ВВЕДЕНИЕ Актуальность темы исследования

Иммуновоспалительные ревматические заболевания (ИВРЗ) протекают с высоким риском сердечно-сосудистых осложнений (ССО), в основном связанным с преждевременным развитием атеросклероза. Стандартизированные коэффициенты смертности при ИВРЗ варьируют от 2,8 до 3,6 и нет данных об улучшении выживаемости больных в последние годы, поэтому остро стоит вопрос о разработке новых подходов к ранней диагностике сердечно-сосудистых заболеваний (ССЗ) [64; 303].

Многочисленные исследования показывают, что относительный риск (ОР) развития ССЗ у пациентов с ревматоидным артритом (РА) колеблется от 1,4 до 3,7 [56], у больных системной красной волчанкой (СКВ) составляет 2,4 (95%-й доверительный интервал (ДИ): 1,95-2,8) [329], у больных системной склеродермией (ССД) - 2,2 (95%-й ДИ: 2,0-2,5) [232] по сравнению с общей популяцией. Более высокие риски очевидны уже с первых лет после постановки диагноза и не снижаются с увеличением длительности болезни [64]. Наиболее часто ССЗ развиваются у больных ИВРЗ с низким или умеренным кардиоваскулярным риском, но с высокой клинико-иммунологической активностью заболевания [81; 329].

Привлекает внимание схожесть патогенетических механизмов классических ИВРЗ и низкой степени хронического воспаления при атеросклерозе. По современным представлениям, хроническое воспаление, развитие которого связывают с неконтролируемой активацией как врожденного, так и приобретенного иммунитета, играет фундаментальную роль на всех стадиях ИВРЗ и атеросклероза.

Прогрессирование иммунопатологического процесса при ИВРЗ сопровождается развитием широкого спектра иммуновоспалительных нарушений, которые могут иметь патофизиологическое значение в развитии атеросклероза. Дисбаланс цитокинов и накопление медиаторов воспаления способствуют развитию сосудистых нарушений, составляющих основу атерогенеза: дисфункции эндотелия, вазоконстрикции, перекисного окисления липидов и липопротеидов, гиперкоагуляции, в дальнейшем - формированию и «дестабилизации» атеросклеротических бляшек (АСБ) и развитию ССО.

Среди многочисленных иммунных клеток и медиаторов, участвующих в иммунопатогенезе ИВРЗ, важное место занимают моноциты/макрофаги и цитокины, продуцируемые ими [461]. Дисбаланс между фенотипами макрофагов рассматривается в качестве одной из причин развития ИВРЗ. Проводятся поиски возможных механизмов нарушений воспалительного ответа моноцитов при различных заболеваниях. Изучение провоспалительного ответа моноцитов и

мутаций митохондриального генома, ассоциированных с клинической активностью, вариантами ИВРЗ, иммунологическими маркерами воспаления и сопутствующим атеросклерозом, может дать важную информацию об участии этих клеток в развитии ИВРЗ и атеросклероза при этих заболеваниях.

Степень разработанности темы исследования

В настоящее время получены данные о сложной гетерогенности и высокой пластичности макрофагов моноцитарного происхождения. Замена резидетной популяции с толерогенным фенотипом макрофагами моноцитарного происхождения в эксперименте может рассматриваться как модель экспериментально вызванной активации и персистирования воспаления [18]. Макрофаги обладают мощной воспалительной активностью, вызывая местные и системные иммунные реакции. Провоспалительный эффект макрофагов достигается, в первую очередь, выработкой цитокинов, хемокинов, а также презентацией антигенов адаптивным иммунным клеткам, что свидетельствует об единстве врожденной и приобретенной иммунных систем. К наиболее важным цитокинам, секретируемыми активированными макрофагами, относят фактор некроза опухоли - альфа (ФНО-а), интерлейкин 6 (ИЛ-6), интерлейкин 1 бета (ИЛ-1Р), интерлейкин 8 (ИЛ-8) и CCL2 (C-C motif ligand 2) или MCP-1 (Monocyte Chemoattractant Protein 1).

