ВВЕДЕНИЕ

Ювенильный идиопатический артрит (ЮИА) — тяжелое хроническое, неуклонно прогрессирующее заболевание неизвестной этиологии, со сложным аутоагрессивным патогенезом. ЮИА — одна из наиболее частых и самых инвалидизирующихревматических болезней у детей [1].ЮИА характеризуется преимущественно деструктивным поражением суставов, а также патологией других органов и тканей с формированием полиорганной недостаточности различной степени выраженности [3]. Заболевание приводит к резкому ограничению возможности передвижения и самообслуживания, физической, психической, социальной дезадаптации детей(Насонов Е.Л., Насонова В.А, 2010).

Лечение ЮИА остается одной из наиболее сложных и актуальных проблем ревматологии. В терапии используют широкий спектр противоревматических препаратов. Патогенетическая иммуносупрессивная терапия существенно улучшает клиническое состояние и качество жизни большинства больных ювенильным артритом[4], однако у многих пациентов возможность достижения стойкой ремиссии с помощью традиционных базисных противоревматических препаратов по-прежнему остается маловероятной [5 -9].

Важная роль цитокинов в патогенезе ЮИА привела к возможности использовать блокаторы цитокинов в лечении данного заболевания. Антицитокиновая терапия обеспечивает вполне удовлетворительные результаты и способна существенно улучшить прогноз даже при тех тяжелых формах ЮИА, при которых общепринятые терапевтические стратегии лечения часто не давали положительного результата.

Особый интерес вызывает применение моноклональных антител, максимальная избирательность которых обеспечивает селективное действие на иммунную систему, позволяющее устранять необходимое звено в патогенетической цепи. Высокая специфичность антител исключает возможность их влияний на другие физиологические механизмы

функционирования иммунной системы.

В ревматологии, несмотря на ряд специфических проблем, включающих <^АаЬе1»-статус ряда генно-инженерных биологических препаратов (ГИБП) для детей, значение этих новых медикаментов постоянно возрастает. В первую очередь это касается препаратов, способных эффективно лечить системный вариант ЮИА — болезнь Стилла — и полиартикулярный вариант ЮИА тяжелого течения.

ГИБП произвели революцию в лечении ревматоидного артрита (РА). Важно отметить, что эти препараты статистически значимо улучшают функциональную активность больных и качество их жизни. Многими клиническими исследованиями подтверждено, что повышение качества жизни на фоне лечения биологическими агентами связано с увеличением физической активности, уменьшением утомляемости и усталости, повышением эмоционального фона и трудоспособности пациентов[10-12].

Внедрение ГИБП в клиническую практику позволяет не только купировать симптомы заболевания, но и сдерживать развитие эрозивного процесса в суставах, улучшать функциональный статус и качество жизни пациента, увеличивать продолжительность жизни до популяционного уровня.

Введение ГИБП в клиническую практику чрезвычайно продвинуло лечение ЮИА и осуществило смену парадигмы. Исследования цитокинового профиля, клеточного состава крови у больных ЮИА в процессе заболевания, могут способствовать разработке критериев диагностики и контроля иммунного статуса у пациентов при подборе наиболее адекватной терапии, что определяет продолжительность и качество их жизни (Алексеева Е.И. и соавт., 2004, Кельцев В.А. 2008, 2010).

Данные литературы о проведении сравнительного анализа влияния традиционных базисных препаратов и биологических агентов на течение ЮИА немногочисленны. Эти положения свидетельствуют о необходимости дальнейших исследований по выбору тактики ведения больных с этими заболеваниями с использованием новых, наиболее эффективных методов

лечения, в том числе моноклональных антител, а так же прогнозирования побочных эффектов при их применении и определения наиболее эффективных и безопасных препаратов в лечении различных форм ЮИА.

Цель исследования:

Провести сравнительный анализ влияния генно-инженерного биологического препарата тоцилизумаба и базисного препарата метотрексата на клинико-иммунологические и цитокиновые показатели у детей и подростков, больных полиартикулярным и системным вариантами ювенильного идиопатического артрита, и разработать рекомендации по наиболее эффективному и безопасному их использованию при лечении различных вариантов данного заболевания.

Задачи исследования:

1. Дать оценку клинико-иммунологическим параметрам гомеостаза у детей и подростков, больных полиартикулярным и системным вариантами ювенильного идиопатического артрита.

2. Провести сравнительный анализ эффективности препаратов метотрексата и тоцилизумаба на течение заболевания и качество жизни детей, больных полиартикулярным и системным вариантами ювенильного идиопатического артрита.

3. Дать сравнительную оценку нежелательных осложнений при лечении детей, больных полиартикулярным и системным вариантами ювенильного идиопатического артрита, метотрексатом и тоцилизумабом.

Научная новизна

Разработаны критерии целенаправленного включения ГИБП в лечение больных полиартикулярным и системным вариантами ЮИА в зависимости от иммунологического статуса.

По данным динамики клинико-иммунологических показателей у больных

с полиартикулярным и системным вариантами ЮИА разработаны критерии эффективности проводимой терапии с включением метотрексата и тоцилизумаба. Анализ клинико-иммунологических показателей позволил оптимизировать курсы лечения полиартикулярных и системных вариантов ЮИА и прогнозировать нежелательные явления при проведении терапии данным больным.

Практическая значимость:

Анализ клинических и иммунологических показателей активности полиартикулярного и системного вариантов ЮИА позволяет дифференцировано назначать базисную терапию данным больным. Мониторинг с помощью клинико-иммунологических показателей состояния больных способствует раннему выявлению нежелательных явлений при проведении базисной терапии метотрексатом и тоцилизумабом больных ЮИА. Динамическое наблюдение за состоянием иммунологических показателей патологического процесса позволяет свести к минимуму осложнения при проведении базисной терапии полиартикулярного и системного вариантов ЮИА. Рефрактерность к терапии метотрексатом детей, больных полиартикулярным вариантом ЮИА в течение 3 месяцев является показанием к назначению им генно-инженерных препаратов.

