ВВЕДЕНИЕ

Согласно данным ЮНЭЙДС, к концу 2011 года в мире проживало около 34 миллионов человек, инфицированных вирусом иммунодефицита человека (ВИЧ). Примерно 0,8% населения Земли в возрасте 15-49 лет имели ВИЧ-положительный статус. В России, согласно данным Федерального научно-методического центра по профилактике и борьбе со СПИДом, на ноябрь 2012 года было зарегистрировано уже более 700 тысяч случаев ВИЧ-инфекции. Показатель распространенности заболевания за 10 месяцев 2012 года в среднем по Российской федерации составил 491,9 на 100 тыс. населения.

До появления высокоактивной антиретровирусной терапии (ВААРТ, APT) с момента инфицирования человека ВИЧ и до развития у него глубокого иммунодефицита проходило около 10-12 лет, а смерть пациента наступала в течение 2-4 лет после первых проявлений СПИДа (Munoz et al., 1989; Hendriks et al., 1998). Введение APT в клиническую практику позволило существенно увеличить продолжительность жизни ВИЧ-позитивных больных. Антиретровирусная терапия снижает вирусную нагрузку в крови пациентов и удерживает ее на уровне ниже определяемого, что сопровождается подавлением воспалительных реакций (Graziosi et al., 1996). Нормализация количества провоспалительных цитокинов приводит к перераспределению Т-клеток между лимфоидными тканями и кровью (Вису et al., 1999), что отражается в быстром увеличении числа периферических Т-лимфоцитов в течение первых трех месяцев лечения. В дальнейшем скорость подъема численности Т-клеток замедляется, так как основным источником лимфоцитов становится вилочковая железа (Pakker et al., 1998; Smith et al., 2000; Douek et al., 1998). Однако, несмотря на высокую эффективность подавления репликации вируса при применении терапии, количество периферических CD4+ Т-лимфоцитов у ВИЧ-инфицированных пациентов не возвращается к нормальному уровню, характерному для здоровых людей (French et al., 2009).

Вместе с тем, у 10-30% ВИЧ-инфицированных больных, получающих ВААРТ, не происходит прироста количества CD4+ Т-клеток на фоне полного подавления вирусной нагрузки (Autran et al., 1999; Piketty et al., 1998). Эти пациенты часто начинают лечение, имея низкий исходный уровень CD4+ Т-лимфоцитов в периферической крови, что негативно отражается на продуктивности восстановления иммунитета при проведении терапии (Kelley et al., 2009; Florence et al., 2003). Ввиду сохраняющегося иммунодефицитного состояния, такие больные подвержены большему риску развития осложнений, чем ВИЧ-инфицированные пациенты, у которых наблюдается восстановление числа периферических CD4+ Т-лимфоцитов в ответ на лечение. Среди осложнений можно отметить тяжелые инфекции, злокачественные новообразования, сердечнососудистые заболевания и смерть (Lapadula et al., 2013; Tan et al., 2008). Высокий риск заболеваемости и смертности ВИЧ-инфицированных пациентов со слабым восстановлением иммунитета при провдении ВААРТ, а также их существенная доля среди больных, получающих терапию, обуславливает большую актуальность проблемы. В то же время причины и механизмы нарушения регенерации иммунитета на фоне антиретровирусной терапии на сегодняшний день остаются неясными.

Цель работы: охарактеризовать субпопуляционный состав Т-лимфоцитов ВИЧ-инфицированных пациентов с неэффективным восстановлением иммунной системы при проведении антиретровирусной терапии.

Основные задачи исследования:

1. Определить субпопуляционный состав CD4+ и CD8+ Т-лимфоцитов ВИЧ-позитивных пациентов с нарушением восстановления количества CD4+ Т-клеток на фоне терапии.

2. Проанализировать параметры активации, апоптоза и пролиферации СБ4+ и СЭ8+ Т-лимфоцитов ВИЧ-инфицированных больных с нарушением восстановления пула Т-клеток при проведении лечения.

3. Оценить наличие маркеров функциональных нарушений СЭ4+ и СЭ8+ Т-лимфоцитов ВИЧ-позитивных людей с неэффективным ответом на антиретровирусную терапию.

