Актуальность темы. При адаптации к систематическим мышечным нагрузкам происходит оптимизация функционирования аппарата кровообращения, связанная с экономизацией функций в покое и при субмаксимальных нагрузках и максимизацией функций при предельных режимах мышечной деятельности (Карпман B.JL, Любина Б.Г., 1982). Реологические свойства крови как составное звено кровообращения под влиянием физической тренировки также оптимизируются, в результате чего вязкость крови снижается. Механизмы, ответственные за повышенную эффективность функционирования аппарата кровообращения и его различных подсистем, у спортсменов полностью не ясны.

Реологические свойства крови играют важную роль в обеспечении транспортных и гомеостатических функций кровообращения, особенно на уровне микрососудистого русла (Чернух A.M. и др., 1984). Вязкость крови и плазмы вносит существенный вклад в сосудистое сопротивление кровотоку и влияет на минутный объем крови (Гайтон А.С., 1969; Фолков Б., Нил Э., 1976). На уровне микроциркуляции деформируемость эритроцитов является абсолютно необходимой для поддержания жизнеспособности тканей (Schmid-Schonbein Н., 1988). Снижение деформируемости эритроцитов повышает местное сосудистое сопротивление при входе и прохождении клетками капилляров, лимитирует транспорт кислорода к тканям (Hakim T.S., 1988; Frank R.S. et al., 1988; Lipowsky H.H. et al., 1993). Агрегация эритроцитов наряду с деформируемостью играет важную роль в перфузии микрососудов и в венозном кровотоке: нормальный уровень агрегации способствует снижению сосудистого сопротивления в одиночных сосудах за счет увеличения осевого тока эритроцитов (Cokelet G.R. and Goldsmith H.L., 1991; Baskurt O.K. et al., 1999), однако чрезмерная агрегация ведет к повышению органного и венозного сопротивления за счет роста сопротивления кровотоку в местах ветвления сосудов (Vicaut Е. et al., 1994; 1995) и повышения вязкости крови в венозном отделе (Cabel М. et al., 1997; Bishop J J. et al., 2001). Повышение текучести крови увеличивает кислородтранспортные возможности крови, что может играть важную роль в повышении физической работоспособности. С другой стороны, гемореологические показатели могут быть маркерами ее уровня и синдрома перетренировки (Муравьев А.В. и др., 1995; Dintenfass L. et al., 1977; Brun J.F., 2002).

Снижение вязкости крови у спортсменов зарегистрировано многими исследователями как в срезовых, так и лонгитудинальных исследованиях (Викулов А.Д., 1997; Dintenfass L., Lake В., 1977; Ernst Е. et al., 1986; 1987; Brun J.F. et al., 2000). Вместе с тем, физиологические механизмы, ответственные за данные перестройки, полностью не ясны, особенно при разном характере мышечной деятельности. Общепринято, важным механизмом снижения вязкости крови, вязкости плазмы и гематокритного показателя является аутогемодилюция - накопление воды в сосудистом русле (Convertino V.A. et al., 1980; 1991; Ernst E. et al., 1991; Brun J.F. et al., 2000). С другой стороны, ряд авторов показывает, что гемодилюция при физической тренировке может быть уравновешена повышенным эритропоэзом и протеиновым синтезом (Назаров С.Б. и др., 1988; Mairbaurl Н. et al., 1983; Vergouwen Р.С. et al., 1999; Nagashima К. et al., 2000). Увеличение текучести крови у спортсменов может быть обусловлено повышенной деформируемостью (Викулов А.Д., 1997; Smith J.А. et al., 1999) и сниженной агрегацией эритроцитов (Hardeman M.R. et al., 1995). Однако работ, исследующих механизмы изменения реологических свойств эритроцитов, не много. Результаты клинических и экспериментальных исследований показывают, что реологические свойства эритроцитов, плазмы могут модулироваться липидно-белковым и ионным составом плазмы, составом тела, активностью фибрино(гено)лиза, возрастным составом эритроцитов, гормональной активностью, функциями эндотелия, оксидативным стрессом (Brun J.F., 2002). Настоящая работа предпринята для анализа факторов, которые участвуют в механизмах изменений реологических свойств крови (РСК) у спортсменов.

