Проблема дефицитных состояний, связанных с недостаточностью поступления в организм соответствующих нутриентов, имеет огромный интерес, научную и практическую значимость. Среди микроэлементозов, по расчетам экспертов ВОЗ, ведущие позиции занимают дефицит железа и цинка [261]. Проблемы дефицита отдельных нутриентов во многом связаны, так как касаются одних групп риска и популяций населения; имеют влияние социально-экономических и экологических факторов [7,26,131,208,211,261]. Эта общность объясняет сопутствующее возникновение дефицитных состояний и требует комплексного подхода к решению. При определении стратегии профилактики и лечения немаловажными являются вопросы взаимодействия обмена микронутриентов [79,107,111,132,164,180,194,218,221,251,290].

Железодефицитная анемия (ЖДА) - полиэтиологичное заболевание, являющееся результатом снижения общего количества железа в организме и характеризующееся прогрессирующим микроцитозом и гипохромией эритроцитов [1].

Экспертами ВОЗ выделено 20 факторов, наиболее влияющих на здоровье человека. В докладе ВОЗ 2002 г. говорится, что посредством влияния на эти факторы будет существенно уменьшена заболеваемость, инвалидность и смертность, значительно увеличена продолжительность жизни. Среди них девятым по важности является проблема дефицита железа (ДЖ) [2]. ДЖ остается наиболее распространенным состоянием, связанным с дефицитом питания [4,52,86,261]. По данным ВОЗ примерно 4-5 миллиардов людей на планете страдает ДЖ, из них около 30% имеют ЖДА, которая является причиной смерти почти 1 млн. человек в год [52, 254,261]. Экспертами ВОЗ ЖДА отнесена к четвертой по значимости среди болезней, связанных с питанием [4,52,261].

В районах, где анемия встречается часто, ДЖ является наиболее распространенной ее причиной [255]. ЖДА остается самым распространенным заболеванием крови [118,146,261]. По данным разных авторов ЖДА составляет около 80%, а в детском возрасте - 90% всех анемий [34,118,146,261].

Чаще всего и наиболее серьезно от нее страдают дети младшего возраста и их матери вследствие больших потребностей в железе в период роста младенцев и беременности [261].

ДЖ приводит к задержке физического, нервно-психического, психомоторного и полового развития, сказывается на иммунном статусе, нарушает работу желез внутренней секреции, нервной системы, увеличивает абсорбцию тяжелых металлов и др. [4,52,130,139,261].

Последствия перенесенного в детстве ДЖ могут быть необратимы в течение всей последующей жизни [1,5,62,169,183,342]. ДЖ у беременных может привести к преждевременным родам, стать причиной низкой массы ребенка при рождении [1,4,52,130,139,261]. К последствиям ЖДА относят увеличение перинатальной, детской и материнской смертности [52,66,226,261,265]. Проблему ДЖ связывают с ухудшением качества жизни. ДЖ объясняют снижение производительности труда и большие трудопотери [52, 261]. Согласно расчетам экспертов ВОЗ лечение и профилактика ЖДА должны привести к увеличению производительности на 20 % [52,261].

ДЖ сопровождается нарушением обмена других эссенциальных микроэлементов, имеющих прямое отношение к эритропоэтической функции, таких как медь и цинк [7,26,79,100,164,194,221,290]. Метаболизм их напрямую связан с составом воды и пищи, экологической ситуацией в том или ином регионе. Взаимосвязь этих состояний и ответ на лечение препаратами железа в доступной литературе освещены неоднозначно [107,114,180,195,218].

В том числе мы не нашли сведений, касающихся эффективности применения растворимого комплексного микроэлементного препарата, содержащего глюконат железа, меди и марганца, у детей раннего возраста с ЖДА. Известно, что медь необходима для усвоения и транспорта железа; ее влияние осуществляется через цитохромоксидазу и церулоплазмин. Действие марганца на процесс кроветворения неспецифично и связано с его высокой окислительной способностью [28].

