Актуальность исследования. В структуре патологии пищеварительного тракта у детей ведущее место занимают хронические воспалительные заболевания верхних отделов пищеварительного тракта. Заболевания чаще всего диагностируются в подростковом возрасте, являющимся одним из критических периодов в онтогенезе, когда начинается половое созревание, и организм ребёнка вследствие имеющейся функциональной и психологической неустойчивости, подвергается более высокому риску развития пограничных и патологических состояний [Баранов A.A., Щеплягина J1.A., 2003; Запруднов A.M., Григорьев К.И., 2011].

В последние годы активно обсуждается ведущая роль Н. pylori в этиологии и патогенезе хронической гастродуоденальной патологии [Küsters J.G. et al., 2006]. Разработаны и внедрены в практику схемы эрадикации этого микроорганизма [Маев И.В. и соавт., 2011; Malfertheiner Р. et al., 2007, 2012; Chey W.D., Wong B.C.Y., 2007].

H.pylori, колонизируя ЖКТ, вызывает у всех инфицированных развитие хронического воспаления в слизистой желудка, значительно повышает риск развития язвенной болезни желудка и двенадцатиперстной кишки, атрофического гастрита, кишечной метаплазии и характеризуется Всемирной организацией здравоохранения как канцероген I класса в отношении риска развития аденокарциномы желудка [Wroblewski L.E. et al., 2010]. Однако известно, что более 50% всего населения земного шара инфицированы H.pylori, при этом лишь у меньшей части отмечаются клинические проявления этой колонизации. Подсчитано, что риск развития язвенной болезни желудка и/или двенадцатиперсной кишки у инфицированных H.pylori составляет от 10% до 20%, а аденокарцинома желудка разовьётся у 12% H.pylori-позитивных людей [Küsters J., 2006].

Вопрос о факторах, предопределяющих развитие той или иной формы заболевания, является наиболее сложным и до настоящего времени не решен.

Но очевидно, что исход колонизации будет зависеть от специфического 5 взаимодействия потенциального патогена и макроорганизма, т.е. обусловлен не только вирулентными и патогенными свойствами микроорганизма, но и особенностями развития в отношении него иммунного ответа [Küsters J. et al., 2006; Peek R.M. et al., 2010].

Анализ литературы демонстрирует тот факт, что, несмотря на пристальный интерес к хеликобактерной инфекции на протяжении более двух десятилетий, остаётся ещё много пробелов в понимании роли этого микроорганизма в этиологии и патогенезе заболеваний человека [Amieva M.R., El-Omar Е.М., 2008; Peek R.M. et al., 2010]. В первую очередь это касается отсутствия целостного представления о взаимодействии H.pylori с другими представителями микрофлоры желудочно-кишечного тракта и об условиях реализации данной бактерией своих патогенных свойств, связанных с изменением микробного окружения, состоянием местных и общих иммунных факторов защиты.

Таким образом, цель исследования - выявить взаимосвязь характера поражения слизистой оболочки верхних отделов пищеварительного тракта, наличия H.pylori с изменениями микрофлоры толстой кишки и ротовой полости и цитокинового профиля крови и слюны для оптимизации диагностики хронических воспалительных заболеваний верхних отделов пищеварительного тракта.

В соответствии с целью были определены задачи исследования:

1. Исследовать особенности микрофлоры ротовой полости у детей I-II групп здоровья и детей с хроническими воспалительными заболеваниями верхних отделов ЖКТ.

2. Изучить особенности микрофлоры толстой кишки у детей I-II групп здоровья и детей с хроническими воспалительными заболеваниями верхних отделов ЖКТ.

3. Определить содержание про- и противовоспалительных цитокинов в крови у детей I-II групп здоровья и детей с хроническими воспалительными заболеваниями верхних отделов ЖКТ. 6

4. Оценить содержание про- и противовоспалительных цитокинов в слюне у детей 1-П групп здоровья и детей с хроническими воспалительными заболеваниями верхних отделов ЖКТ.

5. Установить корреляционные связи между показателями цитокинового профиля крови и слюны с содержанием различных представителей микробиоты толстой кишки у детей 1-П групп здоровья.

6. Определить взаимосвязь между показателями цитокинового профиля крови и слюны с содержанием различных представителей микробиоты толстой кишки у детей с хроническими воспалительными заболеваниями верхних отделов ЖКТ в зависимости от характера поражения слизистой оболочки гастродуоденальной зоны и наличия Н.ру1оп.

