Социальная острота проблемы сахарного диабета (СД) связана с кумулятивным характером заболеваемости, высокой инвалидизацией пациентов, а также с существенными финансовыми издержками, обусловленными ведением этих пациентов [1]. На протяжении последних десятилетий во всех возрастных и этнических группах отмечается неуклонный рост заболеваемости СД [387]. По данным Международной диабетической федерации (International diabetes federation), в 2011 году в мире зарегистрировано 366 млн. человек, страдающих СД [266].

Известно, что не у всех больных СД одинаково быстро формируется потребность в инсулине, и развиваются поздние осложнения. Это прежде всего определяется степенью повреждения ß-клеток поджелудочной железы. В настоящее время в качестве ключевых звеньев патогенеза повреждения ß-клеток рассматриваются аутоиммунные механизмы, глюкозо- и липотоксичность, а также системный окислительный стресс и цитокиновый дисбаланс [253]. Нет никаких сомнений в том, что простое подразделение СД на 1 и 2 типы является упрощенным. Если дебют заболевания приходится на период от 30 до 55 лет, могут иметь место различные патогенетические варианты СД, требующие дифференцированной терапевтической тактики [31].

Сравнительно недавно среди клинических форм СД был выделен медленно прогрессирующий аутоиммунный диабет взрослых (LADA) [226]. Имеющиеся в литературе данные позволяют считать, что медленное повреждение ß-клеток при LADA не является случайным, а отражает патогенетические особенности функционирования Т-звена иммунитета, отличные от механизмов развития СД1 [576]. Однако молекулярные механизмы нарушения цитокиновой регуляции при различных патогенетических вариантах СД все еще остаются невыясненными [22, 80, 117, 118, 143, 190, 421, 436, 505, 568].

До настоящего времени в классификации сахарного диабета LADA отдельно не выделяется [2], его иммунологическая диагностика рутинно не проводится. Зачастую, больные LADA не получают инсулинотерапию, которая считается патогенетически адекватным методом лечения при данном варианте СД [540]. Относительно использования у пациентов с LADA метформина и таизолидиндионов данные литературы противоречивы [103, 642]. Одним из перспективных путей патогенетического лечения пациентов с LADA является иммуноинтервенция [40, 248, 304, 345].

В целом, уточнение роли иммунологических нарушений в прогрессировании повреждения ß-клеток островков Лангерганса при LADA целесообразно для разработки новых стратегий диагностики и лечения не только этого заболевания, но и СД1 с классическим дебютом.

Цель исследования

Установить клинико-патогенетические особенности медленно прогрессирующего аутоиммунного сахарного диабета взрослых; оптимизировать алгоритм диагностики LADA.

Задачи исследования

1. Выявить клинические и метаболические особенности LADA у больных с учетом возраста дебюта, индекса массы тела, наличия осложнений сахарного диабета и потребности в инсулине в соответствии со стажем заболевания.

2. Установить диагностически значимые закономерности изменений профиля аутоантител к антигенам островков Лангерганса у больных LADA по сравнению с таковыми у пациентов с сахарным диабетом 1 типа.

3. Оценить изменения субпопуляционного состава лимфоцитов крови, общие закономерности и особенности изменений концентрации цитокинов (IL-2, -4, -10, TNF-a, IFN-y) в супернатантах культур мононуклеарных лейкоцитов крови и количества лимфоцитов, несущих комплементарные им рецепторы (IL-2Ra; TNF-Rl; IL-4Ra; IL-10R; IL-12R), а также системы Fas/Fas-L/sFas-L у больных LADA, сахарным диабетом 1 и 2 типа.

4. Оптимизировать и внедрить в клиническую практику патогенетически обоснованный алгоритм идентификации LADA у пациентов с клиническим фенотипом сахарного диабета 2 типа.