Научная новизна исследования

В результате сравнительного анализа характеристик Т-лимфоцитов у ВИЧ-инфицированных пациентов с нарушением прироста числа С04+ Т-клеток при проведении антиретровирусной терапии по сравнению с больными с эффективным ответом на лечение впервые было обнаружено следующее:

- увеличение доли СЭ4+ Т-клеток центральной памяти;

- уменьшение абсолютного количества наивных Т-лимфоцитов, клеток центральной и эффекторной памяти, а также терминальных эффекторов в субпопуляции СЭ4+ Т-лимфоцитов;

- увеличение относительного количества активированных СБ4+ Т-клеток и доли СЭ4+ Т-лимфоцитов, находящихся в состоянии апоптоза;

- рост относительного числа пролиферирующих СБ4+ Т-клеток памяти;

- уменьшение численности СЭ4+ Т-лимфоцитов, несущих рецептор к 1Ь-7;

- накопление РЭ1+Т-лимфоцитов в субпопуляциях СЭ4+ и СЭ8+ Т-клеток памяти.

У ВИЧ-инфицированных пациентов с дискордантным ответом на антиретровирусную терапию впервые установлена связь дефицита наивных Т-лимфоцитов с угнетением продуктивной функции тимуса. Кроме того, обнаружено, что у ВИЧ-инфицированных пациентов с подавленной репликацией вируса доля пролиферирующих СБ4+ Т-клеток памяти имеет обратную зависимость от общего числа СБ4+ Т-лимфоцитов.

Теоретическая и практическая значимость работы

Полученные результаты расширяют представления о механизмах нарушения восстановления уровня CD4+ Т-лимфоцитов у ВИЧ-инфицированных больных, принимающих терапию. Установлено, что у пациентов, не дающих адекватного ответа на APT, наблюдалась низкая эффективность работы двух наиболее важных механизмов восстановления пула периферических Т-клеток, а именно: поступления наивных Т-лимфоцитов из тимуса и гомеостатической пролиферации Т-клеток на периферии. При этом у данной категории больных, по сравнению с людьми с эффективным ответом иммунной системы на APT, была увеличена доля умирающих CD4+Т-лимфоцитов, и CD4+ Т-клеток, несущих маркеры CD38, HLA-DRhPDI.

В практическом плане работа демонстрирует необходимость назначения антиретровирусной терапии ВИЧ-инфицированным пациентам в более ранние сроки заболевания.

Внедрение в практику

Материалы диссертации используются в лекционных курсах «Иммунология» кафедры микробиологии и иммунологии биологического факультета ГОУ ВПО «Пермский государственный национальный исследовательский университет» (614000, г.Пермь, ул. Букирева, 15) и кафедры иммунологии ГБОУ ВПО «Пермская государственная медицинская академии им. Е.А. Вагнера» министерства здравоохранения и социального развития (614000, г. Пермь, ул. Петропавловская, 26).

Положения, выносимые на защиту:

1. У ВИЧ-инфицированных пациентов с нарушением восстановления числа CD4+ Т-клеток на фоне антиретровирусной терапии по сравнению с пациентами, эффективно отвечающими на лечение, снижено абсолютное

количество CD4+ наивных Т-лимфоцитов, клеток центральной и эффекторной памяти, а также терминальных эффекторов. Основной причиной сокращения числа наивных Т-лимфоцитов является угнетение продуктивной активности тимуса.

2. Среди CD4+ Т-клеток ВИЧ-инфицированных больных с нарушением восстановления иммунной системы при проведении антиретровирусной терапии увеличена доля активированных, пролиферирующих и умирающих CD4+ Т-лимфоцитов по сравнению с соответствующими показателями ВИЧ-позитивных людей с эффективным ответом на лечение.

3. У ВИЧ-инфицированных пациентов с нарушением восстановления численности CD4+ Т-клеток на фоне терапии происходит накопление PD1-позитивных CD4+ и CD8+ Т-лимфоцитов.

Апробация работы и публикации

Основные результаты представлены в 11 печатных работах, из них 4 статьи в рецензируемых журналах, рекомендуемых ВАК РФ для публикации материалов диссертационных исследований.

Структура и объем диссертации

Диссертация изложена на 192 страницах, иллюстрирована 22 таблицами и 26 рисунками. Состоит из введения, обзора литературы, 3-х глав собственных исследований, выводов, списка цитированной литературы, включающего 528 источников, в том числе 2 работ отечественных и 526 зарубежных авторов.