Накапливаются данные об участии макрофагов в патогенезе различных хронических воспалительных заболеваний и метаболических нарушений. Известно, что активация моноцитов у пациентов с субклиническим атеросклерозом и больных сахарным диабетом (СД) 2 типа развивается по воспалительному типу в отличие от лиц здорового контроля [140]. Изучение провоспалительного ответа моноцитов может дать важную информацию об участии этих клеток в развитии ИВРЗ. Роль, которую циркулирующие моноциты играют в качестве биомаркеров ИВРЗ, позволяет предположить, что они также могут быть инструментом для выявления доклинических стадий заболеваний. Следовательно, метаболическая характеристика подмножеств моноцитов/макрофагов и данные по мутациям мтДНК могут привести к новым стратегиям диагностики ИВРЗ [590].

Ожидается, что у больных ИВРЗ нарушение функционального состояния и реактивности моноцитов будет заключаться в повышении провоспалительной активности клеток не только в отношении иммуновоспалительного, но и атеросклеротического процесса. Важным представляется выявление особенностей воспалительного ответа моноцитов у больных ИВРЗ в зависимости от присутствия субклинического атеросклероза. До настоящего времени подобных исследований у больных ИВРЗ в сочетании с коморбидным атеросклерозом не проводилось.

Проводятся поиски возможных механизмов активации моноцитов и макрофагов по воспалительному типу. В качестве одного из предполагаемых механизмов активации моноцитов рассматривается дисфункция митохондрий иммунокомпетентных клеток вследствие мутаций митохондриальной ДНК (мтДНК) [322]. Установлена роль мутаций митохондриального генома в развитии предрасположенности к атеросклерозу [362; 500]. Можно предположить, что именно дефектная митофагия в результате мутаций в мтДНК может вызвать нарушения врожденного иммунитета, ведущее к хронизации воспаления при ИВРЗ и атеросклерозе. Мутации митохондриального генома в циркулирующих моноцитах у больных ИВРЗ ранее не исследовались, связь митохондриальной гетероплазмии с активацией моноцитов не описана. До настоящего времени не определено прогностическое значение проатерогенных вариантов гетероплазмии мтДНК и активации моноцитов в развитии атеросклеротических поражений сонных артерий у пациентов с ИВРЗ. Изучение данного механизма хронического аутоиммунного воспаления может стать перспективным направлением для разработки новых патогенетических подходов к терапии ИВРЗ и атеросклероза.

Цель и задачи исследования

Цель исследования - установить вклад нарушений воспалительного ответа моноцитов/макрофагов и проатерогенных вариантов гетероплазмии мтДНК у больных РА, СКВ, ССД в клинические проявления, лабораторные показатели ИВРЗ и субклиническое атеросклеротическое поражение сонных артерий.

Для достижения поставленной цели были сформулированы и решены следующие задачи:

1. Оценить частоту субклинического атеросклероза и его связь с клиническими проявлениями и лабораторными показателями у больных ИВРЗ (РА, СКВ и ССД).

2. Изучить нарушения воспалительного ответа макрофагов и гетероплазмию мтДНК по проатерогенным мутациям у больных РА, СКВ, ССД и сопоставить их с клиническими проявлениями и лабораторными показателями ИВРЗ.

3. Проанализировать традиционные факторы риска сердечно-сосудистых заболеваний в зависимости от дисфункции моноцитов/макрофагов у больных РА, СКВ, ССД.

4. Исследовать липидный спектр крови, уровни липопротеина (а) (Лп(а)) и антител к окисленным липопротеинам низкой плотности (анти-оЛПНП) в зависимости от дисфункции моноцитов/макрофагов у больных ИВРЗ.

