ВВЕДЕНИЕ

Актуальность исследования

Одной из наиболее актуальных задач современной физиологии является изучение механизмов пластичности мозга. Под нейропластичностью понимают способность нейронов адаптивно изменять свои морфо-функциональные свойства в онтогенезе, при обучении, а также при дегенерации (Trojan and Pokorny, 1999). Исследования последних десятилетий показали, что механизмы пластичности мозга в норме и при патологии качественно не отличаются, однако в последнем случае они выражены количественно в гораздо большей степени. Яркой иллюстрацией высокой эффективности пластичности мозга является длительное бессимптомное течение нейродегенеративных заболеваний на фоне гибели подавляющего большинства специфических нейронов, обеспечивающих контроль соответствующей функции. Особый интерес вызывают резервы пластичности нигростриатной дофаминергической (ДА-ергической) системы, поскольку её деградация лежит в основе одного из наиболее распространённых нейродегенеративных заболеваний - болезни Паркинсона (БП) (Ehringer and Hornykiewicz, 1960).

Нигростриатная система представлена ДА-ергическими нейронами компактной части черной субстанции (ЧС) и их аксональными проекциями в стриатуме (Rosegay, 1944). Она является ключевым звеном центральной регуляции моторного поведения (Суворов, 1980). Дофамин, действуя на нейроны стриатума, играет роль стимулирующего нейромедиатора, способствующего повышению двигательной активности, уменьшению двигательной заторможенности и скованности, снижению гипертонуса мышц (Арушанян и Отеллин, 1976).

При достижении порогового уровня деградации нигростриатной системы (на 60-80%) начинает проявляться её функциональная недостаточность в виде моторных симптомов БП: тремора, ригидности мышц, брадикинезии и постуральной нестабильности (Крыжановский и др., 2002; Bernheimer et al.,

1973; Riederer and Wuketich, 1976). В результате заболевание приводит к инвалидизации и гибели больного.

Надпороговая гибель ДА-ергических нейронов ЧС может продолжаться на протяжении многих лет (Ehringer and Hornykiewicz, 1960). Длительный период бессимптомного течения БП связывают с компенсаторной активацией сохранившихся нейронов нигростриатной системы, выраженной в усилении синтеза и выделения ДА в синаптическую щель, а также в снижении скорости его обратного захвата (Stachowiak et al., 1987; Zigmond, 1997; Bezard and Gross, 1998; Trojan and Pokorny, 1999). Данные процессы направлены на поддержание внеклеточной концентрации ДА в ЧС и в стриатуме на уровне, достаточном для регуляции произвольной двигательной активности.

Компенсаторные резервы сохранившихся ДА-ергических нейронов остаются неизвестными, т.к. в настоящее время БП диагностируется при их «исчерпании» - после появления моторных симптомов заболевания. Оценка компенсаторных резервов сохранившихся нейронов представляется возможной только при сравнительном анализе функциональных показателей ДА-ергических нейронов на двух стадиях развития БП - досимптомной и симптомной, соответствующих надпороговому и пороговому уровням деградации нигростриатной системы соответственно. Поскольку изучение состояния нигростриатной системы человека на досимптомной стадии развития БП невозможно ввиду отсутствия доступной своевременной диагностики, выходом из положения является экспериментальное моделирование данного заболевания на животных. Модели БП служат не только для изучения компенсаторных резервов нигростриатной системы, но и для поиска эндогенных биомаркеров заболевания с целью разработки методов ранней диагностики и превентивного лечения БП.

Наиболее близко клиническую картину заболевания воспроизводят модели БП, на которых дегенерацию нейронов вызывают с помощью специфических нейротоксинов (Gerlach and Riederer, 1996; Strome and Doudet, 2007). Однако подавляющее большинство существующих моделей основано на

использовании полулетальных доз нейротоксинов, что приводит к быстрой гибели порогового числа ДА-ергических нейронов нигростриатной системы и проявлению симптоматики, т.е. к развитию выраженной симптомной стадии заболевания, минуя досимптомную (Bezard et al., 1997; Petroske et al., 2001; Bezard et al., 2003; Rousselet et al., 2003; Gibrat et al., 2009). И только ограниченное число работ посвящено моделированию доклинической стадии паркинсонизма и её изучению с использованием ряда методов - поведенческих, биохимических и морфологических. При этом исследований, в которых одновременно моделировали бы несколько стадий БП (и досимптомную, и симптомную) с целью их сравнительного анализа, - единицы (Pifl and Hornykiewicz, 2006; Perez et al., 2008). Причем, ни в одном из указанных исследований не дана комплексная оценка всех трёх предполагаемых механизмов компенсации функций погибших нейронов нигростриатной системы - синтеза ДА, его выделения и обратного захвата. Поэтому первая часть настоящей работы посвящена изучению компенсаторных резервов сохранившихся ДА-ергических нейронов нигростриатной системы в процессе прогрессирования её деградации (при переходе досимптомной стадии БП в симптомную).

