ВВЕДЕНИЕ

Актуальность и степень разработанности темы исследования

Церебральная микроангиопатия (ЦМА)/ болезнь мелких сосудов, ассоциированная с возрастом и сосудистыми факторами риска (англ. - cerebral small vessel disease/ cerebral microangiopathy) является главной причиной сосудистых когнитивных расстройств (КР), основным модифицируемым фактором риска болезни Альцгеймера с развитием смешанных форм заболеваний [Gorelick P. B. et al., 2011; Deramecourt V. et al., 2012; De Reuck J. et al., 2018; Iadecola C. 2019; Azarpazhooh M. R. et al., 2018]. ЦМА является причиной 45% всех случаев деменций (изолированно сосудистой или смешанной с нейродегенерацией) [Gorelick P. B. et al., 2011; Bos D. et al., 2018; Hachinski V. et al., 2019; Wardlaw J. M. et al., 2019]. Это обосновывает диагностику КР на ранних стадиях, уточнение факторов и механизмов прогрессирования, разработки мер своевременной профилактики.

Определение ранних КР, не достигающих стадии УКР, не имеют единообразия среди исследователей мира и, главным образом, исследуется при нейродегенерации. В России значимый вклад в изучение данной стадии КР внесли ведущие неврологи - академик РАН Н. Н. Яхно, профессор В. В. Захаров и другие [Яхно Н. Н., 2006; Яхно Н. Н. и др., 2017; Локшина А. Б. и Захаров В. В., 2006; Парфенов В.А. и др., 2014; Захаров В. В. и др., 2015; Захаров В. В. и Кабаева А.Р., 2017; Старчина Ю. А., 2017; Громова Д. О. и Вахнина Н. В., 2017 Коберская Н. Н. и Остроумова Т. М., 2020; Старчина Ю. А., Захаров В. В., 2021; Коберская Н. Н., 2022]. В последние годы для их обозначения был предложен термин предумеренные (доумеренные) КР (преУКР), который включает стадии субъективных КР (субКР) и легких КР. Их диагностика основывается на наличии жалоб пациента на снижение когнитивных функций (КФ), которое в первом случае не подтверждается отклонениями от возрастной нормы при выполнении нейропсихологических тестов, а во втором выявляет их снижение относительно исходного уровня при отсутствии или незначительных отклонениях от

среднестатистической возрастной нормы [Яхно Н. Н., 2006; Яхно Н. Н. и др., 2017; Захаров В. В. и др., 2015; Коберская Н. Н. и Остроумова Т. М., 2020; Старчина Ю. А., Захаров В. В., 2021; Коберская Н. Н., 2022].

За рубежом, наиболее часто субКР рассматривается стадией, предшествующей УКР [Jessen F. et al., 2014; Munro C. E. et al., 2023], но также наблюдается рассмотрение субКР и преУКР (premild cognitive impairment) стадиями нарастания тяжести КР [Tondo G. et al., 2022]. Другим подходом в характеристике стадии, предшествующей УКР, является выделение субКР и субКР+. Последнее предполагает наличие дополнительных факторов, ассоциированных с ускоренным переходом к более выраженным КР [Jessen F. et al., 2020].

Таким образом, стратификация сосудистых субКР при ЦМА не разработана. Очевидно, что их диагностика возможна только при МРТ-подтверждении ЦМА в соответствии с критериями STRIVE (2013 г.). В доступных источниках нам не встретилось исследований, оценивающих данную стадию КР с позиций доказательной диагностики ЦМА. Сложности стратификации стадий субКР и легких КР при ЦМА обусловлены отсутствием общепринятых рекомендаций по использованию нейропсихологических тестов, однократность

нейропсихологического тестирования для абсолютного числа случаев, ограничениями в стандартизации результатов нейропсихологического тестирования. Последняя требует доступности результатов когнитивного обследования здоровой популяционной выборки достаточной мощности, позволяющий получать референсные значения используемых тестов с учетом возраста, уровня образования, этнической принадлежности, социокультурных и других факторов.