ГЛАВА 1. ОБЗОР ЛИТЕРАТУРЫ 1.1. Классификация ВИЧ

Вирус иммунодефицита человека относится к семейству Ретровирусов рода Лентивирусов. Он появился в результате множественных зоонозных заражений человека вирусами иммунодефицита обезьян в Западной и Центральной Африке. Межвидовой перенос, вероятно, происходил в процессе охоты и свежевания добычи, а также ловли, продажи и содержания обезьян в качестве домашних питомцев (Hahn et al., 2000). Сегодня более 40 видов нечеловекообразных приматов являются переносчиками видоспецифичных вирусов иммунодефицита обезьян (Aghokeng et al., 2010).

вирус иммунодефицита человека 1 (ВИЧ-1/HIV-1) и вирус иммунодефицита человека 2 (BH4-2/HIV-2).

ВИЧ-1 является самым распространенным и наиболее патогенным видом вируса. Его принято делить на несколько групп (М, О, Р, N), которые, в свою очередь, разделяются на подтипы. Согласно современным представлениям каждая группа является результатом независимого переноса вируса иммунодефицита обезьян человеку (Sharp, Hahn, 2011). Первая группа - М (от «major» - основной, относящийся к большинству). Предполагают, что для всех вирусов этой группы существовал один общий предшественник, который в процессе распространения подвергся дивергенции, дав начало подтипам А-К. К группе М также относят 15 циркулирующих рекомбинантных форм, появившихся путем перераспределения участков генетического материала между вирусами разных подтипов (McCutchan, 2000). В мире около 90% всех случаев ВИЧ-инфекции обусловлены заражением вирусами группы М. Вторая группа - N (от «new» - новый). Вирус идентифицирован в Республике Камерун в 1998 году (Simon et al., 1998). По данным 2006 года было отмечено лишь 10 случаев инфекции вирусами этой группы (Yamaguchi et al, 2006). Группы М и N независимо

произошли от вируса иммунодефицита обезьян SIVcpz, широко распространенного среди шимпанзе {Pan troglodytes troglodytes) в Западной и Центральной Африке (Gao et al., 1999; Keele et al., 2006). Третья и четвертая группы - О и Р - являются малораспространенными. Они родственны другому вирусу иммунодефицита обезьян - SIVgor, встречающемуся среди западных равнинных горилл {Gorilla gorilla gorilla) в Республике Камерун (Plantier et al., 2009; Van Heuverswyn et al., 2006; Vallari et al., 2011).

ВИЧ-2 получил распространение преимущественно в Западной Африке (O'Brien et al., 1992). Для патогенеза инфекции, вызванной этим видом вируса, характерны более низкая, чем при ВИЧ-1, вирусная нагрузка, меньшая контагиозность (вероятность передачи вируса) и более длительный период бессимптомного течения (De Cock et al., 1993). ВИЧ-2, по-видимому, произошел от вируса иммунодефицита SIVsmm дымчатых мангобеев {Cercocebus atys atys), населяющих прибрежные леса Западной Африки. К 2010 году было выделено 8 групп ВИЧ-2 (А-Н). Из них широко распространены только А и В группы (Plantier et al., 2009; Marx et al., 2001). Остальные были обнаружены у людей однократно.

ВИЧ обладает большой генетической вариабельностью (Robertson et al, 1995). Она становится возможной благодаря высокой частоте мутаций и рекомбинаций, вызванных работой вирусной обратной транскриптазы, у которой отсутствует механизм проверочного считывания синтезируемой последовательности (Roberts et al., 1988). Эта особенность, в сочетании с высокой скоростью репликации, приводит к быстрой генерации генетически разнообразных вирусных популяций в каждом зараженном индивиде: до 10% вариабельности (Но et al., 1995; Korber et al, 2001). В пределах подтипов разница на аминокислотном уровне составляет порядка 8-17%, но может достигать 30%. Между подтипами вариации, обычно, находятся в пределах 17-35%, но могут достигать 42% (Korber et al, 2001). Несмотря на крайне высокую изменчивость вируса, случаев перехода одним из вариантов ВИЧ границ своего подтипа не отмечено.