Положения, выносимые на защиту:

1. Клинико-иммунологические показатели являются основой прогрессирования полиартикулярного и системного вариантов ювенильного идиопатического артрита у детей.

2. Тоцилизумаб при полиартикулярном и системном вариантах ювенильного идиопатического артрита у детей является более эффективным и безопасным препаратом по сравнению с метотрексатом

3. Тоцилизумаб эффективен у больных при системном и полиартикулярном вариантах ювенильного артрита у детей независимо

от длительности заболевания, тем не менее более раннее назначение дает более быстрый эффект, что дает основание к рекомендации назначения данного препарата на ранних этапах развития патологического процесса в случае неэффективности лечения метотрексатом в течение 1 -3 месяцев лечения.

Внедрение результатов исследования

Материалы исследования и рекомендации по диагностике и лечению полиартикулярного и системного вариантов ЮИА у больных внедрены в лечебно - диагностическую работу детского кардиоревматологического отделения Самарского областного клинического кардиологического диспансера.

Результаты проведенных исследований включены в программу учебного курса кафедры факультетской педиатрии и при преподавании ревматологии слушателям ИПО СамГМУ.

Личный вклад автора

Проведено наблюдение и анализ клинического обследования 95 больных с использованием современных клинических, функциональных и инструментальных методик.

Публикации

Основные положения диссертации изложены в 16 работах, опубликованных в областных, региональных и республиканских сборниках, в том числе 4 работы в научных журналах, рецензируемых ВАК РФ.

Апробация работы

Материалы исследования доложены и обсуждены на областных научно-практических конференциях «День кардиоревматолога» (Самара 2010, 2011, 2012); на

межрегиональной конференции молодых ученых и специалистов «Аспирантские чтения» (Самара 2011); на Всероссийской конференции ревматологов России (Нижний Новгород 2010,2011); на XVIII и XIX Российских национальных конгрессах «Человек и лекарство» (Москва 2011,2012); на XIV, XV съездах педиатров России (Москва, 2011, 2012).

Объем и структура работы

Диссертация изложена на 113 страницах машинописного текста и иллюстрирована 9 таблицами и 20 рисунками. Состоит из введения, обзора литературных источников, материалов и методов исследования, клинической характеристики больных, главы собственных наблюдений, заключения, выводов и практических рекомендаций. Список использованных источников включает в себя 273 наименований, из которых 21 отечественных и 252 иностранных авторов.

ВЫВОДЫ

1. Мониторинг цитокиновых показателей в качестве диагностических критериев при полиартикулярных и системных вариантах ювенильного идиопатического артрита является необходимым как для диагностических, так и прогностических позиций. Уровень провоспалительных цитокинов ИЛ-10 (57,6±4,2пг/мл), ФНО-а (36,9±1,7пг/мл), ИЛ-61(1,6±0,7пг/мл), а также противовосполительного цитокина ИЛ-10 (11,5±2,2пг/мл) выше уровня контрольной группы больных детей в сыворотке крови является прогностически неблагоприятным.

2. Тоцилизумаб при лечении больных полиартикулярным и системным вариантами ювенильного идиопатического артрита является препаратом выбора и жизненно необходимым и должен быть введен в стандарт лечения данных больных

3. Назначение тоцилизумаба в ранние сроки заболевания полиартикулярным и системным вариантами ювенильного идиопатического артрита более эффективно, чем назначение в более поздние сроки заболевания, что дало основание к рекомендации назначения данного препарата на ранних этапах развития патологического процесса в случае неэффективности лечения метотрексатом в течение 1-3 месяцев лечения.

4. Высокая частота гепатотоксичности при лечении детей, больных полиартикулярным и системным вариантами ЮИА метотрексатом требует более тщательного мониторированияуровня печеночных ферментов в сыворотке у больных в процессе их диспансерного наблюдения.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

1. Анализ клинических и иммунологических показателей активности полиартикулярного и системного вариантов ювенильного артрита позволяет дифференцировано назначать базисную терапию данным больным. В качестве диагностических критериев полиартикулярного варианта ЮИА можно использовать повышение провоспалительных цитокинов ИЛ-1(3, ФНО-а, ИЛ-6, а также противовосполительного цитокина ИЛ-10 выше уровня контрольной группы в сыворотке крови - как для диагностики, так и для прогноза заболевания.

2. Мониторинг с помощью клинико-иммунологических показателей состояния больных способствует раннему установлению нежелательных явлений при проведении базисной терапии метотрексатом и тоцилизумабом больных ювенильным идиопатическим артритом

3. Динамическое наблюдение за состоянием иммунологических показателей патологического процесса позволяет свести к минимуму осложнения при проведении базисной терапии полиартикулярного и системного вариантов ювенильного идиопатического артрита. Рефрактерность к терапии метотрексатом детей, больных полиартикулярным и системным вариантами ЮИА в течение 3 месяцев, является показанием к назначению им генно-инженерных препаратов.

4. При назначении ГИБП тоцилизумаба на ранних этапах заболевания полиартикулярным и системным вариантами ЮИА эффект у больных был более значимым, что дало основание к рекомендации назначения данных препаратов на ранних этапах развития патологического процесса в случае неэффективности лечения метотрексатом в течение 1 -3 месяцев лечения.

5. Высокая частота гепатотоксичности при лечении метотрексатомдетей, больных полиартикулярным и системным вариантами ЮИА, требует более тщательного мониторированияуровня печеночных ферментов в процессе их диспансерного наблюдения.