Научная новизна

Получены новые данные о клинических проявлениях (абдоминальное ожирение, артериальная гипертензия и наличие микроангиопатий) и метаболических нарушениях (гиперхолестеринемия и снижения секреции С-пептида) у больных LADA в зависимости от длительности заболевания. Впервые доказано, что дифференциальная диагностика LADA на основании клинических и метаболических признаков эффективна только при стаже заболевания более четырех лет.

Впервые установлена высокая частота ассоциации LADA с аутоиммунным тиреоидитом (32,3%).

Получены новые данные об изменении субпопуляционного состава лимфоцитов крови, изменении цитокинпродуцирующей способности мононуклеарных лейкоцитов и количестве лимфоцитов, несущих комплементарные цитокинам рецепторы, в крови у пациентов с LADA.

Установлено, что у больных LADA имеет место повышение содержания в супернатантах культур мононуклеарных лейкоцитов IL-2, IFN-y и IL-10 при неизмененном количестве в крови лимфоцитов, несущих комплементарные им рецепторы, аналогичное таковому у пациентов с СД1 и СД2. При LADA имеет место повышение концентрации IL-4 в супернатантах культур мононуклеарных лейкоцитов и количества IL-4Ra+-лимфoцитoв в крови, по сравнению с таковым у здоровых лиц. Впервые показано, что при декомпенсации углеводного обмена имеет место увеличение ФГАстимулированной концентрации IL-2 в супернатантах культур мононуклеарных лейкоцитов, а при формировании микрососудистых осложнений СД - повышение количества 1Ь-211а+-лимфоцитов в крови. Установлено, что при раннем развитии микрососудистых осложнений LADA имеет место повышение концентрации IL-2, IL-4 и TNF-a в супернатантах культур мононуклеарных лейкоцитов.

Получены новые данные о дисбалансе в системе индукторов апоптоза лимфоцитов при LADA, характеризующемся увеличением количества TNF-R1+- и Раз-Е+-лимфоцитов в крови и снижением содержания в супернатантах клеточных культур sTNF-Rl, по сравнению с таковым у больных СД1, что может обусловливать особенности течения аутоиммунного инсулита.

Предложен патогенетически обоснованный алгоритм диагностики LADA, включающий иммунологический скрининг при стаже заболевания до 4 лет и клинико-метаболическую идентификацию при большем стаже заболевания.

Теоретическая и практическая значимость

Полученные в ходе исследования фактические данные уточняют значимость клинических признаков (абдоминального ожирения, артериальной гипертензии, формирования инсулинопотребности и микроангиопатий) и метаболических нарушений (гиперхолестеринемии и снижения секреции С-пептида) в дифференциальной диагностике LADA в зависимости от длительности болезни. На основании результатов исследования оптимизирован алгоритм диагностики LADA с учетом стажа заболевания, включающий рекомендации по одновременной идентификации в сыворотке крови двух типов аутоантител (ICA и GADAb), проведению динамической оценки остаточной секреции инсулина и скрининг аутоиммунных тиреопатий, а также по раннему назначению инсулинотерапии с целью сохранения секреции инсулина.

Результаты исследования могут служить основанием для дальнейшей оценки молекулярных и клеточных механизмов формирования LADA, разработки на основе полученных фактических данных патогенетически обоснованных подходов к диагностике и лечению больных LADA посредством идентификации специфичных молекулярных диагностических маркеров и фармакологических мишеней.

Положения, выносимые на защиту

1. Клинические признаки (меньшая частота абдоминального ожирения и артериальной гипертензии, по сравнению с таковыми у больных классическим СД2, а также наличие микроангиопатий), снижение секреторного резерва С-пептида и формирование потребности в инсулинотерапии позволяют формировать группы скрининга LADA лишь среди пациентов с клиническим фенотипом СД2 с длительностью болезни более четырех лет. С целью верификации LADA при меньшем стаже заболевания целесообразна одновременная идентификация GADAb и ICA в сыворотке крови у всех больных, позволяющая выявлять LADA с высокой чувствительностью (90,3%).