1.2. Строение и репликация ВИЧ 1.2.1. Геном и структура вируса

Вирион ВИЧ представляет собой объект сферической формы диаметром около 100 нм. Его внешняя оболочка является двухслойной мембраной, сформированной из липидов клеток человека-носителя. В мембрану встроены в среднем 72 копии белкового комплекса Env, пронизывающего поверхность вириона. Комплекс Env образован головкой, состоящей из трех гликопротеидов gpl20, и стержнем, составленным из трех гликопротеидов gp41, закрепляющих структуру оболочки (Zhu et al., 2006; Zanetti et al., 2006; Subramaniam et al., 2006; Zhu et al., 2008). Данный комплекс играет важную роль в проникновении вируса в клетки во время заражения.

Под липидной мембраной располагается капсид, состоящий из вирусных белков р24, и окруженный матриксом из белка р17. Капсид охватывает белковонуклеиновый комплекс из двух полноценных одноцепочечных молекул РНК, и нуклеокапсидных белков рб и р7. Присутствующие в вирионе обратная транскриптаза, интеграза р32 и протеаза pi 1, обеспечивают вирусу возможность репликации.

Генетический материал представлен двумя несвязанными нитями РНК длиной 9749 нуклеотидов каждая (Ratner et al., 1985; Turner, Summers, 1999). В составе генетической последовательности закодированы общие для всех ретровирусов структурные белки и дополнительные - уникальные для ВИЧ -последовательности (Wong-Staal et al., 1985). Геном содержит три структурных гена (gag, pol и env), необходимых для создания структурных белков вирусных частиц. Также ВИЧ имеет шесть регуляторных генов (tat, rev, nef, vif, vpr и vpu), содержащих информацию для создания протеинов, контролирующих контагиозность, размножение и т.д. Девять генов ВИЧ кодируют, по крайней мере, 15 белков (Muesing et al., 1985). Концы каждой

цепочки РНК представлены длинными концевыми повторами, которые управляют продукцией новых вирусов.

1.2.2. Жизненный цикл ВИЧ

Цикл репликации ВИЧ начинается с момента прикрепления вирусного белка gpl20 к молекуле CD4 на поверхности клетки-мишени (Dalgleish et al., 1984). CD4 - основной рецептор ВИЧ - экспрессируется на поверхности многих Т-лимфоцитов, тимоцитов, дендритных клеток, моноцитов, макрофагов, клеток микроглии ЦНС и т.д. Однако CD4 - не единственная молекула, необходимая вирусу для внедрения. В этом процессе также участвуют другие молекулы, такие как CCR5 и CXCR4 (Berson et al., 1996; Bleui et al., 1996). При взаимодействии gpl20 с CD4 и CCR5 на поверхности Т-лимфоцита, вирусные белки претерпевают конформационные изменения, что приводит к слиянию двух липидных мембран и внедрению вирусного капсида в клетку-мишень.

После проникновения начинается процесс «раздевания» капсида, то есть освобождения вирусной РНК от защищающих ее белков. Внедренная в клетку вместе с генетическим материалом ревертаза инициирует процесс обратной транскрипции, последовательно создавая сначала минус-, а затем и плюс-цепь провирусной ДНК ВИЧ (Isel et al., 1995). Образовавшаяся в результате генетическая цепочка тесно связана с целым рядом вирусных (интегразой, обратной транскриптазой, матриксным белком, нуклеокапсидным протеидом, вспомогательным белком Vpr) и клеточных белков (Ku, INI1, HMGal [HMG I(Y)]) - так называемый нуклеопротеиновый преинтеграционный комплекс. В его составе ДНК ВИЧ транспортируется в ядро клетки (Farnet, Bushman, 1997), где вирусная интеграза катализирует процесс ее превращения в функциональный провирус (Brown et al., 1990). ВИЧ способен встраиваться в любую из хромосом человека, отдавая предпочтение более доступным, транскрипционно активным участкам ДНК клетки хозяина.