Цель работы - провести комплексный анализ факторов, лежащих в основе изменений реологических свойств крови у спортсменов.

Задачи исследования:

1. Изучить РСК у спортсменов в покое с учетом направленности тренировочного процесса.

2. Исследовать взаимосвязь РСК с аэробной и анаэробной работоспособностью спортсменов.

3. Исследовать взаимосвязь РСК с показателями липопротеидного и белкового профилей плазмы у спортсменов.

4. Изучить взаимосвязь РСК с показателями минерального обмена у спортсменов.

5. Исследовать взаимосвязь РСК с половыми гормонами (тестостероном, эстрадиолом), тиреотропным гормоном, трийодтиронином и кортизолом у спортсменов.

6. Исследовать взаимосвязь РСК с показателями системы гемостаза, противосвертывающей системы крови и эндотелиальных функций у спортсменов.

7. Исследовать взаимосвязь РСК с показателями кардио- и гемодинамики у спортсменов с учетом направленности тренировочного процесса.

8. Изучить РСК у лиц старшего возраста (>40 лет) с повышенным режимом двигательной активности.

Научная новизна. Впервые установлены особенности РСК у спортсменов с разным характером мышечной деятельности. Выявлено, что при повышении общей физической работоспособности у спортсменов вязкость крови снижается. При тренировке силового характера установлено повышение вязкости цельной крови за счет роста гематокритного показателя. Общей закономерностью у спортсменов разных специализаций было повышение деформируемости эритроцитов. Впервые установлена связь РСК у спортсменов с возрастным составом эритроцитов.

Впервые выполнено комплексное исследование взаимосвязей РСК с ионным, гормональным и антикоагулянтным профилями плазмы, а также функциональным состоянием эндотелия сосудов у спортсменов. Показано, что состояние РСК связано с показателями минерального обмена: концентрациями ионов фосфора, натрия, калия, магния и железа. Связи РСК с половыми гормонами, кортизолом, тиреоидным и тиреотропным гормонами обусловлены их участием в метаболических процессах. Повышение вязкости плазмы у спортсменов взаимосвязано с активацией сосудистого эндотелия и ростом плазменных уровней антикоагулянтов: ATIII, nS, ПС.

Впервые показано, что повышение деформируемости и снижение агрегации эритроцитов у спортсменов взаимосвязаны с улучшением липидного профиля сыворотки: снижением ХС, ХСЛПНП и повышением ХСЛПВП.

Впервые проведено комплексное исследование взаимосвязей между РСК и показателями аэробной, анаэробной работоспособности. Выявлена взаимосвязь физической работоспособности с деформируемостью эритроцитов.

Получены новые данные о взаимосвязи системной гемодинамики с реологическими свойствами крови: снижение вязкости крови сочеталось с уменьшением сердечной производительности и повышенным периферическим сосудистым сопротивлением у спортсменов в состоянии покоя. Установлены новые факты об особенностях реологических свойств крови у физически активных лиц старшего возраста.

Теоретическая и практическая значимость. Полученные результаты существенно дополняют знание о механизмах изменения РСК при адаптации организма к регулярным мышечным нагрузкам разного характера. Выявленные взаимосвязи РСК с различными системами организма позволяют глубже понять физиологический смысл изменений в результате тренировки. Результаты работы помогут выявить те физиологические мишени, воздействуя на которые можно корригировать и управлять текучестью крови, а через нее - системой транспорта кислорода, ключевой для физической работоспособности, спортивных результатов.

Результаты исследования могут быть использованы в спортивной медицине и практике врачебно-педагогического контроля. Важное значение они могут иметь в диагностике степени восстановления спортсменов после физических нагрузок. Данные можно применять в профилактической медицине: повышение текучести крови средствами физической культуры способствует снижению риска сердечно-сосудистых расстройств. Обследование лиц старшего возраста показывает, что регулярные физические нагрузки способны поддерживать высокую текучесть крови и деформируемость эритроцитов, существенно нивелировать ряд возрастных изменений. Новые сведения можно использовать при написании учебных пособий по физиологии спорта и спортивной медицине.

Основные положения, выносимые на защиту.

