Актуальность исследования.

Течение и прогноз острого инфаркта миокарда (ИМ) тесно связаны со своевременной и эффективной реперфузией миокарда. Общепринятая терапия, включая применение тромболитических препаратов, имеет существенные ограничения и отрицательные стороны. В связи с этим в последнее время нашла широкое применение механическая реперфузия с помощью транслюминальной баллонной коронарной ангиопластики (ТБКА) без предшествующего тромболизиса (TJ1T) - первичная ТБКА. Однако, несмотря на развитие техники ТБКА и адекватную антикоагулянтную терапию, у многих пациентов после первичной ТБКА остается существенный резидуальный стеноз, который вместе с гиперплазией интимы приводит в течение полугода наблюдения к рестенозу в 30-50% случаев. В результате требуется либо повторная коронарная ангиопластика, либо хирургическое вмешательство.

Использование в клинической практике стентирования коронарных ар1ерий уменьшает число случаев рецидивов ишемии и ранних рестенозов по сравнению с первичной ТБКА. Однако первичное стентирование при остром ИМ нашло применение лишь в последнее время. Остается спорным вопрос об эффективности применения первичного стентирования при остром ИМ, осложненном сердечной недостаточностью, и в частности -кардиогенным шоком (КШ). Неясны вопросы течения заболевания и прогноз у больных после первичного стентирования. В литературе мало данных о применении ингибиторов гликопротеиновых llb/IIIa рецепторов тромбоцитов во время экстренной (спасительной) ангиопластики со стентированием коронарных артерий при остром инфаркте миокарда после безуспешной TJ1T.

Потому сравнение консервативных и инвазивных методов лечения различных форм острого ИМ, оценка ближайших и отдаленных результатов является очень актуальным в настоящее время.

Цель исследования: сравнение эффективности и безопасности медикаментозных и инвазивных методов лечения острого инфаркта миокарда.

Задачи исследования:

1. Изучить течение и исходы острого ИМ при консервативном лечении в стационаре в зависимости от форм и сроков заболевания, применения тромболитической терапии.

2. Провести анализ выживаемости и частоты осложнений в течение 6 месяцев от развития острого ИМ при консервативном лечении в стационаре.

3. Оценить эффективность и безопасность первичной баллонной коронарной ангиопластики (со стентированием и без него) у больных с различными клиническими формами острого ИМ.

4. Оценить эффективность и безопасность первичного стеншровапия у больных с острым ИМ, осложненным кардиогенным шоком.

5. Провести анализ внутрибольничных и отдаленных исходов острого ИМ у больных после консервативной терапии, первичной и спасительной коронарной ангиопластики и коронарного стентирования.

6. Изучить влияние использования ангитромботических препаратов из группы блокаторов гликопротеиновых Ilb/IIIa рецепторов тромбоцитов во время проведения первичной и спасительной коронарной ангиопластики, а также коронарного стентироания в остром периоде ИМ на отдаленные (6 месяцев) исходы заболевания.

Научная новизна:

Впервые на большой группе больных с острым ИМ в двух клиниках проведено проспективное сравнительное изучение эффективности и безопасности консервативных (медикаментозных) и инвазивных (ТБКА и коронарное стентирование) подходов к лечению острого ИМ в зависимости от сроков заболевания, его клинической формы и наличия осложнений.

Установлено, что инвазивные вмешательства, выполненные в первые 12 часов острого ИМ, значительно улучшают ближайший и отдаленный прогноз у больных с трансмуральным ИМ (по сравнению с тромболитической терапией). Инвазивные вмешательства особенно эффективны у больных кардиогенным шоком, у которых в сочетании с внутриаортальной баллонной контрпульсацией они увеличивают выживаемость до 82%.

Имплантация стента в инфаркт-связанную коронарную артерию и применение тирофибана улучшают эффективность первичной ангиопластики у больных острым ИМ.