2. LADA ассоциируется с аутоиммунным тиреоидитом чаще (32,3%), чем СД1 (5,4%) и СД2 (10,4%), что обуславливает целесообразность ежегодного скрининга аутоиммунных тиреопатий у данной группы пациентов.

3. У больных LADA имеют место изменения субпопуляционного состава лимфоцитов крови (повышение количества СОЗ+-лимфоцитов и снижение числа CD16+CD56+-KJieTOK, по сравнению с таковым у здоровых лиц и больных СД2), системы цитокинов и комплементарных им рецепторов (повышение концентрации IL-2, IFN-y, IL-10 и IL-4 в супернатантах культур мононуклеарных лейкоцитов и числа IL-4Ra+-лимфоцитов, по сравнению с таковым у здоровых лиц), а также дисбаланс в системе рецепторопосредованного апоптоза лимфоцитов увеличение количества TNF-R1+- и Раз-Ь+-лимфоцитов в крови и уменьшение содержания в супернатантах клеточных культур sTNF-Rl, по сравнению с таковым у больных СД1). 4. Раннее развитие микрососудистых осложнений LADA характеризуется выраженными метаболическими (резкое падение секреции С-пептида после 4-го года заболевания) и иммунологическими изменениями (высокий уровень IL-2, IL-4 и TNF-a в супернатантах культур мононуклеарных лейкоцитов).

Внедрение и апробация работы

Основные положения диссертации были доложены и обсуждены на научной конференции «Нейрогуморальные механизмы регуляции висцеральных органов и систем в норме и при патологии» (Томск, 2009), конференции с международным участием «Актуальные проблемы медицины» (Абакан, 2010), межгородской конференции молодых ученых «Актуальные проблемы патофизиологии» (Санкт-Петербург, 2010), XI конгрессе молодых ученых и специалистов «Науки о человеке» (Томск,

2010), V Всероссийском диабетологическом конгрессе (Москва, 2010), II i

Съезде терапевтов Сибири и Дальнего Востока (Новосибирск, 2010), конференциях «Актуальные вопросы эндокринологии» (Томск, 2010 и

2011), всероссийской научно-практической конференции с международным участием «Сахарный диабет, метаболический синдром и сердечнососудистые заболевания» (Томск, 2012) и на заседаниях общества эндокринологов (Томск, 2009-2012).

Исследование выполнено в рамках Федеральной целевой программы «Научные и научно-педагогические кадры инновационной России» на 20092013 годы (проект «Разработка технолргической платформы молекулярной диагностики и лечения социально значимых заболеваний и подготовка на ее основе научно-исследовательских кадров для молекулярной медицины», ГК № 02.740.11.0311) и проекта «Молекулярные механизмы цитокинопосредованной дисрегуляции апоптоза лимфоцитов при поляризации иммунного ответа по ТЫ- или ТЬ2-пути» (ГК № 02.120.11.3842-МД), поддержанного Советом по грантам при Президенте Российской Федерации.

Результаты работы внедрены в учебный процесс на кафедрах эндокринологии и диабетологии, патофизиологии по направлению «Патофизиология обмена веществ», молекулярной медицины и клинической лабораторной диагностики по направлению «Лабораторная диагностика эндокринопатий», а также в лечебный процесс в эндокринологической клинике и исследовательскую деятельность Научно-образовательного центра молекулярной медицины ГБОУ ВПО СибГМУ Минздрава России.

По материалам диссертации опубликовано 14 научных работ, из которых 6 - в центральных рецензируемых журналах, рекомендованных ВАК Минобрнауки России.

Объем и структура работы

Диссертация изложена на 226 страницах машинописного текста и состоит из введения, 4 глав (обзор, литературы, материал и методы исследования, результаты собственных исследований, обсуждение полученных результатов; иллюстрированы 29 таблицами и 21 рисунками), выводов и списка литературы, включающего 645 источника, из них 33 -отечественных и 612 - зарубежных.