5. Определить комплекс факторов риска (клеточных, генетических, традиционных и обусловленных ИВРЗ) атеросклеротического поражения сонных артерий у больных РА, СКВ и ССД.

6. Выделить и описать «проатеросклеротический» фенотип пациентов, страдающих

РА, СКВ и ССД, с нарушениями воспалительного ответа моноцитов/макрофагов и проатерогенными вариантами гетероплазмии мтДНК.

Научная новизна

Впервые в отечественной медицине установлено клиническое значение нарушений воспалительного ответа макрофагов и гетероплазмии по проатерогенным мутациям мтДНК в хронизации аутоиммунного воспаления и ускоренном развитии атеросклероза у больных РА, СКВ и ССД.

Впервые к факторам высокой активности ИВРЗ наряду с болезнь-специфическими (С-реактивный белок (СРБ), позитивность по аутоантителам) были отнесены выявленные в работе клеточные биомаркеры, представленные секрециями воспалительных цитокинов ФНО-а, ИЛ-6, ИЛ-1Р, С^2 и ИЛ-8 макрофагами в базальном и стимулированном состояниях.

Получены данные об единстве нарушений функциональных свойств макрофагов, заключающихся в снижении активации и повышении резистентности клеток, у больных РА. СКВ и ССД. Показана связь показателей секреции, активации и резистентности клеток по цитокинам с традиционными факторами риска, такими как пол, возраст, курение, артериальная гипертензия (АГ) и гиперлипидемия.

У больных ИВРЗ впервые определена степень гетероплазмии по десяти известным митохондриальным мутациям: m.652delG, m.1555A>G, m.3336T>C, m.3256C>T, m.5178C>A, т.123^>А, т.135^>А, m.14459G>A, m.14846G>A и m.15059G>A, связанным с атеросклеротическим поражением сосудов, а также ассоциация более высокой степени гетероплазмии по отдельным мутациям мтДНК с воспалительным ответом макрофагов.

Впервые были выделены «проатеросклеротические» фенотипы больных РА и СКВ и «фенотип, ассоциирующийся с увеличением ТИМ сонных артерий», больных ССД с учетом биомаркеров дисфункции моноцитов/макрофагов и проатерогенных вариантов гетероплазмии мтДНК. Впервые к факторам риска атеросклеротического поражения сонных артерий у больных РА, СКВ и ССД были отнесены биомаркеры дисфункции моноцитов/макрофагов и повышенный уровень гетероплазмии мтДНК m.1555A>G. Согласно анализу литературных источников, в мире нет ни одной научной статьи, где бы изучался пороговый уровень гетероплазмии мутаций митохондриального генома и их связь с предрасположенностью к атеросклерозу у больных ИВРЗ.

Теоретическая и практическая значимость работы

В целом результаты исследования позволили расширить представления о роли макрофагов в развитии иммуновоспалительного процесса и атеросклеротического поражения сосудов у больных ИВРЗ.

Перспективно определение секреции провоспалительных цитокинов макрофагами, полученными из периферической крови пациентов с РА, СКВ и ССД, которые в отличие от сывороточных уровней цитокинов, ассоциируются с клиническими, лабораторными проявлениями ИВРЗ, традиционными факторами риска и субклиническим атеросклерозом.

Исследованные маркеры дисфункции моноцитов могут способствовать совершенствованию методов ранней диагностики, определению активности и органных поражений при ИВРЗ.

Показана взаимосвязь дисфункции макрофагов и проатерогенных вариантов гетероплазмии мтДНК с клиническими проявлениями и иммунологическими нарушениями при ИВРЗ, что позволяет рассматривать механизм развития этих заболеваний с новых позиций.