Доступность маркеров крови, ассоциированных с механизмами инициирования ЦМА с развитием субКР, может быть полезным инструментом оценки активности заболевания при наблюдении за пациентами. Учитывая облигатную связь ранней ЦМА с эндотелиальной дисфункцией, наибольшим потенциалом маркера обладает оксид азота (NO), поскольку снижение его биодоступности является эквивалентом эндотелиальной дисфункции [Forsberg K. M. E. et al., 2018; Cyr A. R. et al., 2020; Bai

T., Yu S., Feng J., 2022]. Нестойкость и сложный механизм образования NO не позволяет использовать его количественное содержание в качестве маркера. Одним из недавних подходов в поиске показателей биодоступности NO является оценка уровня в крови его предшественника L-аргинина. Проведенные к настоящему времени исследования показывают противоречивые результаты [Tang W. H. et al., 2009; Notsu Y et al., 2015]. Роль циркулирующего L-аргинина, а также его расчетных коэффициентов (L-аргинин к его метаболитам и конкурентным ингибиторам) в развитии КР при ЦМА ранее не уточнялась.

Актуальность углубленного изучения стадии субКР при ЦМА обусловлена ее потенциальной обратимостью. Целесообразным является поиск наиболее чувствительных для данной стадии нейропсихологических тестов среди таковых, характеризующих клинически значимые КР при ЦМА [Salvadori E. et al., 2021; Добрынина Л. А. и др. 2019; 2022]. Поскольку субКР ожидаемо имеют не высокое бремя ЦМА по МРТ-признакам, обоснованным является уточнение эквивалентов субКР по микроструктурным изменениям визуально неизмененного белого вещества головного мозга методом диффузионно-тензорной МРТ (ДТ-МРТ), показавшим свою высокую чувствительность в оценке КР разной тяжести [Добрынина Л. А. и др., 2016, 2019-2022; Pasi M. et al. 2016; Jing Du et al., 2021; van den Brink et al., 2023].

Цель

Уточнить когнитивный и психоэмоциональный статус, изменения вещества головного мозга и маркеры, ассоциированные со снижением биодоступности оксида азота, у пациентов с ЦМА и субКР.

Задачи

1. Оценить распространенность, факторы сосудистого риска и МРТ-признаки субКР при ЦМА.

2. Оценить когнитивный профиль у пациентов с субКР при ЦМА и уточнить тесты, имеющие наибольшую диагностическую ценность и их пороговые значения, соответствующие стадиям риска прогрессирования КР.

3. Сопоставить стадии риска прогрессирования субКР с выраженностью МРТ макроструктурных признаков и микроструктурных изменений вещества головного мозга.

4. Оценить эмоциональные нарушения у пациентов с субКР при ЦМА, их связь с результатами нейропсихологического тестирования и диагностическими МРТ-признаками ЦМА.

5. Проанализировать показатели крови, характеризующие метаболизм оксида азота (N0) у пациентов с субКР при ЦМА.

6. Уточнить связи показателей, характеризующих метаболизм N0, с когнитивным статусом, поражением вещества головного мозга у пациентов с субКР при ЦМА.

Методология и методы исследования

В период с 2022 по 2025 годы в ФГБНУ «Российский центр неврологии и нейронаук» (Москва) при участии лаборатории «Евротест» было проведено одномоментное сплошное исследование, в котором приняли участие три сопоставимые по полу и возрасту группы: 80 пациентов с субКР при ЦМА, 60 пациентов с ЦМА и УКР и 36 здоровых добровольцев, не имеющих КР и каких-либо патологических изменений по результатам МРТ головного мозга. Было проведено комплексное клиническое обследование, включающее следующие этапы: 1) сбор анамнеза, направленный на выявление факторов, предрасполагающих к развитию сосудистых заболеваний; 2) оценку неврологического статуса; 3) оценку когнитивного статуса с использованием батареи тестов, показавших ранее чувствительность в оценке КР при ЦМА и эмоционального профиля. Протокол нейровизуализационного обследования включал рутинную, диффузионно-тензорную МРТ головного мозга, волюмометрию, повоксельную и поверхностную морфометрию. В плазме крови

методом высокоэффективной хроматографии с тандемным масс-спектрометрическим детектированием количественно определялся уровень аминокислот, участвующих в метаболизме NO: L-аргинина, L-цитруллина, L-орнитина, асимметричного диметиларгинина.

Научная новизна

Впервые проанализированы субКР при ЦМА, диагностированной в соответствии с международными МРТ-критериями ЦМА.

Впервые сформирована и использована при нейропсихологическом тестировании стадии субКР батарея тестов, проведено их сопоставление, установлены преимущества в диагностике субКР теста ТМТ В (Trail Making Test B, тест прослеживания пути В) (AUC-0,713).

