На сегодняшний день ишемическая болезнь сердца (ИБС) является главной причиной инвалидизации и смертности населения всех развитых стран, что заставляет искать новые возможности прогнозирования, оценки риска, диагностики, профилактики и лечения. Наиболее значимое и опасное проявление ИБС - острый коронарный синдром (ОКС). Он преобладает и в структуре смертности от всех сердечно-сосудистых заболеваний, и в структуре частоты госпитализаций, связанных с ИБС [10, 11, 28]. Универсальным механизмом, через который реализуется действие всех факторов риска развития ИБС, признана эндотелиальная дисфункция [1, 184, 185, 682]. Даже до появления видимых атеросклеротических поражений эта дисфункция может проявляться в виде количественного изменения определённых биохимических показателей крови [1]. Преимущественное нарушение функционального состояния эндотелия зависит от локализации патологического процесса, наличия гемодинамических сдвигов, преобладания различных гуморальных факторов, повреждающих эндотелий [13].

По современным представлениям в патогенезе атеросклероза и его осложнений решающую роль играет воспаление. В частности, воспаление способствует разрыву нестабильных атеросклеротических бляшек и эрозированию интимы, оба эти процесса могут привести к коронарному тромбозу [19, 250, 252]. Воспалительный процесс при атеросклерозе является следствием ответной реакции эндотелия на повреждающее действие атерогенных факторов, к которым относятся, прежде всего, окисленные липопротеины низкой плотности, а также С-реактивный белок, антифосфолипидные антитела, гипергомоцистеинемия, липопротеин (а), цитокины [318, 382, 383, 384, 613].

Также показано, что данные атерогенные факторы вызывают апоптоз эндотелиоцитов [68, 69]. Апоптоз эндотелия представляет форму повреждения, в результате которого увеличивается проницаемость сосудистой стенки для цитокинов, факторов роста, липидов, повышается адгезия лейкоцитов, а также активируется система коагуляции и снижается выработка N0 [71, 72, 73, 75, 241].

Как правило, в конкретной клинической ситуации могут сочетаться несколько вариантов нарушения функциональной активности эндотелия, поэтому в крови происходит изменение содержания различных факторов. Не исключено, что типовые формы эндотелиальной дисфункции (вазомоторная, гемостатическая, адгезионная, ангиогенная) возникают в связи с преимущественным действием различных факторов дисфункции эндотелия.

Необходимо отметить, что исследования, посвященные анализу особенностей влияния факторов повреждения сосудистой стенки (гомоцистеина, высокочувствительного СРБ, липопротеина (а) и антител к окисленным липопротеинам низкой плотности) на функциональную активность и апоптоз эндотелия при остром коронарном синдроме, немногочисленны, результаты их нередко противоречивы.

Таким образом, актуальность темы диссертационной работы определяется важностью дальнейшего исследования эндотелиальной дисфункции, её форм и их зависимости от профиля факторов, влияющих на эндотелий на стадии острых осложнений атеросклероза.

Цель исследования: установить особенности дисфункции эндотелия, обусловленной повреждающим действием гомоцистеина, липопротеина (а), вчСРБ и антител к окисленным липопротеинам низкой плотности, при остром коронарном синдроме.

Задачи исследования:

1. Определить содержание липидов, аполипопротеинов и антител к окисленным липопротеинам низкой плотности при остром коронарном синдроме.

2. Установить роль факторов повреждения сосудистой стенки в развитии плазмо-коагуляционной и тромбоцитарной тромбофилии при остром коронарном синдроме.

3. Исследовать функциональное состояние эндотелия путем определения в крови факторов, повреждающих эндотелий (гомоцистеина, липопротеина (а), вчСРБ, антител к окисленным липопротеинам низкой плотности) и биохимических маркеров дисфункции эндотелия (растворимой формы клеточно-сосудистой молекулы адгезии, антигена тканевого активатора плазминогена, ингибитора тканевого активатора плазминогена и тромбомодулина).