Заманчиво предположить, что полученные в первой части работы данные могли бы быть использованы в терапии БП - медикаментозном управлении компенсаторными процессами. Так, в случае снижения эффективности того или иного функционального показателя сохранившихся ДА-ергических нейронов его искусственное усиление могло бы продлить стадию бессимптомного течения заболевания. Вероятно, данный терапевтический подход имел бы успех при его применении до момента срыва компенсаторных процессов, а следовательно, на досимптомной стадии развития БП. Однако единственный метод, который в настоящее время может позволить диагностировать БП в доклинической стадии - позитронно-эмиссионная томография (ПЭТ) (DeKosky and Marek, 2003), является довольно дорогостоящим, а поэтому не может быть использован при диспансеризации населения с целью выявления людей со

скрытой формой БП. В связи с этим второй задачей нашего исследования была доклиническая разработка легкодоступного метода диагностики БП на досимптомной стадии течения заболевания. Суть подхода заключается в кратковременном частичном выключении синтеза ДА в ДА-ергических нейронах нигростриатной системы путем системного введения обратимого ингибитора тирозингидроксилазы (ТГ) - ключевого фермента синтеза ДА. При этом предполагается, что ингибитор в той дозе, в которой он не вызывает изменения моторного поведения в норме при наличии 100% ДА-ергических нейронов, спровоцирует кратковременное нарушение моторного поведения у мышей на модели досимптомной стадии БП с надпороговым уровнем погибших нейронов.

Цель и задачи исследования

Целью настоящей диссертационной работы было изучение компенсаторных процессов в нигростриатной системе при различной степени её деградации, а также доклиническая разработка доступного метода диагностики БП на досимптомной стадии течения заболевания. Для достижения цели были поставлены следующие задачи:

1. Оценка основных функциональных характеристик нигростриатной ДА-ергической системы на моделях досимптомной и симптомной стадий БП у мышей: скорости синтеза, выделения и обратного захвата ДА.

2. Фармакологическая «провокация» кратковременных нарушений моторного поведения у мышей на модели досимптомной стадии БП.

Научная новизна полученных результатов

В настоящей работе впервые получена информация о функциональном состоянии отдельных сохранившихся ДА-ергических нейронов при различной степени деградации нигростриатной системы мышей, соответствующей досимптомной и симптомной стадиям БП у человека. Дана

оценка активности ТГ, скорости синтеза, выделения и обратного захвата ДА. На основании полученных данных проведен сравнительный анализ двух стадий паркинсонизма, что позволило выяснить, изменение какого(-их) из свойств нейронов может быть одной из причин появления нарушений моторного поведения.

В исследованиях на экспериментальной модели досимптомной стадии БП успешно разработана принципиально новая, легко воспроизводимая и экономичная технология диагностики скрытой формы заболевания - так называемый «провокационный тест», основанный на кратковременном частичном выключении синтеза ДА в ДА-ергических нейронах нигростриатной системы путем введения обратимого ингибитора ТГ -ключевого фермента синтеза ДА. При этом впервые показано отсутствие длительных и необратимых эффектов выбранного ингибитора ТГ на активность фермента в сохранившихся ДА-ергических нейронах деградировавшей нигростриатной системы, а также на общее содержание ДА в телах и терминалях нейронов и моторную активность животных.

Практическая значимость полученных результатов

Полученные в ходе наших исследований данные расширяют и углубляют представления о потенциальных возможностях ДА-ергических нейронов изменять эффективность основных функциональных свойств -скорости синтеза, выделения и обратного захвата ДА при прогрессирующей деградации нигростриатной системы. Это, в свою очередь, позволяет приблизиться к пониманию механизмов, лежащих в основе появления нарушений двигательной активности при БП, а следовательно, может быть использовано при разработке новых или совершенствовании существующих методов терапии заболевания.

Разработанный нами метод краткосрочной фармакологической «провокации» нарушений произвольной двигательной активности на досимптомной стадии развития БП ввиду успешной апробации в

исследованиях на животных может быть рекомендован для клинических испытаний в специализированных неврологических клиниках с целью выявления скрытой формы данного заболевания у людей.

1. Охарактеризовано функциональное состояние нигростриатной ДА-ергической системы у мышей при различной степени её деградации, соответствующей досимптомной и симптомной стадиям БП у человека (дана оценка скорости синтеза, выделения и обратного захвата ДА): а) в черной субстанции на обеих стадиях БП все функциональные показатели соответствуют уровню в контроле за счет активации сохранившихся тел ДА-ергических нейронов на фоне прогрессирования деградации нигростриатной системы; б) в стриатуме на досимптомной стадии БП скорость стимулированного выделения и обратного захвата ДА выше контроля, а на симптомной стадии стимулированное выделение нейромедиатора снижается при неизменной скорости его обратного захвата, что может приводить к уменьшению содержания ДА в синаптической щели и к появлению нарушений двигательной активности.

2. Разработан метод диагностики БП на досимптомной стадии течения заболевания: а) на модели досимптомной стадии БП у мышей путем применения аМПТ - обратимого ингибитора тирозингидроксилазы, спровоцировано появление моторных симптомов заболевания, протекающего в скрытой форме; б) показан кратковременный и обратимый эффект аМПТ на активность тирозингидроксилазы в сохранившихся ДА-ергических нейронах частично деградировавшей нигростриатной системы, а также на содержание катехоламинов в телах и терминалях нейронов и моторную активность животных.