Как и у любого другого гена, экспрессия провирусной ДНК начинается с этапа транскрипции. В этом процессе важное место занимают длинные концевые повторы, в которых локализованы промотор, энхансер, сигнал остановки транскрипции и полиаденилирования. Образующиеся транскрипты мРНК ВИЧ могут быть множественно или однократно сплайсированы, или же остаются полноразмерными РНК. Короткие молекулы кодируют вирусные регуляторные белки, длинные - являются матрицами для трансляции вирусных структурных протеинов, или могут быть включены в дочерние вирионы в качестве геномной РНК. Вирусный регуляторный белок Rev обеспечивает экспорт мРНК из ядра в цитоплазму клетки (Malim et al., 1990).

Белки ВИЧ транслируются в виде длинных молекул-предшественников: Gag (р55), Gag-Pol (р 160) и Env (gpl60). В ходе созревания эти молекулы расщепляются протеазами и дают начало зрелым регуляторным и структурным белкам. Протеин gpl60 транслируется в эндоплазматическом ретикулуме, гликозилируется, расщепляется в аппарате Гольджи на gp41 и gpl20, и, в виде нековалентно связанных молекул, переходит на наружную поверхность клетки-хозяина, оставаясь в ее мембране до момента сборки и отпочковывания вируса.

Морфогенез (сборка новых вирусных частиц) происходит вблизи плазматической мембраны, где под гликопротеидами внешней оболочки накапливаются белки-предшественники Gag и Gag-Pol. Так как в условиях культуры клеток для формирования неинфекционных вирионов, внешне похожих на «настоящие», достаточно только молекул Gag, считается, что «программа» сборки вируса содержится именно в этом белке (Wills, Craven, 1991; Hunter, 1994). Следует отметить, что необходимым условием является связывание между собой порядка 1500 молекул Gag. Вирусная РНК доставляется к месту сборки вириона и укладывается в нуклеокапсид (Berkowitz et al., 1996). Другие необходимые белки также транспортируются к месту сборки вирусных частиц.

ВИЧ проходит этап почкования, в результате которого дочерние вирионы отсоединяются, захватывая часть мембраны клетки-хозяина. После отделения белки-предшественники, оказавшиеся включенными во вновь образованную вирусную частицу, подвергаются протеолизу, что ведет к созреванию полноценных, способных заражать другие клетки, вирусов.

Время, которое ВИЧ затрачивает на процесс репликации, определяется несколькими часами. Каждая инфицированная клетка успевает сформировать около 250 новых вирионов. Таким образом, при высокой плотности клеток-мишеней, вирус взрывообразно распространяется по организму.

1.3. Кинетика развития ВИЧ-инфекции

При отсутствии лечения ВИЧ-1 вызывает хроническое, прогрессирующее инфекционное заболевание, которое, с течением времени, ведет к тяжелому иммунодефициту. Естественное течение инфекции можно разделить на три основные фазы: острую, хроническую и СПИД (синдром приобретенного иммунодефицита).

1.3.1. Острая фаза ВИЧ-1-инфекции

После инфицирования у многих пациентов развивается острый ретровирусный синдром. Самые частые клинические симптомы этого периода: лихорадка, недомогание, макулопапулезная сыпь, язвы на слизистой оболочке полости рта, лимфоаденопатия, артралгия, фарингит, недомогание, потеря веса, асептический менингит и миалгия (Kahn, Walker, 1998; Hecht et al., 2002; Daar et al., 2001a). Симптоматический период обычно продолжается 7-10, реже - более 14 дней. Для острой фазы характерны высокие уровни репликации вируса (вирусная нагрузка может подниматься до 106-107 копий/мл) и интенсивный вирусоспецифический ответ (Koup et al., 1994; Borrow et al., 1994; Pitcher et al., 1999). В этот период резко снижается количество периферических CD4+ Т-клеток (Cooper et al., 1985; Но et al., 1985; Henrard et al., 1995) и устанавливается резервуар латентно

инфицированных лимфоцитов (Chun et al., 1998; Finzi et al., 1999; Chun et al., 1997).