4. Изучить содержание биохимических маркеров апоптоза sApo-1/Fas и sBcl при остром коронарном синдроме.

5. Определить содержание в крови больных острым коронарным синдромом аннексина А5 и антител к аннексину А5.

6. Оценить количество циркулирующих аннексии А5-«позитивных» мононуклеаров при остром коронарном синдроме.

7. Исследовать количество циркулирующих CD59 и CD 105-«позитивных» мононуклеаров при остром коронарном синдроме.

Диссертация выполнена в соответствии с планом научного исследования Санкт-Петербургского государственного медицинского университета имени академика И.П. Павлова по теме кафедры патофизиологии 96.01.02.04 «Функциональная активность сосудов микроциркуляторного русла и тромбоцитов в норме и патологии» в рамках программы «Изучение прогностической значимости и путей коррекции ремоделирования сердца и сосудов у больных артериальной гипертензией и сопутствующими нарушениями углеводного и липидного обмена», № Гос. Регистрации 012002 12902.

Научная новизна. Данная работа является первым систематическим исследованием, позволившим рассмотреть различные формы дисфункции эндотелия в условиях сочетанного действия факторов повреждения сосудистой стенки при остром коронарном синдроме.

В результате исследований получены новые данные об особенностях дисфункции эндотелия при остром коронарном синдроме, вызванной действием факторов повреждения сосудистой стенки (гомоцистеина, липопротеина (а), вчСРБ и антител к окисленным липопротеинам низкой плотности). Установлено, что у 83,6% больных повышался антиген тканевого активатора плазминогена. Значительно реже отмечалось увеличение содержания растворимой формы клеточно-сосудистой молекулы адгезии (41,5%), тромбомодулина (31,5%) и ингибитора тканевого активатора плазминогена - (28,9%). Показано, что одновременное повышение нескольких маркеров эндотелиальной дисфункции при остром коронарном синдроме, особенно при остром инфаркте миокарда с подъёмом сегмента ST, свидетельствует о развитии гемостатической и адгезионной форм дисфункции эндотелия при данном заболевании в результате повреждающего действия гомоцистеина, липопротеина (а), вчСРБ и антител к окисленным липопротеинам низкой плотности.

Впервые проведено исследование содержания циркулирующих в периферической крови CD59 - и СБ105-«позитивных» мононуклеаров для оценки активности воспалительного процесса при остром коронарном синдроме.

Впервые установлено, что увеличение количества циркулирующих аннексии А5-«позитивных» мононуклеаров, наиболее выраженное при остром инфаркте миокарда, сопровождается повышением в крови растворимых форм sApo-l/Fas, sBcl-2 и аннексина А5, обладающих антиапоптотическими свойствами.

Впервые показано, что после лечения, включающего статины, у больных острым коронарным синдромом отмечается снижение количества циркулирующих CD59- и СВ105-«позитивных» мононуклеаров, а также мононуклеаров в раннем апоптозе (аннексии А5-«позитивных»), что свидетельствует о противовоспалительной и антиапоптотической эффективности данной группы препаратов.

Показано, что дисфункция эндотелия при остром коронарном синдроме, обусловленная повреждающим действием гомоцистеина, липопротеина (а), вчСРБ и антител к окисленным липопротеинам низкой плотности, сопровождается активацией клеточных и гуморальных противовоспалительных и антиапоптотических механизмов, направленных на снижение тромбофилии при данном заболевании.

Научно-практическая значимость.

1. Полученные в исследовании результаты расширяют современные представления о механизмах дисфункции эндотелия при остром коронарном синдроме.

2. Установлено, что наиболее значимая роль в повреждении эндотелия и развитии гемостатической и адгезионной форм эндотелиальной дисфункции принадлежит липопротеину (а), вчСРБ и антителам к окисленным липопротеинам низкой плотности.

3. Доказана целесообразность одновременного определения содержания вчСРБ, липопротеина (а) и антител к окисленным липопротеинам низкой плотности при остром коронарном синдроме в первые 72 ч после дестабилизации клинического состояния для скринингового исследования степени выраженности эндотелиальной дисфункции.