ВЫВОДЫ

1. Течение медленно прогрессирующего аутоиммунного диабета взрослых (LADA) характеризуется отсутствием специфичных клинических признаков до четвертого года заболевания, сопоставимой частотой манифестации заболевания в разных возрастных группах (19-30, 30-50 и 50-63 лет) и отсутствием достоверных различий по частоте возникновения диабетических микроангиопатий, по сравнению с СД1 и СД2.

2. Регистрируемые у пациентов с клиническим фенотипом СД2 при стаже заболевания более четырех лет клинические признаки (меньшая частота абдоминального ожирения, артериальной гипертензии и гиперхолестеринемии), а также сравнительно быстрое формирование инсулинопотребности и микроангиопатий, вызванное снижением стимулированной секреции С-пептида, позволяют формировать группы скрининга LADA.

3. Спектр аутоантител в сыворотке крови у больных LADA характеризуется превалированием антител к глутаматдекарбоксилазе (38,7%), антител к островковым клеткам (71%) и меньшей частотой встречаемости в сыворотке крови антител к инсулину (9,7%), по сравнению с их уровнем у пациентов с СД1 (8,1; 24,3 и 37,8%, соответственно). Одновременная идентификация антител к островковым клеткам и антител к глутаматдекарбоксилазе позволяет диагностировать LADA в 90,3% случаев.

4. LADA ассоциируется с хроническим аутоиммунным тиреоидитом статистически значимо чаще (32,3%), чем СД1 (5,4%) и СД2 (10,4%).

5. При СД аутоиммунного генеза (LADA и СД1) имеют место изменения субпопуляционного состава лимфоцитов крови, которые характеризуются повышением числа CD3+-лимфоцитов и снижением количества CD16+CD56+-KneTOK, по сравнению со значениями соответствующих показателей у здоровых доноров и больных СД2.

6. К общим закономерностям изменений системы цитокинов и комплементарных им рецепторов у больных LADA, СД1 и СД2 относится повышение концентрации IL-2, IFN-y и IL-10 в супернатантах культур мононуклеарных лейкоцитов крови при неизмененном количестве лимфоцитов, несущих комплементарные им рецепторы. При LADA имеет место повышение содержания в супернатантах культур клеток IL-4 и количества 1Е-4Ка+-лимфоцитов в крови, по сравнению с таковыми у здоровых доноров. При выраженной декомпенсации углеводного обмена (HbAlc выше 10%) отмечается увеличение ФГА-стимулированной концентрации IL-2 в супернатантах. LADA также характеризуется дисбалансом системы рецепторопосредованного апоптоза с увеличением количества TNF-R1+- и РаБ-Е+-лимфоцитов и снижением содержания sTNF-Rl в супернатантах клеточных культур, по сравнению с таковым у больных СД1.

7. Раннее развитие микрососудистых осложнений LADA характеризуется выраженными метаболическими (резкое падение секреции С-пептида после 4-го года заболевания) и иммунологическими изменениями (высокий уровень IL-2, IL-4 и TNF-a в супернатантах культур мононуклеарных лейкоцитов).

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

1. С целью верификации диагноза LADA целесообразно одновременно идентифицировать присутствие в сыворотке крови двух типов аутоантител (ICA и GADAb).

2. Для ранней диагностики инсулиновой недостаточности у больных LADA следует с периодичностью в 6 мес определять в сыворотке крови уровни С-пептида натощак и на 120-й мин ТТСП.

3. Пациенты с LADA требуют проведения ежегодного скрининга с целью своевременного выявления аутоиммунных тиреопатий, включающего проведение ультразвукового исследования щитовидной железы, определение в сыворотке крови уровня тиреотропного гормона и антител к тиреопероксидазе.

4. С целью сохранения остаточной секреции инсулина при LADA показано раннее назначение инсулинотерапии в средних дозах 0,5 ЕД на I килограмм массы тела с отменой производных сульфонилмочевины.