Впервые доказательно показано, что субКР при ЦМА, диагностированные по общему когнитивному уровню (MoCA>26) могут быть разделены на стадии тяжести по пороговому значению теста ТМТ В без и с отклонениями, соответственно субКР <89 сек и субКР+ >90 сек, что может рассматриваться как нарастающий риск прогрессирования.

Впервые установлено, что повышенный уровень L-аргинина (>51,25 мкмоль/л) ассоциирован с ЦМА, что подтверждено изменениями при морфометрии головного мозга, диффузионно-тензорной МРТ, а также результатами когнитивного тестирования.

Теоретическая и практическая значимость

Теоретическая значимость работы связана с разработкой подходов к диагностике субКР при ЦМА по общего когнитивному уровню (MoCA >26) и стадий их тяжести по пороговому значению теста ТМТ В (Trail Making Test B, тест прослеживания пути В) (89,5 сек), имеющего лучшие характеристики в отношении субКР.

Практическая значимость заключается в создании алгоритма выявления лиц с субКР на основе порогового значения теста ТМТ В, позволяющего разделить пациентов на подгруппы без отклонений от нормы - субКР (<89 сек) и с

отклонениями - субКР+ (>90 сек), что открывает возможности для персонализированного подхода к более точной оценке риска прогрессирования в УКР, динамического наблюдения за пациентами.

Установлена связь повышенного уровня L-аргинина (>51,25 мкмоль/л) с развитием ЦМА с большей выраженностью ГИБВ, микрокровоизлияний и худшими результатами при выполнении нейропсихологических тестов. Это указывает на патогенетическое значение высоких уровней L-аргинина, позволяет рассматривать фактором риска субКР при ЦМА, обосновывает поиск причин его повышения, возможно, связанных с хорошо известным феноменом аргининового парадокса, и разработку подходов к восстановлению гомеостаза L-аргинина.

Положения выносимые на защиту

1. СубКР при ЦМА характеризуются высокой распространенностью и составляют 33% среди всех КР при ЦМА. Среди классических сосудистых факторов риска артериальная гипертензия является облигатным, тогда как гиперхолестеринемия и сахарный диабет 2 типа встречаются в 72,5 % и 10% соответственно.

2. Нейропсихологический профиль субКР при ЦМА характеризуется по сравнению с контролем статистически значимыми отклонениями в тестах на управляющие функции мозга (Trail Making Test, TMT А, В, B-A, тест Струпа) и память (тест 10 слов). Тест ТМТ В имеет преимущества среди всех используемых тестов в дифференцировании субКР от контроля, а его пороговые значения позволяют разделить пациентов на стадии без отклонений (<89 сек.), соответствующих субКР и с отклонениями (>90 сек.), условно обозначенными как субКР+. Наличие у пациентов обеих подгрупп МРТ-признаков поражения головного мозга указывает на риск их прогрессирования, более высокий при субКР+.

3. Эмоциональные нарушения, чаще проявляющиеся субклинической тревогой и депрессией, встречаются менее, чем у половины пациентов. Они не показали

корреляций с результатами абсолютного большинства нейропсихологических тестов и выраженностью МРТ-признаков.

4. Установленный у пациентов с субКР и УКР по сравнению с контролем, но не между собой статистически значимо более высокий уровень в крови донатора N0 - L-аргинина, позволяет предполагать его патогенетическую значимость в развитии ЦМА и связанных с ней КР.

5. Уровень в крови L-аргинина >51,25 мкмоль/л ^ОС-анализ, АиС - 0,83) связан с худшими результатами при выполнении тестов и большим МРТ-поражением головного мозга - выраженностью микрокровоизлияний, ГИБВ, повреждением микроструктуры белого вещества, уменьшением коры, что позволяет предполагать значение нарушений гомеостаза L-аргинина в механизмах когнитивных расстройств.

Личный вклад автора

Автор принимал непосредственное участие во всех этапах данного исследования, начиная с разработки протокола и формулирования целей и задач. Автором лично осуществлялся отбор и комплексное клинико-инструментальное обследование пациентов, включавшее подробный неврологический осмотр, когнитивное тестирование и оценку сосудистых факторов риска. Автор участвовал в сборе и первичной обработке образцов крови, а также в интерпретации результатов лабораторных анализов. Автором был проведён тщательный анализ научной литературы по теме исследования, а также аналитическая и статистическая обработка данных, что позволило сделать выводы и разработать практические рекомендации.

Степень достоверности и апробация результатов исследования

Достоверность и обоснованность результатов исследования подкреплены достаточным объемом выборки, современными высокотехнологичными методами нейровизуализации и лабораторной оценки, а также корректной статистической обработкой с применением проверенных статистических методов и программ.