7. Выявить группы риска по тяжёлым поражениям слизистой оболочки гастродуоденальной зоны.

Научная новизна исследования. Значительно расширены представления о механизмах симбиоза макроорганизма и микробиоты ЖКТ.

Выявлены коррелятивные связи между содержанием отдельных представителей микробиоты друг с другом и с про- и противовоспалительными цитокинами.

Впервые показаны механизмы нарушения симбиоза между микрофлорой ЖКТ и макроорганизмом у детей с хроническими воспалительными заболеваниями верхних отделов органов пищеварения.

Показано, что облигатная микрофлора толстой кишки у детей с ХГД+ГЭРБ характеризуется не только количественными, но и качественными изменениями функционирования, выражающимися в уменьшении защитных антагонистической и противовоспалительной функций и нарастании её провоспалительного потенциала.

Показана главенствующая роль факультативной флоры толстой кишки в регуляции про- и противовоспалительных реакций с преобладанием провоспалительной их направленности у детей с ХГД+ГЭРБ.

Впервые показана ассоциация низкого уровня ИЛ-8 в крови и слюне с наличием антител к Н.ру1оп и эрозивных поражений у детей с ХГД+ГЭРБ.

Практическая значимость. Определён диапазон содержания про- и противовоспалительных цитокинов в слюне у детей.

Показана диагностическая значимость определения уровня цитокинов в слюне для определения характера поражения слизистой оболочки верхних отделов ЖКТ.

Показано, что оценка состояния микробиоценоза кишечника должна включать определение уровня про- и противовоспалительных цитокинов в крови и/или слюне.

Показано, что низкое содержание ИЛ-8 в крови и слюне является фактором риска развития серопозитивной Н.ру1оп-инфекции и эрозивных поражений слизистой оболочки гастродуоденальной зоны.

Установленные при помощи статистических методов информативные диагностические критерии ХГД+ГЭРБ позволяют совершенствовать их диагностику.

Предложен комплекс мероприятий по восстановлению симбиоза между макроорганизмом и его микрофлорой.

Положения, выносимые на защиту:

1. ХГД+ГЭРБ характеризуются наличием дисбиотических нарушений толстой кишки, особенности которых зависят от характера поражения слизистой оболочки эзофагогастродуоденальной зоны и наличия инфицированности Н.ру1оп.

2. ХГД+ГЭРБ сопровождаются изменением цитокинового профиля крови и слюны, зависящим от характера поражения слизистой оболочки эзофагогастродуоденальной зоны и наличия инфицированности Н.ру1оп.

3. Нарушение симбиоза макроорганизма с микробиотой ЖКТ при ХГД+ГЭРБ сопровождается снижением антагонистической и толерогенной функций и провоспалительной направленностью функционирования микрофлоры толстой кишки.

Апробация работы. Основные положения диссертационной работы представлены на XVI Российской Гастроэнтерологической Неделе (Москва,

2010), IX Российском конгрессе «Инновационные технологии в педиатрии и детской хирургии» (Москва, 2010), XV Конгрессе педиатров России с международным участием «Актуальные проблемы педиатрии» (Москва,

2011), научно-практической конференции «Актуальные вопросы педиатрии» (Ставрополь, 2011), XVII Российской Гастроэнтерологической Неделе (Москва, 2011), юбилейной научно-практической конференции детских врачей, посвящённой 100-летию со дня рождения д.м.н., профессора Евтодьевой М.Я. (Ростов-на-Дону, 2011).

Внедрение результатов. Результаты работы внедрены в учебный процесс кафедры педиатрии с курсом неонатологии ФПК и ППС, кафедры детских болезней №1, кафедры общей и клинической биохимии №2 ГБОУ ВПО РостГМУ Минздравсоцразвития России, а также в практику педиатрического соматического отделения МБУЗ "Городская больница №20 города Ростова-на-Дону".

Публикации результатов исследования. По теме диссертационного исследования опубликовано 10 работ, в том числе 3 статьи в периодических журналах из перечня ведущих рецензируемых журналов, рекомендованных для публикации основных научных результатов диссертации на соискание учёной степени.