При заражении первыми мишенями для вируса становятся CD4+ Т-клетки в зоне проникновения (Stahl-Hennig et al, 1999; Veazey et al., 2000; Veazey et al., 1998; Zhang et al., Z. 1999). В течение двух недель ВИЧ распространяется по организму, заражая клетки слизистых, лимфатических узлов, тимуса, селезенки, и т.д. (Reimann et al., 1994; Schacker et al., 2001). Он также инфицирует клетки врожденного иммунитета, например, фолликулярные дендритные клетки (Haase, 1999; Embretson et al., 1993; Maddox, 1993; Temin, Bolognesi, 1993). Скорость распространения ВИЧ-1 зависит от плотности распределения клеток-мишеней (Grossman et al., 1998). Слизистые, и особенно желудочно-кишечный тракт (ЖКТ), являются наиболее благоприятными местами для репликации вируса, так как содержат большую часть всех Т-лимфоцитов организма (Guy-Grand, Vassalli, 1993; Mowat, Viney, 1997). Спустя три недели после начала инфекции из кишечника удаляются почти все CD4+CCR5+ Т-клетки, что слабо отражается на количестве лимфоцитов в периферической крови (Kewenig et al., 1999; Smit-McBride et al., 1998; Vajdy et al., 2000).

В конце острой фазы ВИЧ-1-инфекции уровень виремии снижается на несколько порядков, достигая так называемой «установочной точки». Эта величина является прогностическим признаком скорости развития заболевания (Mellors et al., 1995; Mellors et al., 2007). Снижение вирусной нагрузки совпадает по времени с интенсивной экспансией вирус-специфичных CD8+ Т-лимфоцитов (Koup et al., 1994; Pantaleo et al., 1994; Borrow et al., 1994). В когортном исследовании, проведенном с участием 420 ВИЧ-инфицированных пациентов, установлена обратная зависимость между активностью цитотоксических Т-клеток в острую фазу заболевания и величиной установочной точки. Ранний иммунный ответ замедляет прогрессирование заболевания (Streeck et al., 2009). Следует отметить, что С08+-опосредованные реакции находятся под контролем CD4+

Т-лимфоцитов. В острую фазу ВИЧ-инфекции происходит резкое снижение количества Т-хелперов и нарушается их функциональная активность, что может приводить к снижению реактивности цитотоксических Т-лимфоцитов (Gupta, 1993; Rosenberg et al., 1997; Lichterfeld et al., 2004).

Уменьшение вирусной нагрузки приводит к постепенному увеличению количества CD4+ Т-клеток, хотя без введения высокоактивной антиретровирусной терапии их уровень редко достигает нормальных значений. В дальнейшем, по мере перехода заболевания в хроническую фазу, число CD4+ Т-клеток постепенно уменьшаться.

1.3.2. Хроническая фаза ВИЧ-1-инфекции

По динамике Т-клеток хроническая инфекция принципиально отличается от острой. Как было описано выше, для последней характерно экспоненциальное увеличение вирусной нагрузки и быстрое опустошение пула CD4+ Т-лимфоцитов. При хронической инфекции количество вируса на несколько порядков ниже, чем в предыдущую фазу, но постепенно растет. Вернувшийся к относительно высоким значениям уровень периферических CD4+ Т-клеток постепенно снижается на протяжении примерно десяти лет, после чего начинается присоединение СПИД-ассоциированных заболеваний и болезнь переходит в фазу СПИД.

Основной характеристикой хронической фазы ВИЧ-инфекции является постоянная активация Т-клеток. Другими словами хроническая инфекция -это состояние хронической иммунной активации (Ascher, Sheppard, 1988). В этот период болезни для CD4+ и CD8+ Т-лимфоцитов характерен так называемый «ускоренный оборот» (Kovacs et al., 2001; Mohri et al., 2001). Он включает в себя увеличение скорости пролиферации и смерти многих медленно делящихся Т-клеток, что приводит к сокращению времени их полужизни. Последнее было показано при in vivo мечении Т-лимфоцитов стабильным изотопом дейтерия (McCune et al., 2000; Hellerstein et al., 1999). Таким образом, хроническая иммунная активация ускоряет прохождение

клетки через все стадии развития: наивная клетка —► эффекторная / клетка памяти —► апоптоз, - что приводит к постепенному истощению пула периферических Т-лимфоцитов.

Вирус иммунодефицита поражает не только зрелые Т-клетки. Почти все тимоциты экспрессируют молекулы CD4 и CXCR4, через которые Х4-тропный вирус может проникать в клетки (Zaitseva et al., 1998; Berkowitz et al, 1998; Kitchen, Zack, 1997). В экспериментах in vitro показано, что в то время как Х4-тропные штаммы вызывают быстрое истощение тимоцитов, другие - R5 - вирусы приводят к появлению аномалий клеток стромы железы (Uittenbogaart et al., 1996; Berkowitz et al., 1998). Так как и те и другие штаммы обычно присутствуют в организме зараженного, ВИЧ оказывает значительное влияние на тимус - источник наивных Т-клеток.

