Актуальность проблемы.

Вирус гепатита С (ВГС) является одним из наиболее частых этиологических факторов, вызывающих гепатит с парентеральным путем заражения. Актуальность изучения этого заболевания обусловлена: 1-широкой распространенностью гепатита С (до 3% популяции населения инфицированы ВГС); 2 - высоким риском хронизации инфекции - от 50% до 85% случаев острого гепатита С принимает хроническую форму [27, 161]; 3 - неблагоприятным исходом (у 20-30% больных гепатит С приводит к циррозу печени или гепатоцеллюлярной карциноме) [201]; 4 -отсутствием специфических средств профилактики и лечения.

У 70% ВГС инфицированных лиц отмечено хроническое течение заболевания печени и только в 20% случаев наблюдают явления острого гепатита [39]. Летальные исходы при остром гепатите С (ОГС) встречаются редко. Однако, по данным Европейского комитета по профилактике вирусных гепатитов, гепатит С по причине смертности среди больных с хроническим поражением печени занимает второе место, уступая только хроническому алкоголизму [1]. Гепатит С характеризуется преобладанием случаев бессимптомного течения: так, до 95% ОГС протекает в безжелтушной форме [1], в 85% случаев хроническая инфекция также является бессимптомной [39]. Отсутствие клинических проявлений осложняет специфическую диагностику этого заболевания.

В настоящее время первичная диагностика вирусного гепатита С основывается на выявлении антител к белкам ВГС. Однако, обнаружение анти-ВГС в сыворотке больных оказывается недостаточным, не позволяя разграничить текущую инфекцию от перенесенной в прошлом и закончившейся реконвалесценцией больного [66]. Кроме того, отсутствие антител к вирусным белкам может быть обусловлено периодом "окна" при сероконверсии. Известно также, что часть (около 10%) больных, особенно при иммунодефицитных состояниях, остается серонегативной [251]. В связи с этим возникает необходимость определять у больных РЖ ВГС и вирусные белки. В настоящее время основным методом, позволяющим установить наличие вируса гепатита С в организме, является полимеразная цепная реакция (ПЦР). С использованием этого метода возможно как качественное, так и количественное определение РНК ВГС. Однако, применеиие ПЦР имеет свои ограничения, связанные со значительным расхождением результатов в разных лабораториях мира. Другим подходом для прямого определения вируса в организме больных является определение белков ВГС. Эффективным инструментом для решения этой задачи являются моноклональные антитела (МКА) к вирусным белкам, обладающие высокой специфичностью. Использование МКА позволяет выявлять вирусные антигены в клетках и тканях больных ВГС.

Цель и задачи исследования.

Цель настоящей работы состояла в получении моноклональных антител к белкам ВГС и в разработке на их основе иммуногистохимического метода детекции вирусных белков в клетках ВГС-инфицированных лиц. Для достижения цели ставились следующие задачи:

1. Получить гибридомные клеточные линии, стабильно секретирующие МКА, взаимодействующие с белками ВГС.

2. Изучить иммунохимические свойства МКА к структурным и неструктурным белкам ВГС.

3. Определить способность МКА выявлять белки ВГС, экспрессированные в эукариогических клетках, трансфицированных рекомбинантной ДНК, содержащей гены белков ВГС.

4. Исследовать способность МКА взаимодействовать с натуральными вир> сными белками в клетках печени и лимфоцитах периферической крови (ЛПК) больных ХГС.

5. Сравнить накопление и распределение белков ВГС в клетках печени и ЛПК больных ХГС.

6. Изучить экспрессию поверхностных маркеров Т- и В-лимфоцитов больных с различной активностью гепатита С.

7. Провести статистический анализ полученных результатов для оценки связи между обнаружением вирусных маркеров (РНК ВГС и вирусные белки) и активностью патологического процесса.

Научная новизна и практическая значимость.

1. Получена панель МКА, направленных к 11 индивидуальным эпитопам рекомбинантных белков Core, NS3 и NS5 ВГС. С помощью МКА установлена локализация 9 не описанных ранее антигенных эпитопов белков ВГС.

2. Показана способность МКА взаимодействовать с белками ВГС, экспрессированными в клетках после введения в них ДНК, содержащей гены ВГС.

3. На основе полученных МКА разработан метод выявления белков ВГС на криостатных срезах печени и в препаратах ЛПК больных ХГС.

Основные положения, выносимые на защиту.

1. МКА, полученные в ходе выполнения работы, направлены к 11 различным антигенным детерминантам белков Core, NS3 и NS5 ВГС.

2. Разработан иммуногистохимический метод выявления вирусных белков на криостатных срезах печени и в ЛПК больных ХГС с использованием полученных МКА.

3. Индивидуальные белки ВГС выявляются в клетках печени пациентов с ХГС с различной частотой. Не обнаружено корреляции между накоплением вирусных белков в печени и активностью хронического гепатита С.

4. Установлено, что частота выявления вирусных белков в ЛПК значительно ниже, чем в клетках печени больных ХГС. При этом в ЛПК частота обнаружения белка нуклеокапсида выше, чем неструктурных белков ВГС, тогда как в клетках печени чаще других белков выявляется неструктурный белок NS4.

Выводы

1. Получено 13 гибридом, продуцирующих моноклональные антитела (МКА) к рекомбинантным белкам вируса гепатита С, экспрессированным в бактериальных клетках E.coli. МКА выявляют 11 индивидуальных и 2 перекрестно реагирующих эпитопов на белках ВГС: 3 -на белке нуклеокапсида, 5- на белке NS3 и 3- на белке NS5.

2. Установлено, что все МКА способны взаимодействовать с белками ВГС в эукариотических клетках культуры, трансфицированных рекомбинантной ДНК-плазмидой, содержащей гены NS3, NS4 и NS5 ВГС.

3. Обнаружено, что индивидуальные белки ВГС в печени пациентов с ХГС выявляются с различной частотой: белок нуклеокапсида - у 33% больных, белок NS3 - у 35%, белок NS4 - у 64%, белок NS5 - у 46% больных ХГС.

4. Белки ВГС выявлены в лимфоцитах периферической крови (ЛПК) у

23% больных ХГС. С наибольшей частотой (21%) обнаруживался белок нуклеокапсида, неструктурные белки обнаружены в лимфоцитах 13-16% пациентов.

5. Показано, что накопление белков ВГС в печени и в ЛПК больных ХГС не зависит от уровня АЛТ, гистологических показателей и количества РНК в сыворотке. Одновременное выявление белков в печени и РНК ВГС в сыворотке прямо коррелирует с активностью АЛТ.

6. Установлено, что количественные изменения в субпопуляциях лимфоцитов не коррелируют с показателями активности ХГС (ИГА, ГИС и АЛТ).

7. Выявлена обратная зависимость между экспрессией антигена CD95 (фактор апоптоза) на лимфоцитах периферической крови и накоплением белков Core и NS4 в печени.

На основе МКА ldll и 6Ы1 и 2с9, наиболее эффективно выявляющих натуральные белки, разработан иммунный реагент для иммуногистохимического выявления белков ВГС, позволивший обнаружить вирусные белки в криостатных срезах печени у 72% больных с ХГС.
