ходе заболевания вносят вклад в понимание механизмов возникновения аутоиммунных заболеваний в целом. Также полученные данные о патогенных клонах Т-клеток при АС могут быть использованы для разработки средств терапии, профилактики и методов ранней диагностики.

Анкилозирующий спондилит: история открытия и современное представление.
Анкилозирующий спондилит (АС) - заболевание известное человечеству еще с античных времен, впервые было обнаружено при археологических изучениях египетских мумий. В 1559 году Реалдо Коломбо обнаружил характерные изменения для АС при описании скелета человека, которые были детально задокументированны в книге «Анатомия». Бернард Коннор, ирландский врач, в 1693 году описал скелет человека, в котором тазовая кость, поясничные позвонки, крестец, грудные позвонки с ребрами были сращены в единую кость. В конце восемнадцатого века появляются несколько клинических описаний АС, но на сегодняшний день первым описанием анкилозирующего спондилита считается описание сделанное русским врачом Владимиром Бехтеревым в 1893 году, тогда же болезни было присвоено второе название «Болезнь Бехтерева», которое до сих пор используется.
Анкилозирующий спондилит - хроническое воспалительное заболевание, поражающее осевой скелет и крестцово-подвздошные сочленения человека. Распространенность АС достаточно велика от 0,05% до 1,1%, в зависимости от региона. Основной особенностью протекания заболевания является постоянное воспаление суставов, при котором происходит постепенное ограничение их подвижности, которое в дальнейшем приводит к образованию анкилозов - сращиванию костей друг с другом. При уменьшении гибкости позвоночника, так же ограничивается подвижность суставов, связывающих ребра с грудными позвонками, что приводит к нарушению дыхания и ослаблению вентиляции легких.
Основное отличие АС от различного рода артритов это ограничение подвижности суставов, редко сопровождающиеся разрушением их, что является характерным для артритов.
Генетические факторы развития анкилозирующего спондилита
Анкилозирующий спондилит, одно из заболеваний, для которого был определен наследственный маркер, которым является один из генов главного комплекса гистосовместимости НЬА-В27. Есть предположение, что анкилозирующий спондилит является мультигенным заболеванием и НЬА-В27 не единственный ген, который определяет предрасположенность и развитие АС. Гены, с которыми установлена на данный момент ассоциация с АС представлены в таблице 1.

Выводы
1. Создан новый экспериментальный подход к оценке репертуара периферических Т-лимфоцитов и идентификации клональной экспансии основанный на масштабном секвенировании кДНК р-цепей Т-клеточных рецепторов и сравнительном структурном анализе антиген-распознающего домена.
2. Показано, что индивидуальные репертуары Т-лимфоцитов больных анкилозирующим спондилитом характеризуются выраженной устойчивостью и долговременно олигоклональной экспансией. В составе репертуаров двух больных выявлен 21 преобладающий клонотип Т-лимфоцитов.
3. Определено фенотипическое разнообразие выявленных клонотипов. Установлено, что единственный найденный клонотип Т-хелперов представлен исключительно эффекторными клетками памяти, а каждый из шести охарактеризованных клонотипов С08+ цитотоксических лимфоцитов состоит из нескольких субпопуляций Т-лимфоцитов с преобладанием либо эффекторных клеток, либо клеток памяти.
4. Впервые выявлен сложный клонотип цитотоксических Т-лимфоцитов, включающий шесть подтипов независимо сформировавшихся клонов Т-клеток, Т-клеточные рецепторы которых обладают высокогомологичными антиген-связывающими доменами.
