С современных позиций ГЭРБ рассматривают как социально значимую патологию, что обусловлено высоким уровнем ее распространенности и все возрастающим экономическим бременем [12,20,171,213,249,275]. В странах Западной Европы и Северной Америки частота ГЭРБ достигает 20-27%, Юго-Восточной Азии - 2,3-10,5% [178,473]. В России этот показатель соответствует уровню в 13,3% [23]. Уже сейчас стоимость медицинского обслуживания больных ГЭРБ в связи с ее осложнениями и влиянием на болезни других систем органов в два раза выше, чем у больных без ГЭРБ [285].

Результаты недавних эпидемиологических исследований однозначно свидетельствуют о том, что регистрирующиеся в последние годы эпидемические тренды роста числа случаев и тяжести заболевания наиболее заметны в старших возрастных группах. Так, за семилетний период эрозивные эзофагиты у лиц в возрасте от 40 до 49 лет стали диагностировать в 2,6 раза, 50-59 лет - в 3,1 раза, старше 60 лет - в 4,3 раза чаще [276]. Если в 1994-2006 г.г. заболеваемость ПБ в возрастной группе 21-30 лет возросла до 2 на 100000 населения, то в группе 61-70 лет она достигла пикового уровня - 31 на 100000 [390]. В России проживает свыше 26 миллионов пожилых граждан, что составляет около 18% общей численности населения. Динамика их удельного веса, согласно отечественным и мировым данным, имеет устойчивую тенденцию к росту. К 2015 году количество жителей планеты старше 60 лет превысит 1 миллиард человек [8,16]. Учитывая прогрессивное повышение доли геронтов в общей популяции, ГЭРБ в старших возрастных группах перерастает в крупную проблему современной гастроэнтерологии. Однако на фоне тысяч ежегодных публикаций число исследований, проведенных у лиц старше 60 лет, исчисляется единицами [21,25,108,379,401,440,489]. Их, в большинстве случаев, отличает фрагментарность, небольшая выборка, частое отсутствие групп сравнения. Дискуссионными являются практически все аспекты заболевания. Более того, ни в одном из представленных исследований не анализировались вся совокупность, выделенных недавними международными консенсусами форм ГЭРБ [56,473,499]. Мало изучены факторы риска развития заболевания в старших возрастных группах. В какой мере они могут повлиять на возникновение у пожилого больного НЭРБ, ЭРБ или ПБ до сих пор остается не ясным. Возможность трансформации НЭРБ, ЭРБ и ПБ между собой с течением времени в старших возрастных группах нуждается в изучении. Опубликованные работы, выполненные в этой возрастной категории, ограничиваются лишь описанием динамики симптомов рефлюкса за пятилетний [306] и рецидивов эрозивного эзофагита за трехлетний [359] период наблюдений.

Влияние возраста на появление внепищеводных синдромов ГЭРБ расценивается неоднозначно. J.E. Richter (2000) считает их более характерными для лиц пожилого возраста. Мнение D. Garscia-Compean et al. (2000) противоположно.

Возрастной аспект морфологической диагностики ГЭРБ практически не изучен и не менее дискутабелен [25,215]. Крайне запутан вопрос влияния инфекции Н. pylori на ГЭРБ в целом и в старших возрастных группах в частности [6,156,239,329,505]. Характер патологических изменений в слизистой желудка у пожилых больных ГЭРБ изучен недостаточно. Является ли хеликобактериоз единственным фактором, влияющим у таковых на морфологическое состояние антрума и тела желудка, остается не ясным. В современной литературе отсутствуют прямые сопоставления в старших возрастных группах структурных изменений в слизистой пищевода и желудка при разных формах ГЭРБ.

Имеются лишь отдельные исследования по особенностям назначения ИПП и НЩГБ при ГЭРБ у лиц пожилого возраста [22,25,46,116]. Информации о применении других групп препаратов, а также общей концепции инициального и поддерживающего лечения ГЭРБ в старших возрастных группах в условиях продолжающейся терапии заболеваний других систем органов до сих пор нет.