Определены предикторы субклинического атеросклеротического поражения сонных артерий у больных РА, СКВ и ССД, к которым наряду с традиционными факторами риска были отнесены биомаркеры дисфункции моноцитов/макрофагов. Представлена более высокая частота повышенных уровней липопротеина (а) (Лп(а)) и антител к окисленному липопротеину низкой плотности (анти-оЛПНП) у больных РА, СКВ и ССД по сравнению с группой сравнения. Уточнены пороговые значения анти-оЛПНП для прогнозирования АСБ сонных артерий у больных РА и СКВ.

Выделены «проатеросклеротические» фенотипы пациентов с ИВРЗ, определение которых в клинической практике позволит своевременно диагностировать атеросклеротическое поражение сосудов и улучшить исходы заболеваний.

Перспективно определение степени гетероплазмии мтДНК по m.1555A>G у больных ИВРЗ для прогнозирования развития атеросклероза. Информация о пороговом уровне гетероплазмии мутаций митохондриального генома, ассоциированных с атеросклерозом и его факторами риска, может помочь в оценке предрасположенности к этой патологии. Полученные данные могут быть использованы при создании тест-систем для диагностики атеросклероза. Результаты работы закладывают фундамент для будущих исследований клеточного иммунитета, направленных в итоге на разработку новых лекарственных препаратов для лечения ИВРЗ и атеросклеротического поражения сосудов при этих заболеваниях.

Методология и методы исследования

Работа была выполнена в рамках многоцентрового исследования между ФГБНУ НИИР им. В. А. Насоновой и ГНЦ РФ ФГБНУ «РНЦХ им. акад. Б. В. Петровского». Исследование проведено в соответствии с этическими положениями Хельсинской декларации и Национальным стандартом Российской Федерации «Надлежащая клиническая практика Good Clinical Practice» государственный стандарт Р52379-2005.

Проведение исследования было одобрено локальным этическим комитетом при ФГБНУ НИИР им. В. А. Насоновой. Все участники предоставили письменные информированные согласия до включения в исследование.

Тип исследования - одномоментное поперечное. исследование состояло из четырёх основных разделов. В рамках первого с целью оценки структурных изменений сонных артерий проведено дуплексное сканирование сонных артерий в режиме высокого разрешения с использованием специализированного программного обеспечения M'Ath (M'Ath, Франция).

Во втором разделе работы у больных ИВРЗ в сравнении с группой контроля. были уточнены нарушения воспалительного ответа культивируемых моноцитов периферической крови, включающего определение секреции цитокинов в культуральной жидкости, расчет активации и резистентности клеток в базальном и стимулированном состоянии. В ходе эксперимента определялась секреция цитокинов ФНО-а, ИЛ-6, ИЛ-ip, ИЛ-8, CCL2 в культуральной жидкости методом иммуноферментного анализа (ИФА).

В рамках третьего раздела работы была исследована степень гетероплазмии мтДНК по десяти, ассоциированным с атеросклерозом, мутациям m.652delG, m.1555A>G, m.3336T>C, m.3256C>T, m.5178C>A, m.12315G>A, m.13513G>A, m.14459G>A, m.14846G>A и m.15059G>A.

Четвёртый раздел диссертационного исследования включал определение факторов риска атеросклеротического поражения сонных артерий и выделение «проатеросклеротических» фенотипов больных РА, СКВ и ССД.

В ходе исследования использованы общенаучные методы, такие как количественное и качественное описание признаков, измерение, анализ, синтез, статистические методы (описательная и выборочная статистика). Анализ литературных данных проведен с учетом принципов формализации, типологизации, анализа и синтеза. Статистическая обработка данных проводилась с помощью программ SPSS Statistics 26.0 for Windows (IBM Corporation, USA) и Statistica 12 (StatSoft Inc., США).

Положения, выносимые на защиту

1. Активность и клинические проявления ИВРЗ, традиционные факторы сердечнососудистого риска и показатели липидного обмена ассоциированы с повышением базальной секреции провоспалительных цитокинов ФНО-а, ИЛ-6, ИЛ-1Р, CCL2, ИЛ-8, снижением активации и увеличением резистентности клеток, характеризующих дисфункцию макрофагов.