Широкая распространенность ДЖ и ЖДА, последствия, которые они вызывают у людей групп риска, требуют разработки программ эффективного вмешательства в ряд первоочередных мероприятий национального и мирового уровня [284]. Для достижения успеха в этом целью ставится уравновешивание и координация проводимых исследований, больше внимания предлагается уделять оценке практической выполнимости вмешательств и доказательства их эффективности [72,284].

В настоящее время в нашей стране не существует единой схемы лечения и профилактики ЖДА у детей. Отсутствуют так же обоснованные доказательства преимущества какой-либо из применяемых схем. Исходя из этого, на данном этапе лечение ЖДА представляется эмпирическим и с точки зрения доказательной медицины требует обоснованности. Сравнительно недавно появившиеся в отечественной клинической практике препараты гидро-ксид-полимальтозного комплекса (ГПК) трехвалентного железа получили популярность благодаря незначительному количеству побочных эффектов при лечении. Однако данные об их терапевтической эффективности противоречивы [123,131,156,210,192,179].

В современной медицинской науке доля рандомизированных контролируемых клинических исследований составляет 30%. Проведение их позволяет ответить на ряд нерешенных вопросов, устраняет неоднозначные трактовки, отчетливо выявляет наиболее эффективные и наименее токсичные методы вмешательства [20,34,102]. По данным исследования, проведенного П. Калдуэлл и соавт. [20], выгода педиатров от участия в рандомизированных клинических исследованиях, по мнению их самих, включает профессиональный интерес, улучшение качества жизни их пациентов и их семей, а так же содействие научному прогрессу.

Цель исследования: оптимизация тактики лечения ЖДА у детей и подростков с помощью выбора на основе методов доказательной медицины наиболее эффективного терапевтического плана.

Задачи исследования:

1. Определить причины развития и клинические особенности ЖДА у детей и подростков.

2. Изучить обмен некоторых эссенциальных микроэлементов и установить взаимосвязь меди, цинка и железа у детей раннего возраста с ЖДА.

3. Оценить эффективность и определить сроки лечения растворимым комплексным микроэлементным препаратом двухвалентного железа детей раннего возраста с ЖДА.

4. Организовать рандомизированное клиническое исследование по лечению детей и подростков, больных ЖДА по двум терапевтическим режимам. Разработать критерии включения и исключения из протокола.

5. Оценить терапевтическую эффективность и переносимость гидроксид-полимальтозного комплекса трехвалентного -железа.

6. Провести сравнительный анализ эффективности и переносимости двух терапевтических режимов для детей и подростков больных ЖДА.

Научная новизна

ЖДА у детей раннего возраста обусловлена комплексом причин, включающих в равной степени как неблагоприятное течение беременности и отягощенный социальный анемнез, так и алиментарный фактор и повышенные потребности организма в железе.

Впервые изучен обмен эссенциальных микроэлементов (медь и цинк) у детей грудного и раннего возраста, страдающих ЖДА, с учетом геохимических особенностей региона. Методом корреляционного анализа выявлена взаимосвязь уровней эссенциальных микроэлементов у детей с ЖДА при лечении комплексным микроэлементным препаратом двухвалентного железа.

Продемонстрирована нормализация обмена цинка при лечении ЖДА у детей раннего возраста. Показана эффективность назначения препаратов меди совместно с препаратами железа при одновременном дефиците микроэлементов и для профилактики развития дефицита меди при заместительной терапии ЖДА.

Доказана эффективность применения комплексного микроэлементного препарата двухвалентного железа для лечения ЖДА у детей раннего возраста. Впервые определена длительность лечения исследуемым препаратом для достижения нормализации гематологических показателей и восстановления запасов железа.

Впервые проведено рандомизированное клиническое исследование по лечению детей и подростков с ЖДА ГПК трехвалентного железа по двум терапевтическим режимам.

Продемонстрирована эффективность применения ГПК трехвалентного железа для лечения ЖДА у детей и подростков. Доказано, что несолевые препараты трехвалентного железа в виде ГПК могут считаться средством выбора для лечения ЖДА у детей.