Отсутствуют отечественные данные об эффективности оказания медицинской помощи пожилым больным ГЭРБ в обычной врачебной практике, качестве жизни и приверженности пожилых пациентов к лечению.

Значительное количество открытых вопросов по проблеме ГЭРБ у лиц пожилого возраста предопределило наш интерес. Очевидно, что ее решение требует комплексного подхода.

Цель исследования: изучить клинические проявления, факторы риска и эффективность лечения гастроэзофагеальной рефлюксной болезни у лиц пожилого возраста.

Задачи исследования:

1. Определить частоту НЭРБ, ЭРБ, ПБ, их осложнений и распределение типичных клинических симптомов при данной патологии у пожилых больных в сравнении с пациентами зрелого возраста.

2. Выявить факторы риска типичных клинических симптомов ГЭРБ у пожилых больных в сравнении с пациентами зрелого возраста.

3. Установить особенности действия факторов риска НЭРБ, ЭРБ, ПБ у пожилых больных в сравнении с пациентами зрелого возраста.

4. Определить частоту, клинические проявления, . факторы риска внепищеводных симптомов ГЭРБ у пожилых больных в сравнении с пациентами зрелого возраста.

5. Выделить в ходе проспективного наблюдения факторы, обуславливающие прогрессирование заболевания у пожилых больных ГЭРБ. Разработать математическую модель прогнозирования течения патологии.

6. Исследовать морфологические изменения в слизистой оболочке дистального отдела пищевода, антрального и фундального отделов желудка у больных ГЭРБ пожилого и зрелого возраста.

7. Оценить качество оказания медицинской помощи пожилым больным ГЭРБ в обычной врачебной практике, качество жизни и комплаентность пожилых больных к медикаментозной терапии. Выполнить проспективное рандомизированное исследование эффективности и переносимости некоторых современных схем для проведения инициальной и поддерживающей терапии заболевания у пожилых больных.

Научная новизна. На основании исследования большого клинического материала в условиях сопоставления данных больных разных возрастных групп определены особенности клинической симптоматики и факторов риска НЭРБ, ЭРБ, ПБ; пищеводных и внепищеводных синдромов заболевания у пожилых больных.

В ходе проведения наблюдения за когортой пожилых больных ГЭРБ со средним периодом 4,8+1,2 лет установлено прогрессирующее течение заболевания. На основании выделенных факторов отрицательной динамики ГЭРБ впервые разработаны раздельные модели прогнозирования риска неблагоприятного течения патологии у пожилых больных с типичным симптомокомплексом и наличием внепищеводных синдромов, основанные на оригинальных математических формулах.

Показана взаимосвязь структуры слизистой оболочки дистального отдела пищевода, антрального и фундального отделов желудка у пожилых больных ГЭРБ. Выявлены особенности распределения частоты и степени колонизации Н. pylori слизистой оболочки в антральном отделе желудка при различных клинических формах ГЭРБ.

Обнаружена недостаточность знаний врачей амбулаторно-поликлинического звена об особенностях течения ГЭРБ у лиц пожилого возраста, а также значительное снижение качества жизни и крайне низкий уровень комплаентности пожилых больных к медикаментозной терапии ГЭРБ.

Разработаны и апробированы схемы комплексного инициального и поддерживающего терапевтического ведения пожилых больных ГЭРБ. Установлено положительное влияние «Школы для больных ГЭРБ» на комплаентность и качество жизни пожилых больных.

Практическая значимость. Полученные в результате исследования данные о клинических особенностях и факторах риска различных форм ГЭРБ в старшей возрастной группе являются основой для разработки дифференцированного подхода к диагностическим и лечебным мероприятиям у каждой категории больных.

Возможность прогнозирования неблагоприятного течения заболевания позволяет своевременно выявлять таких лиц и создает условия для модификации факторов риска путем формирования индивидуального комплекса профилактических мероприятий у конкретного больного.

Выявленные в работе данные о качестве оказания медицинской помощи и комплаентности пациентов необходимы для оптимизации диспансерного наблюдения за больными ГЭРБ пожилого возраста.