выводы

1. Инфицированность H.pylori у детей ассоциирована с высокой частотой эрозивных поражений слизистой оболочки желудка и двенадцатиперсной кишки (53%), максимальной при наличии антител к H.pylori (77%).

2. Дети с ХГД+ГЭРБ характеризуются наличием дисбиоза толстой кишки в части содержания бифидобактерий, лактобактерий, бактероидов, пептострептококков, клостридий, гемолитической кишечной палочки, клебсиелл и аэробных бацилл. Группа больных с ЭГД+ГЭРБ, ассоциированными с H.pylori, характеризуется наибольшими отличиями от группы контроля в составе микробиоценоза толстой кишки.

3. У детей с ХГД+ГЭРБ снижена колонизационная резистентность ротовой полости, что выражается в значительном снижении содержания Neisseria spp. в составе микрофлоры ротовой полости, наиболее выраженное в группе H.pylori-позитивных больных с ЭГД+ГЭРБ.

4. Группа детей с ХГД+ГЭРБ характеризуется провоспалительной направленностью функционирования мукозального иммунитета, о чём свидетельствует значительное повышение в слюне в этой группе больных провоспалительного ИЛ-10, на фоне снижения системных противовоспалительных реакций в виде уменьшения концентрации ТФР-0! в крови.

5. Эрозивные поражения слизистой оболочки верхних отделов ЖКТ при наличии антител к H.pylori сопряжены со снижением уровня основного хемокина - ИЛ-8 - в слюне и в крови.

6. У здоровых детей формируются отрицательные корреляции между содержанием лактобактерий и фузобактерий с уровнями провоспалительныго ИЛ-8, клостридиями и провоспалительного ИЛ-10 и положительные корреляции между противовоспалительным ИЛ-10 с условно-патогенными энтеробактериями и дрожжеподобными грибами, свидетельствующие о защитной противовоспалительной роли микрофлоры толстой кишки.

7. У детей с ХГД+ГЭРБ формируются положительные корреляции между содержанием эубактерий и провоспалительным ИЛ-6 и отрицательные корреляции между противовоспалительными цитокинами и другими представителями облигатной флоры толстой кишки (ИЛ-4 с фузобактериями и типичными кишечными палочками, ТФР-(3] с фузобактериями и энтерококками, ИЛ-10 с бифидобактериями), что свидетельствует об участии резидентной флоры в развитии и поддержании воспаления в ЖКТ.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

1. В комплекс обследования детей с хроническими воспалительными заболеваниями верхних отделов желудочно-кишечного тракта рекомендуется включать исследование кала на дисбактериоз.

2. Для оценки функционального состояния микробиоценоза и взаимоотношений дисбиотически изменённой микрофлоры с макроорганизмом рекомендуется исследовать уровень про- и противовоспалительных цитокинов в крови и в слюне.

3. Для выявления больных ХГД+ГЭРБ целесообразно исследовать диагностические модели ИЛ-10 в слюне (АиС=0,728) при значении критерия > 72 пг/мл с чувствительностью 68,75% и специфичностью 76%; ТФР-01 в крови (АиС=0,670) при значении критерия < 10700 пг/мл с чувствительностью 68,85% и специфичностью 70,83%; отношения противовоспалительного ТФР-0, в крови к провоспалительному ИЛ-10 в слюне (АЦС=0,771) с чувствительностью 75,9%) и специфичностью 78,26%).

4. Необходимо выделять группу риска по развитию эрозивных поражений гастродуоденальной зоны, ассоциированных с инфекцией Н.ру1оп, при низких значениях провоспалительного ИЛ-8 в крови и в слюне.

5. Для выявления серопозитивных к Н.ру1оп больных среди детей с ХГД+ГЭРБ целесообразно использовать диагностическую модель ИЛ-8 (АиС=0,763) при значении критерия < 572 пг/мл с чувствительностью 88,89%) и специфичностью - 61,22%.

6. Разработан комплекс мероприятий по восстановлению симбиоза между макроорганизмом и его микрофлорой при ХГД+ГЭРБ, включающий на фоне базисной терапии ХГД+ГЭРБ применение препаратов-метабиотиков, смесей для энтерального питания, содержащих трансформирующий фактор роста-0, а затем мероприятия, направленные на восстановление количественных характеристик облигатной микрофлоры ЖКТ пре- и пробиотическими препаратами.