2. Дисфункция макрофагов у пациентов с РА проявляется максимальной секрецией цитокинов ФНО-а, ИЛ-6 и ИЛ- 1р на начальных точках эксперимента со снижением секреции при повторном стимулировании клетки. «Самый выраженный» воспалительный ответ макрофагов установлен у больных РА, в то время как у больных ССД определены самые высокие уровни резистентности макрофагов по ИЛ-1Р и ИЛ-8 и высокий процент (43%) больных с нарушением резистентности иммунного ответа макрофагов по ССЬ2. У пациентов с СКВ дисфункция макрофагов по изученным цитокинам менее выражена, чем у пациентов с РА и с ССД, а по ряду параметров не отличалась от группы сравнения.

3. У пациентов с РА высокая степень гетероплазмии мтДНК по т3256С>Т, у больных СКВ по т3256С>Т и m5178G>A, у больных ССД по m14459G>A и т3336С>А ассоциировалась с воспалительным ответом макрофагов, что может свидетельствовать о важной роли мутаций мтДНК в развитии нарушений функции иммунокомпетентных клеток у больных ИВРЗ.

4. «Проатеросклеротический фенотип» больных РА и СКВ наряду с традиционными факторами сердечно-сосудистого риска, болезнь-специфическими проявлениями, характеризуется дисфункцией макрофагов с повышением базальной секреции хемокина CCL2 и рестимулированной секреции ИЛ-6 при РА, повышением базальной секреции ИЛ-1Р и снижением активации по ФНО-а при СКВ.

5. Различные патогенетические механизмы приводят к утолщению интима-медиа или образованию атеросклеротических бляшек сонных артерий у больных ИВРЗ. Увеличение толщины интима-медиа (ТИМ) сонных артерий ассоциируется с гетероплазмией мтДНК по т3336С>А и провоспалительной активацией макрофагов с высокой секрецией при РА цитокинов ФНО-а, ИЛ-6, ИЛ-1Р, при СКВ - ИЛ-1Р, при ССД - ИЛ-6, ИЛ-1Р и СШ, образование атеросклеротических бляшек сонных артерий - с гетероплазмией мтДНК по m1555A>G, высокой базальной секрецией ССЬ2 и рестимулированной секрецией ИЛ-6 при РА и активацией клетки по ФНО-а и базальной секрецией ИЛ-1Р при СКВ.

6. Дисфункция макрофагов у больных ССД определяет выделение фенотипа, характеризующегося увеличением ТИМ сонных артерий, но не субклинического атеросклеротического поражения артерий.

Степень достоверности и апробация результатов

Достоверность результатов диссертационного исследования основана на достаточном числе участников. В общей сложности в исследование были включены 274 человека. Среди них 200 пациентов с ИВРЗ, не получающих противоревматическую терапию и не имеющих клинических признаков связанных с атеросклерозом сердечно-сосудистых заболеваний. Группа сравнения была сформирована из относительно здоровых добровольцев без ИВРЗ, аутоиммунных и сердечно-сосудистых заболеваний, без острых и хронических инфекций. Всем пациентам был проведен полный спектр обследований, предусмотренный клиническими рекомендациями Ассоциации ревматологов России. Материалы первичной документации стали базисом основных выводов и положений диссертационной работы. Проведение тщательного анализа и современной статистической обработки с использованием надлежащего программного обеспечения обеспечивают достоверность полученных выводов и рекомендаций.

Апробация работы состоялась 18 марта 2025 г. на заседании ученого совета ФГБНУ НИИР им. В.А. Насоновой (протокол заседания № 6). По итогу - диссертация рекомендована к публичной защите.

Личный вклад автора

Герасимова Е.В. лично определяла все этапы проведенного диссертационного исследования: принимала участие в выборе направления научного исследования, в формулировке цели и задач, разработке дизайна работы и её проведении. В соответствии с задачами исследования была изучена научная литература по теме диссертации, подготовлены и опубликованы в ведущих российских и мировых медицинских журналах 12 больших литературных обзоров.