Впервые показано преимущество назначения перорально применяемых препаратов по схеме, где доза элементарного железа для ежедневного приема составляет 5 мг/кг в течение всего курса заместительной терапии как по восстановлению гематологических показателей, так и запасов железа. Переносимость терапевтических курсов и приверженность пациентов проводимому лечению не различались.

Практическая значимость

Результаты работы выявили ряд факторов, способствующих развитию ЖДА у детей раннего возраста и подростков, своевременное обнаружение и коррекция которых может служить основой профилактических мероприятий. Среди таких факторов у детей грудного и раннего возраста наиболее значимыми являются: состояние здоровья матерей, патологическое течение беременности, сидеропения у беременных, алиментарный фактор и повышенные потребности ребенка в железе в периоды интенсивного роста. Для развития ЖДА у подростков большее значение имеют высокие темпы роста, занятия спортом, а также нарушения менструального цикла у девушек.

Типичными в клинической картине у обследованных детей были бледность кожи и слизистых оболочек. Характерными для детей грудного и раннего возраста явились мышечная гипотония, кишечная диспепсия, тахикардия и/или функциональный систолический шум; для подростков - трофические изменения кожи и ее придатков. Выявление указанных симптомов полезно для диагностики ЖДА и контроля эффективности лечения при сопоставлении с гематологическими показателями.

Показана целесообразность учета геохимических особенностей региона и определения микроэлементного состава крови для назначения комплексных препаратов микроэлементов. Обосновано применение ферропрепаратов детям с ЖДА для нормализации показателей цинка. Назначение препаратов меди, одним из которых может служить комплексный микроэлементный препарат двухвалентного железа, целесообразно при одновременном дефиците обоих эссенциальных микроэлементов и для профилактики развития дефицита меди при заместительной терапии у детей с ЖДА.

Продемонстрирована эффективность и определены сроки назначения растворимого комплексного микроэлементного препарата двухвалентного железа для лечения детей раннего возраста с ЖДА. Установлена высокая частота побочных эффектов со стороны желудочно-кишечного тракта при назначении данного препарата.

Показана эффективность применения препарата трехвалентного железа в виде ГПК для лечения ЖДА у детей и подростков. Использование указанного лекарственного средства сопровождается небольшой частотой побочных явлений, что позволяет достичь высокой приверженности пациентов лечению и рекомендовать его как средство выбора для лечения ЖДА у детей и подростков.

Полученные данные позволяют считать схему назначения перорально вводимого препарата железа в суточной дозе с расчетом на элементарное железо 5мг/кг в течение всего курса заместительной терапии предпочтительной для лечения ЖДА у детей и подростков.

Внедрение результатов работы

Результаты исследования внедрены в практику Республиканской детской клинической больницы МЗ Удмуртской Республики, детских поликлиник № 5, 8 г. Ижевска, а также используются в учебном процессе на кафедре педиатрии Ижевской государственной медицинской академии.

Апробация работы

Основные положения диссертации доложены и обсуждены на III межвузовской конференции молодых ученых и студентов (Ижевск, 2003); VIII Конгрессе педиатров России (Москва, 2003); международной российско-американской научно-практической конференции (Тула, 2004); II межрегиональной межвузовской научной конференции молодых ученых и студентов (Ижевск, 2005); X съезде педиатров России (Москва, 2005).

По материалам диссертации опубликовано 9 научных работ, из них 6 статей.

ВЫВОДЫ

1. У детей раннего возраста в современных условиях большую роль в качестве факторов риска развития ЖДА играют состояние здоровья матери, патологическое течение беременности, анемия во время беременности и отягощенный социальный анамнез. Ведущая значимость алиментарного фактора отмечена у менее половины детей. У подростков причинами развития ЖДА служат высокие темпы роста, занятия спортом, а также нарушения менструального цикла у девушек.

2. У детей раннего возраста с ЖДА концентрация цинка в сыворотке крови была нормальной (у 46,7%) или повышенной (у 50,0%>). Содержание меди в сыворотке крови у 70,0% больных не отличалось от показателей здоровых детей, было значительно снижено у 16,7% и повышено у 13,3% пациентов. Не обнаружено существенной разницы в содержании цинка и меди у детей с легкой и среднетяжелой анемией. Не выявлено достоверных корреляций между уровнем железа, меди и цинка у детей с ЖДА.