Работа была апробирована и рекомендована к защите на совместном заседании научных сотрудников клинических отделений и лабораторий ФГБНУ «Российский центр неврологии и нейронаук» (протокол № 8 от 20.11.2025 года). Основные положения диссертации были представлены на конференциях Нейрофорум-2024, Нейрофорум-2025 и The 10th Congress of the European Academy of Neurology, Helsinki, 2024.

Внедрение результатов работы

Полученные результаты внедрены в практическую деятельность 3-го неврологического отделения и консультативно-диагностического отделения ФГБНУ РЦНН.

Структура и объем диссертации

Диссертационная работа объёмом 139 страниц машинописного текста включает 18 таблиц и 11 рисунков. Диссертация состоит из следующих разделов: введение, обзор литературы, материалы и методы исследования, результаты собственных исследований, обсуждение полученных результатов, выводы и практические рекомендации. Список литературы содержит 53 отечественных и 280 зарубежных источников, включая 6 авторских публикаций по теме диссертации.

ГЛАВА 1. ОБЗОР ЛИТЕРАТУРЫ

Церебральная микроангиопатия (ЦМА) / болезнь мелких сосудов (анг. сerebral Small Vessel Disease, SVD), ассоциированная с возрастом и сосудистыми факторами риска - хроническое прогрессирующее сосудистое заболевание головного мозга, ассоциированное с поражением мелких перфорантных артерий, артериол, капилляров и венул [Гулевская Т. С., 2017; Pantoni L., 2010]. В отечественной научной литературе для обозначения ЦМА наиболее часто используется термин «дисциркуляторная энцефалопатия».

ЦМА вносит значительный вклад в развитие сосудистых и смешанных с нейродегенерацией когнитивных расстройств (КР), составляя до 45% всех случаев деменции [Gorelick P. B. et al., 2011; Bos D. et al., 2018; Hachinski V. et al., 2019; Wardlaw J. M. et al., 2019]. Снижение когнитивных функций (КФ), связанное с ЦМА, оказывает значительное влияние на продолжительность и качество жизни [Wardlaw J.M. et al., 2019; Zhang C.E. et al., 2019; Ma and Chan, 2020], что обосновывает диагностику КР на ранних стадиях с целью проведения своевременной профилактики. Кроме того, ЦМА является причиной 25% ишемических инсультов, ведущей причиной нарушения походки, тазовых и эмоциональных расстройств в старшем и пожилом возрасте [Cannistraro R. J. et al., 2019; Fuellen G. et al., 2019]. Особенностями течения ЦМА является длительный асимптомный период, характеризующийся запуском эндотелиальной дисфункции и связанных с ней механизмов дисгомеостаза сосудистой стенки с ее повреждением, последующим накоплением микроструктурных изменений и формированием МРТ-признаков в веществе головного мозга [De Groot M., et al. 2013, Van Leijsen E.M.C. et al., 2018].

ЦМА на ранних этапах может протекать бессимптомно или проявляться в виде жалоб на снижение когнитивных способностей, которые не находят объективного подтверждения при нейропсихологическом исследовании (субъективные когнитивные расстройства - субКР), также возможны эмоциональные расстройства, включающие тревогу и депрессию. МРТ-признаки ЦМА являются самыми ранними проявлениями сосудистого поражения головного мозга [Добрынина Л. А.

и др, 2016а; 2016b; Гнедовская Е. В. и др., 2018]. Распространенность ЦМА нарастает с возрастом и у более, чем половины лиц в возрасте старше 65 лет выявляются МРТ-признаки ЦМА [Savva G. M. et al., 2009].

Наибольшие сложности в изучении ЦМА является ее доказательная диагностика. Принадлежность заболевания к ЦМА проводится на основании МРТ-признаков головного мозга согласно критериям STRIVE-1 от 2013 года и дополнительных STRIVE-2 от 2023 года (STandards for Reporting Vascular changes on nEuroimaging) [Wardlaw J. M. et al., 2013; Duering M. et al, 2023]. Данные критерии включают в себя лакунарные инфаркты, гиперинтенсивность белого вещества (ГИБВ), церебральные микрокровоизлияния (МКР), корковые микроинфаркты, расширенные периваскулярные пространства (ПВП) и атрофию головного мозга.