Выполнена оценка различных схем для инициального и поддерживающего лечения клинических форм ГЭРБ у пожилых больных.

Внедрение результатов работы. Материалы работы используются в клинической, научной и педагогической деятельности на кафедрах пропедевтики внутренних болезней ИГМУ, геронтологии и гериатрии ИГИУВа, УРАМН НИИ медицинских проблем Севера СО РАМН. Результаты исследования и практические рекомендации внедрены в работу МУЗ КБ № 1, МУЗ КГВВ в Иркутске, МУЗ РКБ в Улан-Удэ, МУЗ ККБ в Чите.

Положения, выносимые на защиту:

1. Симптоматика, структура, частота осложнений пищеводных синдромов, частота и структура внепищеводных синдромов, а также их факторы риска имеют выраженные отличия у пожилых больных в сравнении с пациентами зрелого возраста.

2. У половины пожилых больных ГЭРБ с исходным отсутствием эндоскопических изменений пятилетнее проспективное наблюдение позволило обнаружить прогрессировать патологии с развитием эрозий и пищевода Барретта. Ведущим фактором риска прогрессирования ГЭРБ явилось отсутствие поддерживающей терапии ИПП.

3. Структура слизистой оболочки антрального, фундального отделов желудка и дистального отдела пищевода, частота обнаружения Н. pylori значительно отличаются у больных ГЭРБ пожилого и зрелого возраста.

4. Пятилетнее проспективное исследование установило, что поддерживающая терапия ИПП не сопровождается отчетливыми побочными явлениями и позволяет добиться контроля над симптомами у пожилых больных ГЭРБ. Длительная комбинированная терапия ИПП и УДХК повышает вероятность сохранения клинико-эндоскопической ремиссии у больных с эрозивной формой заболевания и предотвращает дальнейшую прогрессию пищевода Барретта.

Личное участие автора в получении результатов. Автором выполнен весь объем клинических и большая часть инструментальных исследований, сформирована база данных, произведена статистическая обработка полученных результатовгподготовлены и оформлены тезисы и статьи, заявки на изобретения, написана диссертация.

Апробация работы. Публикации. По теме диссертационного исследования опубликовано 43 работы, из них 10 статей в журналах, рекомендованных ВАК России. Получены положительные решения на выдачу патента РФ от 18.05.2010 г. по заявке №2009129118 и от 3.11.2010 по заявке № 2009123544.

Объем и структура диссертации. Диссертация изложена на 309 страницах. Текстовая часть состоит из введения, обзора литературы, материалов и методов исследования, 7 глав собственных исследований, обсуждения полученных результатов, выводов и практических рекомендаций. Работа проиллюстрирована 94 таблицами, 32 рисунками, 5 клиническими примерами. Указатель литературы содержит 506 источников, из них 38 работ отечественных и 468 зарубежных авторов.

ВЫВОДЫ

1. Структура основных пищеводных синдромов и распределение типичных клинических симптомов у пожилых лиц и больных зрелого возраста имеет выраженные отличия. Доля тяжелых эрозивных эзофагитов была выше в 1,2 раза, частота пищевода Барретта в 3,4 раза, а незлокачественных осложнений в 2,5 раза у пожилых больных по сравнению с пациентами зрелого возраста. Вместе с тем, частота изжоги среди пожилых пациентов при ЭРБ и ПБ была в 1,3-1,6 раза ниже, а частота отрыжки в 2,3-1,4 раза выше, чем у больных зрелого возраста. Столь выраженное отличие клинических проявлений ГЭРБ ассоциировалось с отчетливой дифференциацией кислотной продукции в желудке и превалированием щелочных рефлюксов желудочного содержимого в пищевод у пожилых больных по сравнению с пациентами зрелого возраста.

