ВЫВОДЫ

1. В крови больных острым и хроническим гепатитом С выявлен широкий спектр аутоантител (ASMA, ANA, АМА, anti-LKM-1) , а также криоглобулины и ревматоидный фактор.

2. При остром вирусном гепатите С наличие аутоантител сопровождалось более тяжёлым течением заболевания. У 30,7% больных ОВГС к периоду клинического выздоровления наблюдалось снижение титра аутоантител, однако, у 14,5% больных к этому периоду титры аутоантител или сохранялись без изменений, или выявлялись впервые. Цитолитический синдром при ОВГС более выражен у больных с наличием аутоиммунных изменений, чем у пациентов без этих нарушений.

3. У больных ОВГС среднетяжелого течения болезни на высоте интоксикации достоверно выше частота выявления в высоких титрах ревматоидного фактора и криоглобулинов, чем у больных легкого течения. К периоду ранней реконвалесценции установлено снижение титра или исчезновение ревматоидного фактора и криоглобулинов независимо от тяжести болезни.

4. Установлены более высокие показатели уровней цитокинов (ИЛ-1(3 и ФНО-а) у лиц, в крови которых обнаружены аутоантитела по сравнению с пациентами, у которых они не выявлялись. Выявлена прямая средняя корреляционная связь между уровнем ИЛ-1р, ФНО-а и наличием аутоиммунных нарушений.

5. У больных хроническим гепатитом С в 54 % случаев в крови выявляются аутоантитела (ASMA, АМА, ANA, anti-LKM-1). У больных хроническим гепатитом С с минимальной степенью активности не установлена связь между выявлением аутоантител и цитолитическим синдромом, в то же время у больных в группах умеренной и высокой степени активности патологического процесса синдром цитолиза более выражен у пациентов с наличием аутоантител.

6. У больных ХВГС получена прямая корреляционная связь между наличием РФ, ФНО-а и выраженностью цитолитического синдрома, что определяет тяжесть заболевания. Дисбаланс в продукции ИЛ-1, ИЛ-6 и ФНО-а приводит к неадекватному соотношению клеточных и гуморальных факторов иммунитета, и, в конечном итоге, способствует персистенции вируса гепатита С.

7. Высокий уровень спонтанной ХЛ на фоне высокого уровня ЦИК с преобладанием комплексов средних и мелких размеров при ОВГС, является одной из причин длительной циркуляции антигенов вируса в организме, что находит отражение в высоком хрониогенном потенциале, вируса гепатита С.

8. При среднетяжелом течении ОВГС выявлено возрастание функциональной активности ПМЯЛ периферической крови, оцениваемого по индексу стимуляции с использованием РМА, с одновременным сохранением значительного запаса потенциальных возможностей Fc и С рецепторов и отсутсвием их истощения к моменту клинического выздоровления, учитывая высокий уровень стимуляции OZ. Низкий ИС, установленный при использовании FMLP, свидетельствует о падении хемотаксической функции ПМЯЛ, что приводит к их экзоцитарной дегрануляции.

9. При ОВГС установлено, что на фоне высокого уровня ЦИК среднего и мелкого размера, аутоиммунных нарушений, сохраняется снижение показателей СЗ и С5 компонентов комлемента и нормальный уровень С4. Это свидетельствует, с одной стороны, о том, что активация комплемента по классическому пути не доминирует, а с другой стороны, выявленные изменения ведут к формированию аллергических реакций II и III типа (антитело опосредованной цитотоксичности и иммунокомплексной), лежащих в основе патогенеза вирусного гепатита С уже на ранних этапах инфекционного процесса и определяющих длительную персистенцию вируса.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ.

Выявленное наличие аутоиммунных проявлений у больных ВГС на ранних этапах заболевания позволяют рекомендовать исследование в инфекционных стационарах и гепатологических центрах сыворотки крови этих больных, в периоде реконвалесценции и при диспансерном наблюдении на наличие маркеров аутоиммунных нарушений (неорганоспецифические аутоантитела - ASMA, АМА, ANA, anti-LKM-1, ревматоидный фактор) для решения вопроса о прогнозировании течения заболевания и необходимости дальнейшего наблюдения.

У больных перенесших ОВГС при выявлении мочевого синдрома необходимо дополнительное обследование сыворотки крови на криоглобулины и уровень ЦИК для своевременной диагностики поражения почек. Имеет значение исследование сыворотки крови больных ХВГС на наличие ревматоидного фактора и ФНО-а при диспансерном обследовании, так как выявлена прямая корреляционная связь уровней РФ и ФНО-а с выраженностью и интенсивностью патологического процесса в печеночной ткани при хроническом вирусном гепатите С. Исходя из того, что значительное количество аутоиммунных нарушений присутствует у больных ХВГС вне зависимости от активности процесса, желательно не реже одного раза в год при диспансерном наблюдении этих больных в гепатологических центрах обследовать сыворотку крови на наличие неорганоспецифических аутоантител (ASMA, АМА, ANA, anti-LKM-1) и ревматоидного фактора, что имеет значение для подбора противовирусной терапии.
