Рецептор прогестерона (ПР) играет решающую роль в развитии, дифференцировке и функционировании женской половой системы. У мышей с делецией гена рецептора прогестерона нарушены процессы дифференцировки молочной железы, овуляции, имплантации, отсутствует сексуальное поведение и затронута иммунная система (Lydon et al, 1996). Мутации в генах семейства рецепторов стероидных/тиреоидных гормонов приводят к ряду клинических проявлений, таких как микседема (рецептор гормона щитовидной железы) (Oppenhimer et al, 1979), ERKO синдром (рецептор эстрогена) (Korach et al, 1996), рак простаты и бесплодие у мужчин (рецептор андрогена) (Wilson et al, 1983, Patterson et al, 1994). По-видимому, вследствие того, что ПР исключительно важен для репродуктивных функций млекопитающих, отклонения от дикого типа в гене ПР встречаются чрезвычайно редко. До сих пор не было обнаружено мутаций в гене ПР человека. В представленной работе впервые описаны две новых мутации входящих в мутационный комплекс аллеля гена ПР, называемого PROGINS.

В настоящее время клонировано большинство рецепторов стероидных гормонов, определены их структура и функциональные районы. Установлено, что рецепторы стероидных гормонов содержат лиганд- и ДНК-связывающих домены, соединённые подвижным хиндж районом. Для некоторых рецепторов картированы также районы, ответственные за транскрипционную активацию и репрессию (Meyer et al, 1992, Sartorius et al, 1994); сигналы ядерной транслокации (Guiochon-Mantel et al, 1989), гомо - и гетеро-димеризации (Tetel et al, 1997), участки связывающие ко-репрессоры, ко-активаторы и молекулярные чапероны (Jakson et al, 1997). Изучение мутантного варианта ПР позволит точнее установить значимость районов, в которых произошла мутация, для различных ступений взаимодействия и функционирования рецептора и лиганда. Такой подход использовался и для анализа других стероидных рецепторов. В данной работе сочетается изучение физиологических последствий наличия мутации на уровне целостного организма и функциональных отличий на уровне рецептора.

Ранее было показано, что в популяции здоровых женщин существует вариант гена ПР с инсерцией Alu повтора в интрон G гена рецептора прогестерона, называемый PROGINS (PROGesteron gene INSertion) (Fuqua et al, 1991). Мы предположили, что дополнительные изменения в кодирующей части гена ПР могут изменить функции рецептора и, таким образом, повлиять на риск заболевания раком груди или яичников. Естественно ожидать, что изменения в рецепторе могут также повлиять на репродуктивную функцию, иммунную систему, процессы развития и дифференциации тканей экспрессирующих ПР. Цель работы.

Целью настоящей работы было охарактеризовать структуру аллеля PROGINS гена ПР, охарактеризовать влияние структурных изменений на функцию рецептора и изучить их физиологические последствия.

Для достижения этой цели были поставлены следующие конкретные задачи:

1. Установить является ли PROGINS аллель ПР фактором риска в возникновении рака груди и рака яичников в различных этнических группах.

2. Проанализировать частоту встречаемости мутантного аллеля ПР у женщин с идиопатическим бесплодием.

3. Определить, существуют ли дополнительные изменения в кодирующей части гена мутантного рецептора прогестерона PROGINS, приводящие к аминокислотным заменам.

4. Изучить функциональную значимость обнаруженных изменений в структуре белка.

Научная новизна.