2. У пожилых больных изжога была четко связана с частотой жалоб на отрыжку, ретростернальную боль и одинофагию, тогда как у больных зрелого возраста подобная закономерность не наблюдалась. Клиническая симптоматика у пожилых больных ГЭРБ являлась более многообразной, чем у пациентов зрелого возраста. У пожилых больных ГЭРБ частота диспепсии (82,7%), билиарной дисфункции (50,9%), запоров (56,3%) была значительно более высокой, чем у пациентов зрелого возраста (36,0, 8,4, 3,5%). Это свидетельствовало о системном характере и о более значительной роли нарушений моторики у пожилых больных ГЭРБ по сравнению с пациентами зрелого возраста.

3. Факторы риска типичных симптомов ГЭРБ имели отличия у пожилых лиц и пациентов зрелого возраста. Факторами риска изжоги у пожилых больных были злоупотребление алкоголем и курением табака, прием НПВП и аспирина, абдоминальное ожирение. У больных ГЭРБ зрелого возраста появлению изжоги способствовали курение табака при отсутствии влияния дозозависимости и избыточная масса тела.

4. Факторами риска НЭРБ у пожилых больных явились употребление алкоголя, табакокурение и избыточная масса тела; у лиц зрелого возраста -женский пол. Появлению ЭРБ способствовали у пожилых больных: с эффектом дозозависимости - злоупотребление алкоголем и табакокурением, ожирение и прием аспирина; у больных зрелого возраста - ГПОД и мужской пол. Факторами риска возникновения ПБ у пожилых больных явились: злоупотребление алкоголем и табакокурением, избыточная масса тела; у больных зрелого возраста - ГПОД, злоупотребление алкоголем и табакокурением.

5. Внепищеводные синдромы были более характерными для пожилых больных ГЭРБ (58,5%), чем для пациентов зрелого возраста (24,7%). Внепищеводные синдромы у пожилых лиц были представлены преимущественно сочетанием двух и более форм (67,9%), из них чаще в комбинации бронхопульмональных и оториноларингофарингеальных синдромов. У больных зрелого возраста частота моноформы достигала 86,6%) с превалированием кардиальной формы. Частота внепищеводных синдромов у пожилых пациентов была одинаковой у лиц с НЭРБ, ЭРБ и ПБ. Среди больных зрелого возраста частота внепищеводных синдромов у больных с ПБ была в 3 раза выше, чем у больных с НЭРБ и ЭРБ. Факторами риска внепищеводных синдромов у пожилых больных установлены: злоупотребление табакокурением, прием небольшого объема алкоголя и наличие ожирения, причем в большей мере абдоминального. У пациентов зрелого возраста единственным фактором, влияющим на появление внепищеводных синдромов, явилось табакокурение у мужчин.

6. Во время пятилетнего наблюдения за пожилыми больными вероятность прогрессирования НЭРБ в ЭРБ составила 46,4%, в ПБ -2,5%. ЭРБ являлась стабильной формой у 66,8% больных, регрессировала в НЭРБ у 26,0% лиц, прогрессировала в ПБ — у 6,4% пациентов. Дисплазия эпителия нижней трети пищевода возникла за период наблюдения у 8,1% больных с ПБ. Вероятность регрессирования ГЭРБ под влиянием пятилетнего медикаментозного лечения ПБ составила 19,3%. Ведущими факторами риска прогрессирования ГЭРБ у пожилых больных были выявлены: отсутствие поддерживающей терапии ИНН (ОШ 6,2; ДИ 1,8-8,8), абдоминальное ожирение (ОШ 3,1; ДИ 2,3-3,9), злоупотребление табакокурением (ОШ 2,3; ДИ 1,5-3,1), регулярное употребление алкоголя (ОШ 2,0; ДИ 1,1-2,7). В результате применения дискриминантного анализа составлены оригинальные формулы, подтвержденные патентом на изобретение РФ, позволяющие прогнозировать неблагоприятное течение ГЭРБ у пожилых больных с точностью 97,1%).