В свою очередь, в соответствии с признанными критериями, диагностика клинических проявлений ЦМА предполагает их соответствие МРТ-признаков. Диагностика лакунарных инсультов не вызывает сложностей и проводится при наличии недавних субкортикальных малых инфарктов и/ или лакун в соответствии с критериями TOAST [Adams H. P. Jr et al.,1993] и ASCOD [Amarenco P. Et al., 2013]. Суррогатным маркером тяжести ЦМА и КР, используемым в интервенционных исследованиях, является ГИБВ. В качестве таковой она закреплена в международных критериях сосудистых КР VASCOG (2014 г.), которые наряду с этим описывают их особенности - снижение скорости обработки информации, управляющих функций и внимания, а также указывают на необходимость присутствия одного из трех признаков - эмоциональных изменений, нарушений ходьбы, функции тазовых органов [Sachdev P. et al., 2014]. Однако, наличие последних отмечается только на развернутой стадии ЦМА, характеризующихся деменцией или УКР. В связи с чем значительная когорта пациентов на стадии не достигающих стадии УКР и даже УКР исключаются из этого классификатора.

Рекомендации по их доказательной диагностике при ЦМА отсутствуют. Данные ограничения усугубляются имеющейся терминологической несогласованностью среди отечественных и зарубежных исследователей в определении стадий ранних

КР, не достигающих УКР различного генеза, в том числе вследствие сосудистого поражения мозга.

1.1 Сосудистые когнитивные расстройства: классификации и типы

КР являются одной из наиболее актуальных проблем современной медицины и общества, приводящих к нарушению трудоспособности и инвалидизации населения [Tucker-Drob E. M., 2011]. По текущим оценкам, почти 36 миллионов человек во всем мире страдают деменцией, и ожидается, что к 2030 году это число достигнет 66 миллионов, а к 2050 году — 115 миллионов [Wortmann M., 2012].

По распространенности сосудистые КР занимают второе место в мире, уступая лишь болезни Альцгеймера. Сосудистые КР характеризуются разнообразием причин и механизмов развития. В настоящее время ЦМА признана ведущей причиной сосудистых КР и смешанных с нейродегенеративными заболеваниями. На долю ЦМА приходится до 45% всех случаев деменции в общей популяции населения [Cannistraro R. J. et al., 2019; Esiri M. M. et al., 1997; Ighodaro E. T. et al., 2017; Lamar M. et al., et al., 2022].

Начиная с 70-х годов прошлого столетия, с определенной периодичностью предлагались разные варианты диагностических критериев сосудистых КР. В историческом аспекте остановимся на тех, которые впервые актуализировали изучение сосудистых КР и тех, которые предприняли попытки их дифференцирования на основе нейровизуализации и признанием гетерогенности форм. Так, созданная в 1975 году клиническая ишемическая шкала Хачинского (Hachinski ischemic score, HIS) предлагала дифференцирование мультиинфарктной от дегенеративной деменции, которая на тот период времени стала рассматриваться единственной ее причиной [Hachinski V.C. et al., 1975]. Позднее понадобилось внесение уточнение в данный термин, поскольку «мультиинфарктная деменция» стала использоваться как синоним сосудистой деменции, что привело к гипердиагностике данной причины и недооценки других. Впоследствии, расширение знаний о неоднородности сосудистых нарушений послужило стимулом для разработки новых критериев. В начале 1990-х годов группа экспертов

из Национального института неврологических расстройств и инсульта (NINDS) и Международной ассоциации научных исследований и образования в области нейронаук (AIREN) предложила критерии, принимающие во внимание данные нейровизуализации, указывающие на поражение мозга сосудистого генеза. Согласно этим критериям, учитывались не только крупные и средние инфаркты, но и изменения вещества головного мозга, обусловленные поражением мелких сосудов, такие как лакунарные инфаркты, а также ГИБВ, занимающая более 25% от общего объема белого вещества [Erkinjuntti T. et al., 1994].

В 2014 году международное общество сосудистых поведенческих и когнитивных расстройств (Vascular Behavioral and Cognitive Disorders, VASCOG) предложило диагностические критерии сосудистых КР [Sachdev P. et al., 2014], основанные на оценке когнитивных функций и нейровизуализационных признаков (ГИБВ, лакунарные инфаркты). В отношении ЦМА они включают постепенное ухудшение внимания, скорости обработки информации, управляющих функций мозга (планирование, принятие решений, рабочая память, гибкость мышления), наличие ГИБВ стадии Fazekas 2-3, а также присутствия одного из трех признаков -эмоциональных расстройств, нарушений походки, несвязанной с гемипарезом и/ или функции тазовых органов. Также было предложено определять степень выраженности КР по отклонению в тестах: 1-2g - УКР, >2g - деменция [Sachdev P. et al., 2014].