- 134. Показана диагностическая значимость определения гомоцистеина для оценки плазмо-коагуляционной и тромбоцитарной тромбофилии при остром коронарном синдроме.

5. Обоснована необходимость изучения наряду с биохимическими показателями дисфункции эндотелия клеточных и гуморальных маркеров воспаления и апоптоза для оценки выраженности тромбофилии при остром коронарном синдроме.

Основные положения, выносимые на защиту.

1. Наиболее значимая роль в повреждении эндотелия и развитии гемостатической и адгезионной форм эндотелиальной дисфункции принадлежит липопротеину (а), вчСРБ и антителам к окисленным липопротеинам низкой плотности.

2. Информативность одновременного определения уровней вчСРБ, липопротеина (а) и антител к окисленным липопротеинам низкой плотности для оценки функционального состояния эндотелия выше, чем прогностическое значение каждого из этих показателей в отдельности.

3. Роль гомоцистеина как фактора повреждения сосудистой стенки заключается в усилении плазмо-коагуляционной и тромбоцитарной тромбофилии при остром коронарном синдроме.

4. Биохимической основой дисфункции эндотелия при остром коронарном синдроме, обусловленной воздействием изученных факторов повреждения сосудистой стенки, является увеличение продукции тромборегуляторов, молекул адгезии и ускорение апоптоза эндотелиоцитов, что приводит к усилению тромбофилии при данном заболевании.

5. Усиление клеточного апоптоза при остром коронарном синдроме сопровождается активацией противовоспалительных и антиапоптотических механизмов, направленных на уменьшение выраженности тромбофилии за счёт снижения тромбогенного потенциала эндотелиоцитов, подвергшихся апоптозу.

Апробация работы. Материалы диссертации представлены на II Международной научной конференции «Клиническая гемостазиология и гемореология в сердечно-сосудистой хирургии» (Москва, 2005), на 1-м Всероссийском научном форуме «Инновационные технологии медицины XXI века» (Москва, 2005), на VII Всероссийской научно-практической конференции «Актуальные вопросы клиники, диагностики и лечения больных в многопрофильном лечебном учреждении» (Санкт-Петербург, 2005), на V Всемирном конгрессе по иммунопатологии и аллергии и V Европейском конгрессе по астме (Москва, 2007), на Всероссийской научно-практической конференции с международным участием «Острый коронарный синдром: механизмы развития и современная тактика лечения» (Санкт-Петербург, 2007), на XI Всероссийском научном форуме «Молекулярные основы иммунорегуляции, иммунодиагностики и иммунотерапии» (Санкт-Петербург, 2007).

Результаты работы обсуждены и одобрены на заседании проблемной комиссии «Патология сердечно-сосудистой системы» совместно с кафедрой патофизиологии СПбГМУ им. акад. И.П. Павлова (2008 г.).

Комплексное обследование больных включало стандартные методы исследования системы гемостаза и уровня липидов, а также определение содержания факторов эндотелиального повреждения (гомоцистеина, вчСРБ, липопротеина (а) и антител к окисленным липопротеинам низкой плотности), показателей дисфункции эндотелия (клеточно- сосудистой молекулы адгезии 1 типа, антигена тканевого активатора плазминогена, ингибитора тканевого активатора плазминогена, тромбомодулина) и апоптоза (sApo-l/Fas, sBcl-2, аннексина А5, антител к аннексину А5).

Определение содержания маркеров дисфункции эндотелия (тканевого активатора плазминогена, ингибитора тканевого активатора плазминогена, тромбомодулина, клеточно-сосудистой молекулы адгезии) и апоптоза (sBcl-2, sApo-1/Fas, аннексина A5), а также уровня гомоцистеина и антител к окисленным липопротеинам низкой плотности и аннексину А5 в плазме крови больных острым коронарным синдромом проводилось автором самостоятельно.

Анализ полученных результатов проведён автором самостоятельно.