Герасимова Е.В. в качестве врача-ревматолога оказывала квалифицированную медицинскую помощь пациентам, включенным в исследование, или проводила консультативную помощь в качестве старшего научного сотрудника. Автор самостоятельно подбирал участников исследования, осуществлял сбор и оценку анамнестических характеристик больных ИВРЗ, определял степени активности заболеваний и функционального статуса пациентов, проводил оценку традиционных факторов риска ССЗ и курацию больных на протяжении сбора биоматериала. Самостоятельно были разработаны информированные согласия пациентов, карты

участников и электронная база с более 250 данных, заполняемых на каждого пациента, и 50 данных на лиц группы сравнения.

Под личным контролем соискателя проведены сбор и пробоподготовка биологического материала. Все полученные результаты тщательно проанализированы, статистически обработаны, определены общие и нозологически специфические паттерны активации моноцитов и вариантов гетероплазмии мтДНК в сопоставлении с активностью, длительностью и клиническими проявлениями ИВРЗ.

На основании полученных результатов Герасимова Е.В. проводила обобщении клинико-инструментальной диагностики, анализ данных, полученных при культивировании моноцитов периферической крови, самостоятельно рассчитывала активацию и резистентность макрофагов, индекса атерогенности крови, соотношения абсолютного числа моноцитов к ХС ЛПНП. Соискателем были сформулированы научные положения трактата, выводы и практические рекомендации. Автореферат и все главы диссертационной научной работы были изложены Герасимовой Е.В. единолично. Автор лично представлял все доклады на передовых российских и международных конгрессах и конференциях, а также внедрял результаты в клиническую ревматологическую практику.

Публикации по теме диссертации

По результатам исследования автором опубликовано 12 литературных обзоров, 16 оригинальных научных статей, в том числе 26 научных статьей в журналах, включенных в Перечень ВАК при Минобрнауки России, в которых должны быть опубликованы основные научные результаты диссертаций на соискание ученой степени доктора наук, из них 22 научные статьи в базе данных RSCI/SCOPUS; 36 публикаций в сборниках материалов международных и всероссийских научных конференций, съездов и конгрессов.

Внедрение результатов исследования

Основные результаты данной работы внедрены и применяются в ФГБНУ «НИИ ревматологии им. В.А. Насоновой», ФГБНУ НИИМЧ им. Ак. А.П. Авцына ФГБНУ «РНЦХ им. Акад. Б.В. Петровского», ФГБОУ ДПО РМАНПО Минздрава России, материалы диссертационного исследования используются при чтении лекций, проведении круглых столов и практических занятий для врачей, студентов и ординаторов в ФГБНУ «НИИ ревматологии им. В.А. Насоновой», ФГБНУ НИИМЧ им. Ак. А.П. Авцына ФГБНУ «РНЦХ им. Акад. Б.В. Петровского» и ФГБОУ ДПО РМАНПО Минздрава России.

Структура и объем диссертации

Диссертационная работа представлена на 244 страницах компьютерного текста и состоит из введения, четырех глав, заключения, списка сокращений, списка литературы (611 источников, из них 579 зарубежных) и двух приложений. В тексте научной работы содержится 54 таблицы и 47 рисунков.

заболеваниях

Иммуновоспалительные ревматические заболевания (ИВРЗ) представляют собой воспалительные заболевания, в основе которых лежит неконтролируемая активация клеточных и гуморальных компонентов иммунной системы, приводящая к хронизации воспаления и, как следствие, к нарушению ремоделирования и повреждению тканей [519]. Относящиеся к ИВРЗ ревматоидный артрит (РА), системная красная волчанка (СКВ) и системная склеродермия (ССД) являются классическими прототипами воспалительных заболеваний человека.