Вилочковая железа не подстраивается под изменения количественного или качественного состава Т-лимфоцитов в организме. Данные, полученные на мышах, свидетельствуют о том, что в день из вилочковой железы на периферию экспортируется примерно 1-2% общего количества тимоцитов (Berzins et al., 1999; Gabor et al., 1997; Berzins et al, 1998). Это позволяет заключить, что выход тимуса зависит в основном от массы органа (Berzins et al., 1999). Снижение продуктивной функции тимуса, вызванное уничтожением тимоцитов, и последующее прекращение постоянного притока новых Т-лимфоцитов на периферию усугубляет истощение пула Т-клеток во время хронической фазы ВИЧ-инфекции, препятствуя реконструкции иммунной системы.

1.3.3 Фаза СПИД

Когда количество периферических Т-лимфоцитов падает ниже 200 клеток/мкл, ВИЧ-инфекция переходит в третью, завершающую фазу -СПИД. По мере того, как разрушается иммунная система, больные становятся всё более подвержены таким заболеваниям как туберкулез, грибковые, вирусные и паразитарные инфекции, опухоли (Hoffmann et al.,

2006). Смерть наступает в результате неспособности организма справиться с сопутствующими заболеваниями.

1.4. Принципы высокоактивной антиретровирусной терапии

В большинстве случаев применение ВААРТ значительно снижает вирусную нагрузку в крови ВИЧ-инфицированных пациентов и удерживает ее на уровне ниже определяемого. Это приводит к восстановлению функций иммунной системы через увеличение числа периферических CD4+ Т-лимфоцитов, замедлению прогрессирования заболевания и, как следствие, увеличению продолжительности жизни больных ВИЧ-инфекцией. Показаниями к назначению ВААРТ являются: количество периферических CD4+ Т-лимфоцитов ниже 350 клеток/мкл, клиническая симптоматика и уровень вирусной нагрузки. Многие исследовательские работы продемонстрировали, что более раннее начало лечения ассоциировано с лучшими прогнозами развития заболевания (Mezzaroma et al., 1999b; Yeni et al., 2004).

ВЫВОДЫ

1. У ВИЧ-инфицированных пациентов с нарушением восстановления С04+ Т-клеток при проведении антиретровирусной терапии по сравнению с больными, дающими адекватный ответ на лечение, снижено абсолютное количество СЭ4+ наивных Т-лимфоцитов, клеток центральной и эффекторной памяти, а также терминальных эффекторов.

2. У ВИЧ-инфицированных пациентов с нарушением восстановления иммунной системы на фоне терапии дефицит наивных СБ4+ Т-лимфоцитов связан с выраженным угнетением продуктивной активности тимуса.

3. У ВИЧ-инфицированных больных, неспособных дать адекватный ответ на антиретровирусную терапию, увеличено относительное количество активированных и находящихся на ранней стадии апоптоза СЭ4+ Т-лимфоцитов.

4. На фоне полного подавления вирусной нагрузки у ВИЧ-инфицированных пациентов, неэффективно восстанавливающих число периферических Т-лимфоцитов при проведении лечения, увеличивается относительное количество пролиферирующих СБ4+ Т-клеток, что, в первую очередь, обусловлено дефицитом СЭ4+ Т-лимфоцитов; однако, из-за недостаточного абсолютного количества делящихся Т-клеток, в том числе СТ)А+СТ)\21+ Т-лимфоцитов, деление непродуктивно.

5. При нарушении восстановления численности СЭ4+ Т-лимфоцитов на фоне антиретровирусной терапии происходит выраженное накопление Р01-позитивных С04+ и СЭ8+ Т-клеток, что обуславливает развитие «иммунной усталости» Т-лимфоцитов.

Практические рекомендации

Метод оценки кольцевых ДНК а-цепи Т-клеточного рецептора для анализа продуктивной функции тимуса необходимо включать в схему обследования ВИЧ-инфицированных больных, получающих антиретровирусную терапию.