1. У спортсменов в покое, тренирующихся в видах спорта с преимущественным проявлением выносливости, вязкость крови снижена за счет изменений гематокрита и деформируемости эритроцитов. У спортсменов, тренирующихся в видах спорта с преимущественным проявлением силы, без повышения аэробной работоспособности снижение вязкости крови отсутствует.

2. Снижение вязкости крови и повышение индекса транспорта кислорода кровью взаимосвязаны с аэробной работоспособностью.

3. Кровь спортсменов отличается относительным сдвигом возрастного состава эритроцитов в сторону омоложения, что обусловливает у них повышение деформируемости и снижение агрегации эритроцитов.

4. Повышение деформируемости эритроцитов у спортсменов взаимосвязано с понижением индекса атерогенности липидов.

5. Главной причиной снижения вязкости плазмы и агрегации эритроцитов у спортсменов является пониженная концентрация фибриногена плазмы.

6. Увеличение вязкости плазмы у спортсменов взаимосвязано с активацией сосудистого эндотелия и повышением антикоагулянтного потенциала.

7. Взаимосвязи реологических свойств крови с кортизолом, половыми и тиреотропным гормонами опосредованы участием этих гормонов в изменении уровней фибриногена, холестерина, ХСЛПНП, ХСЛПВП и триглицеридов, ретикулоцитов.

8. Повышение текучести крови у спортсменов, тренирующихся на выносливость, сопряжено с понижением СИ и повышением УПСС.

9. Лица старшего возраста с повышенным режимом двигательной активности отличаются от более молодых спортсменов по вязкости плазмы и агрегации эритроцитов. Физическая тренировка не может полностью компенсировать физиологические изменения, связанные с возрастом.

253 ВЫВОДЫ

1. Вязкость крови у спортсменов с высоким уровнем общей физической работоспособности в состоянии относительного покоя снижена. Это обусловлено уменьшением гематокритного показателя и увеличением деформируемости эритроцитов. В основе снижения гематокрита лежит аутогемодилюция. При силовой направленности тренировочного процесса вязкость крови повышена за счет гематокритного показателя. Общей особенностью изменений реологических свойств крови у спортсменов является повышение деформируемости эритроцитов. Один из важных механизмов высокой деформируемости эритроцитов - относительный сдвиг возрастного состава эритроцитов в сторону молодых форм.

2. Повышение текучести крови у спортсменов взаимосвязано с высоким уровнем аэробной и анаэробной физической работоспособности, сила взаимосвязи выше с аэробной работоспособностью. Механизм взаимосвязи обусловлен повышением кислородтранспортных свойств крови вследствие увеличения ее текучести. Важными маркерами высокой физической работоспособности являются индекс транспорта кислорода кровью (Ht/BK) и индекс ригидности эритроцитов (Tk).

3. Изменения белково-липидного профиля плазмы, вызванные физической тренировкой, существенно влияют на реологические свойства крови. Ключевым фактором понижения вязкости плазмы и агрегации эритроцитов у спортсменов является уменьшение концентраций фибриногена и холестерина плазмы. Увеличение деформируемости эритроцитов взаимосвязано с понижением индекса атерогенности липидов.

4. Реологические свойства крови взаимосвязаны с состоянием минерального обмена: концентрациями ионов калия, фосфора, натрия, магния и железа сыворотки. Увеличение деформируемости эритроцитов у спортсменов сопряжено с повышением уровней ионов калия и неорганического фосфора сыворотки. Взаимосвязь вязкости крови и вязкости плазмы с уровнем натрия обусловлена эффектами аутогемодилюции: уровень натрия положительно коррелировал с гематокритом, концентрациями общего белка, глобулинов, холестерина. Деформируемость эритроцитов у спортсменов связана с состоянием магниевого баланса и отрицательно коррелирует с концентрацией железа сыворотки.