В работе впервые установлены новые характеристики аллеля PROGINS рецептора прогестерона. Было показано, что наряду с вставкой Alu повтора в интрон G гена рецептора прогестерона, в мутационный комплекс входят синонимическая замена в пятом экзоне и мутация в четвёртом экзоне, приводящая к замене валина 660 на лейцин. При анализе ДНК всех 1603 женщин в данном исследовании установлено, что все три изменения всегда присутствовали вместе. В ходе эпидемиологического исследования пяти этнических групп было впервые показано, что существует достоверная корреляция между частотой алллеля PROGINS и частотой рака яичников в данной популяции. Установлено также, что PROGINS аллель встречается чаще среди пациенток с раком яичников и груди и, таким образом, является фактором риска для этих заболеваний. В случае спорадического рака яичников риск заболевания у носителя PROGINS аллеля является одним из наивысших (3.0) из всех известных факторов риска. Было впервые обнаружено, что PROGINS является фактором риска и в случае идиопатического бесплодия. В ходе функционального анализа было определено, что стабильность рецептора прогестерона изменена в результате мутации. Мутантный рецептор более стабилен в . отсутствии гормона и менее стабилен в присутствии гормона. При этом, сродство рецептора к лиганду, динамика диссоциации рецептора и лиганда, а также взаимодействие с молекулярными чаперонами не меняются в результате мутации. Практическая ценность.

Рак груди является самым распространённым раковым заболеванием у женщин (Muir 1987, N.C.f.H. Statistics 1990). У пациенток с раком яичников наблюдается наивысшая смертность по сравнению со всеми гинекологическими опухалевыми заболеваниями (Boring

1993). При этом, только около 10% заболеваний раком яичников и груди являются наследственными и могут быть предсказаны с помощью анализа мутаций генов В11СА1 и ВКСА2 (Мл1а 1994, рЩгеа1 1994). Для оставшихся 90% спорадических раковых новообразований существует очень ограниченный набор факторов, позволяющих адекватно оценить риск заболевания этими видами рака. Мы установили, что описанный в этой работе аллель является фактором риска для спорадического рака молочной железы и яичников. Мы также обнаружили, что РЯООШБ влияет на фертильность носителей. Таким образом, тестирование на присутствие аллеля РЮОШБ в комбинации с другими факторами риска может выявить женщин, имеющих повышенный риск заболевания раком, для профилактической терапии злокачественных новообразований яичников и молочной железы, а также бесплодия.

Результаты работы докладывались на ежегодных 87 и 88 конференциях Американской Ассоциации Изучения Рака (Вашингтон, США, 1996, Сан Диего, США, 1997), 49 Симпозиуме по Фундаментальному Изучению Рака (Хьюстон, США, 1996), 44 Научном собрании Общества Гинекологических Исследований США (Сан Диего, США, 1997), Годовом Собрании Эндокринологического Общества США (Новый Орлеане, США, 1998), а также на межлабораторных семинарах департамента клеточной биологии и департамента акушерства и гинекологии медицинского колледжа Бэйлор, г.Хьюстон, США, Новосибирского института биоорганической химии, СО РАН, г.Новосибирск и Института цитологии и генетики СО РАН, г.Новосибирск. По материалам диссертации опубликовано и принато в печать 12 работ.

ВЫВОДЫ

1. Впервые описан новый аллель рецептора прогестерона, называемый Р1ЮОШ8. Этот аллель отличается от ПР дикого типа вставкой А1и повтора в интрон О гена рецептора прогестерона, синонимической заменой в пятом экзоне и мутацией в четвёртом экзоне, приводящей к замене валина 660 на лейцин.

2. - Впервые установлено, что РЯООШЗ аллель встречается чаще среди пациенток с раком яичников и груди и, таким образом, является фактором риска для этих заболеваний. В случае спорадического рака яичников риск заболевания у носителя РИООШБ аллеля является одним из наивысших -3.0 - из всех известных факторов риска.

3. В пяти этнических группах установлена достоверная корреляция между частотой РИООПЧБ и частотой заболевания раком яичников в соответствующей популяции.

4. В ходе функционального анализа было определено, что стабильность рецептора прогестерона изменена в результате мутации. Мутантный рецептор более стабилен в отсутствии гормона и менее стабилен в присутствии гормона. Сродство рецептора к лиганду, динамика диссоциации рецептора и лиганда, а также взаимодействие с молекулярными чаперонами не меняются в результате мутации.