3. Растворимый комплексный микроэлементный препарат двухвалентного железа является эффективным лекарственным средством для лечения легкой и среднетяжелой ЖДА у детей раннего возраста. Заместительная терапия этим препаратом в течение двух месяцев приводит к нормализации обмена железа и цинка, а также к существенному повышению сывороточной концентрации меди у больных ЖДА. В ходе лечения 20%> пациентов имели побочные явления в виде желудочной и кишечной диспепсии легкой степени. Приверженность лечению составила 80,0%.

4. Заместительная терапия у детей и подростков с ЖДА ГПК трехвалентного железа эффективна, так как после завершения курсового лечения нормализация концентрации НЬ достигнута у 97,0%>, а СФ у 60,9% пациентов. Отсутствие периода толерантности к проводимому лечению, незначительное количество нежелательных явлений (6,3%) в виде запора и высокая приверженность пациентов лечению (100,0%) позволяют считать его оптимальным препаратом для терапии ЖДА у детей и подростков.

5. Схема лечения ГПК трехвалентного железа в суточной дозе 5 мг/кг в течение всего курса терапии является предпочтительной, так как уже после второго месяца заместительной терапии все больные, имевшие легкую форму анемии и получавшие лечение по этому плану, восстановили уровень гемоглобина до нормальных значений. После завершения курсового лечения значения гемоглобина в этой группе были нормальными у всех пациентов, независимо от степени тяжести анемии, и существенно выше (р<0,05), чем у детей, получающих препарат по традиционной «ступенчатой» методике.

6. Заместительная терапия ГПК трехвалентного железа в суточной дозе 5 мг/кг в течение всего курса лечения имеет преимущество перед традиционной методикой, так как позволила достичь уровня СФ (р<0,05) и восстановления запасов железа у 75,0% детей и подростков с ЖДА.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

Своевременное выявление и коррекция факторов риска, способствующих развитию ЖДА у детей раннего возраста и подростков, таких как состояние здоровья матери, патологическое течение беременности, анемия во время беременности и отягощенный социальный анамнез, занятия спортом, а также нарушения менструального цикла у девушек, позволяют существенно снизить вероятность возникновения ЖДА.

Выявление клинических признаков анемического и сидеропенического синдромов при сопоставлении с соответствующими гематологическими показателями и показателями обмена железа служит надежным критерием диагностики и контроля лечения ЖДА.

Растворимый комплексный микроэлементный препарат двухвалентного железа, назначенный в суточной дозе в расчете на элементарное железо 5 мг/кг, служит эффективным средством для лечения ЖДА у детей раннего возраста. Назначение препарата в указанной дозе в течение 2 месяцев достаточно для восстановления гематологических показателей и запасов железа. Рекомендуется учитывать, что сравнительно высокая частота побочных эффектов, связанных с его приемом, в ряде случаев может ограничивать возможности для его назначения.

Назначение препаратов железа для лечения ЖДА способствует нормализации нарушенного обмена цинка у детей.

Дополнительное назначение препаратов меди необходимо осуществлять с учетом геохимической обстановки соответствующего региона и после оценки уровня маркеров обмена этого микроэлемента у каждого пациента.

Назначение комплексных препаратов железа и меди целесообразно при одновременном дефиците этих микроэлементов.

Препарат в виде ГПК трехвалентного железа, назначенный в адекватном дозовом режиме на срок, соответствующий тяжести анемии эффективен для лечения детей и подростков с ЖДА. Учитывая незначительное число побочных эффектов при его приеме, использование этого препарата может быть рекомендовано в качестве средства выбора для лечения ЖДА у детей и подростков.

Для лечения ЖДА у детей и подростков предпочтительно использование терапевтического режима с назначением препарата в суточной дозе 5мг/кг на срок адекватный степени тяжести анемии (легкая - 3 месяца, среднетяжелая - 4 месяца, тяжелая - 6 месяцев).