Объем и структура диссертации. Диссертация изложена на 287 страницах машинописного текста, состоит из введения, обзора литературы, материалов и методов исследований, результатов собственных исследований и их обсуждения, общего заключения, выводов, рекомендаций по использованию научных выводов и списка цитируемой литературы, включающего 30 отечественных и 672 зарубежных авторов. Работа иллюстрирована 30 рисунками и содержит 31 таблицу.

ВЫВОДЫ

1. При остром коронарном синдроме отмечается достоверное повышение в крови антител к окисленным липопротеинам низкой плотности, наиболее выраженное у больных с инфарктом миокарда без подъёма ST.

2. Острый коронарный синдром сопровождается развитием плазмо-коагуляционной и тромбоцитарной тромбофилии, особенно выраженной у больных с гипергомоцистеинемией.

3. Увеличение содержания антигена тканевого активатора плазминогена (t-РА), ингибитора тканевого активатора плазминогена (PAI-1) и тромбомодулина свидетельствует о развитии у больных острым коронарным синдромом гемостатической формы дисфункции эндотелия, обусловленной сочетанным воздействием гомоцистеина, вчСРБ, липопротеина (а) и антител к окисленным липопротеинам низкой плотности.

4. Повышенный уровень клеточно-сосудистой молекулы адгезии 1 типа (sVCAM-1) отмечался при увеличении содержания в крови как одного, так и нескольких факторов, повреждающих эндотелий, что свидетельствует о развитии при остром коронарном синдроме адгезионной формы дисфункции эндотелия.

5. При остром коронарном синдроме, особенно при инфаркте миокарда с подъёмом сегмента ST, отмечается одновременное повышение нескольких маркеров эндотелиальной дисфункции, что свидетельствует о сочетании гемостатической и адгезионной форм дисфункции эндотелия в результате повреждающего действия гомоцистеина, липопротеина (а), вчСРБ и антител к окисленным липопротеинам низкой плотности.

6. Острый коронарный синдром сопровождается увеличением содержания в крови sApo-1/Fas и sBcl-2, обладающих противовоспалительными и антиапоптотическими свойствами.

7. Повышение в крови больных острым коронарным синдромом аннексина А5 связано с дисфункцией эндотелия в результате повреждающего действия гомоцистеина, вчСРБ, липопротеина (а) и антител к окисленным липопротеинам низкой плотности.

8. Антитела к аннексину А5 являются дополнительным тромбогенным стимулом, усиливающим повреждение эндотелия при остром коронарном синдроме.

9. Установленная взаимосвязь между количеством циркулирующих аннексии А5—«позитивных» мононуклеаров и содержанием факторов повреждения эндотелия свидетельствует о роли гомоцистеина, вчСРБ, липопротеина (а) и антител к окисленным липопротеинам низкой плотности в усилении апоптоза эндотелиоцитов при остром коронарном синдроме.

10. Увеличение количества циркулирующих CD59- и СОЮ5-«позитивных» мононуклеаров при остром коронарном синдроме свидетельствует об активации в условиях дисфункции эндотелия клеточных противовоспалительных механизмов в ответ на повреждающее действие гомоцистеина, вчСРБ, липопротеина (а) и антител к окисленным липопротеинам низкой плотности.

ПРАКТИЧЕСКИЕ РЕКОМЕНДАЦИИ

1. Для оценки функционального состояния эндотелия при остром коронарном синдроме в первые 72 часа после дестабилизации клинического состояния целесообразно определять в комплексе содержание липопротеина (а), вчСРБ и антител к окисленным липопротеинам низкой плотности.

2. Количественное определение циркулирующих аннексии А5-«позитивных» мононуклеаров, а также CD 59- и CD 105-«позитивных» мононуклеаров в динамике представляет собой один из возможных способов оценки эффективности различных классов лекарственных препаратов, в том числе статинов.

3. Определение клеточных и гуморальных маркеров воспаления и апоптоза может быть рекомендовано для оценки выраженности тромбофилии при остром коронарном синдроме.