5. В механизмах повышения текучести крови у спортсменов важную роль играют гормональные изменения, вызванные физической тренировкой. Взаимосвязи реологических свойств крови с кортизолом, половыми, тиреоидным и тиреотропным гормонами опосредованы их участием в метаболических процессах. Повышение текучести крови и плазмы взаимосвязано с пониженными концентрациями общего тестостерона, механизм корреляций обусловлен участием гормона в регуляции уровней гематокрита, триглицеридов и холестерина. Ускорение метаболизма эстрадиола у спортсменов сопряжено с уменьшением концентраций фибриногена, холестерина и ХСЛПНП, а также понижением вязкости крови и плазмы. Рост концентрации кортизола, вызванный физической тренировкой, сопровождался повышением текучести плазмы через гормональные эффекты в регуляции метаболизма триглицеридов и уровня IgG. Корреляция сниженной ригидности эритроцитов с повышенным уровнем тиреотропного гормона у спортсменов опосредована взаимосвязями гормона с ретикулоцитозом.

6. Текучесть плазмы у спортсменов взаимосвязана с функциональным состоянием эндотелия сосудов. Увеличение вязкости плазмы у спортсменов с низкой физической работоспособностью, в основном, за счет концентрации глобулинов сопряжено с компенсаторно-повышенным уровнем антикоагулянтов (ПС, nS, ATIII) и активацией сосудистого эндотелия: повышенным уровнем антигена фактора фон Виллебранда. Снижение уровня фибриногена, ключевого фактора вязкости плазмы и агрегации эритроцитов, у спортсменов с высоким уровнем физической работоспособности тесно связано с повышенной фибринолитической активностью (пониженной активностью плазминогена).

7. Текучесть крови взаимосвязана с состоянием кардиодинамики и системной гемодинамики в покое. Характер взаимосвязей указывает на компенсаторное повышение производительности сердца и снижение периферического сосудистого сопротивления при повышении факторов, определяющих вязкость крови. Снижение вязкости крови играет важную роль в оптимизации функционирования кровообращения в тренированном организме через уменьшение нагрузки на сердце по преодолению вязкостного сопротивления кровотоку и через увеличение преднагрузки на сердце, а также участвует в механизмах понижения сердечного выброса.

8. Возрастные особенности занимающихся физическими упражнениями оказывают модулирующее влияние на реологические свойства крови. Многолетние физические нагрузки у лиц старшего возраста имеют ряд положительных эффектов в отношении вязкости крови и деформируемости эритроцитов, однако повышение уровней фибриногена и холестерина с возрастом обусловливали увеличение вязкости плазмы и агрегации эритроцитов.

В заключение, полученные результаты позволили выявить многие факторы, вовлеченные в физиологические механизмы изменений реологических свойств крови у спортсменов. Показано, что текучесть крови при адаптации к интенсивным физическим нагрузкам, связанной с ростом физической работоспособности, повышена, во многом, за счет увеличения деформируемости эритроцитов. По-видимому, основой механизмов повышения текучести крови является ускорение метаболических процессов в организме спортсмена, что ведет к модуляции состава тела, накоплению воды в сосудистой системе, омоложению клеток крови, снижению уровня плазменных белков и липопротеидов. Важную роль в этих процессах играет гормональная регуляция метаболических процессов, высокая инсулиновая чувствительность тканей. Выявленные различия небольшие в процентном выражении, но статистически достоверные, по-видимому, это связано с относительностью контроля: в нашей работе это - молодые (18-22 года) здоровые лица, имеющие нормальный общий уровень двигательной активности. В случае более старшего контроля различия скорее будут возрастать. Вместе с тем, мы не считаем, что «чем больше, тем лучше», тренировка скорее оптимизирует функционирование как всей системы кровообращения, так и гемореологических свойств так, что-показатели будут стремиться к некоторому оптимуму. По-видимому, имеются индивидуальные особенности оптимума. Возрастные особенности тренирующихся лиц оказывают модулирующее влияние на процессы метаболизма липидов, фибриногена и связанные с ними реологические свойства крови. Однако работа имеет ряд ограничений: 1) наше исследование имеет срезовый характер, что скрывает индивидуальную динамику изменений у спортсменов в процессе тренировок; 2) установленные корреляционные связи не дают ответа на причинность полученных результатов, а только на параллельность/сопряженность изменений. Вместе с тем, статистически достоверный характер полученных различий и корреляционных взаимосвязей позволяет ответить нам на поставленные задачи и сделать выводы.