Практи чее кая знач и мость:

Показано, что ТБКА при различных формах острого ИМ имеет преимущества перед ТЛТ и должна быть методом выбора в клиниках, располагающих катетерными лабораториями.

Применение первичного коронарного стентирования при остром ИМ позволяет сократить время наступления реперфузии, улучшает ближайшие и отдаленные результаты по сравнению с первичной ТБКА. При этом снижаются затраты на лечение острого ИМ и последующих осложнений.

Использование ингибиторов гликопротеиновых Ilb/HIa рецепторов тромбоцитов увеличивает эффективность спасительной ТБКА со стентированием после безуспешной ТЛТ при остром ИМ.

выводы.

1. Проведение тромболитической терапии (стрептокиназа 1500000 Ед в течение 90 мин) у больных острым инфарктом миокарда с зубцом Q в первые 6-12 часов от начала заболевания достоверно уменьшает госпитальную летальность с 18 до 9%.

2. Реперфузия миокарда при проведении тромболитической терапии у больных острым инфарктом миокарда достигается у 59% больных по сравнению с 27% в группе больных без тромболитиков - спонтанной реперфузией (р<0,01).

3. Благодаря применению тромболитической терапии 6-месячная выживаемость больных инфарктом миокарда возрастает с 70 до 86% (р<0,01), по сравнению с больными без тромболитиков. При этом в группе больных подвергнутых тромболитической терапии через 6 месяцев наблюдения отмечается улучшение систолической функции сердца (фракция выброса левого желудочка сердца достоверно возрастала с 44 до 57%). В группе больных без тромболитиков возрастание фракции выброса было недостоверным.

4. Проведение тромболитической терапии не сопровождается возрастанием риска опасных кровотечений, в частности частота внутричерепных кровоизлияний достоверно не различалась между больными получавшими и неполучавшими тромболитики (1,1%) и 1,4% соответственно, р>0,05).

5. Проведение первичной балонной коронарной ангиопластики со стентированием в первые 6-12 часов острого инфаркта миокарда обеспечивает наибольшую частоту реперфузии миокарда (89% по сравнению с 59% в группе больных с тромболитической терапией). При этом сроки наступления реперфузии значительно сокращаются с 269±114 до 157±154 (р<0,05), в среднем на 112 мин.

6. Госпитальная летальность у больных с острым инфарктом миокарда, подвергшихся первичной болонной ангиопластике значительно ниже, чем при проведении тромболитической терапии и составляет 4%

7. Внутривенное введение блокатора гликопротеиновых llb/IIla рецепторов тромбоцитов тирофибана (агграстата) во время спасительной ангиопластики не оказывало существенного влияния на ближайшие исходы, но достоверно улучшало отдаленные (6-месячные) результаты, существенно уменьшая число сердечно-сос) дистых осложнений (р<0,()5).

8. У больных острым инфар.ттом миокарда с кардиогенным шоком проведение первичной коронарной ангиопластики в сопровождении с внутриаортальной контпульсацией является наиболее эффективным вмешательством, которое значительно снижает госпитальную летальность до 17% по сравнению с 70% при консервативной терапии.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ.

1. Всем больным с острым инфарктом миокарда с подъемом сегмента ST и зубцом Q в первые 6-12 часов от начала заболевания необходимо проводить внутривенную тромболитическую терапию в отсутствие противопоказаний.

2. Ургентная первичная балонная ангиопластика со стентированием инфаркт-связанной коронарной артерии является более эффективным методом лечения и может быть рекомендована во всех стационарах, где имеется необходимое оборудование и высококвалифицированные кадры.

3. В лечении больных острым инфарктом миокарда с кардиогенным шоком первичная ангиопластика со с квитированием является единственным эффективным способом вмешательства.

4. Отдаленные рез\льтаты коронарной ангиопластики у больных острым инфарктом миокарда улучшаются при одновременном назначении блокатора гликопротеиновых Ilb/IIIa рецепторов тромбоцитов тирофибана (агграстата).