7. У пожилых больных ГЭРБ была ассоциирована с антральным атрофическим гастритом (71,0%), относительно низкой частотой определения инфекции Н. pylori (62,2%), реальным наличием атрофии в фундальном отделе (20,7%). У пациентов зрелого возраста структура слизистой оболочки желудка имела принципиальные отличия и характеризовалась низкой частотой атрофических изменений в антральном отделе (6,9%), относительно высокими показателями частоты инфекции Н. pylori (77,9%) и отсутствием атрофии в фундальном отделе желудка. У пожилых больных с ПБ по сравнению с пациентами с НЭРБ частота атрофии в антральном отделе была выше в 1,25 раза, кишечной метаплазии в 1,7 раза, в фундальном отделе частота атрофии превышала показатели больных с НЭРБ в 2 раза. У пациентов зрелого возраста аналогичные закономерности подтверждались в антральном отделе, но не определялись в теле желудка.

8. У пожилых больных ГЭРБ, в отличие от пациентов зрелого возраста, слизистая оболочка дистального отдела пищевода характеризовалась более выраженными дисрегенераторными и дистрофическими явлениями в сочетании с более высокой активностью воспалительных изменений. У больных с ПБ в обеих возрастных группах регистрировались однонаправленные изменения, которые проявлялись гиперплазией базального слоя и удлинением сосочков эпителия нижней трети пищевода. Максимальные показатели воспалительной активности наблюдались у пациентов с эрозивным эзофагитом и язвами пищевода как у пожилых, так и пациентов зрелого возраста.

9. Открытое рандомизированное исследование в параллельных группах позволило продемонстрировать более высокую эффективность и безопасность эзомепразола при инициальной терапии у пожилых больных ГЭРБ как для быстрого купирования изжоги, так и для заживления эрозий в пищеводе в течение 4 и 8 недель. Пятилетнее проспективное исследование эффективности различных ИПП для поддержания ремиссии ГЭРБ не обнаружило отчетливых преимуществ при сравнении результатов применения эзомепразола, пантопразола, лансопразола и омепразола. Пятилетняя поддерживающая терапия ИПП не сопровождалась отчетливыми побочными явлениями и позволяла добиться контроля над симптомами у 81,8% пациентов (терапия по требованию) и у 95,1% больных (при ежедневном приеме ИПП). Отсутствие поддерживающей терапии ИПП не давало возможности контроля над симптомами у 76,9% пациентов.

10.Пятилетнее открытое проспективное исследование в параллельных группах позволило обнаружить, что эффективность комбинированной терапии омепразолом в суточной дозе 40 мг и УДХК в суточной дозе 10 мг/кг способствовала положительной динамике, заключающейся в отсутствии определения метаплазированного эпителия, у 32,3% больных с пищеводом Барретта, тогда как монотерапия омепразолом в суточной дозе 40 мг позволила достигнуть аналогичного результата у 6,5% пациентов. Частота диагностики эрозивного эзофагита снизилась через 5 лет терапии с 80,6 до

51,6% (р=0,02) в группе омепразола и с 86,7 до 16,1% (р<0,001) в группе приема омепразола и препарата УДХК

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

1. Значительная клиническая динамика у лиц пожилого возраста с симптомами ГЭРБ диктует необходимость ежегодно производить эндоскопическое обследование верхних отделов ЖКТ и тщательное диспансерное наблюдение.

2. Больным пожилого возраста с диагнозом ГЭРБ целесообразно оценить риск неблагоприятного течения заболевания. Больные, имеющие высокий риск, подлежат динамическому диспансерному наблюдению с поддерживающей терапией ИПП.

3. Всем больным пожилого возраста с трудно контролируемыми заболеваниями сердечно-сосудистой системы, органов дыхания, ЛОР-органов и полости рта должно быть выполнено углубленное гастроэнтерологическое обследование для исключения ГЭРБ.

4. Для инициальной терапии больным ГЭРБ пожилого возраста (особенно при наличии эрозивного эзофагита) целесообразно назначать оригинальные ИПП последнего поколения. Для поддерживающей терапии ГЭРБ эффективные дженерики ИПП могут применяться на равных правах с оригинальными препаратами.

5. В клинической практике ведения больных с ПБ комбинация ИПП в суточной дозе 40 мг и препарата УДХК в суточной дозе 10 мг/кг является более предпочтительной, чем монотерапия ИПП.