В 2017 г. критерии VICCCS-2 (Vascular Inpairment of Cognition Classification Consensus Study) выделили четыре фенотипические подгруппы сосудистой деменции/УКР: постинсультные, мультиинфарктные, субкортикальные, смешанные [Skrobot O.A. et al., 2017].

Субкортикальные деменции и УКР являются наиболее распространенным вариантом сосудистых КР, этиопатогенетически связанным с ЦМА. Поражение мелких перфорантных артерий головного мозга в большей степени сопряжено с поражением его подкорковых ганглиев, глубокого и перивентрикулярного белого вещества. Повреждения длинных проводников и функциональных нейрональных сетей головного мозга приводит к формированию синдрома «разобщения» коры с

нижележащими структурами головного мозга. Формирование данных изменений, в первую очередь, вызывает вторичную дисфункцию лобных долей [Яхно Н. Н., Захаров В. В., 2002; Левин О. С., 2019; Dichgans M., Leys D., 2017]. В связи с чем КР при ЦМА, главным образом, проявляются нарушением УФМ с формированием так называемого дизрегуляторного типа КР [Яхно Н. Н., Захаров В. В., 2005; Захаров В. В., 2009; Парфенов В.А., 2011; 2014; Вахнина Н. В., 2014; Дамулина А. И., Кадыков А. С., 2014; Захаров В. В., Громова Д. О., 2015; Соловьева Э. Ю., 2017; Добрынина Л. А. и др., 2018; 2022; Боголепова А. Н., 2022; Dichgans M., Leys D., 2017; Ter Telgte A. et al., 2018; Hamilton O. K. L. et al., 2021; Zanon Zotin M. C., et al. 2021]. Дизрегуляторный тип КР характеризуется нарушением с сферах планирования, принятия решений, рабочей памяти, гибкости мышления, что связано с поражением лобных долей и подкорковых структур и что, как ранее упоминалось закреплено в классификации сосудистых КР VASCOG (2014 г.) [Sachdev P. et al., 2014].

В то же время при ЦМА не менее редко встречается смешанный тип КР -дизрегуляторно-амнестический, когда отклонения в доменах управляющих функциях мозга и памяти выражены в равной степени [Добрынина Л. А. и др., 2016; Lamar M., 2022]. При любом типе КР при ЦМА они нарастают постепенно, реже ступенеобразно, хотя темп их нарастания существенно отличается между пациентами. Так наше пятилетнее наблюдение за пациентами с ЦМА показало, что за этот период у участников исследования произошло статистически значимое снижение показателей ингибиторного контроля УФМ (тест Струпа) и памяти (тест 10 слов), увеличив число пациентов со смешанным типом КР [Добрынина Л. А. и

др., 2022].

1.2 Додементные сосудистые когнитивные расстройства

Со второй половины 20 века как отечественные, так и зарубежные исследователи стали предлагать выделение додементных форм [Калашникова Л. А., 1989; Яхно Н. Н., 2005, Захаров В. В. и и др., 2015; Захаров В. В. и Кабаева А. Р., 2017; Старчина Ю. А., 2017; Громова Д. О. и Вахнина Н. В., 2017; Захаров В. В. и Локшина А. Б., 2021; Старчина Ю. А. и Захаров В. В., 2021].

В конце 20 века исследователями клиники Мэйо (США) был предложен термин УКР, который предполагал наличие у пациента объективных КР по данным нейропсихологического тестирования при отсутствии выраженных нарушений повседневной активности [Petersen R. S. et al., 1999]. Термин оказался крайне удачным и быстро вошел в научную и клиническую лексику в качестве эквивалента когнитивного статуса между нормальными характеристиками когнитивных функций и деменцией.

Позже стало очевидным, что данная категория тяжести КР имеет большой разброс по когнитивному снижению среди пациентов и не отвечает задаче первичной профилактики КР. Это обосновывало изучение стадий, не достигших УКР.

Предлагаемые российскими и зарубежными исследователями термины стадий, не достигших УКР, имеют расхождения в определении. Следует отметить, что их выделение было инициировано в первую очередь с целью более ранней диагностики вероятной болезни Альцгеймера и обсуждается до настоящего времени, в основном, к ней.
